Abstract

Abstrak Tuberkulosis (TB) masih menjadi masalah kesehatan global dengan sekitar 8,2 juta kasus baru setiap tahun. Strategi End TB WHO menargetkan penghentian epidemi TB pada 2035 dengan tingkat keberhasilan pengobatan ≥90%. Kabupaten Probolinggo menempati urutan kedua kasus TB tertinggi di Jawa Timur. Hingga 18 Maret 2025, dari target 16.466 terduga, ditemukan 3.532 kasus dengan 713 pasien positif (21%). Namun, hanya 638 (89%) yang terdaftar memulai terapi, dan capaian pengobatan tetap 21%. Penelitian ini bertujuan menganalisis pengaruh cognitive behavioral therapy (CBT) terhadap kepatuhan dan kesadaran diri pasien TB kategori “loss to follow-up” (LTFU). Desain yang digunakan adalah metode campuran (sequential exploratory). Tahap pertama berupa penelitian kualitatif fenomenologi untuk menggali fenomena perilaku pasien TB yang tidak berobat melalui FGD dan wawancara yang dianalisis tematis. Tahap kedua merupakan pra-eksperimen dengan desain one-group pre-post test pada 40 pasien LTFU dari populasi 45 orang, dipilih secara purposive sampling. Intervensi berupa konseling oleh pakar keperawatan jiwa selama 4 minggu (1 kali/minggu, 6 sesi, 30–40 menit). Hasil menunjukkan pemahaman dan pengalaman dalam pengobatan TB, pasien merasa malu dan khawatir terhadap lamanya pengobatan. CBT terbukti meningkatkan kepatuhan dan kesadaran diri, sehingga direkomendasikan diterapkan pada pasien maupun keluarga. The WHO End TB Strategy aims to eliminate the epidemic by 2035, with a recommended treatment success rate of ≥90%. Probolinggo Regency ranks second in East Java for TB incidence. As of March 18, 2025, 3,532 suspected cases were identified from a target of 16,466, with 713 confirmed cases (21%). However, only 638 (89%) initiated treatment, resulting in an overall treatment success of 21%. This study examined the effectiveness of cognitive behavioral therapy (CBT) in improving adherence and self-awareness among TB patients categorized as “loss to follow-up” (LTFU). A mixed-method sequential exploratory design was employed. The qualitative phase used a phenomenological approach through focus group discussions and interviews, analyzed thematically. The quantitative phase applied a pre-experimental one-group pre-post test involving 40 purposively selected LTFU patients. Counseling sessions were delivered by psychiatric nursing experts over four weeks (once weekly, six sessions, 30–40 minutes). Results revealed that CBT interventions enhanced understanding, reduced anxiety, and significantly improved adherence and self-awareness, suggesting its applicability for both patients and families. Abstract

Abstrak Pemberian Air susu ibu (ASI) eksklusif merupakan salah satu bagian dari program prioritas kesehatan di Indonesia. Pemberian ASI eksklusif adalah pemberian ASI sebagai sumber nutrisi satu-satunya bagi bayi sampai usia 6 bulan pertama kehidupan tanpa adanya tambahan makanan dan minuman lainnya kecuali obat. Namun, sampai saat ini masih ditemukan ibu yang tidak memberikan ASI secara eksklusif, dengan alasan ibu merasa ASInya tidak cukup untuk memenuhi kebutuhan bayi. Kurangnya keyakinan diri ibu menyusui juga menjadi salah satu faktor yang penting untuk digali kaitannya dengan kegagalan pemberian ASI eksklusif. Tujuan penelitian ini dilakukan untuk mengetahui efektifitas edukasi supportif terhadap self efficacy ibu post partum dalam pemberian asi eksklusif. Metode penelitian ini menggunakan desain pra-eksperimental dengan menggunakan metode one-group pra-post test design, Populasi dalam penelitian ini adalah seluruh ibu post partum di wilayah Puskesmas Pajarakan sejumlah 30 responden. Tehnik sampling yang digunakan yaitu total sampling sampling. Tehnik pengumpulan data menggunakan kuisioner. Analisis menggunakan uji Paired simple T-Test. Hasil menunjukan bahwa self efficacy ibu post partum dalam pemberian asi eksklusif sebelum dan sesudah diberikan edukasi suportif  menunjukkan adanya pengaruh yang bermakna dimana P= 0,000 lebih kecil dari α=0,005. Sehingga edukasi supportif dapat dijadikan sebagai solusi untuk meningkatkan kepercayaan diri ibu dalam pemberian ASI eksklusif. Kata kunci: edukasi supportif, self efficacy, post partum, ASI eksklusif   Abstract Exclusive breastfeeding is a national health priority program in Indonesia. It refers to providing only breast milk as the sole source of nutrition for infants during the first six months of age, without additional food or drink except for medication. However, many mothers still fail to practice exclusive breastfeeding, often due to concerns about insufficient milk supply. Low maternal self-efficacy is also a key factor contributing to this problem. This study aimed to evaluate the effectiveness of supportive education on the self-efficacy of postpartum mothers in exclusive breastfeeding. A pre-experimental design with a one-group pre-test post-test approach was applied. This study population consisted of all postpartum mothers in the working area of Pajarakan Public Health Center, with 30 respondents selected through total sampling. Data were collected using a validated questionnaire and analyzed using paired sample t-test. Results showed a significant improvement in maternal self-efficacy after supportive education, with p = 0.000 (p < 0.005). These findings suggest that supportive education is effective in enhancing the confidence of postpartum mothers to practice exclusive breastfeeding. Integration of this intervention into maternal and child health services is highly recommended. Keywords: supportive education, self-efficacy, postpartum, exclusive breastfeeding
