Abstract

Background: Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease characterized by symmetrical joint involvement and extra-articular manifestations. Methotrexate and leflunomide are disease modifying anti-rheumatic drugs(DMARD) and use as treatment options for rheumatoid arthritis. This report aims to compare the effectiveness of leflunomide with methotrexate as therapy for rheumatoid arthritis. Methods: Literature search was carried out through three electronic databases: PubMed, Cochrane, and EBSCOHost with specific keywords based on PICO. Selection was in accordance with inclusion and exclusion criteria based on the clinical question. Selected literature was critically reviewed using an adapted instrument from the University of Oxford’s CEBM. Result: One systematic review and meta-analysis literature by Alfaro-Lara, et al, (2017) was obtained. The results showed a trend towards methotrexate in achieving response according to American College of Rheumatology (ACR20) (0.88 [95% CI: 0.74-1.06]; p = 0.18), reduction in joint swelling was greater with methotrexate (-0.82 [95% CI: 0.24-1.39]; p = 0.006), no significant difference in joint pain reduction (p>0.05). Liver enzyme elevations were more frequent with leflunomide (0.38 [0.27-0.53]; p = 0.007); gastrointestinal (GI) complaints were more frequent with methotrexate (1.44 [1.17-1.79]; p = 0.0007), no significant difference in the incidence of non-severe infections. Conclusion: Leflunomide has the same efficacy as methotrexate for RA therapy; more reduction of joint swelling with methotrexate. Leflunomide was associated with greater increases in liver enzymes, but fewer GI complaints.

Abstract

Stroke patients are vulnerable to malnutrition, which is associated with poor outcomes, both in the acute and rehabilitation phases. Malnutrition at admission and one week after admission is a predictor of poor outcomes within or after three months of stroke. Adequate nutritional assessment and management are necessary to improve the outcomes of stroke patients. The CONUT score can be used for nutrition assessment in stroke patients.

Abstract

Background: Indonesia reported 1,143 penile carcinoma cases from 2004 to 2013. Bali reported the highest age-standardized incidence rate (ASR) in 2004-2013. Method: A quantitative descriptive retrospective study was conducted at Karangasem General Hospital and Balimed Karangasem Hospital. Results: This study involved 29 patients diagnosed with penile cancer in 2020-2022. The highest number of cases was found in 2020. Most cases were in the 60-69 year age-group (34.5%), with a low educational level (75.9%), a smoking habit (75.9%), no history of circumcision (100%), and squamous cell carcinoma (86.2%). The treatment was mainly partial penectomy (41.3%). Conclusion: The study found that the highest prevalence of penile cancer was 6.02 per 100,000 people in 2020.

Abstract

Background: Stroke is the third most common disease after heart disease and cancer and the main cause of disability worldwide. The most common type of stroke, which accounts for around 85%-87% of cases, is ischemic stroke (IS). This report is to examine the benefits of citicoline for IS patients. Case: A 63-year-old male was admitted with a diagnosis of IS and electrolyte disturbances. General treatment is semi-Fowler’s position, oxygen, and maintaining fluid and nutritional intake. Specific management is RL infusion 20 gtt/minute, candesartan 1x16 mg, clopidogrel 1x75 mg, omeprazole 40 mg/24 h intravenous (IV), and citicoline 500 mg/12 h IV. The patient showed clinical improvement. Method: Articles in PubMed, Cochrane, and Google Scholar in February-April 2022 were searched to find relevant studies to be evaluated with inclusion and exclusion criteria. The full text was obtained and critically reviewed with the evidence-based medicine guidelines. The results are limited to systematic reviews and meta-analyses. Results: Five studies were deemed valid, important, and applicable. However, the quality of these studies varies. Conclusion: All studies show that citicoline use could potentially improve clinical outcomes in IS patients.

