Abstract

Access to professional mental health services was low worldwide, especially in lower-middle-income countries. Indonesia encounters several challenges in providing adequate mental health care. Poor detection could reduce the treatment coverage. This study aimed to determine how many individuals with depression recognize their condition or receive an appropriate diagnosis, as well as the types of treatments they receive. We analyzed secondary data from the Indonesian Family Life Survey 5th edition (IFLS-5), which included socio-demographic data, levels of well-being, subjective experiences of mental health issues, and treatment information. We found that 6,645 respondents (22.8%) exhibited significant depressive symptoms, yet only 15 respondents (0.1%) reported having a lifetime psychiatric disorder that met the criteria for depression. Most lifetime psychiatric diagnoses (LPD) were made by doctors (93.3%). A significant portion of respondents with a history of psychiatric disorders did not receive any treatment (70.5%), and among those who did, medication was the most common approach. There was considerable gap between the identified significant depressive symptoms and LPD, warranting further exploration. Low detection rates and stigma, potentially linked to Indonesian culture and perceptions of mental health, may underlie these issues. A variety of treatment options must be available and accepted to be beneficial for patients.

Abstract

Perinatal mental health issues, particularly depression and anxiety, pose critical challenges to maternal and children’s well-being. It has been reported that psychological distress affects approximately 20.7% of prenatal and 17% of postnatal mothers, attributed to multifaceted emotional, physiological, social, and interpersonal transitions during the perinatal period. Accessibility of digital technologies has increased, prompting innovative approaches to prevention and treatment. This study aims to evaluate the effectiveness of digital psychotherapy interventions compared to traditional care in addressing perinatal mental health outcomes. A comprehensive literature search conducted between September and October 2023 across Scopus, PubMed, and Web of Science databases yielded eleven eligible randomized controlled trials (2017-2023). Meta-analytic findings using Review Manager 5.4 demonstrated significant reductions in anxiety (SMD: -0.41; 95% CI: -0.67 to -0.15; p = 0.002) and depressive symptoms (SMD: -0.41; 95% CI: -0.54 to -0.27; p < 0.00001) through digital interventions compared to traditional care. Future research directions should prioritize developing engaging interventions, possibly incorporating animation and virtual environments, while considering population-specific factors and optimal intervention duration.

Abstract

The demand for shoes must be met by the worldwide shoe manufacturing business, but the inhalation dangers associated with its chemical processes—which use benzene, toluene, and xylene (BTX)—raise health issues for workers. Initial surveys showed that most shoe manufacturing workshops do not have adequate ventilation. Most workers often complain of symptoms such as coughing, shortness of breath, and headaches. This study examined the relationships between worker symptoms and chemical levels at shoe factories in Medan, Indonesia, in 2017. This was an observational cross-sectional study. The total population was 72 workers in shoe workshops in the small industrial center area in Medan city. The sample size in this study was 47 people in seven shoe workshops. BTX concentrations were measured in seven workshops. Statistical tests used in this study were chi-square test and Fisher’s exact test with a significance level of p<0.05. According to the findings, the most common symptoms among employees were coughing (40.4%), dizziness (55.3%), and eye discomfort (55.3% of respondents). The range values were 0.09 ppm to 2.83 ppm, 1.20 ppm to 178.98 ppm, and 0.11 ppm to 50 ppm for benzene, toluene, and xylene, respectively. The two compounds most commonly exposed to levels exceeding the ACGIH (American Conference of Governmental Industrial Hygienists) threshold level value were toluene (55.3%) and benzene (25.5%), according to ambient readings. Of the workshops that were inspected, only two (28.57%) had a functional ventilation system. Exposure to benzene exceeding the quality standard showed a significant association with dizziness (p = 0.003) and watery eyes (p = 0.003). Meanwhile, exposure to toluene exceeding the permissible limit was significantly associated with dyspnea (p = 0.005). Our research showed that over half of the workforce reported subjective complaints and that levels of benzene and toluene exceeded recommendations for occupational exposure.

