Abstract

One of the implementations of controlling medical record documents is by completing medical record documents properly in accordance with SOPs. This study was shown to determine the implementation of medical record document control at Sentosa Baru Health Center. This research is a Quantitative descriptive research. The method used in this study is by Observation. The research was conducted at Sentosa Baru Health Center. The study was conducted from June to August 2023. The population at Sentosa Baru Health Center is 780 medical record documents The sample size in this study was calculated based on the slovin formula so that 80 medical record documents were obtained as research subjects. For the completeness of medical record documents, in the section of writing patient identity, name, time, doctor's signature, there are 80 medical record documents completely (100%). The column of date, month, year, justification for writing medical records there are 40 complete medical record documents (50%) and in the writing section of Assessment, Planning, Screening-vital sign there are 60 complete medical record documents (75%). Thus, the author suggests to the medical record officer at Sentosa Baru Health Center, to re-socialize related to the SOP by filling out medical record documents and then evaluating the suitability between the SOP and the performance of the medical record officer.

Abstract

Chronic renal failure (CRF) is a condition of kidney function that decreases gradually due to kidney damage. Patients undergoing hemodialysis therapy will face problems arising from malfunctioning of the kidneys. Coping mechanism is one way to adapt to stress. Family support is an attitude, action, acceptance of the family and ready to provide help and assistance to one of the families who is sick or needs help and causes a sense of love and affection. The purpose of the study was to determine the effect of individual coping and family support in reducing the anxiety level of hemodialysis patients at Imelda General Hospital Indonesia in 2023. This type of research is quantitative research. The study was conducted at Imelda Indonesia Hospital Medan in August 2023. The study population was 104 hemodialysis patients, the study sample was 52 people with purposive sampling. Bivariate Analysis with Chi Square test. The results of the study The age of the majority of hemodialysis respondents aged 46-55 years (Early Elderly) as many as 26 people (50.0%), the majority sex is male as many as 28 people (53.8%), Long time undergoing hemodialysis the majority of 1-3 years as many as 30 people (57.7%), Adaptive coping (62-100) as many as 49 people (94.2%), Good family support (36-48) as many as 50 people (96.2%), Mild anxiety levels (20-39) as many as 42 respondents (80.8%), Adaptive coping individuals (62-100) had an effect with mild anxiety levels (20-39) as many as 40 people with P Value values of 0.035<0.05. Good family support (36-48) had an effect with mild anxiety levels (20-39) as many as 42 people with P Value 0.013<0.05.

Abstract

Work stress can be experienced by everyone without exception workers in hospitals and is a consequence of workplace events that demand excessive physical and psychological involvement of employees (Triatna, 2015). Work stress that lasts continuously can cause emotional and behavioral changes such as irritability and difficulty concentrating, which can reduce one's work motivation and result in a decrease in work quality The purpose of this study is to determine the picture of work stress of distribution medical record officers at H. Adam Malik Hospital Medan. The type of research conducted is qualitative descriptive. The sample used by researchers took all 5 distribution officers. Research variables are an attribute or trait of people who have certain variations set by researchers, research instruments in the form of questionnaires given to respondents to see the picture of work stress of medical record officers in the distribution section. Data analysis in this study is presented in the form of a distribution table. The results of research conducted on 5 respondents that the majority of work stress can be concluded that as many as 4 respondents (80%) experience work stress, and the minority work stress as many as 1 respondent (20%). From the results of the study above, it can be concluded that the description of work stress of distribution medical record officers at H. Adam Malik Hospital Medan the majority experience work stress. The researcher's suggestion to the hospital is to provide facilities in the form of goods carriers to medical record officers in the distribution department to facilitate the delivery of medical record files to the polyclinic and this can reduce the workload of medical record officers in lifting heavy items.

