Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Infection of the central nervous system in pediatric patients have a high mortality as well as acute and chronic neurological sequelae. Signs of the disease are unclear, so cerebrospinal fluid (CSF) test is used as a gold standard for diagnosis, but the investigation has faced many obtacles. Empiric antibiotic therapy is the key factor in reducing morbidity and mortality. Microbiological culture result is obtained within 5-7 days. The effectiveness of empirical antibiotic use is questionable. Therefore, other investigations are conducted to determine the effectiveness of antibiotics by using one marker, the CRP. This study was to analyze CRP level in supporting antibiotic therapy effectiveness in pediatric patients with central nervous system (CNS) infections. A prospective cohort study was conducted to determine the relationship of CRP with other parameters, including clinical, microbiological and laboratory, in pediatric patients with central nervous system infections. Patients meeting blood samples criteria were taken before (H0), the third day (H3) and the fifth day (H5) after antibiotics administration. This study involved 10 patients with central nervous system infections (meningoencephalitis, encephalitis and encephalitis with cerebral edema). Six patients were male, with ages less than a year. Antibiotic treatment effectiveness was associated with improved condition of the patients' CRP level. It was 3.558 ±3.196 before (H0), 3.878±2.813 on the third day (H3) and 3.891±2.204 on the fifth day (H5) after antibiotic administration. Leukocyte levels were 13.680±1.660 before (H0), 17.832±7.213 on the third day (H5), and 10.546±3.671 on the fifth day (H5) after antibiotic administration. Pearson's correlation test analysis performed on CRP and WBC parameters showed H0 p=0.981, CRP and WBC H3 p=0.621, while CRP and WBC H5 obtained significance p=0.644. There was no significant correlation observed between CRP and WBC parameters before and after antibiotic administration. In conclusion, there was no correlation of CRP levels with clinical, laboratory and micobiological parameters in patients with central nervous system infections.

Abstract

The purpose of this study was to determine the effects of DLBS1033 on blood pressure reduction in hypertension--induced wistar strain rats. Design used in this study was true experimental design. The study was conducted at Biomedical Laboratory, Physiology, Faculty of Dentistry, Jember University, and Clinical and Community Pharmacy Laboratory, Pharmacy Faculty, Jember University. Subjects were taken from male Wistar rats aged 2-3 months, weighing 150-200 grams, as many as 22 rats. Intervention samples were induced into hypertension by giving prednisone in a dose of 1.5 mg/kg/day orally and 2% NaCl followed DLBS1033 of 0.5 grams/kgbw/day orally in the treatment group. T test results on systolic blood pressure (p = 0.00), which was <0.05, showing significant differences in systolic blood pressure in treatment groups. It can be concluded that DLBS1033 can lower systolic blood pressure in wistar rats with induced hypertension using prednisone 1.5 mg/kgbw/day and NaCl 2%.

Abstract

Industry that uses technology nowadays, beside having positive impacts, also having negative impacts such as adverse effects on workers and work environment. Every person who works will have workload and responsibility on their work. This will affect health of the workers, including their blood pressure. The purpose of this study was to study the relationship between work load and work period with blood pressure among workers in ship construction company, PT. X. This study was an observational descriptive study, using cross sectional design. Total population involved 41 individuals, taken from the respondents who met the inclusion criteria. Data collection included pulse and blood pressure measurements. Data analysis used in this study was Pearson and Spearman correlation tests. The result showed that there was a level of correlation between blood pressure and age (0.478), blood pressure and work period (0.432), blood pressure and work load (0.448). The conclusions of this study was that blood pressure related to age, nutritional status, work period, work load and noise intensity. PT X needs to pay attention to the health of the workers, such as blood pressure examination, and provide socialization on nutritious foods that are appropriate to the work performed.

