Abstract

Background: With the detection of tuberculosis (TB) infection in children means that there are transmitting infection by the lung TB carrier with positive acid fast (AF) sputum. The transmission more frequently occurred in one-roof close contact.Objective: To know the relationship between the TB infection in primary school students and the one-roof family member with positive AF.Material and methods: This study was performed from May 1997 to September 1999 using case-control study design. The subjects of the study were primary school students in Yogyakarta Municipality. The primary schools involved were selected using proportional population random sampling method by the area IPDA. The study was performed by active case finding of one-roof family members with over 2 week successive cough and performing the sputum culture. As the controls were the students who were confirmed not infected by TB after BCG test, matched for their age and sex. The statistical analysis using univariable and the clinical significance of the risk factor was inferred from the confidence interval.Result: One hundred and forty one TB-infected students were assigned as the cases and 189 students not infected by TB were assigned as the controls. The one-roof family members with over 2 week successive cough were found more in the case group than in the'control group, with the Odds ratio = 2.83; (95% CI = 1.01-5.611. The one-roof family members with over 2 week successive cough and positive AF sputum were found only 1 person in each group, with the Odds ratio = 1.34; (95% CI = 0.08-21.65); X2 =. 0.04; p = 0.84.Conclusion: There were more one-roof family members with over 2 week successive cough in the case group. The presence of a one-roof family member with over 2 week successive cough and positive AF sputum culture, was not proved statistically and not conclusive clinically but tendency to be conclusive as the risk factor of the incidence of TB infection in children.Keywords: TB in children - BCG test - risk factor - contact person - positive sputum culture

Abstract

A 2.5 year old female child was admitted to Dr. Sardjito Hospital with complaint of enlarging abdomen since 6 months. She was apparently well until 3 months prior to admission when she had anorexia and weight lost. Physical examination revealed a solid intrabdominal mass (15x13 cm) with smooth surface and no tenderness. USG of the abdomen showed a large mass in paraaortal region andpushed the aorta. Operation was done and during exploration the mass was located as high as the stomachlevel and extended into the pancreas. Gross examination showed the tumor was 11x11x8 cm, encapsulated,nodular, white cut surface, some parts were brown and most of them were fragile. The diagnosis ofpancreoblastoma was established by immunohistochemical examination. The presenting features of pancreoblastoma are generally nonspecific and clinically difficult to distinguishfrom other intraabdominal tumors such as neuroblastoma, non-Hodgkin lymphoma, Wilms tumor, hepatoblastoma and desmoblastoma. Some clinical tests might suggest these tumors, i.e. multiorgan involvement for non-Hodgkin lymphoma; renal origin, the propensity for venous invasion and for thepulmonary metastasis for Wilms tumor. a–feto protein that is positive in either hepatoblastoma or pancreoblastoma cannot differ both tumors. Another intrabdominal tumor that should be considered is desmoblastoma that positive vimentin stain. The positive CAM5.2. as well as cytokeratin and the negative vimentin in immunohistochemical examination confirmed the diagnosis of pancreoblastoma.Keywords : pancreoblastoma – child – intraabdominal tumors – immunohistochemical examination

Abstract

The harniful health effects of tobacco smoking have been reported by many experts. Death rates are unifomily higher among smokers than among non-smokers, in both sexes and whatever the age at death. Cigarette smoking is also considered to be responsible for the great majority of cases of lung cancer. There is evidence that the influence of smoking in ischaemic heart disease is not only independent, but also synergistic with other risk factors such as hypertension and high blood cholesterol. Surveys in many countries show that smokers cough more than non-smokers and produce more sputum. Cigarette smoking acts independently and synergistically with other risk factors contributing to bronchitis.Many experts agree that the smoking problem is now a worldwide epidemic and causing serious diseases. Therefore, studying the problem associated with tobacco should include, specifically the harmful health effects of smoking.The study was carried out in Yogyakarta among senior high school's student. The aim of the study is to test the hypothesis that there is a relationship between neurotic tendency with smoking. Randomized sampling method were done for the analytic-descriptive design of the study.Results showed that there is a significant difference between group of smokers and non-smokers according to the tendency of neurosis (p<0.05). Abstract