Abstract

Dengue fever remains a significant public health problem in the tropics, with Aedes aegypti as the primary vector. Insecticide resistance among mosquitoes has been increasing, so an environmentally friendly alternative approach with biolarvicides is needed. This study aimed to evaluate the effects of bromelain and pineapple (Ananas comosus) extracts on the reproductive parameters of Aedes aegypti. Experiments were conducted in a controlled laboratory, where mosquitoes were exposed to various concentrations (0% to 10%) of bromelain and pineapple extracts. The number of egg production, hatching rate, and lifespan of adult mosquitoes were measured. Our results showed Bromelain, especially at higher concentrations, significantly reduced egg production, hatching rate, and longevity compared to the control and pineapple extract. At 6% and 8% bromelain concentrations, egg production and hatchability were very low, and mosquito lifespan was reduced. Pineapple extract also showed an inhibitory effect on egg production, although less consistently than bromelain. The sugar solution used as a control produced more eggs and live longer. In conclusion, bromelain from Ananas comosus showed promising insecticidal potential by reducing mosquito fertility and longevity, indicating its utility as an alternative vector control strategy for managing dengue outbreaks.

Abstract

The problem of domestic wastewater on small islands poses a significant threat to the surrounding marine ecosystem. Generally, residents of small islands tend to overlook the sanitation aspects of domestic wastewater. The contents of domestic wastewater can harm environmental ecosystems and act as vectors for diseases within the community. This study aims to assess the sanitation risk index of domestic wastewater and its impact on environmental and public health. The research was conducted on Bungin Island in May 2024, using a mixed-methods approach with a Sequential Explanatory Design. The study involved 274 households as respondents and six key informants. Research instruments included questionnaires, observation sheets, and interview guides. Data collected were analyzed using Microsoft Excel 2013 and IBM SPSS version 29.0, while interview and observation data were processed using NVIVO version 14.0. The findings indicate that residents generally do not perceive domestic wastewater as a potential problem. Bungin Island falls into the high-risk category for sanitation, impacting fish ecosystems, coral reefs, and contributing to marine pollution. Furthermore, it acts as a driving factor for diarrheal diseases and stunting in children. In conclusion, poor sanitation conditions have direct and indirect impacts on environmental ecosystems and public health. Integrated efforts are thus essential to address domestic wastewater issues on densely populated small islands.

Abstract

Adolescent-friendly Health Services (PKPR’s Nutrition service) are intended to eliminate adolescent malnutrition, including anemia. However, the effect of health program utilization on adolescent anemia has rarely been studied. This study intends to examine the prevalence of anemia, the relation of social and behavioral factors, and the use of PKPR’s nutrition service with the anemia status of adolescent girls in Surakarta. A school-based cross-sectional design was conducted at a senior high school in Surakarta. Samples were 275 female students in grades 10 and 11 who had experienced menstruation. Multiple regression analysis is deployed to model the factors associated with anemia. The prevalence of anemia in female students was 37.82%. The employment status of the father and mother is associated with the anemia status of adolescents. Other factors, such as parent education, family affluence scale, behavior factors, parent support, knowledge, attitude, perception, and self-efficacy, were not associated with anemia status. PKPR’s nutrition service utilization did not correlate with adolescents' anemia status. PKPR’s nutrition service has not yet had an impactful outcome in reducing the frequency of anemia. Further research is needed to evaluate its implementation and impact on adolescent health outcomes.

Abstract

Dengue hemorrhagic fever (DHF) is a major global health challenge, especially in areas endemic to Aedes aegypti mosquito. This study analyzes the environmental factors influencing larval presence and investigates the effectiveness of natural larvicides from castor seed (Ricinus communis) in controlling Ae. aegypti mosquito larvae. Using a cross-sectional survey of households by purposive sampling between those located near and far from public places and a quasi-experimental study compared households in the intervention group (using castor seed powder larvicide) with those in the comparison group (not using castor seed powder larvicide) were conducted in Medan Amplas District, Medan City. The study found that location significantly impacts mosquito larvae (p = 0.045, OR 3.26, 95% CI), with households near public places at higher risk. Applying castor seed-based natural larvicide at 100 mg/L of water (p = 0.0001, OR 37.76, 95% CI 17.9-79.2) significantly reduced larvae, with a 37.76-fold higher likelihood of larval absence than the comparison group. Notably, the use of castor seed powder demonstrated exceptional effectiveness in reducing the presence of Ae. aegypti mosquito larvae. These findings highlight the potential of castor seed natural larvicides as a sustainable and environmentally friendly alternative to chemical larvicides, particularly for households near public places.

