Abstract

Latar belakang: Alat musik drum merupakan alat musik yang memiliki nilai kebisingan. Instruktur drum dapat terpapar bising yang tinggi, sehingga dapat mengalami gangguan pendengaran. Tujuan: Mengetahui hubungan antara lama paparan bising terhadap gangguan pendengaran pada instruktur drum di Surakarta dan sekitarnya. Metode: Jenis penelitian yang digunakan adalah obervasional non experimental dengan desain cross sectional pada 71 instruktur drum di 14 sekolah musik di Surakarta. Sampel berupa hasil pemeriksaan audiometri nada murni yang dilaksanakan di studio musik kedap suara dengan NAB kurang dari 40 Dba SPL. Data dianalisis menggunakan  uji statistik Chi Square. Hasil analisis statistik bermakna bila didapatkan nilai p=0,001<0,01. Hasil: Pada rerata tingkat kebisingan yang terukur dengan alat Sound Level Meter di 14 sekolah musik di Surakarta. Pada saat memainkan drum adalah sebesar 111,48±3,84 dB. Terdapat responden dengan durasi ≤2 jam tanpa GPAB sebanyak 10 orang (14,1%) dan paling banyak terjadi dengan GPAB pada durasi >4 sampai dengan 6 jam sebanyak 38 orang (51,2%). Nilai p=0,001<0,01 yang berarti ada hubungan yang bermakna antara durasi dengan GPAB pada instruktur drum dengan coefficient of contingency CC sebesar 0,687 (68,70%). Kesimpulan: Terdapat hubungan antara lama paparan bising terhadap gangguan pendengaran pada instruktur drum.  Background: Drum is one of musical instruments producing a high level of noise. Drum instructors are exposed to this loudness, which might give them a high risk of hearing impairment. Objective: To investigate the correlation between loudness exposure time and hearing impairment of drum instructors in Solo area. Methods: This was an observational non experimental research using cross sectional design on 71 drum instructors in 14 music schools in Surakarta. The samples were the result of pure tone audiometry examination conducted in a soundproof music studio with NAB less than 40 Dba SPL. Data were analyzed using Chi Square statistical tests. The results of statistical analysis were significant if  p = 0.001 <0.01. Result: The average noise level measured by a Sound Level Meter tool in 14 music schools in Surakarta, while playing the drum was 111.48 ± 3.84 dB. There were respondents with a duration of ≤ 2 hours without Noise Induce Hearing Loss (NIHL) as many as 10 people (14.1%), and the highest  occurence with NIHL was at a duration of  >4 to 6 hours as many as 38 people (51.2%). The value of p = 0.001 <0.01, revealed that there was a significant correlation between the duration of noise exposure and NIHL in drum instructors with coefficient of contingency (CC) of 0.687 (68.70%). Conclusion: Noise exposure time was correlated with hearing impairment in drum instructors.

Abstract

Abstract Introduction: Undifferentiated nasopharyngeal carcinoma is the most common malignant tumor in nasopharynx, in which most patients come to a hospital in advanced stage. Cisplatin is one of the most effective widely used chemotherapy drugs for advanced stage undifferentiated nasopharyngeal carcinoma. Although it provides a successful outcome against cancer, there is serious side effect namely ototoxic hearing loss. Ginkgo biloba is a powerful antioxidant which may prevent ototoxic hearing loss. Objective: The study aims to determine the effect of ginkgo biloba against ototoxic hearing loss on advanced stage undifferentiated nasopharyngeal carcinoma receiving cisplatin chemotherapy. Methods: This study was double blind Randomized Control Trial with pre-post test design, was conducted in 22 patients with advanced stage undifferentiated nasopharyngeal carcinoma receiving cisplatin chemotherapy at the Department of Otorhinolaryngology Head and Neck Surgery dr. Moewardi Hospital Surakarta between June 1st and October 1st 2019. The subjects were allocated into control (plasebo) and treatment (80 mg Ginkgo biloba extract) groups. Hearing function was examined with pure tone audiometry, tympanometry and Distortion Product Otoacoustic Emission before and after the first, the second and the third cisplatin chemotherapy. The results of the examination of the two groups were tested using Friedman and Chi Square test. P value of <0.05 was considered significant. Results: The study subjects were mostly male (63.6%), with the age range of 40-59 years (90.9%). The baseline characteristics of the study subjects were homogeneous (p>0.05). There was a significant difference in the incidence of ototoxic hearing loss between control and treatment group after getting the second and the third chemotherapy (p=0.043 and p=0.033, respectively). Conclusion: There is a significant effect of ginkgo biloba in term of preventing ototoxic hearing loss on advanced stage undifferentiated nasopharyngeal carcinoma patients receiving cisplatin chemotherapy.