Abstract

Disorders that occur in nerves and muscles are a problem that is often encountered in society and one of them is facial nerve dysfunction that can affect a person's quality of life called Bell's Palsy. Improving the functional ability of Bell's Palsy patients is managed physiologically, namely patients are given massage and education such as mirror exercises. The purpose of the study was to determine the effect of mirror exercise and massage on the facial functional ability of Bell's Palsy patients in the practice of Physiotherapy Anugerah Sehat. The type of research used is a quantitative type of research, with the research design used is Quasi Experimental with a pre-test and post test one group approach. This research was carried out at the Anugerah Sehat Physiotherapy Practice in April 2023. The population in this study is 30 Bell's Palsy patients.The sampling technique in this research is Total Sampling. Scale used in the assessment of facial functional ability with Ugo Fisch. Bivariate analysis uses the Paired Sample T Test. The results of the research on the frequency distribution of facial functional abilities of Bell's Palsy patients before being given the Mirror Exercise and Massage modalities, the majority had moderate category facial functional abilities at a value of 30-70 as many as 27 people (90%), the frequency distribution of facial functional abilities of Bell's Palsy patients after being given the Mirror Exercise modality and Massage, the majority of whom had normal facial functional abilities at a value of 70-100 as many as 25 people (83.3%), there was an effect of mirror exercise and massage on the facial functional ability of Bell's Palsy patients in the Anugerah Sehat Physiotherapy Practice with a significant value (2-tailed) 0.000<0.05.

Abstract

Quality of health services is the perfection of services in accordance with professional standards and health service standards, this aims to increase the satisfaction of patients who come for treatment. The aim of this research is to determine the relationship between the quality of BPJS health services and the satisfaction of BPJS patients in class III inpatient installations at the RSU Imelda Pekerja Indonesia Medan in 2023. This research method uses a quantitative correlation analytical type method with a cross sectional approach. The population in this study was 4,325 class III BPJS Health inpatient participants at RSU Imelda Pekerja Indonesia Medan. Based on the results of sample calculations using the Slovin formula, the sample to be studied was 98 respondents. The sampling technique in this research uses a probability sampling technique with a simple random sampling type. The research instrument uses a questionnaire created by researchers based on Tjiptono's (2017) theory. Data were analyzed using univariate analysis and bivariate analysis using the chi-square statistical test. The results of this study show the quality of BPJS health services with the satisfaction of BPJS patients in class III inpatient installations at RSU Imelda Pekerja Indonesia. The results obtained are reliability (p-value= 0.01), guarantee (p-value = 0.00), evidence physical (p-value = 0.00), empathy (p-value = 0.04), responsiveness (p-value = 0.02). It can be concluded that there is a relationship between the quality of BPJS Health services and the satisfaction of BPJS patients in the Class III Inpatient Installation at the RSU Imelda Pekerja Indonesia Medan in 2023. It is hoped that the results of this research can be input for the Imelda Workers Indonesia RSU to improve the quality of service, especially in the Inpatient Installation. One of the class III stays is improving facilities at the RSU Imelda Pekerja Indonesia Medan.

Abstract

The completeness of filling out the informed consent form is very important because it can affect the quality of medical records and legal aspects in the medical records themselves. This study aims to determine the factors that affect the completeness of the inpatient surgical patient's informed consent form at Mitra Sejati Medan General Hospital in 2023. The research method used is descriptive quantitative, which is to describe a situation objectively. The population of this study was an informed consent form for inpatient surgical cases (235) divided by 1+235 (precision rate/10%=0.1) resulting in a sample of 70. The sample of subjects in this study was 13 officers and for the object in this study 70 documents informed consent hospitalization. The type of data in this study is primary data obtained by distributing questionnaires and using checklist sheet observations. The results of the study were 71% complete, and 29% incomplete. The influencing factors are the officer's knowledge in the completeness of filling out the inpatient informed consent form illustrates that there are 7 people who show good knowledge, and there are 6 people who show poor knowledge.

Abstract

Pulmonary tuberculosis, one of the infectious diseases, is ranked fifth as an infectious disease that causes the most deaths in the world. Treatment with a long enough period of time allows non-adherence behavior in taking medication in pulmonary TB patients. Self-efficacy is a person's belief in his ability to achieve a desired goal. The purpose of the study was to determine the relationship between the level of self-efficacy on medication adherence in tuberculosis patients at the Imelda General Hospital of Indonesian Workers in 2023. Quantitative research method with correlational descriptive. The study was conducted at the Imelda Pekerja Indonesia General Hospital in August 2023. Population of 282 Pulmonary TB patients, study sample 73 people with Purposive Sampling. Bivariate test using Chi Square. The age of the majority of pulmonary tuberculosis respondents aged 26-35 years (Early Adulthood) was 25 people (34.2%), the gender of the majority of pulmonary tuberculosis respondents was 40 people (54.8%), the majority of pulmonary tuberculosis respondents were employed as 48 people (65.8%). The level of self-efficacy in pulmonary tuberculosis patients was the majority of high self-efficacy scores >77 as many as 55 people (75.3%), followed by low self-efficacy scores of <63 as many as 10 people (13.7%) and moderate self-efficacy scores of 64-76 as many as 8 people (11.0%). Adherence to taking medication in pulmonary tuberculosis patients the majority had moderate adherence scores 1-2 as many as 42 people (57.5%), followed by high adherence score 0 as many as 21 people (28.8%) and low adherence score >2 as many as 10 people (13.7%). There was no relationship of self-efficacy on medication adherence with a p value of 0.324>0.05.

