Abstract

Thyroid nodules as discrete lesions within the thyroid gland. Nodules in the thyroid gland are detected in approximately 5% to 7% of the adult population by physical examination alone. Thyroid nodules are approximately 4 times more common in women than men Thyroid nodules can be classified as neoplastic and non-neoplastic. The initial evaluation of a patient with a thyroid nodule should include a history, physical examination, thyroid stimulating hormone (TSH) measurement, and thyroid ultrasonography to characterize the nodule. TSH measurement by itself may detect subtle thyroid dysfunction. Diagnostic tests include serum markers, fine needle aspiration (FNA) cytology, genetic markers, immunohistochemical markers, and several imaging modalities, most commonly ultrasonography, but also elastography, MRI, CT, and 18FDG-PET scanning. Thyroidectomy is the most common endocrine surgery. Due to compression on the trachea, airway management can be difficult.[1] Preoperative evaluation and management are critical when planning elective thyroidectomy, where changes in the anatomical location are expected because of a large or substernal goiter. Anatomical changes, laryngeal edema and an inexperienced team can all contribute to difficulty in intubation. The incidence of difficult tracheal intubation (DTI) in the patient population undergoing thyroid surgery varies. Several studies found a similar value for the incidence of DTI (5.3% to 6.8%). Previous studies have evaluated risk factors for DTI in patients undergoing thyroid surgery. Increasing age, high Mallampati score, Grade III or IV Cormack score, reduced mouth opening (<4.4 cm), goiter cancer and tracheal stenosis (≥30%) are independent predictors of DTI. Neck circumference (NC) appears to be an important predictor of DTI.

Abstract

Nurses who work in the Psychiatric Clinic are also prone to burnout, where burnout is a problem caused by prolonged work pressure. There are several conditions that people experience burnout, namely emotional saturation, loss of motivation, and reduced commitment which results in a decrease in performance at work. This study is a correlative analytical study with a cross-sectional approach that analyzes the relationship between free and dependent variables using the Maslach-Trisni Burnout Inventory (M-TBI) instrument. The sample was taken by the Non-Probability consecutive sampling method. The study had 6 independent variables, and the sample size was determined based on the highest bivariate relationship. Data analysis was carried out by the Chi-Square test.   Pene; ITI found a relationship between gender (p = 0.001), age (p = 0.021), and job position (p = < 0.001) to burnout syndrome in nurses working in the Psychiatric Clinic. This study found a relationship of burnout syndrome in nurses who care for people with mental problems.

Abstract

The morbidity and mortality rates of mothers and children remain critical public health concerns in Indonesia. Posyandu (Integrated Health Service Post), as a Community-Based Health Effort (UKBM), plays a vital role in child health maintenance, especially through mothers' active participation in bringing their toddlers for health services. This study aims to identify the factors associated with mothers’ activeness in bringing toddlers (aged 12–36 months) to Posyandu in Helvetia Village, Sunggal Subdistrict, Deli Serdang Regency, in 2023. An analytical survey with a cross-sectional approach was employed, involving 72 mothers selected through purposive sampling. Data were collected using questionnaires and analyzed using the chi-square test. The results showed that the majority of mothers (66.7%) were inactive in taking their toddlers to Posyandu. There was a significant relationship between mothers’ activeness and knowledge (p=0.001), employment status (p=0.000), husband’s support (p=0.000), and attitude (p=0.000). Mothers with good knowledge, who were unemployed, received support from their husbands, and had a positive attitude were more likely to actively attend Posyandu. These findings highlight the importance of educational interventions and social support in increasing mothers’ participation in Posyandu, which is essential for optimizing child growth and development.

Abstract

Stress is the result of an imbalance between demands and the resources an individual possesses—the greater the gap, the higher the level of stress experienced, which can pose a threat to well-being. The aim of this study was to describe the work stress experienced by medical record officers in the coding section at Haji General Hospital Medan in 2024. This study employed a qualitative research method. The informants included four individuals: one outpatient coding officer, two inpatient coding officers, and one head of the medical record installation. The findings revealed that the causes of work stress among coding officers included workload, work-related conflicts, inadequate work equipment, and poor working environment. It is recommended that the hospital provide adequate workspace and facilities that ensure comfort for the officers. Additionally, coding officers should pay more attention to task distribution and provide education to other healthcare workers to ensure timely delivery of medical record files for coding. Officers must also maintain effective communication with other healthcare personnel to prevent conflicts and misunderstandings.

