Abstract

Cancer is important problem in public health worldwide especially in developed countries. About 10 millions new cases was diagnosed every year. Colorectal carcinoma is more common in man than woman. Despite significant advances in both surgical methodology and adjuvant therapy regimes, long-term survival for CRC patients remains in the range of 50-60%. Based on WHO, 90 % colorectal carcinoma was adenocarcinoma. Colorectal carcinoma is caused by a collection of various genetic changes and the most common cause is a loss of function of the p53 tumor suppressor gene. P53 tumor suppressor gene plays an important role in cell cycle and apoptosis. This gene encodes for a 53 kDa phosphoprotein and is frequently targeted for inactivation in a wide range of tumours. It is the target of point mutations and small deletions and insertions that lead to total or partial abolition of protein function. Inactivation is believed to abolish the ability of p53 to maintain genomic integrity through regulation of various activities, including the control of cell cycle arrest, DNA repair and apoptosis. Accumulation of p53 in tumor cells can be detected with a specific p53 antibody. Mutation on p53 gen and overekspression of p53 is common incolorectal carcinoma tissue then p53 mutant and p53 wild type can be targeted by p53 specific antibody. P53 overexpression associated with colorectal carcinoma histologycal grade and tend more common in colorectal carcinoma with high proliferation activity. To assess p53 mutan expression in determining differentiation grade and prognose adenocarcinoma colorectal and the prognose of adenocarcinoma colorectal at Anatomic Pathology laboratory of RSUP Haji Adam Malik Medan. The study design was observational design with cross sectional approach. All variables were immunohistochemically stained with p53 mutant. The variables were measured only once and at one moment. The samples used in this study were the sample preparation of paraffin blocks from 59 colorectal carcinoma tissues which diagnosed patologically according inclusion criteria. All variables were immunohistochemically stained with p53 mutant. Distribution of patients by age group mostly aged over 60 years which is 24 cases (40.68%). Distribution of patients by sex mostly male which is 36 cases (61.02%). 36 cases (61,02%) were located on colon. The majority, 27 cases (45,76%) were diagnosed as well differentiated adenocarcinoma. Colorectal carcinoma samples based on p53 mutant expression were score +1 (weak) were 35 cases (59,32%). Calculation of chi-square with 2x2 crosstab table, p value = 0.427 (p value> 0.05). No difference between p53 mutant expression with histopathological grade in colorectal adenocarcinoma.

Abstract

Breast care is an act of massaging the back along the spine which is useful for facilitating breastfeeding, mothers are relaxed and comfortable in post partum mothers. The purpose of this study was to describe nursing care for post partum mothers with the application of back rolling massage to increase milk production. Respondents in this study were 2 people who were taken by purposive sampling technique. Data were collected by interview, physical examination, observation and documentation studies. The results of the assessment show that the client says the milk has not come out. The act of back rolling massage which is carried out for 3 days of treatment can increase milk production. The application of back rolling massage can overcome the problem of ineffective breastfeeding in post partum mothers. It is hoped that back rolling massage will be an alternative source in increasing breast milk production.

Abstract

Congestive Heart Failure (CHF) or called congestive heart failure where the heart is unable to pump enough blood to the human body's organs and other tissues. Symptoms that usually appear in sufferers of Congestive Heart Failure (CHF) are shortness of breath, weakness and fatigue, swollen ankles or stomach due to fluid buildup, increased respiratory frequency. The non-pharmacological action taken for Congestive Heart Failure (CHF) patients is giving the semi-Fowler position. This research design uses a case study method with 2 respondents through interviews and physical examination. The results of the study showed that after the nursing action was carried out, Mrs. N and Mr. A found a decrease in respiratory frequency in Mrs. N 27x/minute becomes 20x/minute while in Mr. A 24 x/minute becomes 20 x/minute. Abstract

