Abstract

Diabetes mellitus is a disorder in the metabolism of carbohydrates, lipids and proteins, which is characterized by hyperglycemia. Its prevalence in the world is increasing, espe- cially type 2 diabetes, causes an increased incidence of morbidity and mortality throughout the world due to complications of hyperglycemia. Hyperglycemia stimulates the release of superoxide (O2-) at the level of mitochondria which is the trigger early onset of oxidative stress in patients with diabetes mellitus type 2. Therefore one of the therapeu- tic modalities that can be developed in type 2 diabetes mellitus is a substance as potent antioxidant. Geraniin of rambutan peel extract is known to have the ability as a potent antioxidant and radical scavenging, so it is possible to be a prime candidate antidiabetes in the ype 2 diabetes melitus. We searched Cochrane Library data bases, Google Scholar, Medline and PubMed, from 2006 to 2016, to assess the effect of rambutan peel extract for type 2 diabetes melitus. We obtained 17 articles in accordance with the inclusion criteria. In the peel of rambutan (Nephelium lappaceum), geraniin is the main phenolic compounds that have the potential ability as an antioxidant and antihyperglycemic through inhibition of alfa-glucosidase and alfa-amylase, preventing the polyol (aldol reductase inhibition) and prevents the formation of advanced glicosilation products (AGEs). Thereis a very strong correlation between the ability as an antioxidant with the ability to inhibit alfa-glucosidase and alfa-amylase, so geraniin of rambutan peel extract (Nephelium lappaceum) expressed as the ideal candidate for the management of hyperglycemia in individuals with diabetes.

Abstract

Business competition is now entering the era of high competition. The company has always tried to maintain maximum customer who has achieved. Various methods through excellent service, competitive prices, indirect aspects such as brand image with the purpose of providing satisfaction to customers through the services or products offered. This research is explanatory or confirmatory or testing the hypothesis that explains the influence or causal relationships between variables through hypothesis testing. This study describes the relationship between the exogenous variables (quality of service), endogenous intervening (satisfaction), and the endogenous variables bound (loyalty). The relationship between these variables is described in the structural relationships directly or indirectly, partially or simultaneously. The study population includes outpatient clinics oral surgery, dental conservation, prosthodontics, perio and oral medicine at the Hospital University of Jember. The sample size of 120 people were taken to the cluster and purposive technique. This study using path analysis or flow charts. Then make equation modeling and analysis include: 1) designing a model based on the theory; 2) prepare the path diagram; 3) change the path diagram into a structural equation; 4) selecting an input matrix for data analysis; 5) assess the model identification; 6) evaluating the model estimation; 7) the interpretation of the model. There is a direct effect of service quality on satisfaction (0.215). The direct effect of service quality on loyalty (0.290). The direct effect of satisfaction on loyalty (0.636). There is no direct influenceon the quality of service loyalty (0,447). There are a total effect of service quality on satisfaction (0.703). Effect of total quality of service loyalty (0.736). Effect of total satisfaction on loyalty (0.636). Direct effect on the quality of service satisfaction, andsatisfaction effect on patient loyalty Dental Hospital of the University of Jember. Abstract

Hypoglycemia is an acute complication of diabetes mellitus which frequently occur repeatedly identified by blood glucose level less than 70 mg/dl. Diabetic patients experience dilemmatic situation where they are required to obtain diabetes drug therapy tocontrol blood sugar level remain normal, but facing another concerns about the side effects of treatment which can lead to complication of hypoglycemia. This condition will affect the psychological fearness of repeated attacks that result a traumatic feelings in diabetic patients. The study aimed to gain in depth understanding of diabetic patients experiences who had experienced of hypoglycemia episodes in Persadia Depok. This was a qualitative research which applied phenomenology method and involved six participants with diabetes mellitus experienced hypoglycemia episodes. Data were collected using in-dept hinterview and field notes. A Colaizzi's content analysis method was operated across data andrevealed 6 themes: decline in physical function while in response to hypoglycemia, traumatic feelings when experiencing hypoglycemia, participants' understanding that caused of hypoglycemia, awareness of hypoglycemia prevention, internal beliefs became the main source of coping strategy to face hypoglycemia and nursing care needs. The results of this study suggested a need of improvement innursing education for patients with hypoglycemia.

Abstract

Post-operative patients with general anesthesia are highly risk of mucus hypersecretion and respiratory distress as the effect of anesthesia caused by secretions accumulation in oropharynx. One of nursing interventions to decrease accumulation of mucus is by oral hygiene. The purpose of this study was to determine the differences of using chlorhexidine 0.2% and povidone iodine 1% as oral decontamination to the colonization of Staphylococcus aureus of post-operative patients in mawar ward at Dr. Abdoer Rahem Hospital Situbondo. This study was a quasy experimental research with non-equivalent control group design. The samples in this study were 20 samples divided into two groups 10 as the treatment group was given chlorhexidine 0.2% and 10 as the control group was given povidone iodine 1%. Data were analyzed by using Mann-Whitney. The result of Mann-Whitney test showed p value (0.490) > alpha (0.05). However, there was no difference between chlorhexidine 0.2% and povidone iodine 1% as oral decontamination for colonization of Staphylococcus aureus of post-operative patients in mawar ward at Dr. Abdoer Rahem Hospital Situbondo. The conclusion is chlorhexidine 0.2% and povidone iodine 1% can be used as oral decontamination for post-operative patients with general anesthesia.

