Abstract

PENDAHULUAN Penyakit tiroid merupakan masalah kesehatan masyarakat yang harus diwaspadai karena dapat mempengaruhi berbagai organ tubuh. Penyakit COVID-19 dapat memacu gangguan tiroid secara direk atau melalui respon inflamasi imun yang memacu autoimun. Tujuan penelitian ini untuk menganalisis profil tiroid, autoantibodi tiroid dan RT-PCR SARS CoV-2 serum pada pasien COVID-19 METODE Penelitian potong lintang dengan sampling berurutan pada bulan Juli 2022 sampai dengan Oktober 2022 di laboratorium Patologi Klinik RSUD Dr. Moewardi di Surakarta. Kriteria inklusi yaitu pasien dewasa dengan COVID-19. Kadar TSH, FT4, FT3, dan TPO antibody diperiksa menggunakan alat imunologi Mindray CL-900i metode CLIA. RT-PCR SARS CoV-2 serum menggunakan alat Roche Cobas Z 480. Data diproses menggunakan komputer. HASIL Penelitian ini terdiri 85 subjek dengan median usia 50 (19-68) tahun dan laki-laki lebih banyak daripada perempuan (55,3% vs 44,7%). Disfungsi tiroid pada 16 subjek (18,8%), dan kasus hyperthyroid lebih banyak daripada hypothyroid (12,9% vs 5,9%). Subjek dengan euthyroid, hyperthyroid dan hypothyroid didapatkan rerata usia 49 (19-68); 58 (38-66); dan 50 (21-60) tahun (berurutan). Subjek dengan autoimmune dan non autoimmune thyroid sebanyak 12 (14%) vs 73 (86%) orang. Keseluruhan subjek didapatkan hasil RT-PCR SARS CoV-2 serum negatif (100%). SIMPULAN Diantara subjek dengan thyroid dysfunction didapatkan gangguan terbanyak yaitu hyperthyroid, dengan prevalensi autoimmune sebanyak 14% dan tidak didapatkan bukti transmisi virus SARS CoV-2 melalui darah. Perlu dilakukan skrining autoimmune thyroid pada pasien COVID-19.

Abstract

PENDAHULUAN Perkembangan SARS-CoV-2 telah ditandai dengan pengenalan "Variant Of Concern," atau perubahan sifat virus seperti penularan penyakit dan antigenisitas. VOC Delta SARS-CoV-2 40% -60% lebih mudah menular daripada VOC Alpha (B.1.1.7) dan dapat dikaitkan dengan peningkatan risiko rawat inap. TUJUAN Untuk mengetahui karakteristik klinis pasien COVID-19 varian Delta dan Omicron di High Care Unit (HCU) dan Intensive Care Unit (ICU) Rumah Sakit Dr Moewardi. METODE Penelitian deskriptif analitik kuantitatif dengan pendekatan retrospektif yang dilakukan pada Juni 2021-Maret 2022. Sampel penelitian adalah pasien terkonfirmasi positif COVID-19 yang di rawat di ICU dan HCU isolasi pada Agustus-Oktober 2021 dan Januari-Maret 2022. Diukur tanda vital, komorbid, hasil laboratorium, status intubasi dan mortalitas. HASIL Angka kematian pasien yang terinfeksi Covid-19 varian delta dan varian omicron masing-masing adalah 10% dan 10,8%. Varian Delta lebih signifikan berdampak terhadap CVA (p=0.001) dan penyakit jantung (p<0.001) dibandingkan pada CVA (p=0.015) dan penyakit jantung (p=0.190) pada pasien SARS-CoV-2 varian Omicron. Varian Omicron lebih signifikan berdampak terhadap sepsis (p=0.012) dibandingkan varian Delta (p=0.026). KESIMPULAN Tidak didaptakan perbedaan angka mortalitas antara varian COVID-19 omicron dan delta. Kematian pada kelompok omicron sebagian besar disebabkan penyakit komorbid, sedangkan pada kelompok delta, kematian disebabkan karena COVID-19 menyebabkan disfungsi endotel.

Abstract

BACKGROUND Clinical assessment of the delivery of tissue oxygenation is very challenging for clinicians to study. Anaemia reflects a decrease in oxygen carrying capacity in the blood which is very at risk of oxygen delivery (DO2) insufficiency and cellular hypoxemia, to assess tissue hypoxia in addition to being a reliable tool, the clinician must also adjust to the patient's clinical and hemodynamic. Blood transfusions are generally used to correct anaemia precisely and efficiently, but in certain circumstances that make it impossible for clinicians to perform blood transfusions, at that time we have to think of other methods of therapy in patients. METHODS 20-year-old woman came with anaemia, with a haemoglobin (Hb) level of 6.0 g/dl, a stable clinical condition and hemodynamic within normal limits, a blood transfusion was planned but the results of the Comb test was incompatible, so it was an absolute contraindication for transfusion. The patient experienced clinical changes that indicated the occurrence of tissue hypoxia when the Hb level was 2.0gr/dl. The clinician decided to maximize DO2, and reduce oxygen demand/consumption (VO2), so that there is a balance. The patient was treated with steroids, suspected autoimmune haemolytic anaemia (AIHA). On the eighth day of clinical treatment the patient improved, hemodynamically stable, and Hb increased. CONCLUSION Clinical valuation of the distribution of tissue oxygenation is very important for AIHA patient.