Abstract

This study evaluated the implementation of the Clinical Pathway for acute appendicitis at Mitra Medika General Hospital, Medan Tanjung Mulia, in 2022. The objective was to assess the compliance level of healthcare professionals with the established clinical pathway and to identify factors influencing its application in patient management. A descriptive retrospective design was employed, using medical records of acute appendicitis patients treated during the specified period. Data analysis focused on adherence to diagnostic, therapeutic, and postoperative care standards outlined in the clinical pathway. The results showed that overall compliance was high, particularly in preoperative and operative phases, while postoperative follow-up displayed moderate adherence due to variations in discharge planning and documentation. Factors affecting implementation included staff workload, availability of medical resources, and differences in clinical judgment. It was concluded that the clinical pathway was effectively implemented, although improvements are required to enhance consistency in postoperative care. Strengthening monitoring systems and conducting regular training are recommended to improve adherence and optimize patient outcomes.

Abstract

Chronic Energy Deficiency (CED) among women remains a major nutritional problem in developing countries and has significant impacts on maternal health, pregnancy outcomes, and future generations. The high prevalence of CED indicates that this issue is not solely related to inadequate nutrient intake, but is also influenced by structural social and cultural determinants that interact with one another. This article aims to examine the role of social and cultural determinants in the occurrence of CED among women in developing countries. This article was developed using a narrative review approach based on open-access journal articles, official reports from the World Health Organization (WHO) and UNICEF, as well as population-based and qualitative studies published between 2016 and 2025. The keywords used included chronic energy deficiency, women, social determinants, and cultural factors. Relevant articles were selected based on topic relevance, developing country context, and a focus on women, and were then thematically analyzed to identify recurring patterns of social and cultural determinants. The findings indicate that poverty, low levels of education, limited access to nutritious food, gender inequality, and women’s workload significantly contribute to the occurrence of CED. In addition, cultural norms and practices related to intra-household food distribution and specific food taboos further exacerbate women’s vulnerability to CED, particularly during the reproductive age. Chronic Energy Deficiency among women is a structural problem that requires nutrition interventions sensitive to social and cultural contexts as well as gender equity in order to achieve sustainable improvements in nutritional status.

Abstract

Adequate nutritional status in pregnant women is essential for optimal fetal growth and development as well as for ensuring a safe and smooth delivery process. Good maternal nutritional status can be achieved through a balance between nutritional intake and physiological needs. Chronic Energy Deficiency (CED) in pregnant women is a condition of long-term undernutrition, characterized by a mid–upper arm circumference (MUAC) of less than 23.5 cm, and is associated with an increased risk of preterm delivery, low birth weight (LBW) infants, and even maternal and neonatal mortality. This study aimed to identify factors associated with the occurrence of Chronic Energy Deficiency (CED) among third-trimester pregnant women in the working area of Tanjung Rejo Public Health Center, Percut Sei Tuan. The research employed a quantitative method with a cross-sectional design. The population consisted of all third-trimester pregnant women registered at the Tanjung Rejo Public Health Center in 2025, totaling 120 individuals. A sample of 20 pregnant women was selected using accidental sampling techniques. The study was conducted from May to August 2025. The results of statistical analysis using the chi-square test showed a p-value of 0.004 (p < 0.05), indicating a significant relationship between dietary patterns and the incidence of Chronic Energy Deficiency (CED) among pregnant women at the Tanjung Rejo Public Health Center, Percut Sei Tuan District, Deli Serdang Regency. It is recommended that pregnant women consume a balanced and nutritious diet according to their household capacity, with particular emphasis on adequate energy, protein, and iron intake.

