Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Watermelon rind water extract contain of citrulline. Citrulline is a precursor of generating Nitric Oxide (NO). NO possibly reduced the level of blood glucose in streptozotosin-induced diabetic rats.This study was a laboratory experimental study completely randomized design. A number of 30 male albino Wistar rats weighting 100 – 200 gr were devided into six groups (N=5), with group 1 served as normal control group. The other groups were given streptozotosin to induced diabetes mellitus. The rats in control diabetic group were given CMC 0,5%, the others were given watermelon rind extracs for 8 days with different doses for each group. The results showed that serum glucose levels in posttest control group was significantly different, from that group of the watermelon rind extract 250 mg/kg bw/day (p = 0,000), groups of watermelon rind extract 500 mg/kgbw/day (p = 0,000) and groups of watermelon rind extract 1000 mg/kgbw/day (p = 0,000). The level of triglyceride in postest control was significantly different from that group of the watermelon rind extract 250 mg/kg bw/day (p = 0,000), groups of watermelon rind extract 500 mg/kgbw/day (p = 0,000) and groups of watermelon rind extract 1000 mg/kgbw/day (p = 0,000). The conclusion, the watermelon rind water extrac can be used to reduced glucose serum level (250, 500, 1000mg/kg bw) with the optimal dose was 500 mg /kgbw/day with the optimal dose was 1000 mg /kgbw/day.

Abstract

Hirschsprung disease is a rare developmental disorder of the enteric nervous system. This researched purpose to give an epidemiological database and current trend analysis from radiological perspective of Hirschsprung disease at our center. A prospective descriptive study was conducted for a year in 2017 at Sanglah Public General Hospital Bali. Colon in loop examination using either water-soluble or barium was conducted. All baseline characteristics, contras media used, and HD type were the parameter being evaluated in this study. Hirschsprung accounted for 2.16% from all 1018 newborn. Male is 3.4 times more prone to Hirschsprung disease. Age distribution predominant during 0 – 4 weeks after birth (36.4%). Ultrashort, short, long segments were 45.5%, 45.5%, and 9% respectively. Contras media used was mainly barium rather than water soluble 8:3, due to standard operating procedure adapted by some pediatric surgeon. Ultrashort and short segment HD has the same contribution. Barium is dominated at our center although water soluble is chosen in most literature for infant, due to its ability as diagnostic as well as therapeutic. Hence further research should be done to support and change the perspective in making the diagnosis.

Abstract

Highlight: • Dermoscopy is a practical, non-invasive and accurate method for reducing the number of unnecessary excision of benign skin neoplasms. • Knowledge about vascular pattern and description setting can lead to a prompt benign skin neoplasm diagnosis. Abstract: Benign skin neoplasms are commonly found in the population. It has a well-differentiated and slow growth nature. The patients often come seeking treatment when the tumor has developed into malignancy. This usual delay in diagnosis and therapy frequently happens because early-stage mass has not generated any complaints by the patients. Detection and monitoring of benign skin neoplasms can be carried out earlier and more effectively if the clinician or dermatologist has the knowledge of distinguishing benign from malignant lesions. The histopathological examination can help to establish the diagnosis, but this method is invasive and requires an extended amount of time. Dermoscopy is a practical, non-invasive and accurate method for early detection of skin disorder which reduces the number of unnecessary excisions of benign skin neoplasms. Knowledge of the vascular pattern and arrangement description, combined with the additional dermoscopic feature can lead to the prompt diagnosis of benign skin neoplasms.