Abstract

Highlights: 1. This study investigated the etiology of paralytic strabismus, a condition that requires further research to prevent its incidence, particularly in adults. 2. The findings of this study provided valuable insight into the primary etiology of paralytic strabismus and its underlying factors. Abstract Paralytic strabismus is the most common cause of adult-onset strabismus. It can result from paralysis of the third, fourth, or sixth cranial nerves, either separately or in combination, which are responsible for innervating the ocular motor muscles. Paralysis of these cranial nerves occurs due to congenital disorders, neoplasms, trauma, intracranial processes, idiopathic diseases, and other causes. This study aimed to determine the etiology of paralytic strabismus incidence at the Department of Ophthalmology of Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, between 2017 and 2022. This study employed a retrospective methodology and utilized total sampling. The medical records of patients diagnosed with paralytic strabismus were used to collect secondary data. Out of the total of 282 cases, the most common condition was combined ocular motor cranial nerve palsy, which accounted for 102 (36.2%) cases. The main factors contributing to this condition were neoplasms (37.2%), intracranial processes (25.5%), trauma (25.5%), other causes (8.8%), idiopathic diseases (2%), and congenital disorders (1%). The incidence of third cranial nerve palsy and sixth cranial nerve palsy was equally prevalent, each accounting for 86 (30.5%) cases. Meanwhile, fourth cranial nerve palsy was the least frequent with just eight (2.8%) cases. This study concluded that the most frequent etiology of paralytic strabismus was combined ocular motor nerve paralysis, which primarily results from neoplasms.

Abstract

This study aimed to identify human DNA from mixing human and chicken blood samples by utilizing Polymerase Chain Reaction (PCR) and cytochrome b gene primer. The cytochrome b gene is a gene located in mitochondrial DNA and has high variation of sequence relation between one species and another. PCR analysis was performed using human cytochrome b gene primer in variation of DNA templates (0 ng, 0.01 ng, 0.1 ng, 1 ng, 10 ng and 100 ng), human blood percentages (100%, 50%, 40 %, 25%, 10%, 5%, 1%, 0%) and sample age before analysis (0 day, 3 days, 7 days, 10 days, and 15 days). The minimum DNA template obtained in this study was 0.01 ng and minimum percentage of human blood in the mixture was 1%. Blood spots on cloth isolated on days 0, 3, 7, 10 and 15 could still be analyzed and the resulting of DNA band (157 bp) had the same intensity/thickness. From the results of this study, it can be concluded that human blood in the mixture of human and chicken blood can be identified using PCR with specific primers of cytochrome b gene. PCR using specific primer of cytochrome b gene may help forensic practitioners to identify human sample in mixed biological samples.

Abstract

Caesarean section is the delivery through a surgical incision in the abdomen and uterus with various risks, such as Surgical Site Infection (SSI) which either occurs rapidly (24-48 hours postoperatively) or delayed. To reduce the risk of various post-cesarean section infections, prophylactic antibiotics are given. The administration of prophylactic antibiotics with recommended regimens, such as in developed countries as in the United States and Europe, with first generation of cephalosporins has not been applied in Dr. Iskak Hospital, Tulungagung. This hospital still used other types of antibiotics, such as cefotaxime or ceftriaxone. In addition, for several reasons, prophylactic antibiotics were continued with other antibiotics to reduce the risk of delayed infection. This study was designed to analyze the effectiveness of prophylactic antibiotics as recommended in cesarean section. This was a prospective observational study with consecutive sampling type, examined 41 mothers with cesarean section at the Department of Obstretics and Gynecology, Dr. Iskak Hospital, Tulungagung, Indonesia. This study was conducted from May to July, 2017. The results of the study showed that the effectiveness of prophylactic antibiotic use was 98% based on the clinical parameters and maternal laboratory outcome, which showed that SIRS was in normal category. Whereas, there was one mother (2%) who had SSI on day 10 postoperatively.

