Abstract

Perhatian yang lebih bcsar seyogyanya diberikan kepada hipertensi wanita (HW), karena berlainan dengan perkiraan yang lazim berlaku, HW merupakan pula penyebab sedemikian- banyak morbiditas• dan mortalitas pada wanita, tidak saja pada urrrur seten.gah tua dan umur tua, tetapi juga pada umur muda dalam periode mampu hamil.Sejalan dengan hipertensi pria (HP) kerusakan organis yang terjadi sebagai komplikasi hipertensi pads Oigans vital seperti pembuluh darah arteriola, jantung, otak, ginjal, mata, kelenjar endoktrin dengan segala akibatnya, prognotis sangat menentukan pada kelangsungan kesehatan dan kehidupan wanita.Dalam bentuk murni atau berlandasan penyakit hipertensip, toxaemia gravidarum dengan kesudahan terakhir eklampsia atau solutio placentae, merupakan sebab penting gangguan pertumbuhan janin, prematuritas dan kcmatian perinatal, sclain menjadi salah satu sebab utama kematian ibu di samping perdarahan dan infcksi.Hipertensi sebagai komplikasi pemakaian pil oral kontrasepsi yang mengandung oestrogen, meski klinis bukan merupakan persoalan besar dan akan surut bila pemakaiannya dihentikan, perlu mendapat perhatian seksama, tidak saja karena problema psikis pemakai dan dari segi epidemiologi penting dalam pelaksanaan program keluarga berencana, tetapi juga karena sebenarnya ini menunjuk kepada adanya faktor dasar dalam konstitusi wanita yang pada kondisi lain nanti dapat menjadi landasan terjadinya hipertensi.

Abstract

Background: It is important to diagnose testicular feminization due to other abnormalities with similar clinical findings which needs different treatments.Aim: To diagnose early testicular feminization syndrome by clinical findings, hormone levels, and chromosome analysis.Design: Review of reported cases.Setting: A university hospital. Patient: Phenotypic girl aged 9 years with abnormal external genitalia revealed large clitoris-like structure, normal labia majora, a blind vaginal pouch and no palpable uterus and adnexa. Results: The uterus and adnexa of the patient at ultrasonography and laparoscopy examination were proved to be absent. The serum testosterone concentration was not substantially increased and the karyotype was 46XY. Further examination found left testis in inguinal canal and the right one in labia majora. From biopsy it was found that both seminiferous tubules were in development phase mostly with spermatogonia, and several part in spermatocyte phase. The diagnosis was testicular feminization (androgen insensitivity syndrome). Conclusions: Cases of primary amenorrhea, incomplete external genitalia, and vagina agenesis/hypogenesis, are important to be early diagnosed by investigating the sex chromosome, gonads, sex hormone, genitalia, and sex psychology. Because of the increased risk of malignancy, an orchidectomy has to be performed.Key words : testicular feminization - normal male 46XY - female external genitalia - orchidectomy

Abstract

This article described the medical effects of nuclear arms race and of a probable, intended, unintentional or self-ignited, nuclear war. The danger prior CO and after the explosion of the nuclear arsenal is brought forth. Among the post-war effects the global and prolonged ecological damages are elaborated, affecting both the physical-biological and cultural ecosystem. Considering these effects, post-war survival would not be preferable to immediate death. Medicine would be rendered impotent in such a nuclear holocaust; hence, all efforts have to be concentrated on prevention. Indonesia despite its geographical and political position would not be well off in a total nuclear confrontation. Even here, therefore, the physician as teacher-healer, biologist and scientist or technologist, and especially as a human being, cannot escape the social responsibility to contribute his utmost effort towards the prevention of nuclear war and its preparation. Abstract

The objective of this study is to provide evidence on whether the subfomical organ is a neuroendocrine gland, by which the neurosecretion process to be carried out in this organ. This investigation involved the use of light microscopy by using an immunohistochemistry as well as histochemistry methods and transmission electron microscopy. Eighty adult male and female albino rats of the Lembaga Makanan Rakyat strain (Jakarta) weighing between 150-250 g, and 9 rats of Wistar strain, weighing between 200-300 g, were studied.According to morphology and by using immunohistochemistry and histochemistry methods, the results revealed that the subfomical organ was not a neuroendocrine gland that secreted a neurohormone or neurosecretory substances through a neurosecretion process.Key Words: subfomical organ-neurosecretion process - transmission electron microscopy - inununohistochemistry technics - histochemistry technics.