Abstract

Latar belakang: Rinosinusitis kronis (RSK) merupakan inflamasi mukosa hidung dan sinus paranasal dengan jangka waktu gejala 12 minggu atau lebih. Transpor mukosiliar merupakan suatu mekanisme pertahanan lokal pada hidung dan sinus paranasal. Pada RSK terjadi disfungsi transpor mukosiliar. Tujuan: Mengetahui perbedaan transpor mukosiliar pada pemberian larutan garam hipertonik dan isotonik pada penderita RSK. Metode: Penelitian eksperimental murni dengan desain uji klinis randomisasi. Sampel terdiri dari dua kelompok, yaitu RSK yang mendapat larutan garam isotonik dan RSK yang mendapat larutan garam hipertonik. Tiap kelompok terdiri dari 25 sampel. Waktu transpor mukosiliar dilakukan pra dan pasca terapi memakai uji sakarin. Hasil: Secara keseluruhan, selama dua minggu masa terapi, didapati penurunan waktu transpor mukosiliar pada kelompok pasien yang diberi larutan garam hipertonik (13,00±2,12 menit) lebih besar dibandingkan pada kelompok pasien yang diberi larutan garam isotonik (6,84±2,54 menit) dan secara statistik terdapat perbedaan yang signifikan (p<0,001). Kesimpulan: Pemberian larutan garam hipertonik dapat menurunkan waktu transpor mukosiliar lebih besar dibandingkan larutan garam isotonik pada penderita rinosinusitis kronis.Kata kunci: Transpor mukosiliar, larutan garam isotonik, larutan garam hipertonik, rinosinusitis kronis ABSTRACT Background: Chronic rhinosinusitis (CRS) is an inflammation of the nasal and paranasal sinuses mucosa with periods of symptoms 12 weeks or more. Mucociliary transport plays a role as local defense mechanism of the nasal and paranasal sinuses. In CRS, there are dysfunctions on the mucociliary transport. Purpose: This study aimed to find out the difference of mucociliary transport after nasal washing with hypertonic and isotonic saline solutions in patients with CRS. Methods: This was a pure experimental study with randomized controlled trial designs. The sample was divided into 2 groups: CRS with isotonic saline solution and CRS with hypertonic saline solution treatment. Each group was consisted of twenty five samples. The mucociliary transport time examination were performed pre and post treatment by using saccharin test. Result: During the 2 weeks of therapy, the overall of mucociliary transport time reduction in the group of patients given hypertonic saline solution (13.00±2.12 minutes) were bigger than in the group of patients who were given isotonic saline solution (6.84±2.54 minutes) and there was a significant difference statistically (p<0.001). Conclusion: Hypertonic saline solution is better than isotonic saline solution in reducing mucociliary transport time.Keywords: Mucociliary transport, isotonic saline solution, hypertonic saline solution, chronic rhinosinusitis