Abstract

Background: The toddler period represents a critical window for social-emotional development, which serves as the foundation for social interaction and emotional expression. Nurturing care practices are recognized as essential contributors to optimal child development. However, evidence regarding the relationship between nurturing care and social-emotional outcomes in acutely ill children remains limited. Objective: This study examined nurturing care practices and social-emotional development in acutely ill toddlers, explored their relationship, and assessed potential confounding factors. Methods: A descriptive-analytical cross-sectional study was conducted from November 2024 to January 2025 at Banguntapan II Health Center, Bantul Regency, Indonesia. Sixty-four mother-child dyads were recruited through consecutive sampling. Mothers of acutely ill toddlers (aged 15-39 months) completed validated questionnaires assessing nurturing care practices and child social-emotional development using the Ages and Stages Questionnaire: Social-Emotional (ASQ:SE). Chi-square tests examined associations between variables. Results: Among participating mothers, 57.8% demonstrated good nurturing care practices. Social-emotional development screening revealed that 59.4% of children were not at risk for developmental concerns. A significant association was found between nurturing care practices and children's social-emotional development (p=0.038; OR=0.338, 95% CI: 0.120-0.954). Maternal physical health was significantly associated with nurturing care practices (p=0.018), while maternal depression, education, and family income showed no significant associations with either nurturing care practices (p=0.180-0.395) or social-emotional development (p=0.115-0.705). Conclusion: Good nurturing care practices were significantly associated with reduced risk of social-emotional developmental concerns in acutely ill toddlers. Maternal physical health emerged as a significant factor influencing caregiving practices. These findings underscore the importance of supporting maternal health and promoting comprehensive nurturing care strategies, even during episodes of acute childhood illness, to optimize social-emotional development outcomes.

Abstract

Background: Community nurses play a collaborative role in developing holistic complementary therapies for smoking cessation. Lemongrass (Cymbopogon citratus) essential oil, known for its anxiolytic properties, was formulated into a herbal inhaler as a candidate Nicotine Replacement Therapy (NRT) to complement psychological and behavioral interventions for smokers.Objective: This study aimed to evaluate smokers' acceptance of the lemongrass-based herbal inhaler as a potential NRT. Methods: Threeinhaler formulations were developed, with lemongrass essential oil as the active ingredient. Stability testing was tested among 90 participants over a 4-week storage period, assessing homogeneity and power of Hydrogen (pH) levels. Results: Results indicated no significant differences in homogeneity or pH across the storage period, with p-values at week 0, 1, 2, 3, and 4 being 0.716, 0.754, 0.771, 0.710, and 0.674, respectively (p> 0.05). Conclusion: The findings demonstrate that the lemongrass essential oil herbal inhaler exhibits favorable physical stability and is well-accepted by smokers as a smoking cessation therapy.

Abstract

Background: Spiritual beliefs in schizophrenia can be both positive, negative. Numerous individuals with schizophrenia engage spirituality as a central coping resource to manage stress, adversity throughout recovery process. Nevertheless, a subset of patients exhibits maladaptive or pathological spiritual beliefs, which may predispose them to employ negative spiritual coping strategies, thereby impeding their overall recovery. Objective: Aimed to explore role of spirituality in recovery from schizophrenia. Methods: Systematic review followed criteria from Centre for Review and Dissemination and Joanna Briggs Institute, as well as PRISMA, to assess quality of included studies. A comprehensive search across 5 databases: Scopus, Web of Science, ProQuest, Science Direct, SpringerLink was conducted to identify relevant articles. A total of 623 articles were screened, with inclusion criteria: studies comprising schizophrenia patients; related to spiritual experiences, descriptions from first-person perspective; journal publications from 2020 to 2025; qualitative or quantitative studies; published in English. 15 articles met inclusion criteria and included in final review. Results: Identified 3 main findings: 1) positive and negative experiences of spirituality in schizophrenia; 2) spiritual coping related to recovery of schizophrenia; 3) relationship between spirituality and recovery of schizophrenia. Conclusion: Spirituality plays a vital role in helping individuals with schizophrenia enhance their self-confidence, self-control, strength,hope, which contributes to their recovery.
ANALYZING BIOPSYCHOSOCIAL FACTORS INFLUENCING SELF-MANAGEMENTBEHAVIORS OF PATIENTS UNDERGOING HEMODIALYSIS

Abstract

Background:Continuous hemodialysis is a lifelong treatment, causing medical, psychological, social, and economic consequences for patient. Effective self-management is a critical strategy to improve patient outcomes, as it empowers individuals to actively participate in their care. Strong selfmanagement is directly linked to better adherence, fewer treatment-related complications, an enhanced quality of life by giving patients greater control over their condition. Objective: Aimed to investigate biopsychosocial factors that influence hemodialysis patients' self-management practices. Methods: This study utilized a cross-sectional, observational analytical design. Psychological factors were self-efficacy, depression, social factors were family support. Data were collected using a questionnaire and analyzed to find associations with self-management behavior. Chi-square test was used for categorical biological factors, while the Spearman's rank test was used to analyze relationship with ordinal variables of family support, self-efficacy, depression. Results: There is a correlation between self-management behavior with education level (p=0.035), occupation (p=0.031), marital status (p=0.048), family support (p=0.039), depression (p=0.001), and self-efficacy (p=0.001). Self-management behavior is unrelated to age (p=0.538), gender (p=0.273), length of illness (p=0.529), length of hemodialysis (p=0.529), complications (p=0.675), and comorbidities (p=0.694). Conclusion: There is a relationship betweeneducation level, occupation, marital status, family support, depression, selfefficacy with self-management behavior in CKD patients undergoing hemodialysis at KRMT Wongsonegoro Hospital, Semarang.