Dengue Haemorrhagic Fever (DHF) is a disease caused by the dengue virus, belonging to the Flaviviridae family and there are 4 sterotypes, namely DEN-1, DEN-2, DEN-3, and DEN-4. The design of this research is quantitative - descriptive using a case study approach. The research sample consisted of 2 respondents with the inclusion criteria of DHF children, using warm  pediatric nursing assessment formats, thermometers, and temperature measurement observation sheets. The sampling used in this research was purposive sampling. Data was collected using observation techniques, interviews, physical examinations, supporting examinations, documentation carried out on April 23 2024 and June 27 2024 at the TK IV Army Hospital 01.07.01 Pematangsiantar. This research was conducted for 3 days. The results of the research that have been obtained confirm the nursing diagnosis, namely hyperthermia with warm compress intervention given 3 times a day for 3 days of treatment to client 1 which decreased to 36.5 0C and to client 2 it decreased to 360C. The main problem of hyperthermia in both clients was resolved by using warm compresses. Researchers , especially parents, are expected to be able to apply the warm compress technique as a measure to treat high fever in children who experience Dengue Haemorrhagic Fever (DHF), especially when caring for sick children at home apart from giving antipyretics.

Abstract

Stroke is a disturbance in the function of the nervous system caused by a disturbance in blood circulation in the brain due to rupture of blood vessels or due to blockage of blood vessels in the brain. One of the nursing problems that arises in cases of hemorrhagic stroke is ineffective airway clearance. Efforts made to overcome airway clearance are the use of suction techniques. This research design is quantitative-descriptive using a case study approach. The research sample consisted of 2 respondents, namely hemorrhagic stroke sufferers. The sampling used in this research was purposive sampling. Data was collected using observation techniques, interviews, physical examination, supporting examinations, documentation carried out on May 22 2024 and June 21 2024 at Vita Insani Pematangsiantar Hospital. The results of this study show that after implementing suction for 3 days, it was found that both patients experienced a decrease in respiratory frequency, in patient 1 the first day from 22 x/minute to 20 x/minute, patient 2 respiratory frequency 23 x/minute to 21 x/minute minutes and mucus can be expelled, vesicular breath sounds, regular breathing rhythm and sonor lung percussion in both patients. Suction technique can improve airway clearance in haemorrhagic stroke patients, Researchers recommend that nurses be able to implement suction techniques in hemorrhagic stroke patients to overcome the problem of ineffective airway clearance.

Abstract

Gout arthritis is an inflammation of the joints caused by the buildup of crystals in the joints due to impaired purine metabolism in the body. Purines are proteins that undergo metabolism in the body that are converted into uric acid. The purpose of this study is to describe Nursing Care for the elderly with Gout arthritis in the provision of warm water compresses to reduce pain in Upt Yansos for the Deaf and Speech Pematangsiantar. The design of this study is descriptive using a case study method on 2 clients with Gout arthritis who experience acute pain with purposive sampling techniques. Data were collected by interview, observation, and physical examination techniques carried out using the gerontic askep assessment format, warm compress SOP. The results of the assessment found that the client complained of pain with a pain scale of client I and client II, which was 5. After being given warm compresses, the pain scale in client I and client II decreased, namely on the first day with a pain scale of 5, on the second day the pain scale was 4, and on the third day the pain scale became 3. After 3 days of nursing treatment, the act of giving warm water compresses is very effective in reducing acute pain in patients with Gout arthritis. For nurses to teach care to patients with gout arthritis. For patients about maintaining health and a healthy lifestyle.

Abstract

Hospital reporting is an organizational tool that aims to produce fast, precise, and accurate reports. This study aims to identify the factors that hinder the reporting of hospital service data (RL 3.1) at RSIA Artha Mahinrus Medan, as well as evaluate the impact of the condition of facilities and infrastructure on the effectiveness of the reporting process using the online SIRS application, by considering three indicators of infrastructure: completeness, condition, and use. The approach used is qualitative descriptive, with data collection through interviews and observations of research subjects including the head of medical records, registration officers, poly officers/nurses, and IT officers. The instruments used are interview guidelines and observation sheets. The results of the study show that the reporting of hospital service data (RL 3.1) in the online SIRS at RSIA Artha Mahinrus Medan experienced obstacles, such as the lack of number of computers in the medical record unit, computer system failures, and limited internet connections, which caused slow networking. Based on the results of the study, delays in making reports can be caused by problems with existing facilities and infrastructure. Therefore, the researcher suggests that the reporting of hospital service data (RL 3.1) be carried out outside of the collection time to prevent additional delays.