Abstract

Latar belakang: Neuropati adalah salah satu komplikasi yang paling sering dari Diabetes Melitus(DM) tipe 2. Disamping neuropati perifer dan otonom, pasien dengan DM tipe 2 juga dapat menderitagangguan pendengaran tipe sensorineural hearing loss (SNHL) pada frekuensi tinggi. Pada studi inihubungan HbA1c dengan hasil pemeriksaan brainstem evoked response audiometry (BERA) dan transient evoked otoacoustic emissions (TEOAE) digunakan untuk mendeteksi neuropati akustik dan sentral pada pasien DM tipe 2. Tujuan: Untuk mengetahui hubungan kadar HbA1c pada DM tipe 2 dengan masa laten gelombang BERA dan respon TEOAE. Metode: Desain penelitian cross sectional study, dilakukan dari Maret 2018 hingga jumlah sample terpenuhi. Penelitian dimulai dengan mengidentifikasi pasien DM tipe 2 dengan profil kadar HbA1c, kemudian dilanjutkan dengan pemeriksaan TEOAE dan BERA. Hasil: Dari hasil pemeriksaan BERA terdapat hubungan signifikan antara HbA1c dengan masa laten gelombang BERA di intensitas 70 dB dan 80 dB kecuali latensi antar-puncak I-V, tetapi tidak berhubungan signifikan pada intensitas 90 dB. Didapatkan pada pemeriksaan TEOAE di telinga kanan kadar HbA1c berhubungan dengan hasil pemeriksaan TEOAE kategori refer rata-rata 8,67+1,14%, sedangkan pasien dengan hasil pemeriksaan TEOAE kategori pass rata-rata 7,73+0,95%. Hasil uji t test didapatkan nilai p=0,021 (p<0,05). TEOAE di telinga kiri kategori refer rata-rata 8,82+1,09%, sedangkan pasien dengan hasil pemeriksaan TEOAE kategori pass rata-rata 7,66+0,88%. Hasil uji t test didapatkan nilai p=0,003 (p<0,05). Kesimpulan: Terdapat hubungan yang signifikan secara statistik antara kadar HbA1c dengan hasil pemeriksaan gelombang BERA dan TEOAE. Background: Neuropathy is complication of type 2 diabetes mellitus (DM). Besides peripheraland autonomic neuropathy, patients with type 2 DM may also have sensorineural hearing loss at highfrequency. In this study the association of HbA1c level with the results of BERA and TEOAE tests was used to detect acoustic and central neuropathy in type 2 DM patients. Objective: To find out the relationship of HbA1c level in type 2 DM with BERA and TEOAE. Method: Research design cross sectional study, conducted from March 2018 until the number of samples fulfilled. The study began by identifying patients with type 2 DM with the HbA1c profile, followed by TEOAE and BERA examinations. Result: There was a significant relationship between HbA1c and the latent period of waves at the intensity of 70 dB and 80 dB except on the interpeak I-V latency, but not significantly related to the 90 dB intensity. Results of TEOAE test in the right ear: the average levels of HbA1c with refer category results on TEOAE test was 8.67+1.14 %, whereas patients with pass category results on TEOAE was 7.73+0.95 %. The result of t test value was p=0.021 (p<0.05). TEOAE on the left ear: average refer category 8.82+1.09%, whereas patients with TEOAE examination results in the mean pass category 7.66+0.88%. The result of t test value was p=0.003 (p<0.05). Conclusion: There was a statistical significant relationship between HbA1clevel and BERA and TEOAE.

