Abstract

Primary brainstem hemorrhage is an intracerebral hemorrhage that affects the brainstem, primarily caused by chronic hypertension. Clinical manifestations include decreased consciousness, motor weakness, respiratory problems, hyperthermia, and pupillary abnormalities. Diagnosis is with a head CT scan/MRI, bleeding was found in the mesencephalon, pontine, or medulla oblongata. Management is conservative with stabilization of vital signs, correction of hemostatic abnormalities, and intensive monitoring of neurological symptoms. Surgery is still controversial because of the lack of clinical and research evidence. The prognosis is generally grave depending on the degree of initial loss of consciousness, location, and volume of bleeding.

Abstract

Ocular complications rarely occur in dengue fever, but were reported increasing. Several studies showed that cross-reactive antibody is responsible for ocular involvement in dengue. Blurred vision, followed by posterior segment involvement was seen in most cases. A simple examination in primary medical care such as the Amsler grid may detect scotoma and metamorphopsia. Early recognition of ocular involvement in dengue cases may prevent morbidity.

Abstract

Introduction: COVID-19 can be life-threatening in older age group and those with comorbidities. This is a study on the clinical characteristics of COVID-19 patients in rural field hospitals in Indonesia. Method: A retrospective, non-experimental study on COVID-19 patients in LeonaKefamenanu Hospital, East Nusa Tenggara, Indonesia from January 2021 to December 2021. Multivariable logistic regression analysis was used to analyze the factors associated with severe COVID-19. Results: During the period, 153 patients were registered, 66 (43.1%) were male; mostly 40 to 59 years of age. The most common symptom was cough 144 (94.1%), followed by fever (n=105, 68.9%), fatigue (n=92, 60.1%), and nausea (n=80, 52.3%). Most patients had normal leukocyte, lymphocyte, and platelet counts; 17.6% of patients had hypokalemia, more frequently seen in severe cases (p<0.029). Most patients had an abnormality in their chest x-ray on admission (63.4%) and all were severe COVID-19. Multivariatelogistic regression analysis showed that age ≥60 years (OR = 8.98, 95%CI: 1.81–44.56, p =0.007) and dyspnea (OR = 16.83, 95%CI: 1.95–144.69, p =0.010) were independently associated with severe cases. Fatigue (OR = 4.606, 95%CI: 1.589–13.354, p <0.005) was also independently associatedwith disease deterioration. Conclusion: Age ≥60 years, fatigue, and dyspnea were associated with severe cases in Leona Kefamenanu Hospital, East Nusa Tenggara, Indonesia.

Abstract

Atrial fibrillation (AF) is one of the most common arrhythmias globally, and its prevalence is projected to increase, including in Indonesia. AF progression is related to lower quality of life and less responsive to therapy or even becomes irreversible. The progression of AF is significantly slower with the rhythm control strategy than with the rate control strategy alone. The options for therapy in Indonesia are limited to amiodarone and propafenone. Amiodarone is one of the most common drugs prescribed for AF rhythm control, but associated with multiple organ toxicities, thus increasing the risk of multiple side effects, especially on long-term usage, potentially leading to drug discontinuation. Flecainide is an antiarrhythmia with a favourable effectiveness and safety profile, especially for AF management, used for acute cardioversion, long-term rhythm control, and as a therapeutic modality for AF with preexcitation syndrome. Flecainide is a potential drug to address the unmet needs of amiodarone as a long-term management for AF.

Abstract

Background: Chorea hyperglycemia basal ganglia (C-H-BG) syndrome is an uncommon manifestation of diabetes mellitus seen in patients with poor glycemic control. Case: A 74-year-old poorly controlled diabetic women with acute onset of movement disorder in the form of uncontrolled choreiform movements of her left upper and lower extremity. Her initial glucose level was 289 mg/dL and HbA1C was 9.5%. Discussion: Most C-H-BG patients are older Asian female. Female predisposition is probably related to the estrogen decline during menopause, resulting in gamma-aminobutyric acid (GABA) depletion or dopamine receptors hypersensitivity and enhancement of dopaminergic system. Its pathophysiology has not been entirely defined. Conclusion: C-H-BG is a rare manifestation of poorly controlled diabetes. Early recognition and treatment of elevated blood glucose levels appear to lead to total resolution.