Abstract

PENDAHULUAN Twin-Twin transfusion syndrome (TTTS) merupakan tantangan tersendiri bagi perifer. Hingga saat ini Fetoscopic laser photocoagulation (FLP) baru tersedia satu di seluruh Indonesia, yaitu di RSAB Harapan Kita, Jakarta. Sementara menurut perhitungan rasio kejadian TTTS terhadap angka kelahiran di Indonesia, tidak kurang dari 3000 kasus TTTS terjadi pertahunnya. Artinya, 10 kasus perhari membutuhkan tindakan tersebut. Maka bila di satu senter layak mengerjakan maksimal 3 kasus maka dibutuhkan 3-4 senter fetoskopi di Indonesia. Sementara kita menunggu terbitnya senter fetoskopi selain RSAB Harapan Kita Jakarta, maka apa yang mungkin dan bisa kita lakukan? TUJUAN Memberikan gagasan manajemen TTTS untuk daerah perifer berbasis telaah literatur beserta telaah kasus serial yang ditangani. Metode: Serial Kasus HASIL Empat kasus TTTS terdiagnosis rata-rata pada minggu 26.5 kehamilan (dari 23 – 29 minggu). Satu kasus datang sudah dalam persalinan tanpa intervensi. Satu kasus dilakukan amnnioreduksi. Dua kasus dapat dilakukan 50% dilakukan septostomi dan amnioreduksi. Lama waktu dari intervensi sampai dengan bayi lahir 19.6 hari. Didapatkan 100% (8 bayi) lahir hidup, tetapi hanya 37.5% (3 bayi) yang dapat bertahan hidup. Dua bayi lahir dari ibu yang sama dengan usia kehamilan 36 minggu, sayang bayi resipien meninggal beberapa saat setelah dilahirkan. Pada kasus lain Satu bayi donor juga berhasil bertahan hidup pasca tindakan septostomi yang lahir pada usia 32 minggu sementara resipien meninggal pasca salin. Cara persalinan, 50% (2 kasus) pervaginam dan 50% (2 kasus) perabdominam. KESIMPULAN Tindakan pertama terpilih untuk TTTS adalah fetoskopi dan laser ablasi dengan angka keselamatan kedua janin lebih dari 90%, hanya saja dikarenakan fasilitas ini belum tersedia di RSUD Dr. Moewardi maka diambil pilihan kedua, yaitu melakukan septostomi dan amnioreduksi. Tindakan septostomy dapat mencegah kompresi tali pusat pada janin donor akibat oligohidramnion, sehingga mengurangi probabilitas kematian intrauterin. Tindakan amnioreduksi dilakukan simultan setelah septostomy dapat mengurangi dypsnea dan nyeri perut akibat overdistensi serta prematuritas. Timing septostomi dan amnioreduksi ternyata sangat menentukan untuk setidaknya dapat menolong salah satu janin.

Abstract

PENDAHULUAN Neuropati auditori merupakan bagian dari tuli sensorineural, dimana suara dapat masuk hingga telinga dalam, tetapi transmisi sinyal dari telinga dalam ke otak terganggu pada jaras tertentu. Evaluasi klinis serta pemeriksaan yang tepat dibutuhkan pada penegakkan diagnosis neuropati auditori sehingga tatalaksana yang tepat dapat segera diberikan. LAPORAN KASUS Dilaporkan seorang anak laki-laki umur 1 tahun 8 bulan dengan keluhan belum dapat bicara sesuai usianya dan tidak respon terhadap panggilan ataupun suara keras. Pasien mempunyai riwayat hiperbilirubinemia pada usia 2 minggu. Pasien dilakukan pemeriksaan OAE dengan hasil normal, ABR/BERA terdapat gelombang mikroponik koklea pada intensitas 90-95 dB dan tidak tampak gelombang V pada intensitas 70-95 dB dan ASSR hasil ambang dengar gangguan derajat berat. DISKUSI Diagnosa neuropati auditori dapat ditegakkan dengan pemeriksaan Otoacoustic Emission (OAE) dengan hasil fungsi sel rambut luar koklea normal dan pada auditory brainstem respons (ABR/BERA) didapatkan Cochlear Microphonic (CM) normal atau pernah ada dalam satu waktu tetapi respons gelombang tidak normal atau tidak ada. Pada kasus ini telah dilakukan penegakan diagnosis yang sesuai dengan panduan. KESIMPULAN Diagnosa neuropati auditori dapat ditegakkan dengan pemeriksaan Otoacoustic Emission (OAE) dengan hasil fungsi sel rambut luar koklea normal dan pada auditory brainstem respons (ABR/BERA) didapatkan Cochlear Microphonic (CM) normal atau pernah ada dalam satu waktu tetapi respons gelombang tidak normal atau tidak ada.

