Abstract

Septic arthritis yang disebabkan infeksi bakteri memerlukan kecepatan dan ketepatan diagnosis untuk mengurangi prognosis buruk. Diagnosis pasti tergantung isolasi patogen dari cairan sendi. Tujuan utama tatalaksana adalah sterilisasi sendi, dekompresi sendi, dan melindungi fungsi sendi. Sterilisasi dengan antibiotik intravena minimal 2 minggu, dilanjutkan sesuai hasil kultur. Dekompresi sendi dapat dicapai dengan beberapa metode termasuk aspirasi jarum, arthrotomy, dan arthroscopy.   Septic arthritis caused by bacterial infection reRuired diagnostic speed and accuracy to improve prognosis. Final diagnosis rests on isolation of pathogen from joint fluid. The main management objectives are joint decompression, joint sterilization, and to reserve joint function. Sterilization with empirical intravenous antibiotic for at least 2 weeks, then adjusted based on bacterial culture result. Joint decompression can be achieved by various methods including closed-needle aspiration, tidal irrigation, arthroscopy, and arthrotomy.

Abstract

Aterosklerosis adalah penyakit progresif lambat yang ditandai oleh penebalan bertahap tunika intima. Faktor pemicu aterosklerosis prematur adalah hemolisis kronis, peningkatan cadangan kadar zat besi serum, dan dislipidemia. Aterosklerosis prematur adalah salah satu komplikasi kardiovaskular penting pada penderita thalassemia beta mayor. Pada penderita thalassemia beta, kadar kolesterol total, HDL, dan LDL lebih rendah; sebaliknya kadar trigliserida lebih tinggi dibandingkan individu normal. Dislipidemia pada thalassemia beta disebabkan berbagai faktor, antara lain dilusi plasma akibat anemia, meningkatnya aktivitas eritropoeisis, gangguan fungsi hati karena penimbunan zat besi, gangguan hormonal, dan menurunnya aktivitas lipolisis di luar hati.   Atherosclerosis is a slow progressive disease characterized by gradual intimal thickening. The predisposing factors for premature atherosclerosis are chronic hemolysis, increased serum iron deposit, and dyslipidemia. Premature atherosclerosis has emerged as important cardiovascular complication in beta-thalassemia major. Beta-thalassemia is associated with lower total cholesterol, LDL, and HDL, and higher triglyceride level. Dyslipidemia in beta-thalassemia is caused by several factors, such as plasma dilution due to anemia, increased erythropoietic activity, liver injury due to iron overload, hormonal disturbance, and decreased extra hepatic lipolysis activity.

Abstract

Shoulder impingement syndrome (SIS) adalah sindrom nyeri bahu disebabkan penyempitan celah subakromion. Injeksi steroid adalah prosedur sederhana yang bisa dilakukan pada fasilitas kesehatan primer, tetapi jarang pada pasien SIS di Indonesia. Studi ini bertujuan untuk mengevaluasi efektivitas injeksi steroid bersama program latihan fisik jika dibandingkan dengan latihan fisik saja pada pasien SIS usia >40 tahun. Pencarian artikel sistematis pada tiga basis data: PubMed¥, Cochrane Library¥, dan TRIP Database¥. Dua uji acak terkontrol dipilih untuk dievaluasi berdasarkan kriteria Center of Evidence-based Medicine, University of Oxford. Kedua studi mendapatkan bahwa injeksi steroid efektif menurunkan gejala nyeri dan meningkatkan fungsi bahu hingga enam minggu setelah injeksi. Efek tersebut diprediksi lebih baik jika dosis steroid setara triamsinolon 40-80 mg dengan pendekatan posterolateral.   Shoulder impingement syndrome (SIS) is a syndrome characterized by shoulder pain caused by impingement of subacromial space. Steroid injection is a simple procedure feasible in primary health care, yet seldom used for SIS patients in Indonesia. This study evaluate the efficacy of steroid injection combined with exercise program compared to exercise alone in SIS patients aged >40 years old. Systematic searching was conducted on three databases: PubMed, Cochrane Library, and TRIP Database. Two randomized clinical trials were selected to be appraised based on the criteria from Center of Evidence-based Medicine, University of Oxford. Both studies found that steroid injection is effective in reducing pain and improving shoulder function up until six weeks after the injection. The effect is predicted to be better if steroid dose eRuals to 40-80 mg triamcinolone with posterolateral approach injection.

Abstract

A patient presented with inability to use his right thumb properly after work accident due to ruptured right Extensor Pollicis Longus (EPL). A tendon free grafting from the Palmaris Longus (PL) was performed to repair the ruptured EPL.   Pasien datang dengan keluhan tidak mampu menggunakan ibu jari tangan kanan setelah kecelakaan kerja, dicurigai mengalami ruptur Extensor Pollicis Longus (EPL) kanan. Tindakan free tendon graft dari palmaris longus (PL) dilakukan untuk memperbaiki tendon EPL yang telah ruptur.

Abstract

Penyembuhan luka yang optimal melibatkan berbagai proses biologi dan molekuler berupa migrasi sel, proliferasi, deposisi matriks ekstraseluler, dan remodelling. Berbagai pendekatan penelitian berbasis sel dapat membantu penggunaan sel punca (stem cells) untuk mempercepat proses penyembuhan luka. Tinjauan ini mengenai tipe sel punca yang dapat digunakan untuk penyembuhan luka namun penggunaan klinisnya masih terbatas.   Optimal wound healing includes various biological and molecular processes. Adult stem cells can promote more effective wound therapy. Choosing the right stem cells through in vivo study is important. Further studies is needed for the identification of appropriate adult stem cells, protocols, and building the bioscaffold used for optimal wound healing in the future.