Abstract

Latar belakang: Bayi baru lahir dengan asfiksia perinatal dapat mengalami gangguan fungsi sel rambut luar  pada kokleanya. Tujuan:  Mengetahui   hubungan   asfiksia  perinatal  dengan  gangguan   fungsi  sel rambut luar koklea. Metode: Penelitian analitik observasional dengan desain Case control yang dilakukan di bagian perinatologi RSUD Dr. Moewardi Surakarta, RSUD Karanganyar,          RSUD Wonogiri, dan RSUD Sukoharjo dengan waktu penelitian  pada bulan Oktober 2014 – Januari 2015. Sampel penelitian dipilih dengan cara non probability sampling yaitu dengan teknik consecutive sampling, sebanyak 50 orang yang terdiri dari 25 kelompok kasus dan 25 kelompok kontrol. Diagnosis adanya gangguan fungsi sel rambut luar koklea  ditegakkan dari hasil pemeriksaan fisik THT dan pemeriksaan Distortion Product Otoacoustic Emissions  (DPOAE). Asfiksia perinatal dapat dilihat dengan menggunakan  skor APGAR, sedangkan faktor risiko yang lain dapat dilihat dari  catatan  medis pasien. Analisis statistik menggunakan univariat, bivariat, dengan chi square dan multivariat dengan regresi logistik ganda model faktor risiko. Hasil: Dari 25 kelompok kasus dan 25 kelompok kontrol didapatkan hasil bahwa asfiksia perinatal merupakan faktor risiko yang berpengaruh terjadinya gangguan fungsi sel rambut luar koklea.  Bayi baru lahir yang mempunyai  gangguan fungsi sel rambut luar koklea  dengan  asfiksia perinatal mempunyai faktor risiko 29 kali lebih besar daripada bayi dengan bayi baru lahir tanpa gangguan fungsi sel rambut luar koklea, setelah mengontrol pengaruh dari faktor perancu hiperbilirubinemia dan berat  badan lahir rendah. Hasil tersebut didapatkan bermakana secara statitistik (OR=29,614; CI – 95 % = 5,454 - 160,792; p<0,001). Kesimpulan: Terdapat hubungan antara asfiksia perinatal dengan  gangguan fungsi sel rambut luar koklea. Background: Newborn  with  perinatal  asphyxiae can occur disturbance of  function outer hair cell in the cochlea. Objective: was to know the relationship between the degree of perinatal asphyxiae  with impaired  function  of  outer hair cell. Methods: The study was  an analytic observational study with case control design, that  took a place in perinatology department in Doctor Moewardi hospital Surakarta, General hospital  in  Karanganyar,  General hospital in Wonogiri, General hospital in Sukoharjo. The study started from October 2014 until January 2015. The sample study were selected with consecutive sampling method, with total sample of 50 newborns which consist of 25 case group and 25 control group. Impaired function of outer hair cell  has been diagnosed from ENT examination and  examined with Distortion Product Otoacoustic Emissions  (DPOAE). Perinatal asphyxiae were measured with APGAR score and the other risk factor were collected from the medical record of the patients. Data were analyzed with univariat, bivariat (chi square) and multivariat statistic with double logistic regression. Result: From 25 case group and 25 control group, were found that perinatal asphyxiae was the influence risk factor to the occurrence of impaired function of outer hair cell. Newborns have impaired function  of  outer hair cell with   perinatal asphyxiae will have the risk of twenty nine times higher than newborns without perinatal asphyxiae, after controlling of confounding factors low birth weight, and hyperbilirubinemiae. The results  was statistically significant. (OR = 29,614; CI – 95 % = 5,454 - 160,792; p < 0,001). Conclusion: There has corellated between perinatal asphyxiae and impaired function of outer hair cell.

Abstract

ABSTRACTBackground: Along with the increasing incidence of chronic renal failure patients receiving hemodialysis, recently, the incidence of sensorineural hearing loss (SNHL) among patients with end-stage kidney disease undergoing hemodialysis had also increased. The cochlea and kidney have many anatomical, physiological, pharmacological and pathological similarities. Dialysis sometimes causes worsening of hearing function. Objective: To determine the correlation between the duration of renal failure patients undergoing hemodialysis and the degree of sensorineural hearing loss. Method: A retrospective study using cross sectional method, conducted from August–September 2018. The study began by identifying patients with chronic renal failure and then continued with Oto Acoustic Emission (OAE) examination and pure tone audiometry. Result: The results of the Spearman rank statistical test for the correlation between duration of hemodialysis and the degree of SNHL in the right and left ears in patients with chronic renal failure found a significant positive correlation r= 0.498 and p=0.001 (right) , and r= 0.400 and p=0.006 (left). Conclusion: There was a significant correlation between the length of hemodialysis and the degree of sensorineural hearing loss. ABSTRAKLatar belakang: Seiring dengan peningkatan angka kejadian pasien gagal ginjal kronik yang mendapatkan hemodialisa, akhir-akhir ini didapati peningkatan angka kejadian Sensorineural Hearing Loss (SNHL) di antara pasien dengan penyakit ginjal stadium akhir yang menjalani hemodialisa. Koklea dan ginjal memiliki banyak kesamaan anatomis, fisiologis, farmakologis dan patologis. Dialisa terkadang menyebabkan memburuknya fungsi pendengaran. Tujuan: Untuk mengetahui hubungan lama pasien gagal ginjal yang menjalani hemodialisa dengan derajat gangguan pendengaran tuli sensorineural. Metode: Penelitian retrospektif dengan metode potong lintang, dilakukan dari Agustus-September 2018. Penelitian dimulai dengan mengidentifikasi pasien gagal ginjal kronik kemudian dilanjutkan dengan pemeriksaan tes Oto Acoustic Emission (OAE) dan audiometri nada murni. Hasil: Hasil uji statistik Spearman rank untuk korelasi lama hemodialisa dengan derajat SNHL telinga kanan dan kiri pada pasien gagal ginjal kronik didapatkan korelasi positif yang signifikan: r= 0,498 dan p=0,001 (kanan) dan r= 0,400 dan p=0,006 (kiri). Kesimpulan: Terdapat korelasi yang bermakna antara lama hemodialisis dengan derajat tuli sensorineural.