Abstract

INTRODUCTION Nasopharyngeal carcinoma (NPC) is a common type of cancer in Southeast Asia, and its treatment often involves a cisplatin chemotherapy regimen. Ototoxicity is a side effect that often occurs in patients receiving this regimen. Antioxidant therapy, both synthetic and natural, such as curcumin, has been studied as having the potential to reduce ototoxicity due to cisplatin. This study was conducted to evaluate the effect of administering mecobalamin and curcumin as antioxidants in patients with nasopharyngeal carcinoma after cisplatin chemotherapy regimen with hearing loss. METHOD This study was an observational study with a cross sectional design, using consecutive sampling with inclusion criteria, namely patients diagnosed with NPC receiving the cisplastin regimen and willing to take part in the study. The data obtained was then analyzed using the Rank-Biserial Correlation's Test. Research approval was obtained from the Medical Research Ethics Commission, Faculty of Medicine, Sebelas Maret University/RSUD Dr. Moewardi Surakarta. RESULTS Of the 30 NPC patients who received cisplatin from the ENT-KL installation section of Dr Moewardi Hospital in the period March - September 2023, 30 data were obtained for each right ear and left ear. The average hearing threshold before intervention was 33.79 ± 10.40 dB and 34.10 ± 9.75 for the right and left ears. There was a difference in the average increase in hearing threshold for all parameters (Group I: Mecobalamin, Group II: Curcumin, Group III: Curcumin + Mecobalamin), with a value of p=0.000 (p<0.05). From these data it was concluded that the combined therapy of both was able to increase the hearing threshold along with the chemotherapy series. CONCLUSION Based on the research results, administration of mecobalamin and curcumin as antioxidants has a significant effect on patients with nasopharyngeal carcinoma after cisplatin chemotherapy regimen with hearing loss.

Abstract

ABSTRAK Autoimmune Inflammatory Rheumatic Disease (AIIRD) is a group of different disorders, which have similar clinical, laboratory and immunological manifestations. In East Asia, the top 3 diseases included in AIIRD are Systemic Lupus Erythematosus, Rheumatoid Arthritis, and Sjogren's Syndrome. Glucocorticoids are still one of the therapeutic modalities for AIIIRD with their antiinflammatory and immunosuppressive effects. However, glucocorticoids have some side effects. One side effect of concern is decreased bone density. The condition of glucocorticoid induced osteoporosis (GIOP) is reported with increasing prevalence. The main effect of the use of glucocorticoids on bones is the presence of impaired bone formation regardless of the role of inflammation.  Secretome stem cells contain bioactives that exhibit diverse physiological functions such as immunomodulation, anti-inflammatory, angiogenesis, anti-apoptotic and anti-oxidation. Secretome can modulate cell differentiation by promoting osteogenesis and inhibiting adipogenesis. In this case report, it showed improvements in markers of bone damage, inflamation, and vitamin D levels after stem cell secretome administration in AIIRD patients with long-term corticosteroid use.

Abstract

Background: The use of trigeminal block with/without ultrasonography guidance as a pain management in outpatient settings are common, mainly for neuralgic pain. Whereas, the use of trigeminal block are not widely performed intraoperatively especially for oral-maxillofacial surgery. Case: We present a case of 16 year old boy diagnose with mandibular abscess prior to Open Reduction Internal Fixation (ORIF) due to mandibular fracture 1 months prior. Patient admitted with complaints of pain and swelling on right jaw, difficult opening his mouth and experienced a salty taste in mouth which we suspect the abscess penetrate into the oral cavity. Patient was in a limp condition, shortness of breath in supine position, with oxygen saturation of  96% room air. We use LEMON method to assess the intubation difficulties, we found swelling on the right jaw, Incisor distance less than 2 cm, make it impossible to evaluate mallampati score, and limited neck mobility due to pain. We decided to avoid general anesthesia since preoperative airway assessment indicates difficult intubation. We sedate the patient with midazolam and administered trigeminal block using 22-G needle with local anesthetic solution of levobupivacaine 0,375% and dexamethasone 2,5mg as an adjuvant with classic landmark trigeminal technique. The patient tolerated the procedure well and return to the ward after the procedure. Conclusion: The use of trigeminal block can be as an alternative general anesthesia oral-maxillofacial surgery area that may have predisposing difficult airway intubation and may be valuable in contributing better patients outcomes.