Abstract

Anxiety during labor is a common psychological condition experienced by pregnant women, particularly primigravida mothers, and may negatively affect the labor process. Poorly managed anxiety can increase pain perception, inhibit uterine contractions, prolong labor, and adversely impact maternal and fetal well-being. Pharmacological management of anxiety has limitations due to potential side effects; therefore, safe and effective complementary therapies are needed. Aromatherapy has been widely used in midwifery care due to its relaxing and anxiolytic effects. Cymbopogon citratus and Citrus aurantifolia essential oils contain bioactive compounds that provide calming, refreshing, and mood-enhancing effects, and their combined use is expected to produce a synergistic effect in reducing anxiety during labor. This study aims to examine the effectiveness of a combination of Cymbopogon citratus and Citrus aurantifolia aromatherapy as a complementary therapy in reducing maternal anxiety during labor. This study employs a quasi-experimental design with a pretest–posttest control group approach. The subjects are laboring mothers who meet the inclusion criteria. Data are analyzed to determine differences in anxiety levels within and between groups. The combination of Cymbopogon citratus and Citrus aurantifolia aromatherapy is expected to significantly reduce anxiety levels among laboring mothers compared to standard care alone.: The use of combined Cymbopogon citratus and Citrus aurantifolia aromatherapy has the potential to serve as a safe, practical, and effective non-pharmacological intervention to reduce maternal anxiety during labor. This therapy may support holistic midwifery care by improving maternal comfort and psychological well-being during childbirth.

Abstract

Hemodialysis is a therapeutic intervention used to replace kidney function when the kidneys are no longer able to perform their physiological roles adequately, and it is initiated when conservative treatment fails to maintain renal function. Uremic pruritus is a commonly used term referring to a dermatological symptom characterized by an itching sensation that provokes the urge to scratch and is highly prevalent among patients undergoing hemodialysis. The administration of olive oil was conducted to evaluate its long-term effect on reducing the severity of pruritus. The objective of this study was to determine the effectiveness of olive oil application in reducing pruritus among hemodialysis patients at Putri Hijau Hospital, Medan. This study employed a pre-experimental design with a one-group pretest–posttest approach. The research was conducted from February to April 2025 at Putri Hijau Hospital, Medan. The total population consisted of 60 patients, and a sample of 37 respondents was selected using purposive sampling. Bivariate analysis was performed using the Wilcoxon signed-rank test. The results showed that on the first day prior to olive oil application, the majority of hemodialysis patients experienced unbearable itching, with 18 respondents (48.6%). On the fifth day after the intervention, the majority of patients reported mild itching, with 19 respondents (51.4%). Based on the Wilcoxon test results, the significance value was less than 0.05, indicating a statistically significant effect. The study findings demonstrate that olive oil application significantly reduced pruritus among hemodialysis patients at Putri Hijau Hospital, Medan, with a two-tailed significance value of 0.000 (<0.05).

Abstract

The continuously increasing population remains a major challenge for countries worldwide, particularly developing nations. Indonesia is a developing country with the fourth largest population in the world, after China, India, and the United States. Based on the 2015 census data, Indonesia’s population reached 238,518,000 and was projected to increase to 271,066,000 by 2020. Long-Acting Reversible Contraception (LARC), including subdermal contraceptive implants, is highly effective in preventing pregnancy. However, the utilization of LARC remains relatively low. The low uptake of Long-Term Contraceptive Methods (LTCM) is often associated with various factors, such as limited knowledge about contraceptive methods, concerns regarding side effects, discomfort, and the husband’s desire to have additional children. Therefore, this study aimed to explore the reasons, perceived benefits, and complaints experienced by contraceptive implant users at the Independent Midwifery Practice of Hj. Dewi Sesmera, Tanjung Mulia, Medan, in 2024. This research employed a qualitative approach with a phenomenological design. The informants, selected for triangulation purposes, consisted of 10 women who were family planning acceptors at the Independent Midwifery Practice of Hj. Dewi Sesmera in Tanjung Mulia, Medan. The findings revealed that implant acceptors obtained information through counseling sessions and direct communication with previous implant users. They demonstrated adequate knowledge regarding the insertion procedure and potential side effects of the implant. The factors influencing the selection of contraceptive implants included their effectiveness in long-term pregnancy prevention and spousal support. In conclusion, contraceptive implant acceptors demonstrated sufficient knowledge, effective information-seeking strategies, and identified long-term pregnancy prevention as the primary reason for choosing the implant method.