Abstract

Introduction: Pancreatic adenocarcinoma stands as one of the most devastating forms of cancer, with a survival rate of less than 5% after 5 years. Despite resection surgery being the only curative option, recurrence occurs in up to 85% of cases. It is crucial to identify factors that can accurately predict the probability of recurrence for more effective management strategies. Method: Following the PRISMA guidelines, a systematic review was carried out by extracting relevant studies from the PubMed MEDLINE database. Results: A total of 1,345 studies were initially identified, and 34 studies were analyzed. A total of thirty-five predictive factors were identified and classified into preoperative and postoperative categories. These factors were be further subdivided into patient characteristics, biomarkers, tumor characteristics, and genetics. The most frequently observed factors were lymph node metastasis, serum CA 19-9 level, and tumor size. Conclusion: Preoperative and postoperative patient characteristics, genetics, biomarkers, and tumor characteristics data can be utilized to predict and implement suitable follow-up strategies aimed at enhancing patient survival rates.

Abstract

Osteomyelitis is a bacterial infection in bone or bone marrow. This study was a retrospective descriptive study of chronic osteomyelitis cases at Kartika Husada Level II Army Hospital of Kubu Raya Regency from January 2015 to December 2017 as well as a description of its management. There were 47 subjects taken based on their medical records. Surgical management was done in 39 cases (83.0%), consisting of debridement and sequestrectomy (33 cases), debridement (4 cases), and amputation (2 cases). Surgical management was combined with non-surgical management.

Abstract

Lesbian, gay, bisexual, transgender (LGBT) face many gaps in sexual health aspects, including an increased risk of human immunodeficiency virus (HIV infection), sexually transmitted infections (STIs), and pregnancy. LGBT often experience social stigmatization, isolation, and rejection, even from their respective parents. Risk of STIs is related to sexual orientation, sexual anatomy, and sexual behaviour. LGBTs sexual activity may vary and with multiple partners, who are vulnerable to transmission of STIs and HIV infection. Health care providers are responsible for an open discussion regarding sexual health issues; including sexual orientation, sexual behavior, and gender identity.

Abstract

Background: Cognitive impairment in acute ischemic stroke is associated with disability, poor functional outcome, and mortality, hence requires effective management. Citicoline is one of pharmacotherapy for central nervous system disorders. This study aims to evaluate current evidence of the effectiveness of citicoline for cognitive impairment in acute ischemic stroke. Methods: Systematic review and meta-analysis on randomized controlled trials searched in PubMed, Cochrane, and Google Scholar databases. Screening was done on 518 articles according to inclusion and exclusion criteria, 3 articles were qualified for review. Results: Cano-Cuenca, et al, (2015) found that citicoline was associated with a mini-mental state examination (MMSE) score of >25 at 12 weeks. Pinzon & Asanyi (2018) found that the use of citicoline can prevent cognitive impairment at 6 and 12 months, especially in attention and temporal orientation. Bonvicini, et al, (2022) found that citicoline is associated with good cognitive outcomes as measured with global cognitive impairment. Conclusion: Citicoline is effective as a therapy for cognitive impairment in acute ischemic stroke.

Abstract

Ischemic stroke, caused by a blockage in the brain’s blood vessels, is a critical emergency that requires immediate treatment. This study examined the efficacy of citicoline as an adjunct therapy to improve outcomes in acute ischemic stroke. A comprehensive search of four databases (Cochrane Library, ScienceDirect, PubMed, and Google Scholar) was conducted, identifying a total of 5265 articles. Three clinical trials were selected for critical appraisal. Agarwal demonstrated a larger reduction in stroke infarct volume in the citicoline group (4.22 cm3) compared to the placebo group (2.63 cm3) (p=0.483). On day 90, the citicoline group exhibited superior results in mRS scores of 0-2 (p=0.732), NIHSS scores of 0-2 (p=0.934), Barthel index ≥95 (p=0.564), and patient mortality (p=0.468). Seifaddini’s study revealed improved peak systolic velocity and mean arterial flow velocity in the cerebrovascular system. Enrico’s study showed improved mean short afferent latency inhibition in the cholinergic circuits of the citicoline group (p=0.004). Citicoline proves effective as an adjuvant neuroprotective and neuroregenerative therapy for acute ischemic stroke.