Abstract

Asthma is a chronic disease that attacks the respiratory tract with marked shortness of breath due to narrowing of the airways. One of the non-pharmacological measures to reduce asthma is the administration of a combination of ginger and honey boiled water. The aim of the study was to identify the effect of consuming a combination of ginger and honey boiled water on asthma sufferers. This study uses a quasi-experimental research design approach with two groups pretest posttest. The sample of this study were 30 asthma patient respondents who were undergoing treatment which were divided into two groups, 15 intervention groups and 15 control groups. The sampling technique uses the Slovin formula. Research data collection used demographic data, informed consent sheets, Asthma Control Test (ACT) questionnaires, which were combined using a modified combination of ginger and honey boiled water. Data were analyzed using descriptive statistics, paired t test and independent t test. The results showed that there was an effect of consuming a combination of ginger and honey boiled water on ineffective breathing patterns, post-test intervention group (t = -23,503, p=0,001) and post-test control group (t = -5,281; p=0,001) and there was a significant difference significantly to the ineffectiveness of breathing patterns between the post-test of the intervention group and the post-test of the control group after the intervention was carried out by consuming a combination of ginger and honey boiled water for ineffective breathing patterns, the intervention group and the control group after filling out the Asthma Control Test (ACT) questionnaire obtained a value (t = 4,707, p=0,001). The results of this study can recommend consuming a combination of ginger and honey boiled water as a non-pharmacological measure to help reduce the ineffective breathing pattern of asthma sufferers.

Abstract

Incompleteness in filling out a medical resume will greatly affect the hospital's accreditation assessment listed in the MIRM 15 SNARS standard sub-group Issue 1.1. This study aims to determine the completeness of filling out the resume form based on the assessment elements of MIRM 15 SNARS Edition 1.1 at Imelda Pekerja Indonesia Hospital in Medan. This study uses quantitative and qualitative methods, with a population of 1,355 medical resume forms at Imelda Pekerja Indonesia Hospital Medan in November-December 2021 and a sample of 93 medical resumes which are completed into 100 medical resume forms. Based on the results of the study, it was found that 76 medical resume forms were filled (70%) and 30 medical resume forms were incomplete (30%). score Based on the assessment of SNARS Edition 1.1, the number of complete medical resume forms was 76% with a score of 5 (partially fulfilled) because the achievement range was 29%-79%. The highest completeness on the medical resume form at Imelda Pekerja Indonesia Hospital is found in the patient history component, diagnostic examination, therapeutic procedures and actions, drugs given and drugs when the patient leaves the hospital, the patient's health condition (current status) and follow-up instructions (100% ) Medical resume forms and the lowest completeness were found on physical examination (15%) indication of patients being treated (6%), comorbidities (12) and patient/family signatures (24%) of medical resume forms. Researchers suggest that there is a need for socialization that continuously increases knowledge to medical officers about filling out the complete medical resume form according to SNARS at Imelda Pekerja Indonesia Hospital in Medan.

Abstract

Dermatophytosis is one of the superficial mycoses caused by fungi that invade keratin-containing tissues such as the stratum corneum of the epidermis, hair, and nails. Dermatophytosis is divided based on its location, namely tinea capitis, tinea barbae, tinea cruris, tinea pedis et manum, tinea unguinum, and tinea corporis. Dermatophytosis is caused by dermatophyte fungi from the arthrodermataceae family. This family consists of more than 40 species divided into three genera: Epidermophyton, Microsporum, and Trichophyton. This study aims to determine the profile of dermatophytosis in the Dermatology and Venereology Polyclinic of Wulan Windy Hospital, Medan, from January to December 2013 based on the classification of location, age, gender, occupation, and therapy given. This study is a retrospective descriptive study based on medical record data from new patients who were treated at the Dermatology and Venereology Polyclinic, Wulan Windy Hospital, Medan, in the period 2021-2024. The number of new dermatophytosis patient visits increased from 2021 to 2023, but decreased in 2024. Tinea corporis was the most common diagnosis at 54.9%. Most patients were women. The most common age group found was 45-64 years in 2021-2023. Most treatments were topical. There was an increase in the number of dermatophytosis patients. The most infected age group was the productive age group, because in this age group there was an increase in physical activity and a tendency to sweat a lot and be humid.