Highlights: 1. This study, conducted at a tertiary hospital, was the first to analyze the correlation between thyroid status and clinical activity score in thyroid eye disease. 2. This study offers valuable information regarding the crucial role of thyroid stimulating hormone (TSH) in determining the clinical activity score of thyroid eye disease. Abstract Thyroid eye disease is an autoimmune disorder characterized by inflammation of the orbital and periorbital tissues. This condition can lead to vision impairment, which is a notable manifestation of thyroid disease. The symptoms of thyroid disease reflected the level of inflammatory activity, whereas the clinical activity score was indicative of active and inactive phases. This retrospective analysis explored the intricate association between thyroid status and clinical activity score in thyroid eye disease patients, with the hope of providing a foundation for further research on the association between thyroid status and the occurrence of TED. This study utilized medical records from the years 2019 to 2022 from the Outpatient Department of Ophthalmic Oncology of Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. The variables examined in this study were thyroid status and the clinical activity score of thyroid eye disease. The statistical analysis was performed using Phi and Cramer's V as well as Spearman's correlation test (p<0.05). A total of 88 patients were diagnosed with thyroid eye disease during the study period. Thyroid eye disease primarily affected female patients (58%) and those who had a mean age of ≥41 years (58%). The Phi and Cramer's V analyses indicated no significant association (p > 0.05) between thyroid status and clinical activity score in thyroid eye disease patients. Intriguingly, the results exhibited the presence of euthyroidism as well as both overt and subclinical hypothyroidism and hyperthyroidism, regardless of the clinical activity score. Additionally, the Spearman test that incorporated the levels of thyroid hormones, i.e., triiodothyronine (T3), thyroxine (T4), thyroid stimulating hormone (TSH), and free thyroxine (FT4), showed a significant negative correlation (p<0.05) between TSH and clinical activity score. In conclusion, THS plays a crucial role in determining the clinical activity score of thyroid eye disease patients. This study underscores the imperative for further research to comprehensively elucidate the intricate correlation between TSH and clinical activity score.

Highlights: • Patients with IRSS I stage have the longest lag time than IVB stage. • Lag time is no significant with stage retinoblastoma. Abstract: Retinoblastoma is a rare neoplasm disease that occurs in children, generally under the age of two. Retinoblastoma is more prevalent in developing countries and is often associated with a late diagnosis. Such delays can lead to a poor prognosis. The time from the appearance of symptoms of retinoblastoma (onset) to the time of diagnosis is called lag time. Early diagnosis of retinoblastoma by paying attention to factors such as age, clinical symptoms, and laterality can help improve retinoblastoma survival rates, especially in developing countries. The purpose of this study was to analyze the relationship between the lag time to the stage of retinoblastoma patients at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. This study was a retrospective analytical observational study using secondary data from retinoblastoma patients at the Ophthalmology Outpatient Unit at Dr. Soetomo General Academic Hospital, Surabaya, from January 2014 to December 2018. The data were analyzed using Kendall's tau-C test. No significant correlation was found between lag time and stage (p = 0.339). Patients with International Retinoblastoma Staging System (IRSS) I stage had the longest lag time (on average 28 months), and patients with stage IVB had the shortest lag time (on average four months). There was no correlation between lag time and retinoblastoma stage. However, there was a trend of patients with early stages delaying hospital visits, while patients with advanced stages in earlier to the hospital.

Abstract

Head and neck reconstruction serves two purposes, namely to restore function and aesthetics. The priority goals for lip reconstruction should be determined before surgery. In cases of large lip defects, maintaining the goal makes lip reconstruction difficult. This is a case report of a 66 year old man who came to ORL-HNS OPD, Dr. Soetomo Hospital, Surabaya with the main complaint of ulcers on the upper lip since one year ago accompanied by pain. A mass biopsy result on the right vestibule in Malang on October 11, 2016 showed adenosquamous cell carcinoma. The patient planned to have a wide excision and reconstruction of the lip defect. Wide excision and lip reconstruction was performed on 15 September 2016. Total resection of the upper lip was performed up to the vestibule of the nose. The results showed that squamous cell carcinoma was poorly differentiated. The evaluation after myocutaneous flap surgery was well combined, there were no signs of inflammation or infection. Patients can open their mouths, eat, drink and speak without complaint. The patient then received regular chemoradiotherapy two weeks after surgery.
