Abstract

Background: Ultraviolet B (UVBI may cause DNA damage due to photooxydative reaction and form 8hydroxy-2'-deoxyguanosine (8-0HdGI as a genotoxic product. Topically extra virgin olive oil (EVOOI can decrease 8-0HdG formation among UVB irradiated-hairless mice epidermal cells. Such effect in human skin has never been discovered.Objective: To know the effect of topically EVOO in UVB induced-8-0HdG formation in human epidermal cells.Method: A simple experiment was performed on cultured human foreskins. The foreskins were divided into 3 groups: group was treated with 100mJ/cm2 UVB + topical EVOO, group B with 100mJ/cm2 UVB, and group C as control. All skin was cultured in completed DMEM and fixation was performed at one and 24 hours. 8-0HdG expression was determined immunohistochemically with antibody anti 8-0HdG and measured based on histogram of selected colour of Adobe photoshop.Results: The mean of 8-0HdG expression in experimental groups: (8.02:t 13.81, (26.95:t 23.231. and 14.96:t 7.141, respectively (p =0.0001 at an hour; (14.91 :t30.63I, (37.58:t 25.631. and (8.76:t 10.941 respectively (p = 0.0001 at 24 hours.Conclusion: Extra virgin olive oil was proved to reduce the 8-0HdG formation in exposed human skin-UVB. and may be developed to be anti-photocarcinogenesis active ingredient topically.Kata kunci:Extra virgin olive oil, UVB, 8-OHdG, human Skin

Abstract

Genetic counselling is a relatively new medical activity. There.are some conditions that usually need genetic counselling.•The classification of genetic disorders, their mode of inheritance and the method of establishing the diagnosis must be well understood before giving the counselling. The mode of inheritance of genic disorders and some difficulties in the diagnosis need special attention. Chromosomic disorders and embryopathies have been shortly explained ,as well.Finally, a short explanation of prenatal diagnosis, carrier detection and some methods of treating genetics disorders have been proposed so that genetic counselling can be given effectively.Key Words: genetic counselling - genetic disorders - genic disorders - prenatal diagnosis - carrier detection.

Abstract

Background: Misoprostol is now widely used as an agent for the induction of labor, although the effectiveness of its use in premature rupture of the membrane is questionable.Objectives: To compare the effectiveness of vaginal versus oral administration of misoprostol for induction of labor in term pregnancy with premature rupture of the membranes.Study Design: A retrospective cohortMaterials and Method: The study was conducted at Dr. Sardjito General Hospital Yogyakarta from January 1999 to November 2001. Data were taken from medical record. The inclusion criteria were gestational age of 37 weeks not yet in labor with premature rupture of membranes, singleton gestation, cephalic presentation, reactive fetal heart pattern, and Bishop score s 4. The pregnancy that is complicated by diseases were excluded. Vaginal administration of misoprostol was considered as the exposed group while oral administration was as control. Misoprostol 50 mg had been at six hours interval, for maximal dose of 200 pg. Variable outcomes were effectiveness, duration of induction, mode of delivery, side effects, and newborn asphyxia. Data were processed with SPSS for Window version 10.0, t-Test, chi-square test, relative risk, and binary logistic regression were used for statistical analysis.Result: During the period of three years, there had been 249 cases of premature rupture of membranes that met the inclusion and exclusion criteria consisting of 88 cases exposed group and 88 cases of control. The success rate of the vaginal misoprostol was 80.70% compared to 87.50% in the oral misoprostol with p > 0.05. The cesarean rate from vaginal misoprostol and oral misoprostol were 8.00% versus 3.40% respectively (RR 2.23 and 95% CI 0.62-8.73), the rate of asphyxia in the newborn was 29.50% versus 26.10% (RR 1.13 and 95% CI 0.70-1.82). The rate of side effect, hyperstimulation, was 5.70% versus 1.10% (RR 5.00 and 95% CI 0.60-41.93) in the vaginal group. The length of induction vaginal and oral misoprostol were 10.00 ± 4.86 and 9.76 ± 4.56 hours. There was one case of cesarean section followed with hysterectomy due to urine atony in the vaginal group.Conclusion: The success rate of vaginal misoprostol induction was slightly lower than oral, but it was not statistically significant, as well as the incidence of the first minute of asphyxia. The interval between the administration of drug to complete dilatation in the vaginal and oral group was not different. The rate of cesarean section and hyperstimulation was higher in the vaginal group.Key words: misoprostol - oral and vaginal - effectiveness - premature rupture of membranes - neonatal .asphyxia