Abstract

Proteus mirabilis is pathogenic bacteria that can cause gastrointestinal infections, bacteremia, and Urinary Tract Infections (UTI). Therefore, it is necessary to have a fast, sensitive, specific, and accurate detection method to identify Proteus mirabilis. This study aims to determine the confirmation, specificity, and sensitivity test of the hpmA gene primer to detect Proteus mirabilis quick and precise using the real-time Polymerase Chain Reaction method. Gradient Polymerase Chain Reaction results showed the hpmA primer has an amplicons length of 195 bp and the optimum annealing temperature at 60°C. The primer pair produced Ct value of 10.40±0.18 and showed one peak in the melting curve with Tm value of 81.84°C±0.02 by real-time PCR. Furthermore, the hpmA primer was also able to distinguish target and non-target bacteria based on the difference in Ct and Tm value formed. Based on these results, the concentration of bacterial DNA that can be detected by primers reached 3.2 pg/μL, equivalent to the concentration of target bacteria that can be detected by primers is 10.24×102 CFU. In the next step, hmpA primer will be developed to detect Proteus mirabilis in artificial contaminated samples using real-time PCR.

Abstract

In the healthcare sector, delivering patient-centered care is vital for enhancing results and ensuring favorable experiences for individuals. Coordinating care plays a critical part in attaining patient-centricity by enabling smooth and efficient interaction and partnership among healthcare professionals, patients, and their families. Efficient care coordination advances patient-centered care by guaranteeing that all aspects of a patient's medical journey are interconnected and tailored to address their specific requirements. This connectivity and personalization can be accomplished through various approaches such as evidence-based practice, electronic medical records utilization, promotion of patient safety, cross-functional teamwork, mutual communication, goal alignment, and shared accountability. Achieving efficient care coordination for cardiac surgery involves assembling a varied team that collaborates to develop personalized treatment plans for every patient. This approach ensures the smooth coordination of all elements of patient care, including pre-operative assessments, surgical procedures, and post-surgery rehabilitation plans. Prioritizing the significance of coordinated care in hospital administration can assist policymakers and healthcare systems in striving to deliver tailored, patient-focused care that contributes to enhanced health results for individuals. Through enhancing communication and teamwork, distributing tasks equitably, and advancing evidence-based practices among medical professionals, the overall coordination of care in cardiac surgery can be notably enhanced resulting in improved personalized patient care and ultimately better health outcomes.

Abstract

Tuberculosis is a major global health threat. Tuberculosis patients frequently suffer hypoalbuminemia. Protein-rich foods like Bloso fish (Glossogobius giuris sp.) can improve patient outcomes by elevating albumin levels and nutritional status. This study aims to evaluate the impact of Bloso biscuit on albumin levels in Tuberculosis patients. This study used a cross-over randomized controlled trial design. Sixty participants were divided into two groups: the treatment group consuming Bloso biscuits and the control group consuming non-Bloso biscuits. Each group was monitored for four weeks, and albumin levels were checked before and after biscuit administration. The formula consists of 15% bloso fish flour, 35% low protein flour, 20% margarine, 7% butter, 10% cheese, 8% egg yolk, 3% cornstarch, and 1% sugar. The study statistically analyzed the albumin level in Tuberculosis patients. This study found that most patients were female (55%) and aged 26-35 (41.7%). The baseline albumin levels in both groups were 4.5 g/dL. The average albumin level in the treatment group increased to 5.07 mg/dL. Based on statistical tests, Bloso fish biscuits significantly increased albumin levels in the treatment group (p<0.05). We concluded that giving Bloso biscuits can increase albumin levels in Tuberculosis patients.
