Abstract

Objective: Pregnancy in advanced maternal age (AMA) was classified as high risk. The study aims to provide a better description of pregnancy outcome in AMA. Method: The cross sectional study was to review the demography, medical and obstetrics problems, mode of delivery, maternal and fetal outcome in Tangerang General Hospital as a referral center in Banten. The data were taken from medical records of 35-year-old and above women age who delivered on the period of January to December 2014. Result: The total number of delivery and live birth was 6,107 and 5,926 respectively, including 1,548 (25.36%) pregnancies in AMA. The prevalence of grandmultiparities was 11.4%. The average education level was mostly elementary. The prevalence of hypertension in pregnancy was 34.6%. There was increasing trend of miscarriages from 10.8% in 35-40-year-old group to 25% in above 45- year-old group. Pregnancy complications were higher consisting of 3.2% multifetal, 16.6% malpresentation, and 7.1% placenta previa. Cesarean section rate was 33.6%. Maternal near-missed cases were 56 of 1000 and the maternal mortality rate (MMR) was 932 of 100,000 live births. The prevalence of stillbirth and perinatal mortality were 5.3% and 12.9%; respectively. Conclusion: Adverse maternal and fetal outcome were higher in AMA as it takes special attention and multidiscipline-approached care started from preconception, antenatal, preparation of delivery, and also postpartum care. Keywords: advanced maternal age (AMA), fetal outcome, maternal outcome, pregnancy

Abstract

Introduction: Nutritional problems in pregnant women in Indonesia reach 50%, this is still a concern. Based on Basic Health Research (Riskesdas) 2018, 17.5% CED pregnant women in Central Java. Mid-Upper Arm Circumference (MUAC) is an examination to determine nutritional status of pregnant women. Poor nutritional status can cause anemic neonates. According to research at RSUP Dr. Hasan Sadikin Bandung 2018, prevalence of anemic neonates is 14.5% which is still a problem. Objectives: This study aimed to determine relationship between MUAC in third trimester and incidence of anemic neonates. Methods: This study used a cohort study of pregnant women and neonates at Dr. Oen Solo Baru. The sampling technique was purposive sampling. The samples were 80 respondents with the independent variable being MUAC in third trimester and the dependent variable being anemic neonates. Data were analyzed using Spearman's correlative test. Results: In 40 (50%) respondents with normal MUAC which is 23.5 – 28.5 cm, there were 31 (77.5%) normal haemoglobin (Hb) neonates and 9 (22.5%) anemic neonates. In 40 people (50%) were malnourished which is <23.5 cm, there were 13 (32.5%) normal Hb levels neonates and 27 (67.5%) anemic neonates. Based on statistical tests, there is a relationship between third trimester MUAC and incidence of anemic neonates with p = 0.000 (<0.05) and the correlation coefficient value is 0.452 which indicates a sufficient correlation. Conclusions: The conclusion is there is a relationship between MUAC for pregnant women in third trimester and incidence of anemic neonates.

Abstract

The use of contraception is important to control population growth, regulate the interval between pregnancies, avoid unwanted births, control the time of birth in a husband and wife relationship and determine the number of children in the family. According to the Central Statistics Agency (BPS) of East Java Province in 2018, the number of active Family Planning (KB) participants of married women of childbearing age who use hormonal contraception in the form of pills was 19.26%, injections were 50.44%, and implants is 11. 45%. Uterine fibroids occur in 20%-25% of women of reproductive age. The prevalence of leiomyoma may increase with hormonal contraceptives containing the hormone estrogen, either pure or combined. Uterine fibroids can affect the quality of life of the sufferer.Purpose: To analyze the relationship between the use of combined pill contraceptives and those who do not use the combined pill contraception in women with the incidence of leiomyoma.Method: This research is an observational analytic carried out retrospectively with a case-control approach. The sampling technique in this study was non-probability sampling, namely purposive sampling. This study uses secondary data, namely medical records of gynecological patients at the RSPAL Dr. Ramelan Surabaya by meeting the inclusion criteria and exclusion criteria. The data collected will be analyzed by looking at whether leiomyoma patients use combined pill contraception.Result: Patients diagnosed with leiomyoma who used the combined pill contraceptive were 48 (63%), while those who did not use the combined pill were 28 (37%). This study's results indicate a significant relationship between the use of combined pill contraceptives and the incidence of leiomyoma (p=0.001).Conclusion: There is a relationship between the use of combined pill contraceptives with the incidence of leiomyoma.

Abstract

Background: Perineal rupture is the most common complication of vaginal delivery. Similar to the wound healing process in general, the perineal wound healing process is influenced by nutritional factors. Nutritional factors that are known to be related to the wound healing process are protein levels which are reflected in albumin levels. In Indonesia, serum protein levels are not a variable that is routinely monitored in pregnant women. With a high prevalence of perineal wounds, measurement of serum albumin levels can be a means of early detection of maternal nutritional factors that can affect wound healing outcomes. Objective  of this study was to determine the relationship between serum albumin levels and the degree of post-salary perineal wound healingMethods: The study was originally a prospective analytic observational study with a cross-sectional design. In response to the pandemic, the research design was changed to a literature study by searching for scientific publications during the last 5 years in Pubmed, Google Scholar, Sciencedirect, Researchgate, and Mendeley with the keywords Perineum, wounds, postpartum, albumin, nutrition and exclusion for literature studies. is inaccessible PDF and Publication not in the last 10 years .Results: There were 10 literatures regarding wound healing and nutritional status of which 6 literatures were perineal wounds and 4 literatures were wounds related to malignancy. From the literature, it was concluded as many as 10 literatures showed that there was a relationship between nutritional status and wound healing. Conclusion: : After reviewing the ten literatures, it can be concluded that there is a relationship between nutritional status and wound healing in mothers with perineal wounds. Keywords: Perineum, wound, postpartum, albumin, nutrition

Abstract

Introduction: Early menarche is increasing throughout the world, including in Indonesia. The average age of early menarche in Indonesia is less than 10 years based on the results of Riset Kesehatan Dasar or Basic Health Research 2013. Early menarche can cause stress, cardiovascular disease, and the risk of breast cancer. Early menarche can be influenced by various factors, one of which is nutritional status which can be assessed by body mass index. Objective: To determine the relationship between Body Mass Index and Menarche Age in elementary school “X” in Krian.Method: This study used a cross-sectional research design, conducted on a population of elementary school children in grades IV-VI aged 9-12 years at primary school “X” in Krian with the inclusion criteria being School Health Unit (Unit Kesehatan Sekolah or UKS) data and the exclusion criteria being having chronic diseases. Data analysis was carried out using the Spearman Correlation test. Results: 49 samples were obtained consisting of 7 people in class IV, 14 people in class V, and 28 people in class VI. From the sample, 8 children had a low BMI (16.3%), 30 children had a normal BMI (61.3%), and 11 children had a high BMI (22.5%). Meanwhile, for early menarche 6 people (12.2%), normal menarche 43 people (87.8%), and no late menarche (0%) obtained a p-value = 0.616 (p>0.05). The percentage of early menarche in obese BMIs is higher than in other BMIs with a percentage of 6.1%.Conclusion: There is no significant correlation between body mass index and age at menarche due to the small number of samples.
