Abstract

Numerous inflammatory, neoplastic, and genetic skin disorders are associated with interstitial lung disease (ILD), a fibrosing inflammation of lung parenchyma with significant morbidity and mortality. There is inconsistent data regarding the association between bullous pemphigoid and pneumonia. This article details the clinical manifestations of bullous pemphigoid (BP) and pemphigus vulgaris (PV) associated with interstitial pneumonia, specific diagnostic strategies, underlying mechanistic connections, and possible treatments.

Abstract

During the menopause transition, several symptoms such as muscle and joint pain, back pain, vasomotor symptoms, urogenital symptoms, and psychological disorders may occur due to declining estrogen levels. Approximately a third of women would experience severe symptoms with a need for treatment. Currently, menopausal hormonal therapy (MHT) is still considered to be the first-line therapy despite its risk of cancer. Other options, such as pharmacotherapy or herbal formulas, have their limitation, urging the need for an effective and safe treatment. Standardized purified cytoplasmic pollen (PCP) extract, otherwise known as PureCyTonin® is a novel, non-hormonal, non-phytoestrogenic option that is proven to be effective in reducing menopausal symptoms while being neutral to the proliferation and apoptosis of breast cancer cells. Recent evidence shows promising results of PureCyTonin® for menopausal symptoms treatment.

Abstract

Varicose veins are characterized by the elongation, widening, and twisting of the venous system, accompanied by impaired blood circulation. It usually occurs in lower extremities, with a prevalence of 10%-30%. It affects mainly women, and the prevalence rate increases with age. The CEAP (Clinical, Etiological, Anatomical, and Pathophysiological) classification allows for a more precise diagnosis and provides better results. Treatment includes conservative therapy and intervention. With appropriate management, postoperative care, and rehabilitation, the prognosis of varicose veins is generally good.

Abstract

Hyperglycaemia is a medical condition of increased blood glucose levels, which is characteristic of several diseases, especially diabetes mellitus. Diabetes mellitus is currently a global threat to health. A fixed-ratio combination a insulin glargine and lixisenatide (iGlarLixi) is a productcontaining glargine insulin (a long-acting insulin) and lixisenatide (a glucagon-like peptide-1 receptor agonist) in one single injection. Fixed ratio combination offers many clinical benefits for adults with type 2 diabetes mellitus.

Abstract

Background: Traumatic brain injury (TBI) is a condition resulting from physical trauma to the head which can cause structural damage to the brain and interfere with cognitive, motor and emotional functions. Citicoline is a natural compound that plays an important role in phospholipid metabolism and neurotransmitter synthesis in the brain. This evidence-based case report (EBCR) study aims to provide scientific evidence of the potential benefits of citicoline in improving clinical outcomes in TBI patients. Method: Literature search using the PICO format. The formulation of clinical questions was arranged by case. Search for articles on March 6–7th 2023 using the Publish or Perish (PoP) software with the target of searching articles for the last 10 years (2013-2023) on the Scopus, Pubmed, Google Scholar databases, and manual searches on the Cochrane database, using the keyword 'traumatic brain' injury' AND 'therapy' AND 'citicoline' AND 'neuroprotective' AND 'treatment' AND 'outcome' along with synonyms and related terms. Results: Four studies were obtained, with the level of evidence 1a which resulted from a meta-analysis or systematic review of various randomized controlled trials, which were then critically reviewed. Two studies reported no significance of clinical assessment and the Glasgow Coma Scale (GCS); two other studies showed significant results at the level of independence. Conclusion: This study shows that citicoline is beneficial in the independence of TBI patients but does not produce a significant difference in the Glasgow Coma Scale assessment.

Abstract

Glaucoma is one of the most common causes of blindness in the world. The most common type is open-angle glaucoma. In open-angle glaucoma, intraocular pressure slowly rises, causing nerve damage which will gradually narrow the visual field. Treatment involves surgical as well as medical options. Several classifications of drugs with different mechanisms of action can be combined. Several treatment options will be discussed in this article.

Abstract

Background: Neonatal hypoglycemia is a metabolic problem that is often found in the NICU and is one of the causes of permanent brain damage in infants. The diagnosis of persistent hypoglycemia and its causes are still controversial. Case: A male baby was born to a mother with severe preeclampsia at 36 weeks of gestation, with a birth weight of 1600 grams. At 1 hour of age, he experienced hypoglycemia that lasted more than 72 hours with the lowest blood sugar of 18 mg/dL, and experienced hyperkalemia (potassium level 6.1 mmol/L). The baby is diagnosed as persistent hypoglycemia. The clinical condition improved after being given dextrose and dexamethasone, and was discharged in stable condition. Conclusion: Management of persistent hypoglycemia depends on the cause. Diagnosis refers to risk factors before or after birth.

Abstract

Stroke is a major cause of disability and cognitive impairment, accounting for 5.2% deaths worldwide. Antiplatelets and anticoagulants have been widely used as prevention of recurrent stroke. Recently, lumbrokinase has been used in stroke and other cardiovascular patients, but further research is needed.

Abstract

Carbon monoxide (CO) poisoning is one of the leading environmental emergencies with high mortality and morbidity. The brain and heart are especially at-risk organs of being affected. Clinical manifestations may closely mimic various nonspecific viral illnesses. Diagnosis is made based on of clinical symptoms, patient history, and the location’s circumstances. Current therapy recommendations for CO poisoning are 100% normobaric oxygen (NBO) or hyperbaric oxygen (HBO). Non-invasive ventilation CPAP (continuous positive airway pressure) method is effective as a therapy for CO poisoning in the emergency room because it can provide tissue oxygenation and eliminate CO quickly.

Abstract

Pituitary apoplexy defined as bleeding or infarct on pituitary gland often occurs in undiagnosed pituitary tumour. Mechanisms thought to trigger pituitary apoplexy are fluctuations in blood pressure, hormonal stimulation of the pituitary gland, coagulation disorders, or vascular disorders. The case is a male, aged 73 years, from India, with complaints of chest pain in the last four hours. Electrocardiography (ECG) and laboratory examination results lead to an acute coronary syndrome (ACS) event. Patients are given fondaparinux, aspirin, and clopidogrel. On the 4th day of hospitalization, the patient experienced a severe headache, projectile vomiting, and double vision. Radiological examination reveals a pituitary tumour with signs of diffuse haemorrhage compressing the optic chiasma, suggesting a pituitary tumour apoplexy. Hormone function tests show a non-functioning pituitary tumour. The blood thinners were discontinued, vitamin K and dexamethasone were given to reduce the patient symptoms. After a few days, the headaches and double vision began to improve. Head MRI showed stable mass of bleeding. Pituitary tumour may be a relative contraindication to dual antiplatelets and anticoagulants in acute coronary syndromes especially in group with renal or liver comorbidities.