Abstract

Background: Choline-containing phospholipids, citicoline, and choline alphoscerate have been proposed as adjuvants in acute stroke treatment. An evidence-based case report (EBCR) was conducted to examine the benefits of citicoline in hemorrhagic stroke (SH) patients. Method: Search for articles published from October 2019-2023 from PubMed, Cochrane, Google Scholar, and ProQuest sources using the keywords “Citicoline”, “Hemorrhagic Stroke”, “Choline”, and “Intracerebral Hemorrhage”. The search results were evaluated using inclusion and exclusion criteria. Selected full-text studies were evaluated. The final selection results were critically reviewed based on validity, importance, and applicability by the three authors. Results: One study was considered valid, important, and applicable. The quality of this study is very high based on the literature review. Conclusion: The study shows that citicoline has the potential as an adjuvant therapy to improve clinical outcomes in SH patients, although it does not sufficiently improve neurological or functional outcomes.

Abstract

Graves ophthalmopathy (OG) is an inflammatory disorder of orbital tissue associated with autoimmune thyroid disease. The complex interaction of endogenous and exogenous or environmental (modifiable) factors are known to trigger the occurrence of OG. The symptoms can have a negative impact on the patient’s quality of life, both physical and psychosocial, and become a burden on public health costs. Early diagnosis, management, and immediate referral to related specialists are very important to prevent the progression of the disease.

Abstract

Acute rheumatic fever is an autoimmune disease following group A β-hemolytic Streptococcal infection. The disease may present with variable symptoms and signs. Cases: The first case is an 8-year-old boy with arthritis and previous streptococcal infection. The treatment was glucocorticoids and antibiotics. The second case is a 17-year-old boy with prolonged fever and palpitations. Carditis was found on echocardiography. The treatment was aspirin and antibiotics. The third case is a 17-year-old girl with a congenital heart defect. The patient had severe carditis as well as arthritis. The treatment was diuretics, antibiotics, steroids, antiarrhythmics, and beta-blockers. Management differed in each case, but all had favorable outcomes. Conclusion: Clinicians should be aware of the various manifestations of rheumatic fever and come to a swift diagnosis to initiate prompt treatment, which thus can decrease disease burden and morbidity.

Abstract

Introduction: Smoking, which involves tobacco as the primary material, has extensive impacts on environmental and human health. This study incorporates literature and research related to the environmental impacts of smoking. Methods: This research uses the literature review method, referencing relevant sources. Document searches were conducted through Google Scholar and manual searching using the keywords “(Smoking) AND (Environmental Health).” Results: The environmental impacts of smoking are highly complex, including air pollution, littering from cigarette butts, and the effects of tobacco processing and production on land and water. Regulatory issues pose challenges in controlling the negative environmental impacts of smoking. Discussion: Air pollution resulting from smoking includes a significant increase in carbon dioxide emissions and a substantial contribution to climate change. Littering cigarette butts leads to the accumulation of harmful plastic waste in marine ecosystems and human health. Tobacco production also carries serious consequences, including deforestation, agrochemical use, and pollution. Controlling tobacco and cigarette use requires comprehensive measures, including cost regulation, advertising bans, and strict monitoring. Stronger regulations and collaborative efforts are needed to address the environmental impacts of smoking effectively. Conclusion: Smoking has significant impacts on environmental health, including air pollution, littering pollution, and the effects of tobacco production. Further efforts are necessary to control these negative impacts through effective regulations and raising public awareness of the environmental consequences of smoking.

Abstract

Koebner phenomenon is defined as the development of psoriatic lesions on previously healthy skin of a psoriasis patient following skin trauma. A related skin reaction, known as Wolf's isotopic response, involves the appearance of a new skin condition in an area of healed dermatosis; in many cases, from herpes zoster. The exact cause of this link is not fully understood, but it may involve neuroimmune system and its neuromediators, particularly substance P. This paper serves as a reminder on the importance of recognizing dermatologic phenomena.

Abstract

Background: Dengue haemorrhagic fever (DHF) is caused by dengue virus type 1-4; it is still endemic in Indonesia. Case: A 1 year and 11 months--old baby-girl with fever, lethargic, nausea and vomiting, and thrombocytopenia. She was diagnosed with dengue hemorrhagic fever. Pleural effusion and ascites were diagnosed after fluid resuscitation. Clinical improvement was observed after furosemide therapy and could be discharged after seven days of treatment. Conclusion: Diuretic administration can be considered in case of dengue haemorrhagic fever with severe plasma leakage. Further research is needed.