Abstract

Introduction: Management of low vision in pediatric patients with greater result can affect their quality of life than in adult low vision patients.. Retinopathy of Prematurity is the main cause of blindness in children. A case study highlights the tertiary prevention (low vision examination and management) in a 5 year old boy with ROP related blindness to optimize his remaining visual capacity by using optical and non optical aids that support daily activity and educational learning. Case Report: A 5 year old boy with ROP related blindness visited to the low vision clinic after receiving the several treatments for ROP before. His visual acuities were light perception in the right eye and hand movement in the left eye using his current spectacle correction. He had anti VEGF bevacizumab intravitreal injection, vitrectomy and complicated cataract extraction history on the left eye. He wasdifficult to learn and read numbers and alphabets caused by visual impairment. Discussion : Upon evaluation by assessment low vision method, his distance visual acuity in the left eye was improved significantly up to 3/40 on LEA Acuity chart with +14.00D. His near vision became 10 M at 30 cm of reading distance (without near correction) and 4 M at 13 cm improved with +3.00 D by spectacle. The non optical aid devices as reading task were also suggested to improove continuous reading text without difficulty. Conclusion: ROP blindness can be preventable if appropriate, adequate and accessible screening programmes are available. Unfortunately, even with the current screening guidelines and recommended treatment of ROP, many babies suffer from blindness by this disease each year. Low vision assessment and management in pediatric patient with visual impairmentaccording to children's need can optimize the recent visual capability of low vision patient and give self care daily activity and educational learning.

Abstract

Highlight: • The management of penetrating ocular injury with IOFB and traumatic cataracts needs a thorough examination of the mechanism of injury, location, size, and composition of IOFB. • Endophthalmitis, retinal detachment, and development of PVR are potentially vision-threatment. Abstract: Most IOFB are metallic and found in males of productive age as a consequence of work-related accidents. A 45-year-old man complained of sudden blurred vision in the left eye (3/60 pinhole 5/12) after getting hit by a foreign body when cutting grass with a lawn mower. Anterior segment examination revealed a 10 mm long, one-plane, straight, full thickness, already sutured inferonasal corneal laceration, inferonasal traumatic iridectomy size 3x7 mm, and opaque lens. Head CT-scan revealed opacity with metallic density intraocularly. Ultrasonography revealed an echogenic lesion, particle-shaped with 100% RCS complex density, located at the inferonasal of the vitreous cavity. Focal laser photocoagulation was performed preoperatively because there was a tear at the superonasal of the retina. The patient underwent cataract extraction, intraocular lens implantation, vitrectomy, and IOFB extraction in a one-step procedure. IOFB was found at the inferonasal side of a vitreous cavity with size 3 x 1 mm, metallic, and not attached to the retina. Silicon oil tamponade was used as a precaution because there were retinal tears. Postoperatively, the left eye's visualacuity was 5/20. After 6 months, the silicon oil was evacuated and the visual acuity became 5/8.5.

Abstract

Obesity is one of the chronic diseases that may have significant impact and affect the quality of life, especially in elderly who already have multidimensional problems. This study aimed to provide information about the influence of obesity in elderly on every component of his quality of life by using the SF-36 quality of life. This study was conducted on 105 patients in outpatient Medical Rehabilitation, Geriatrics and Diabetes at Dr. Soetomo Hospital, Surabaya. Patients aged 60-75 years, consisting of 62 men and 43 women. The number of patients with obesity was 49 persons (46.7%) with an average body mass index (BMI) of 27.16 kg / m2. Whereas, non-obese patients were 56 persons (53.3%) with an average BMI of 21.23 kg/m2. In conclusion, there was no significant difference in the SF-36 life quality of obesity and non-obesity groups after being analyzed using t-test.

Abstract

Red dragon fruit (Hylocereus polyrhizus) peel contains anthocyanin, fiber and vitamin C, so it can be used to improve lipid profile in dyslipidemia. The peel of the dragon fruit is not durable, so in this study we used freeze-dried dragon fruit peel. The aim of this study was to prove that the administration of the red dragon fruit peel in a dose of 0.72 g/200 g BW, 1.08 g/200 g BW, and 1.44 g/200 g BW of rat per day for 28 days may improve lipid profile in male wistar strain white rats with high-fat diet. Lipid profiles were studied by examining of total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. This was a pure experimental study using randomized post test only control group design. This study used experimental animal of 30 male wistar strain white rats which were divided into five groups. Measurements of total, LDL, and HDL cholesterol levels were done by CHOD-PAP method, while triglyceride level was measured with GPO-PAP method. Data were analyzed statistically by One Way Anova test. The results of this study indicated that giving the peel of red dragon fruit as much as 0.72 g lowered total cholesterol levels, 1.08 g lowered total and LDL cholesterol levels, and 1.44 g lowered total cholesterol, LDL cholesterol, triglyceride levels, and raised HDL cholesterol level. Red dragon fruit peel can be used alternatively to improve lipid profile in dyslipidemia.