Abstract

Obstructive sleep apnea (OSA) merupakan terjadinya henti napas saat tidur. Standar baku emas diagnosis OSA adalah polysomnography (PSG). Terapi OSA meliputi terapi kelainan anatomis, penggunaan positive airway pressure (PAP), mouth piece, dan operasi. PAP adalah terapi standar; mengeluarkan udara bertekanan tertentu yang akan mempertahankan jalan napas tetap paten. Titrasi tekanan PAP berdasarkan PS( dan pedoman American Academy of Sleep Medicine (AASM). Penanganan yang tepat dan cepat mempengaruhi outcome dan mencegah komplikasi kardiovaskular termasuk stroke.   Obstructive sleep apnea (OSA) is the occurrence of interrupted breath at sleep. The gold standard for diagnosis was polysomnography (PSG). Therapies include anatomical corrections, use of PAP, mouth piece, and surgery. Positive airway pressure (PAP) is the standard therapy; to provide pressurized air to maintain airway patency. Titration of PAP pressure based on PSG monitoring and American Academy of Sleep Medicine (AASM) guidelines. Appropriate and rapid management will affect outcomes and prevents cardiovascular complications including stroke

Abstract

The most common intraocular surgery is cataract surgery. Endophthalmitis is a rare complication of intraocular surgery, but very threatening to sight. Intravitreal injection and vitrectomy pars plana are recommended especially in patients with poor visual acuity. Early follow up and appropriate therapies are very important. Late management can change the prognosis of eyesight.

Abstract

Air susu ibu (ASI) merupakan rekomendasi dan pilihan utama untuk memenuhi kebutuhan nutrisi bayi prematur. Pada beberapa keadaan, ibu tidak dapat memberikan ASI untuk bayi yang terlahir prematur, sehingga donor ASI menjadi pilihan. Masih banyak orang tua yang tidak mengetahui peran ASI donor dan ragu-ragu memberikan ASI donor untuk bayi prematur.   Human breast milk is regarded as the first choice for preterm babies. If breast milk is not available; donor breast milk is the next option. Nevertheless, most parents still have no idea and reluctant to provide donor breast milk for their preterm babies.

Abstract

Necrotizing enterocolitis (NEC) adalah kegawatdaruratan yang mematikan dan sering ditemukan pada bayi prematur atau berat badan lahir rendah. Patogenesis penyakit ini belum jelas dan dipengaruhi banyak faktor. Diduga faktor risiko utama NEC adalah prematuritas, kolonisasi patogen di luminal, dan pemberian susu formula. Ketidakseimbangan flora komensal pada saluran cerna bayi prematur menjadi dasar pertimbangan pemberian probiotik untuk mencegah NEC. Tinjauan pustaka ini mengulas studi pemberian probiotik untuk mencegah NEC pada neonatus prematur.   Necrotizing enterocolitis (NEC) is a severe emergency condition mostly found in preterm or low birth weight infants. The pathogenesis is not well understood and affected by many factors. Suggested risk factors are prematurity, intraluminal colonization of pathogen, and formula milk feeding. Commensal gut bacteria imbalance in premature infant is the reason to give probiotics to prevent NEC. This article reviews studies on probiotic supplementation in preterm neonates to prevent NEC.

Abstract

Pendahuluan: Cedera kepala merupakan satu penyebab utama kematian dan disabilitas di dunia terutama pada usia produktif. Prediksi awal keluaran pasien cedera kepala yang akurat penting untuk menentukan keputusan klinis, alokasi rasional sumber daya, dan konseling keluarga. Penelitian ini bertujuan untuk membandingkan Glasgow Coma Scale (GCS) dengan Midline-Shift (MLS) sebagai prediktor mortalitas pasien cedera kepala. Metode: Penelitian analitik potong-lintang pada 43 pasien. Data GCS dan status pasien saat masuk IGD diambil dari rekam medis RSUD Dr. Abdul Aziz Kota Singkawang dan data MLS diketahui melalui hasil CT-scan di RS Santo Vincentius Kota Singkawang. Analisis data menggunakan uji korelasi Spearman dan dilakukan perbandingan antara GCS dan MLS terhadap status keluar pasien. Hasil: Nilai GCS memiliki hubungan moderat terhadap status keluar pasien (IK 95%; p = 0,018; r = 0,361). MLS memiliki hubungan kuat terhadap status keluar pasien (IK 95%; p = 0,000; r = 0,531). Makin rendah nilai GCS dan makin tinggi nilai MLS, makin banyak status keluar meninggal. Simpulan: MLS memiliki korelasi lebih kuat sebagai prediktor mortalitas daripada GCS pada pasien cedera kepala.   Introduction: Head injury is one of the main causes of death and disability in productive ages. Early and accurate outcome prediction is very important to aid clinical decision, rational allocation of resources and family counseling. The purpose of this study is to compare Glasgow Coma Scale (GCS) and Midline-Shift (MLS) as the predictor of head injury patients’ mortality. Methods: An analytical cross-sectional study was performed on 43 patients. GCS and patient’s data were obtained from RSUD Dr. Abdul Aziz Singkawang medical records and MLS data were obtained from CT-Scan data, done in RS Santo Vincentius Singkawang. The correlation of GCS and MLS to patients’ outcome was analyzed by Spearman correlation test. Results: GCS had moderate correlation to patients’ outcome (CI 95%; p = 0,018; r = 0,361). MLS had strong correlation to patients’ outcome (CI 95%; p = 0,000; r = 0,531). Lower GCS value and higher MLS correlate to higher mortality. Conclusion: MLS had stronger correlation than GCS to mortality of head injury patients.