Abstract

Background: The use of spices and herbs in the nutrition installation of Dr. Moewardi Regional General Hospital to make kitchen seasoning generates waste. The results of the researcher's interview with officers at the Food Production of Nutrition Installation of Dr. Moewardi Hospital, it was discovered that there was no utilization of spices and herbs waste. The spices and herbs waste was thrown away. Therefore, the researchers initiated to use herbs waste such as onion peels, garlic peels, leek, kaffir lime leaves, lemongrass, pandan leaves, and ginger as an alternative to additional seasoning in the form of oil called magic oil to increase the taste of patients' food, especially plant-based side dishes because based on the results of a study by Martini et al, 2022 it is discovered that there is more than 20% of leftover plant-based side dishes. Hence, a study to be conducted on the nutritional content and organoleptic properties of the variety of plant-based side dishes with the substitution of spice waste oil called magic oil. The plant-based side dish to be used was tofu curry because tofu curry was one of the plant-based side dishes used in previous study. In addition, tofu curry uses one of the basic seasonings, namely yellow seasoning. Purpose of the Study: Discovering the nutritional content and organoleptic properties of plant-based side dish modification with magic oil substitution. Method of the Study: The study used observational analytic design with its experimental nature. This study describes the nutritional content and organoleptic test of the modification of tofu curry with magic oil substitution. Nutritional content data were processed using  the Shapiro-Wilk test because the number of samples <50. The results were normally distributed, subsequently it was continued using the Anova test and Post-Hoc test. Data on organoleptic properties were processed using  the Shapiro-Wilk test  because the number of samples <50 results were not normally distributed, therefore it was tested using the Kruskal-Wallis test and continued with the Mann-Whitney test, providing that it was significant. Results: Based on the results of the Anova test, there was a significant difference in nutritional content data and continued with post-hoc  test. It was discovered that the treatment groups, namely B, C, and D, had high nutritional content compared to group A. The highest water, mineral, and protein content was discovered in group D, the highest fat and fiber content was discovered in group B,  and the highest carbohydrate content was discovered in group C. In organoleptic tests  consisting of color, taste, aroma, and texture, and it had a p value of <0.05 which means the data was not normally distributed therefore it used Kruskal-Wallis test  and continued with Mann-Whitney test,  providing that it was significant. After the Kruskal-Wallis test, the most influential was the taste factor in group D with a median value in taste of 3 which means “like”. Conclusion: The results of the Nutritional Content Test and Oraganoleptic Test of Tofu Curry Magic Oil Substitution show significant differences between group A (standard) and treatment groups, namely groups B, C and D. The higher the level of Magic Oil substitution, the higher the nutritional content and acceptability  of organoleptic  properties to the modification of Tofu Curry.

Abstract

Background: The heart is an organ that pump blood to the entire body system and draw the blood back into the heart. The inability of the heart to perform its functions is generated by damage to the anatomy or physiology of the heart, which induces various diseases of the cardiovascular system. Cardiovascular disease is the leading cause of death in the world. (Dewanti, I.P, 2014). One in ten deaths in the world is caused by heart disease. This makes heart attacks the leading cause of death in the world every year. One of the criteria for Infarc Myocard Acute (IMA)  diagnosis by World Health Organization (WHO) is an increase in heart enzyme levels. Troponin is one of the heart enzymes that will increase if there is a myocardial cell damage. A study conducted at West Sumatra Main Branch of YJI Heart Specialist Hospital in the period of July 2013 – June 2014, discovered 81 patient data with IMA diagnosis and 37 data with Troponin and ECG examination results. (Primananda, M.L, Syafri, M, and Meinapuri, 2014) Dr. Moewardi Regional General Hospital is one of the hospitals in Surakarta City that has specialized wards for patients with heart disease, one of which is the Aster room. Food intake in heart patients is part of the therapy given. The dietary intake of heart patients treated at Dr. Moewardi Hospital is still below 80% of the target quality of the Nutrition Installation. Low food intake can be instigated by low appetite, a feeling of abdominal bloating. Small frequent feeding portions is one way to increase the feeding intake of heart patients. Based on these circumstances, it is necessary to conduct a study to determine the effect of feeding modification and eating frequency on nutrient intake and changes in troponin level of heart patients in the Aster Ward of Dr. Moewardi Regional General Hospital Surakarta. Purpose of the Study: discovering the description of the effect of feeding modification on changes in troponin level of heart patients in Aster inpatient ward of Dr. Moewardi Regional General Hospital. Method of the Study: It was an analytical description study the study design used in this study was "One Group Pretest-Posttest Design", which is a study design that contains pretest before treatment and posttest after treatment.