Abstract

Factors that affect the quality of medical record services are human resources, facilities and infrastructure and policy monitoring and evaluation. The problem of this study is that there are less medical record officers, causing less fast service. The purpose of the study was to determine the factors that affect the quality of medical record service activities at Puskesmas Bestari. This type of research is a descriptive study where the population and sample are the same, namely 4 general practitioners, 3 dentists, 7 nurses and 2 medical record officers. This research collection technique is a field study technique, namely questionnaires, interviews and observations. The data analysis used in this study was univariate analysis. The results obtained were poor service quality by 12.5%, poor doctor human resources by 28.6%, poor nurse resources by 28.6%, good medical recorder resources by 66.6%, poor facilities and infrastructure by 20%, policy monitoring and evaluation by 40%. Based on these results, it can be concluded that the quality of medical record services, doctor human resources, nurse human resources, medical recorder human resources, facilities and infrastructure and policy monitoring and evaluation are still lacking, so it is expected that the Head of the Puskesmas will conduct training on the importance of the quality of medical record services in improving the performance of officers at the Puskesmas.

Abstract

Duplication of medical record numbers is a problem that often occurs in the Integrated Service Unit (UPT) of the Pasar Merah Health Center. This study aims to identify the factors that cause duplication of medical record numbers at UPT Puskesmas Pasar Merah. This research method uses a qualitative approach with in-depth interviews with medical administration officers, patient registration officers, and medical personnel directly involved in managing medical record numbers. In addition, observations were also made to understand the process of managing medical record numbers at UPT Puskesmas Pasar Merah.The results showed that the factors of duplication of medical record numbers involved several aspects, including lack of training of medical administrative officers in database management, recording systems that were not effectively integrated between service units, and the unavailability of medical record number validation when registering new patients. In addition, human error factors such as writing and recording errors are also the cause of duplication of medical record numbers. The recommendations of this study include improving training for medical administrative officers in database management, developing an integrated recording system, implementing direct validation of medical record numbers during patient registration, and strengthening supervision mechanisms to prevent human error in managing medical record numbers. This research is expected to contribute to the improvement of the medical record management system at UPT Puskesmas Pasar Merah, so as to reduce the incidence of duplication of medical record numbers and increase efficiency and accuracy in managing patient data.

Abstract

Medical record documents must be managed properly so that they are not damaged or lost so that they can be reused for treatment or providing other services. From the interview results, damage to medical record documents included the folder or cover of the medical record document being damaged or torn, there were piles of files on the floor so that the medical record folder was dirty and crumpled. This study aims to determine the factors that cause damage to medical record documents at Dr.R.M. Hospital. Djoelham Binjai uses 5 management elements Man, Method, Machine, Material, and Money. This type of research uses qualitative research with a descriptive approach. The subjects in this research were officers in the filling room at RSUD Dr. R.M. Djoelham Binjai, totaling 5 people and the object of this research is medical record documents in the filling room at Dr. RSUD. R.M. Djoelham Binjai. The data collection methods used in this research are observation and interviews. The results of the research show that the factors causing damage to medical records are based on the 5M factor, namely the human factor, there are still many officers who are not D3 medical record graduates, officers do not immediately replace damaged folders. Factors include limited shelf storage and a lot of documents piling up. The storage room method factor already has an SOP but its implementation is not carried out, and retention is not carried out. The material factor of the map material used is in accordance with standard hospital procedures. The money factor does not yet have a special budget for replacing damaged maps. To the officers in the filling room, it is best to retain or shrink medical record documents so that the storage shelves are a little loose and there are no medical record documents piled up on the floor, so that medical record documents are maintained.