Abstract

Post anesthesia agitation is common problem in pediatric post anesthesia care unit. The incidences range from 10 to 80%. EA has been described as a dissociated state of consciousness in which the child is inconsolable, irritable, and uncooperative typically thrashing, crying, moaning or incoherent. This study was done to determine the incidence of emergence agitation and associated risk factors in pediatric patients who underwent general anesthesia. This descriptive and analytic study was performed on 105 pediatric patient aged 1-12 years that underwent general anesthesia for various elective diagnostic and surgeries at Dr. Soetomo Hospital between January and February 2016. The presence of emergence agitation was recorded using Pediatric Anesthesia Emergence Delirium (PAED) scale. The factors that linked with Emergence Agitation were recorded in a questionnaire. The data were analyzed using SPSS software with logistic regression. p - values less than 0.05 were considered as significant. Forty two (40%) children had Emergence Agitation. Preoperative anxiety (p = 0.006) and Pain (p=0.035) were associated with higher rates of post anesthetic emergence agitation. This study identified preoperative anxiety and pain as risk factors, which are associated with emergence agitation in children. To minimize the incidence of post anesthetic emergence agitation, these risk factors should be considered in the routine care by anesthetist.

Abstract

Recitation and relaxation are translated as stimuli that can turn disstraints into uneven conditions (balanced conditions). The purpose of this study was to explain the effect of remembrance and relaxation on improving stress perception and decreasing blood cortisol levels in patients with type 2 diabetes mellitus with Anti Diabetes Medication (OAD). This type of research is quasi-experimental with the non randomized control group pretest posttest design. The population in this study were all type 2 diabetes mellitus patients at the Internal Poly department of Syarifah Ambami Hospital Rato Ebu Bangkalan. The study sample was divided into 2 groups, namely control and intervention, each of which was 14 respondents. First, respondents were asked to fill out an informed consent form, identity, fill in the Depression Anciety Stress Scale 42 questionnaire (DASS 42) and examine cortisol levels as pre-treatment data. Then for 6 weeks the intervention group was given remembrance and relaxation treatment, while the control group was only given health education. After 6 weeks the second group respondents were asked to fill in DASS 42 and examined cortisol levels as data after treatment. Difference data before and after the treatment was tested for normality using Shapiro-Wilk and obtained normal data distribution so that the different tests used independent T test with a = 0.05. Recitation and relaxation can significantly improve stress perception with p value (0.001) <0.05 and reduce cortisol levels with p value (0.024) <0.05. Recitation and relaxation improve stress perception so that the hypothalamic pituitary adrenal (HPA) axis produces a balanced level of the hormone cortisol. Cortisol in a balanced level will improve various metabolic processes.

Abstract

Background: Longterm exposure to aminoglycoside such as kanamycin to cochlear cells is known to be associated with damage to outer hair cells in the organ Cortiand type 1 sensory hair cells in the vestibular organs which ultimately causes permanent damage to hair cells. Hair cell damage occurs from the basal cochlea (high frequency area) to the apex cochlea (low frequency area) and followed by damage from the auditory nerve. Evaluation of cochlear dysfunction on multidrug resistant tuberculosis (MDR TB) patients have been rarelyreported in the literature based on distortion product otoacoustic emission (DPOAE).Objectives: To prove cochlear dysfunction after kanamycin injection in MDR TB patient based on DPOAE examination of the overall frequencies and specific frequency. Methods: An observational longitudinal analytic with pre and post design without control group conducted in the infection division of MDR TB Outpatients Pulmonology Department and Otorhinolaryngology Community division of ORL-HNS Department Dr. Soetomo general hospital Surabaya, within 3 months in 2018, 15 ear with the best baseline examination were taken by consecutive sampling and statistical analysis for cochlear dysfunction based on overall frequency and specific frequency DPOAE examination using Mc Nemar test. Results: Based on DPOAE examination of overall frequencies there was no cochlear dysfunction (p > 0.05) but a significant difference was found at frequency of 10,000 Hertz (Hz) (p = 0.002 ). Conclusion:On ototoxicity monitoring there was no cochlear dysfunction after 4 weeks Kanamycin injection but cochlear dysfunction occurs at a specific frequency of 10,000 Hz.