Simple glaucomas are charaterized by an elevated intra ocular pressure (10P) and, are an important cause of blindness in the world. The blindness is due to the optic nerve atrophy and is found more frequently in women. It has been reported that high activity of G6PD isozyme can reduce the ocular pressure in the calf eyes. The activity of G6PD isozyme in the blood of simple glaucoma patients has been examined, and compared to those of normal persons. This study consisted of 76 patients of simple glaucoma (41 men and 35 women) and 30 normal persons as a control group. The 10P of both patients and control group was measured by using a Schiotz tonometer. The average of the G6PD isozyme activity was 9.40 ulg Hb in the simple glaucoma and 10.94 u/g Hb in control group. The two G6PD isozyme activity showed a significant difference. It can be concluded that the activity of G6PD isozyme of simple glaucoma patients were lower than those of normal persons without glaucoma symptoms.Key Words : isozyme - G6PD - glaucoma

Abstract

Background: Platelet activation plays an important role in plaque rupture and thrombus formation. Activated platelets release soluble CD40 ligand (sCD40LI. It has coagulation and inflammation properties. Enhanced sCD40L activity has been found in acute myocardial infarction, especially in ST-elevation myocardial infarction. The impact of this enhancement on the degree of myocardial damage remains uncertain, although evidence suggests association between sCD40L level and high burden coronary thrombus.Objective: The aim of this study was to find out whether serum sCD40L level on-admission is predictive of subsequent myocardial damage following acute myocardial infarction.Methods: This study was a cross-sectional study. We consecutively enrolled patients with acute myocardial infarction admitted to ICCU Dr. Sardjito Hospital with onset < 24 hours. Blood collection for analysis of sCD40L was withdrawn on-admission before revascularization and heparinization. Serial measurement of creatine kinase (CK-MBllevel in first 24 hours of admission was conducted, the highest level was considered a peak level that reflected myocardial damage. Troponin I was measured in single sample during 72 hours of admission. The association between serum sCD40L and peak CK-MB level was analyzed with Pearson correlation test. Statistical significancy was determined by p value < 0.05.Result: We enrolled 54 consecutive subjects, among them 37 subjects was STEM!. There was a significantly positive correlation between on-admission sCD40L and peak CK-MB level (r = 0.320 ; p value = 0.0201. Nonetheless, sCD40L did not correlate with troponin I level. In STEM I, this correlation was stronger (r = 0.424; p value = 0.0101 and even more stronger in patients with STEMI who underwent revascularization therapy (r = 0.515 ; p value = 0.0291.Conclusion: On-admission serum sCD40L level is predictive of subsequent myocardial damage following acute myocardial infarctionKey words: soluble CD40 ligand - platelet activation - acute myocardial infarction - peak CK-MB myocardial damage

Abstract

Pemakaian IUD Bertembaga dalam waktu lama melewati masa efektip teoretis tidak mengganggu efektifitasnya untuk mencegah terjadinya kehamilan. Hal ini disimpulkan dari penelitian se-lama sepuluh tahun meliputi 52 pasien praktek spesialis pribadi yang setuju untuk tents menggunakan Multiload Cu-250 tanpa ganti setelah pemakaian selama tiga- tahun yang merupakan batasan masa pakai efektip teoretis bagi jenis IUD Mi. Lama pemakaian tercatat berkisar antara 36,08 sampai 100,51 bulan, sedangkan rata-rata pemakaian adalah 52,10 bulan. Sebanyak 25 (48,07%) wanita memakainya selama 3-4 tahun, 19 (36,53%) selama 4-5 tahun dan 9 (17,30%) selama 6-7 tahun. Pemakaian terlama tercatat pada 4 wanita selama 77,60; 72,48; 92,72 dan 100,51 bulan. Tidak terjadi sate pun kegagalan kehamilan dalam kelompok penelitian ini. Kelompok pembanding terdiri atas 63 wanita pemakai jenis IUD yang sama selama waktu kurang dari tiga tahun dan berasal dad praktek pribadi yang sama. Gangguan subyektip, perdarahan, kelainan haid dan ekspulsi tidak menunjukkan peningkatan frekuensi yang berarti. Sebaliknya, infeksi panggul dan sekresi vaginal yang berlebihan bertambah Bering secara bermakna, meski masih dalam derajat yang ringan dan mudah teratasi. Biqa pula dipahami bahwa terjadi lebih banyak permintaan untuk melepas IUD karena ingin hamil lagi. Karena Multiload Cu-250 sudah terpilihsebagai IUD Bertembaga utama yang digunakan dalam program keluarga berencana nasional di Indonesia, basil penelitian ini dapat bermanfaat dalam kaitannya dengan masalah kontinuasi pemakaian, keamanan, efektifitas, penghematan biaya dan penyediaan peralatan, baik dad segi kepentingan pemakai maupun dari segi kepentingan program. Dalarn penelitian lain yang sedang berjalan hasil-hasil ini akan dibandingkan dengan kelompok pemakai lain yang berganti IUD setelah usai masa efektip teoretis tiga tahun. Dianggap perlu diadakannya penelitian•penelidan lain dalam lingkup epidemiologik lapangan yang lebih Was dengan kondisi klinis medic yang mungkin tidak selalu optimal.Key Words: copper intrauterine device — family planning — Multiload Cu-250 — demography —gynecology