Abstract

Background : Traditionally some benalu species were used to treat tumour in Indonesia. Some research report indicated the present of anti tumour activity, although its toxicity study by BST was not performed yet. It is necessary to observe the toxic level of the benalu leaves and whether they are safe to be consumed. Objectives : To determine the toxic level of species of benalu leaves (Dendrophtoe pentandra L. Miq. (1), Macrosolen tetragonus (bL) Miq. (2 and 7), Helixanthera parasitica Lour. (3), Dendrophtoe falcate (Lf.) Ettings (4), Dendrophtoe constricta Dans. (5), Macrosolen cochinchinensis (Lour). Tiegh. (6))Methods : The leaves of benalu were separately extracted with chloroform, followed by methanol. The chloform and methanol extracts toxicity level were screened using Brine Shrimp Lethality Test (BST) at the dose of 1500, 1000 and 500 ug/mI. The level of the toxicity was determined by counting the death percentage of the Artemia saline larvae after 24 hours of adding the extract at the dose indicated. The extract was considered toxic when 100% A. saline larvae was killed at the dose < 1000 ug/ml, and the extract was mildl toxic when 100% death percentage was observed at the dose of 1500 ug/ml.Results : Practically all the chloroform extracts were non toxic since they were not able to kill 100% A. saline larvae at the dose of 1000 ug/mI. Although when the doses were raised to 1500 ug/mI, the 100% death percentage was still not obtained. Similarly, the methanol extracts were also non toxic at the dose of 1000 ug/ml. However, M. cochinchinensis (6) and M. tetragonus (7) were considered to be mild) toxic as their methanol extracts were able to kill 100% A. saline larvae when the dose was raised to 1500 ug/mI.Conclusion: The different test result (at 1500 ug/mI) shown by two methanol extracts of similar species M. tetragonus {2b (killed 52%) & 7b (100%)} should give an idea that the benalu are practically non toxic and save to be consumed for tumor treatment.Key words : toxicity screening, brine shrimp lethality test (bst) - benalu leaves - benalu hosts - anti tumor.

Abstract

Two cases of glaucomas were found by papil disc examination. In the elder patient, more than 40 years old, glaucoma was diagnosed by tonometric examination. In the younger patient, less than 40 years old, suspected glaucoma was diagnosed by water drinking test. Abstract

Thalassemia is a great health problem in the affected countries, especially in the developing countries including Indonesia. There is no curative therapy up to now, while the supportive therapy should be done life4ong and such therapy brings many problems to the patients and an enormous burden to the community who should afford the health services.Ninety thalassemia syndrome, at Dr. Sardjito General Hospital during the period of 1986-1992 were retrospectively studied. The diagnosis was based on clinical manifestations, routine blood examination, HbF investigation either with alkali denaturation test or electrophoresis and on the evidence of trait in the parents. Thirty six patients had undergone molecular studies. Aspects of therapy were also studied.The study found that 57 and 33 patients were major thalassemia and thalassemia-hemoglobin E disease, respectively. There was no difference between major thalassemia and thalassemia-hemogolobin E disease in term of male and female proportion (p > .05), and on the level (p > .05). Foutty eight patients came from Yogyakarta Province, the rest came from various parts of Java, especially nearby Yogyakarta Province. Compared with the estimation, we conclude that many thalassemic patients had not yet been covered. The expenses for supportive therapy were so great, while most of the patients belonged to the middle to low sosioeconomic level, so that only 30-40% had come regularly for routine transfusion. Among the 36 patients who underwent molecular studies, 31 have been genotypically identified. Molecular diagnostic was aimed at the first step of prenatal diagnosis.Key words: thalassemia syndrome-thalassemia trait-blood transfusion-molecular study-prenatal diagnosis

Abstract

Skin to epidural space distance varies in an insertion of epidural needle which can greatly affectthe identification of the epidural space and epidural anesthesia complications. Therefore, acareful prediction of skin to epidural space distance is needed in doing insertions. The purposeof this study was to evaluate the relationship between body mass index (BMI) and the distanceof skin to the epidural space in the 3rd – 4th lumbar epidural anesthesia in nonobstetric surgery ofIndonesian patients. This was a prospective observational study with cross sectional designinvolving patients of both sexes between the age 18-65 years with ASA physical status I-II whounderwent non obstetric surgery with 3rd-4th lumbar epidural anesthesia in Dr. Sardjito GeneralHospital, Yogyakarta and in Satellite Hospital. Weight, height and BMI were considered as theindependent variables, whereas skin to epidural space distance was considered as dependentvariable. Data were collected and analyzed using multiple regression analysis continued usingPearson’s correlation test to evaluate the relationship between BMI and skin to epidural spacedistance. One hundred patients selected from October to December 2011 were evaluated in thisstudy. The mean of weight, height and BMI were 57.35±11.59 kg, 155.98±5.88 cm and23.52±4.26 kg/m2, respectively. The mean of skin to epidural space distance of non obstetricsurgery of Indonesian patients was 40.89±9.95 mm. Multiple regression analysis showed thatBMI had a strongest relationship (r= 0.81; p<0.03) with skin to epidural space distance comparedto weight (r= 0.11; p=0.78) and height (r= 0.04; p<0.83). Further analysis using Pearsoncorrelation test showed that a significantly good correlation of BMI (r=0.92; p=0.001) andbody weight (r=0.87; p=0.001) with the skin to epidural space distance were observed. Inconclusion, BMI and body weight have a significant correlation with the skin to epidural spacedistance in non obstetric surgery patient.

Abstract

The objective of this study is to provide an account of anatomically connections between the subfomical organ and limbic system, using an immunofluorescent study with single fluorescent labeling tracer DAPI (4'-6-diamidino-2-phenylindol dihydrochloride), on seven adult albino rats of Wistar strain weighing between 250 - 350 g.The result of this study revealed that the subfomical organ, anatomically connected to the limbic system.Key Words: subfomical organ - limbic system - DAPI - albino rat - inunUnofluorescent
