Abstract

Background: Otitis media with effusion (OME) is the presence of middle ear fluid without acute ear infection signs or symptoms. OME is one of the causes of hearing loss. Chronic asymptomatic OME could be managed by careful waiting for 3 months because it is a self-limiting disease. During surveillance, autoinflation is a beneficial intervention with minimal side effects and high adherence. Objective: To evaluate the effectiveness of autoinflation to improve hearing in otitis media with effusion and hearing loss. Methods: Literature searching was conducted through PubMed, Cochrane, ScienceDirect, Scopus, and EBSCOHost with inclusion and exclusion criteria. The articles were selected and underwent critical appraisal using rapid critical appraisal of systematic review and level of evidence by the Oxford Centre for Evidence-based Medicine in 2011. Result: One systematic review and meta-analysis, or RCT, met the criteria. This article revealed a pooled estimate of RR 1.74; 95% CI [1.22-2.50] with a Z-score of 3.02 (p=0.0025). This result showed substantial significant (p=0.01) heterogeneity (I2=69%). Conclusion: OME patients with hearing loss who performed autoinflation >1 month had a 1.74 times greater chance of achieving hearing improvement than controls. Autoinflation with the Politzer method gives significant results in improving hearing loss. Large-scale RCT research is recommended to evaluate the effectiveness of autoinflation with the Politzer method or other available methods in OME patients with hearing loss.

Abstract

People with diabetes mellitus have a dysregulated immune system and are more susceptible to infection, which poses a higher risk of severe influenza complications. Current evidence for influenza vaccine effectiveness in people with diabetes mellitus indicates a beneficial reduction of morbidity and mortality. In addition, serological studies show antibody levels elicited from influenza vaccination in diabetes patients show comparable results with healthy adults. Although more controlled clinical trials in diabetes mellitus patients with influenza are needed to reflect the effect of vaccination, current data indicate the need for routine influenza vaccination in diabetes mellitus patients, as recommended by guidelines. Efforts should be made to increase vaccination coverage in this risk group.

Abstract

Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death worldwide. Pulmonary rehabilitation is an essential component of COPD management and can be delivered through telerehabilitation programs. COPD telerehabilitation consists of exercise testing and training that can be performed at home with supervision from a physiotherapist or remotely using a smartphone application. Exercise training includes cardiorespiratory endurance, resistance, flexibility, and respiratory muscle training. COPD telerehabilitation is effective in improving exercise tolerance, reducing the incidence of acute exacerbations, and improving patients’ quality of life.

Abstract

Critically ill patients are patients in life-threatening conditions and will result in significant increment of mortality and morbidity if not given immediate medical intervention. Critically ill patients, especially those using ventilators, often experience pain. Pain can be caused by interventions or procedures in the ICU or by other unknown causes. Pain management in critically ill patients can be non-pharmacological or pharmacological.

Abstract

Due to their aesthetic demand and high training load, ballet dancers are often at risk of low energy availability (LEA), with or without eating disorders (ED). A prolonged state of LEA may alter physiological function and lead to other major health concerns. This study aims to analyze the risk of LEA and eating disorders in female elite dancers at X dance school in Jakarta. This descriptive cross-sectional design study was followed by 11 dancers. Risk of LEA was assessed using the low energy availability in females questionnaire (LEAF-Q), eating behaviors using the eating attitude test (EAT-26) questionnaire, and body composition using the bioelectrical impedance analysis (BIA) instrument. A total of 18.2% of dancers were at risk of LEA (LEAF-Q score ≥8) and no dancers were at risk of eating disorders. The most common characteristics of LEA are gastrointestinal dysfunction (90.9%), followed by menstrual disturbance (45.5%), and musculoskeletal injury (9.1%). Compared to the first study in Indonesia, this study showed lower proportion of dancers at risk of LEA and ED at X dance school in Jakarta. Despite a lower proportion, most dancers experienced other characteristics of LEA, thus prevention and health management are required to reduce further health issues.

Abstract

Primary open-angle glaucoma (POAG) is the second leading cause of irreversible blindness worldwide with increasing prevalence. Oral citicoline has been shown to have a role in glaucoma; however, studies regarding citicoline eye drops for glaucoma are still limited. This is a critical appraisal of the literature on the effectiveness of citicoline in improving the mean deviation (MD) of the visual field and thickness of the retinal nerve fiber layer (RNFL) in POAG. A literature search was done on 4 databases: PubMed, Cochrane, Embase, and Scopus. From the 165 studies initially identified, 3 studies were qualified for critical review. Rossetti demonstrated significant MD (p=0.02) and RNFL (p=0.04) differences after 3 years of citicoline eye drops compared to placebo. Mirachtsis showed that after citicoline administration for 3 months there was a significant difference in RNFL (p=0.029), but no significant improvement in MD (p>0.05). Parisi showed an improvement in MD after 4 months (+0.56 dB) and 6 months (+0.18 dB) of citicoline administration. Citicoline eye drops have the potential to inhibit glaucoma progression by inhibiting RNFL loss, but further research is needed on visual field improvement.

Abstract

Glaucoma is the second-leading global cause of blindness. Lowering intraocular pressure can prevent the worsening of open-angle glaucoma, but alternative neuroprotective therapies are needed; citicoline is supposed to have neuroprotective potential. This evidence-based case report was to review the effectiveness of oral citicoline as an adjunct therapy for glaucoma. From 22 studies identified through PubMed, Scopus, Cochrane, EBSCO Host, and Google Scholar, three articles were selected for critical review. Lanza M. found the RNFL and GCC of the citicoline group 500 mg were significantly higher, 70.39 and 71.19 Pm, p<0,01 compared to controls, after 12 months. Sahin A, et al, showed that the RNFL was significantly higher in the third month than baseline in the oral citicoline 250 mg group, p = 0.038. Putri VC, et al, found that after 60 days, based on sub-group analysis, there was a significant difference in RNFL and GCIPL thickness changes after citicoline 500 mg intervention in the mild glaucoma group and after citicoline 1,000 mg in the moderate-severe glaucoma group. Oral citicoline supplementation prevents significant worsening of glaucoma, it can be given as an adjunct therapy.

Abstract

Background: Stroke is a cause of long-term mortality, morbidity, and disability. Neuroprotectors, such as citicoline, are a choice for patients not eligible for rTPA. This evidence-based case report aims to look at the association of citicoline with the severity and functional status of ischemic stroke. Method: The search is based on the database Scopus. A total of 191 studies were reviewed, and 3 articles were chosen for critical review: Ghosh, et al, (2015), Martynov, et al, (2013), and Mehta, et al, (2019). Results: Significant differences in functional status measured by the Barthel index and modified Rankin Scale (mRS) and severity measured by the NIHSS. Conclusion: Citicoline has a significant effect on the functional status and severity of an ischemic stroke.