Abstract

Benzene is one of the dangerous chemical substances which can cause some health disturbances.Meta-analysis shows that people who are exposed to benzene have a leukemia risk of 1.4 timesmore than the ones who are not. The employees who are exposed to benzene for a long timehave a probability to receive the toxic effect from benzene. This study was conducted to evaluatethe relationship between the intensity of benzene exposure and blood count of employees in theoil and natural gas company in East Kalimantan. This research was conducted to 897 employeeswith case control design. The control group consisted of 521 subjects, whereas the case groupconsisted of 376 subjects. The subjects were 39.91±10.90 years old, with the working termbetween 1 to 36 years. Data were obtained from routine medical check-up and health riskassessment in 2007 – 2010. The independent variable namely benzene exposure and the dependentvariable namely result of blood count, were analyzed by using chi-square bivariat analysis.Intensity of benzene exposure was measured as OVM (Organic Vapour Monitor) for externaldosage and SPMA (S-phenylmercapturic acid) for internal dosage. The intensity of benzene in2007 was 1.07± 5.5 ppm (OVM) and average of SPMA was 11.54±5.28 ìg/g creatinine. Theintensity of benzene in 2009 was 4.15±13.6 ppm (OVM) and SPMA was 55.90±43.6 ìg/gcreatinine. The employees who were exposed to benzene had a risk of having a decrease ofeither hemoglobin, erythrocyte, thrombocyte, or leukocyte 1.71 times (95% CI, 1.18–2.48)compared with the control, although this reduction was still in the normal range (p= 0.004). Inconclusion, there was no relationship between the intensity of benzene exposure with the lowblood count of the employees.Keywords: benzene exposure - complete blood counts - oil and gas earth - mining - toxic effect

Abstract

Dilakukan peninjauan atas 1516 kasus persalinan di Bagian Obstetri-Ginekologi Rumah Sakit Universitas Gadjah Mada, yang dikumpulkan secara bebas dalam waktu delapan belay (!8) bulan (dari bulan Oktober 1969 sampai bulart April 1971), tentang hubungan antara paritas dan perdarahan postpartum.Dari peninjauau tersebut dapat diatnbil kesimpulan, bahwa makirt tinggi paritas wanita makin tinggi pula frekwensi perdarahan postpartum khususnya perdarahan kala III dan IV.Frekwensi perdarahan postpartum di Bagian Obstetri-Ginekologi Riunah Sakit Universitas G-adjah Mada cukup tinggi (17,7%), sehingp perlu Fieningkata.n perhatian dan sikap aktip dalim mengadakan tindakan pencegahan atau pertolongan bila terjadi perdarahan postpartum.

Abstract

Resi,stance to insulin-stimulated glucose uptake is present in the great majority of patients with impaired glucose tolerance or type 2 diabetes mellitus, in first degree relatives of patients with type 2 diabetes mellitus, as well as in approximately 25% of non-obese individuals with normal oral glucose tolerance. Deterioration of glucose tolerance can be prevented if beta cell is able to produce more insulin resulting hyperinsulinemia.The association between hypertension and diabetes mellitus is well recognized. Several reasons for the coexistence of these disorders have been postulated, i.e. as part of aging process, diabetes causes macrovascular and microvascular disease, renovascular stenosis which is enhanced by the development of hypertension. Insulin causes renal sodium reabsorption, stimulates the sympathetic nervous system, increases intracellular calcium, and is a vascular growth factor.Insulin resistance, hyperinsulinemia, impaired glucose tolerance, increased plasma triglyceride, and decreased HDL-cholesterol concentration have been shown to be associated with increased risk of coronary heart disease. Based upon these considerations, it raised the possibility that insulin resistance and hyperinsulinemia are involved in the etiology and clinical course of three major related disease; the type 2 diabetes mellitus, hypertension, and coronary heart disease.Key Words: hyperinsulinemia insulin resistance cardiovascular disease glucose homeostasis aging

Abstract

Objective: To report the success of amniotic membrane transplantation combined with trabekulectomy ina patient with Pseudophakic bullous keratopaty.Case Report: Amniotic membrane transplantation and trabeculectomy followed by tarsoraphy had been successfully performed in an eye of male patient 67 years old with pseudophakic bullous keratopathy, with increased intraocular pressure, the visual acuity for both eyes was hand movement, intraocular pressure in the right eye was 25 mmHg and 18 mmHg in the left eye. Patient felt painful in both eyes also. Pre surgical and post surgical medication with oral and eye drop antibiotic, oral and eye drop steroid, and oral vitamin was given.Result: The painful of both eyes disappeared on the            111 day after amniotic membrane transplantation and no recurrence in 2 week observation. In the right eye amniotic membrane dissolves on the 41n day after surgery, an epithelization occurred on the 51" day after surgery. On the 7th day the visual acuity was 2/60. Amniotic membrane dissolves on the 31d day in the left eye, epithelization occurred on the 4th day. On the 71h day the visual acuity reached 3/60 and remained constant until the 2"tl week of observation.Conclusion: Amniotic membrane transplantation followed by tarsoraphy is effective for pseudophacic bullous keratopathy treatmentKey words: Pseudophacic - bullous keratopathy - amniotic membrane transplantation- trabeculectomy-intraocular pressure- visual acuity-