Hasil uji t test didapatkan nilai p=0,003 (p<0,05). Kesimpulan: Terdapat hubungan yang signifikan secara statistik antara kadar HbA1c dengan hasil pemeriksaan gelombang BERA dan TEOAE. Background: Neuropathy is complication of type 2 diabetes mellitus (DM). Besides peripheraland autonomic neuropathy, patients with type 2 DM may also have sensorineural hearing loss at highfrequency. In this study the association of HbA1c level with the results of BERA and TEOAE tests was used to detect acoustic and central neuropathy in type 2 DM patients. Objective: To find out the relationship of HbA1c level in type 2 DM with BERA and TEOAE. Method: Research design cross sectional study, conducted from March 2018 until the number of samples fulfilled. The study began by identifying patients with type 2 DM with the HbA1c profile, followed by TEOAE and BERA examinations. Result: There was a significant relationship between HbA1c and the latent period of waves at the intensity of 70 dB and 80 dB except on the interpeak I-V latency, but not significantly related to the 90 dB intensity. Results of TEOAE test in the right ear: the average levels of HbA1c with refer category results on TEOAE test was 8.67+1.14 %, whereas patients with pass category results on TEOAE was 7.73+0.95 %. The result of t test value was p=0.021 (p<0.05). TEOAE on the left ear: average refer category 8.82+1.09%, whereas patients with TEOAE examination results in the mean pass category 7.66+0.88%. The result of t test value was p=0.003 (p<0.05). Conclusion: There was a statistical significant relationship between HbA1clevel and BERA and TEOAE.

Abstract

Latar belakang: Bayi baru lahir dengan asfiksia perinatal dapat mengalami gangguan fungsi sel rambut luar  pada kokleanya. Tujuan:  Mengetahui   hubungan   asfiksia  perinatal  dengan  gangguan   fungsi  sel rambut luar koklea. Metode: Penelitian analitik observasional dengan desain Case control yang dilakukan di bagian perinatologi RSUD Dr. Moewardi Surakarta, RSUD Karanganyar,          RSUD Wonogiri, dan RSUD Sukoharjo dengan waktu penelitian  pada bulan Oktober 2014 – Januari 2015. Sampel penelitian dipilih dengan cara non probability sampling yaitu dengan teknik consecutive sampling, sebanyak 50 orang yang terdiri dari 25 kelompok kasus dan 25 kelompok kontrol. Diagnosis adanya gangguan fungsi sel rambut luar koklea  ditegakkan dari hasil pemeriksaan fisik THT dan pemeriksaan Distortion Product Otoacoustic Emissions  (DPOAE). Asfiksia perinatal dapat dilihat dengan menggunakan  skor APGAR, sedangkan faktor risiko yang lain dapat dilihat dari  catatan  medis pasien. Analisis statistik menggunakan univariat, bivariat, dengan chi square dan multivariat dengan regresi logistik ganda model faktor risiko. Hasil: Dari 25 kelompok kasus dan 25 kelompok kontrol didapatkan hasil bahwa asfiksia perinatal merupakan faktor risiko yang berpengaruh terjadinya gangguan fungsi sel rambut luar koklea.  Bayi baru lahir yang mempunyai  gangguan fungsi sel rambut luar koklea  dengan  asfiksia perinatal mempunyai faktor risiko 29 kali lebih besar daripada bayi dengan bayi baru lahir tanpa gangguan fungsi sel rambut luar koklea, setelah mengontrol pengaruh dari faktor perancu hiperbilirubinemia dan berat  badan lahir rendah. Hasil tersebut didapatkan bermakana secara statitistik (OR=29,614; CI – 95 % = 5,454 - 160,792; p<0,001). Kesimpulan: Terdapat hubungan antara asfiksia perinatal dengan  gangguan fungsi sel rambut luar koklea. Background: Newborn  with  perinatal  asphyxiae can occur disturbance of  function outer hair cell in the cochlea. Objective: was to know the relationship between the degree of perinatal asphyxiae  with impaired  function  of  outer hair cell. Methods: The study was  an analytic observational study with case control design, that  took a place in perinatology department in Doctor Moewardi hospital Surakarta, General hospital  in  Karanganyar,  General hospital in Wonogiri, General hospital in Sukoharjo. The study started from October 2014 until January 2015. The sample study were selected with consecutive sampling method, with total sample of 50 newborns which consist of 25 case group and 25 control group. Impaired function of outer hair cell  has been diagnosed from ENT examination and  examined with Distortion Product Otoacoustic Emissions  (DPOAE). Perinatal asphyxiae were measured with APGAR score and the other risk factor were collected from the medical record of the patients. Data were analyzed with univariat, bivariat (chi square) and multivariat statistic with double logistic regression. Result: From 25 case group and 25 control group, were found that perinatal asphyxiae was the influence risk factor to the occurrence of impaired function of outer hair cell. Newborns have impaired function  of  outer hair cell with   perinatal asphyxiae will have the risk of twenty nine times higher than newborns without perinatal asphyxiae, after controlling of confounding factors low birth weight, and hyperbilirubinemiae. The results  was statistically significant. (OR = 29,614; CI – 95 % = 5,454 - 160,792; p < 0,001). Conclusion: There has corellated between perinatal asphyxiae and impaired function of outer hair cell.

