Abstract

The local DRL for Dr. Moewardi Hospital is used as a control of patient radiation safety and to measure the quality of radiological examinations conducted at the Radiology Installation of Dr. Moewardi Hospital. The study conducted an assessment on a conventional examination with a General Radiography X-ray plane. The research methodology used was a quantitative approach by using a sample of 30 patients in each examination. X-ray machine data collection was conducted by looking for the Entrance Surface Air Kerma (ESAK) equation, then inputting the patients' data of kV, mAs, FSD, and BSF into the ESAK equation to obtain Entrance Surface Dose (ESD) value of each patient. From ESD data, DRLs were made for each examination using the 3rd quartile data. The results of the study indicated that the local DRL of Dr. Moewardi Hospital was lower than the National DRL of BAPETEN

Abstract

Introduction Inflammatory autoimmune rheumatic disease (AIIRD) such as rheumatoid arthritis, systemic lupus erythematosus, and Sjogren's syndrome often requires long-term glucocorticoid therapy. Despite its effectiveness, glucocorticoids are recognized to cause decreased bone quality and increase the risk of osteoporosis. This study aims to evaluate the effect of the period of glucocorticoid administration on bone quality parameters in AIIRD patients. Methods This study was an observational analytical study with a cross-sectional design. A total of 28 female patients with AIIRD were divided into two groups based on the period of glucocorticoid administration: less than 24 months and more than 24 months. The parameters to be measured included bone mineral density (BMD), Collagen Type I C-Telopeptide (CTX), Vitamin D3 levels, and Calcium Ions. Data analysis was conducted using the Mann-Whitney test and the unpaired t-test with a significance level of p<0.05. Results There was no statistically significant difference between the two groups on all measured parameters. The mean BMD in the group of less than 24 months was 0.9471 kg/m², while in the group of more than 24 months it was 0.8920 kg/m² (p>0.05). The mean CTX also did not differ significantly between the two groups (0.2474 vs. 0.2508 ng/mL; p>0.05). Vitamin D3 and Calcium Ion levels showed similar results in the absence of significant differences (Vitamin D3: 15,596 vs. 16,255 ng/mL; Calcium Ion: 1,075 vs. 1,040 mmol/L; p>0.05). Conclusions The period of glucocorticoid administration does not show a significant effect on bone quality in AIIRD patients in this study. Nonetheless, regular monitoring and an individualized approach are still necessary to prevent bone complications in patients receiving long-term glucocorticoid therapy.

Abstract

Introduction Medication waiting time is one of the minimum pharmaceutical service standards in hospitals, which, if not achieved, can lead to patient dissatisfaction. Based on a field observation, several things hinder prescription services at the Pharmaceutical Supplies Distribution Unit of the regular outpatient of Dr. Moewardi Hospital. Optimization is carried out to reduce medication waiting times for drug services. Objective: To analyse the effect of optimization to medication waiting time at Pharmaceutical Supplies Distribution Unit of regular outpatient in Dr. Moewardi General Hospital   Method The research method was conducted by comparing the average medication waiting time before and after optimization. Before a comparative test, a normality test was carried out with Kolmogorov – Smirnov. A T-test would be conducted if the data were distributed normally, if the data were not distributed normally, the Mann-Whitney test would be performed. Results The results showed a decrease in the waiting time for compounded medicines, both the time from the doctor ordering the prescriptions to the pharmacy validating the prescriptions (p=0.044), and the time from the pharmacy receiving the prescriptions until the medicines were ready (p=0.000). There was a decrease in the average waiting time for non-compounded medicines, the waiting time from the pharmacy receiving the prescriptions until the medicines were prepared, which was originally 45.54 minutes to 36.9 minutes after the service optimization was conducted. However, the decrease in time was not significantly different, both for the time from the doctor ordering the prescriptions to the pharmacy validating the prescriptions (p=0.386), and the time from the pharmacy receiving the prescriptions until the medicines were ready (p=0.305). Conclusion There is an effect of optimizing the flow of prescription services on waiting time of the compounded medicines at the UDPF of Regular Outpatient of Dr. Moewardi Hospital.

Abstract

Introduction Sexually transmitted infections (STIs) are diseases resulting from bacteria, viruses, or parasites transmitted during vaginal, anal, or oral sexual intercourse. The COVID-19 pandemic markedly influenced STI statistics, leading to potential underreporting of cases and heightened transmission rates. Objective : To analyse the profile of sexually transmitted infections and the influence of COVID-19 on STI assessments at the Dermatology and Venereology (DV) Outpatient Clinic of Dr. Moewardi General Hospital. Method A retrospective study using secondary data derived from medical records and registration logs at DV Outpatient Clinic of Dr. Moewardi General Hospital, spanning from January 2018 to December 2022.   Results 284 new STI cases were documented in the before pandemic lockdown period, however during the pandemic lockdown, there was a significant decline of 149 new cases. After the pandemic lockdown, there was 200 additional cases of STIs. Up to 90 patients with STIs were co-infected with HIV. The predominant instance of STIs was condyloma acuminata., and there was a strong correlation between gender and the incidence of STIs (p=0.000; p<0.05) Conclusion There was a sharp decline in STI cases during the COVID-19 pandemic and a surge in STI cases after the COVID-19 pandemic, predominantly featuring instances of condyloma acuminata and syphilis.

Abstract

Background: The simultaneous occurrence of ST-elevation myocardial infarction (STEMI) and acute ischemic stroke (AIS) was described as Cardio-Cerebral Infarction. Consequently, we aimed to provide an overview of the available treatment option for patients in type c hospital in order to guide the similar cases. Case Description: A 50-year-old man came to the emergency room complaining of chest pain for two hours before. The patient had a history of hypertension and an active smoker. The patient had a left hemiparesis. The patient's electrocardiogram showed ST segment elevation in leads V3 and V4. A non-contrast head CT-scan showed infarct in the right occipital region. Echocardiogram showed regional wall motion abnormality. The patient was admitted to the hospital, administered DAPT, statin, anticoagulant and discharged after relief of symptoms. Intravenous alteplase followed by primary PCI is the gold standard therapy. However, due to limitation, anticoagulants were chosen and showed clinical improvements. Conclusion: In summary, the goal for treat CCI is to achieve more efficacy therapy while minimising the risk of bleeding. Despite the limitations, patient outcome was good.
