Abstract

To detennine whether non-insulin dependent diabetes mellitus (NIDDM) patients with microalbuminuria have significant abnormality in lipid profile which could result in increasing the cardiovascular risk, a cross sectional study had been conducted by matching in-patient and out-patient of the Department of Medicine, Dr. Sardjito General Hospital, Yogyakarta between October 1990 to May 1992.NIDDM patients with urinary albumin excretion rates (UAER) of 30 to 300 mg per 24 hours (microalbuminuria, incipient nephropathy) were matched for age and sex with NIDDM patients with UAER below 30 mg per 24 hours (control group). Venous blood samples were taken to determine blood sugar, total cholesterol, high density lipoprotein (HDL) cholesterol, low density lipoprotein (LDL) cholesterol and triglyceride levels. Sex. age. duration of diabetes, relative body weight, retinopathy and neuropathy were also determined.Four groups of 34 NIDDM with microalbuminuria (17 males and 17 females) were found, and 34 controls (17 males and 17 females) were studied. There were no significant difference in age, duration of diabetes, relative body weight and neuropathy. However, retinopathy was significantly higher in diabetic patient with microalbuminuria. No significant difference was found in LDL-cholesterol (122,58 ± 59,09 mg/di vs. 113,55 ± 42,25 mg/di, p = 0,24) and triglyceride (173.91 ± 89.06 mg/dl vs. 164.69 ± 69.69 mg/di, p = 0.31), fasting blood sugar (202.55 ± 87.01 mg/dl vs. 23061 ± 111.93 mg/dl, p = 0.13) and 2 hours after meal (253.91 + 89.46 mg/di vs. 284.38 + 114.40 mg/di, p = 0.11) in both groups. However HDL-cholesterol levels (35.94 ± 13.65 mg/dl vs. 4.3 ± 12.32 mg/di, p = 0.01) was significantly lower and the ratio of total cholesteroVHDL-cholesterol (6.31 ± 3.08 vs. 4.1 ± 1.13, p = 0.00017) was significantly higher in NIDDM patients with microalbuminuria compared to the control group...Key Words: diabetes mellitus - lipid profile - cholesterol - HDL cholesterol - cardiovascular risk factors

Abstract

Mercury is one of the potent allergens which can induce contact hypersensitivity. This substance however, is commonly used for cosmetics and medical purposes. The exact mechanisms underlying the mercury-induced hypersensitivity remain unclear. Studies on mercury contact hypersensitivity in human were hampered due to ethical reason. This investigation was, therefore, aimed at studying hypersensitivity reactions In animal model. Thirty five female Wistar rats, aged three months were divided into three groups of treated animals and two groups of controls. The treated groups were sensitized with 2,5%, 5%, and 10% HgCl2 In petrolatum for 10 days. At day 14, animals were challenged with 5% HgCl2 in 97% ethanol. Contact hypersensitivity was assessed by measuring the ear swelling before and after ear challenge. Specificity test was carried out by challenging the sensitized animals with K2Cr207. The results showed that the, peak levels of ear swelling could be achieved by applying 10% HgCl2 as compared to other groups of animals. Chromium (K2Cr207) ear challenge following 10% HgC12 sensitization failed to induce ear swelling. It can be concluded that the mercury hypersensitivity reaction in this animal model is antigen-specific.Key words: allergy - contact hypersensitivity-mercury - DTH reaction - Wistar rats

Abstract

Background: Electrocardiographic (ECG)changes have been observed in leptospirosis. The prevalence of ECG alterations may reach 80%. Despite of this finding,few studies have been conducted to assess ECGpatterns in only hospitalized leptospirosis patients in Indonesia. Objective: To reveal the prevalence and type of ECGalterations in hospitalizedleptospirosisand its association with disease severity. Methods: This was a cross-sectional study using medical record data. Patients hospitalized in Dr. Sardjito Hospital (January 2003 - December 2007) with leptospirosis (lCDXA27.9) were enrolled. Electrocardiogramstaken in 24 hours of admission were evaluated. Disease severity was determined based on clinical and laboratory findings. Association between disease severity with ECGrecordings were performed using Kruskal-Wallisand Chi square test. P value < 0.05 was significant. Results: Sixtysamples were analyzed.The prevalence of alterationof ECGrecordingswas 66.7%. Sinustachycardia was the most frequent (36.7 %). The most common arrhythmia was atrial fibrillation(10 %). Other abnormalities were alterationof ventricularrepolarization(8.3%1.extrasystoles (6.6%) and disorderof conduction (4.9%). Although not significant, those with atrial fibrillationtended to have the worst clinicaland laboratory findings. Conclusions: The prevalenceof alterationof ECGrecordingsin hospitalizedleptospirosiswas 66.7 %. Atrialfibrillation was the most frequent arrhythmia, and patients with this arrhythmia were likelyto have the most severe disease.