Abstract

We reported a case of lupus nephritis in a 9-year-old patient with beta-thalassemia trait who was treated in Cipto Mangunkusumo Hospital. The patient presented with bicytopenia, hepatosplenomegaly and lymphadenopathy. Laboratory results revealed hematologic disorder, i.e. hemolytic anemia with positive Coombs' test and thrombocytopenia, positive ANA and antibody to double-stranded DNA, and proteinuria of 1230 mg/24 hrs, which fulfilled 4 criteria of ACR for the diagnosis of systemic lupus erythematosus ISLE). Signs of pericarditis was not found. In the follow up, the patient showed microcytic hypochromic anemia with high transferrin saturation level, leading to hemoglobin analysis which revealed beta thalassemia trait. This case is interesting because this patient didn't present with characteristic clinical features, and because SLE is seldom diagnosed before 16 years of age. Moreover, this patient also suffered from beta thalassemia trait, which is known to be associated with rheumatoid arthritis, another autoimmune disorder.Key words: SLE, lupus nephritis, beta thalassemia trait, childhood age

Abstract

Background: Permanent tooth eruption from the alveolus into the oral cavity is one of the basic growth and developmental processes in humans. The permanent tooth eruption occurs gradually with age and therefore the eruption schedule can be used as an age indicator in forensic cases where only skeletons and teeth of children have been found.Objective: This study examines permanent tooth eruption in Javanese children and compares it with other populations.Material and Method: The subjects were 175 children from the state elementary school of Imogiri II, Bantu! District, Yogyakarta. Examinations were carried out using a mouth mirror under natural light. Data was classified in one-year age interval, based on the questionnaire given to parents to fill out with the birth date of each child. In addition, the mean and median ages of dental eruption in boys and girls maxillary and mandibular dentition were also statistically analyzed.Results: Results show that in their maxillas, boys were more advanced than girls in tooth eruption, except for the upper canine teeth. Similarly, in the mandible, boys were more advanced than girls in tooth eruption, except for lower canine and second premolar teeth. In boys, the upper canines erupt later than upper second molars, which is opposite to the pattern of girls. The order of tooth eruption in boys is 11-M1, M', I', 12, 12, P1, P', P2, P2, CO3 M2, C°, M2. In girls, the order of tooth eruption is 11, M1, M', P, 12,12, PI, P1, Co, P2-P2, co, M2, M2.Conclusion: Age can be determined on the basis of tooth eruption in children and subadults.Keywords: permanent teeth, eruption, children, age, growth, Java

Abstract

Suryadi & Harijadi - The Influence of Ethanol Extract of Purple Nutsedge (Cyperusrotundus) on Epithelization of Skin Wound Healing ProcessBackground: If skin is wounded, epithelial cells will migrate to the wound area in order to keep homeostatic.Epithelization consists of mitotic and proliferation of epithelial cells. Rhizome of purple nutsedge (Cyperusrotundus) contains cyperene, flavonoid, ²-sitosterol dan ascorbic acid that can accelerate epithelization inwound healing process and has been used in traditional medicine.Objective: To investigate the influence of ethanol extract of Purple Nutsedge (Cyperus rotundus) inepithelization of wound healing process.Methods: Fourty five Balb/c male mice 8-10 weeks old, 25-35 g weight, were excised punch biopsy onback 0.5 cm right and left of columna vertebralis, 2.5 cm in cutis from the ear area. The mice weredivided into 5 groups. Negative control group was treated with vehicle of extract, positive control groupwas treated by gel containing placenta extract 1% & neomycin sulfate 0.5% and the others were treatedwith C. rotundus 1%, 2%, and 4% in concentration. Each of the groups was divided into 3 subgroups.Each of the subgroups composed of 3 mice based on the period of termination, i.e.: 3rd, 7th and 12th dayafter wounded. Histological evaluation was done to investigate the thickness of epithelial layer.Results: The data were analyzed by Two-way Anova and the results showed that there was a significantdifference (p<0.05) in every decapitation period, groups, and interaction between decapitation period andgroups. The results of LSD test showed that there was a significant difference (p<0.05) in the thicknessof epithelial layer between negative control and C. rotundus groups.Conclusion: Cyperus rotundus extract accelerates epithelization process of wound healing in mice.Key words: Wound healing - Cyperus rotundus - epithelization - mice