Abstract

ABSTRAK Gangguan pendengaran akibat bising (GPAB) sering dijumpai pada usia pelajar. Adanya kemajuan alat elektronik beserta kemudahan dalam mengakses internet dan sosial media menyebabkan pelajar sering menggunakan personal listening device seperti headphone/earphone untuk mengakses informasi dari piranti elektronik tersebut. Identifikasi gangguan pendengaran secara dini dan intervensi yang sesuai terbukti dapat mencegah konsekuensi tersebut. Tujuan diselenggarakan kegiatan ini adalah untuk memberikan edukasi gangguan pendengaran akibat bising (GPAB) dan untuk mengetahui sejak dini kondisi telinga dan pendengaran pelajar dan memberikan intervensi dan tatalaksana sesuai kebutuhan. Metode dalam kegiatan ini direncanakan dilakukan melalui tahapan - tahapan sesuai dengan petunjuk pengendalian HIRARC (hazard identification, risk assessment, and risk control). Hasil kegiatan ini didapatkan pendengaran 147 siswa dalam batas normal, 31 siswa terdapat serumen, 7 siswa Otitis Eksterna, 3 siswa OMSK, dan 4 orang siswa dicurigai menderita gangguan pendengaran akibat bising. Siswa yang dicurigai menderita gangguan pendengaran akibat bising dianjurkan ke RSUD Dr. Moewardi Surakarta untuk dilakukan pemeriksaan dan tatalaksana lebih lanjut.   ABSTRACT Noise induce hearing loss (NIHL) is often found in students. The advancement of electronic devices along with the ease of accessing the internet and social media has caused students to often use personal listening devices such as headphones/earphones to access information from these electronic devices. Early identification of hearing loss and appropriate intervention have been shown to prevent such consequences. The purpose of this activity was to educate noise induce hearing loss (NIHL) and to know early on the ear and hearing condition of students and provided intervention and management as needed. The method in this activity was planned to be carried out through stages in accordance with the HIRARC (Hazard Identification, Risk Assessment, and Risk Control) control instructions. The results of this activity obtained the hearing of 147 students within normal limits, 31 students with cerumen, 7 Otitis Externa students, 3 OMSK students, and 4 students with suspect of noise induce hearing loss. Students who were suspected of suffering from hearing loss due to noise were recommended to Dr. Moewardi Surakarta Hospital for further examination and management.