Abstract

Carcinoma of the male breast occurs so infrequently. Several articles reported that the relative frequency ranges between 0.46% —2% of all breast carcinoma. The histologic types of the male breast carcinoma were relatively the same as in the female, but in the male, lobular carcinoma and carcinoma with metaplasia were never found. The disease commonly occurs in elderly men, from the late fifth through the seventh decade, and in women it usually occurs before menopause. This situation suggests that estrogen has an important role in the pathogenesis of breast carcinoma. Two cases of male breast carcinoma were reported. One occurred in a man aged 28 years. The histologic type was papillary carcinoma with mucoid degeneration. The other occurred in an older man aged 70 years with infiltrating duct carcinoma. The latter case could support the important role of estrogen in causing carcinoma. But the other one, cannot support this. It must be thought that there are other factors, such as in genetic syndromes. Key Words: male breast carcinoma — estrogen — oncology — genetic syndromes — neoplasm

Abstract

Secara sepintas terdapat kesan kecenderungan meluasnya penggunaan susu botol di kalangan ibu-ibu di kota maupun di pedesaan. Namun belum pernah dilaporkan tentang kontaminasi maupun kadar pengenceran dalam penyediaan susu bowl yang akan diberikan kepada bayi di Indonesia pada umumnya dan di daerah Yogyakarta khususnya.Pemeriksaan terhadap 53 sampel susu botol yang akan diberikan kepada bayi (30 berasal dari daerah kota dan 23 dari pedesaan) menunjukkan basil 72% dengan angka kuman yang tinggi (lebih 100.000/m1).Sepertiga dari jumlah sampel menunjukkan pengenceran di bawah 50%, dan hanya seperempat jumlah sampel dengan pengenceran 80-120%. Tingkat pendidikan pembuat larutan susu dan daerah asal sampel (kota maupun desa) tidak mempunyai hubungan positif dengan besarnya angka kuman maupun pengenceran susu botol.Dari hasil penelitian ini perlu ditekankan bahwa apabila susu bowl terpaksa diberikan, diperlukan penerangan dan pendidikan bagaimana cara menyediakan susu bowl untuk menjaga kesehatan dan pertumbuhan anak.

Abstract

179 cases of inguinal hernia in infants and children, including 20 incarcerated cases during the eleven-year period from January 1961 through December 1971 at the Department of Surgery, Gadjah Mada University Hospital, were reported.In accordance to most of the references attention was given to the incidence of the left side in the report ( 59,8%) which paradoxially was markedly higher than the right side ( 30,7% ).Operations were performed on 160 cases and the mortality rate was percent.

Abstract

The principal objective of this study is to provide more extensive description of the chemopreventive effect of rneniran herbs-hexan extracts against the toxic effect of aflatoxin B1 in the liver of Rattus norvegicus rats. The subjects consisted of sixteen male rats Rattus norvegicus in good general condition, 1 - 2 month old, with body weight around 62 - 68.5 grams, divided into 4 groups of four rats each. Water and ad libid were given to All of the groups of the rats. The first group was given 1 ml of emulsion of coconut oil. The second group was given 30 mg of meniran herbs-hexane extracts in 1 ml emulsion. The third group was given 15 g aflatoxin B, (AFB,) in 1 ml emulsion. The fourth group was given 30 mg of meniran herbs-hexane extracts and 15 g of AFB, in 1 ml of emulsion. Extract and AFBi were given orally by an aplicator for 16 weeks. By analysis of variance the results showed that the effect of meniran herbs-hexane extracts against the toxic effect of aflatoxin B1 demonstrated a significant effect (p < 0,01) in serum alanine aminotransferase (glutamic pyruvictransaminase). Aflatoxin B, increased serum alanine aminotransferase significantly (p < 0.001) in comparison with control (the group of rats treated with 1 ml emulsion and the group of the rats treated with 30 mg extracts). Histological feature in the group of rats treated with AFBi showed bad liver features compared to the other groups. There were altered foci in the hepatocytes (3/5) and the bile duct epithelial cells demonstrated hyperplasia as well as metaplasia (5/5). The group of the rats treated with meniran herbs-hexane extracts and AFB, showed normal hepatocytes and some of bile duct epithelial cells showed proliferation. The other groups of the rats respectively treated with coconut oil emulsion and meniran herbs-hexane extracts showed normal hepatocytes and bile ducts epithelial cells. It is concluded that meniran herbs-hexane extracts could be used to reduce aflatoxin B, toxicity in the rat liver.Keywords: Phyllanthus niruri L. herbs-hexane extracts - aflatoxin Bt - serum alanine aminotransferase - bileduct epithelial cells - hepatocytes - Rattus norvegicus

Abstract

Background: In vitro studies reveal that LMP1 induces the expression of many factors for invasion and metastasis. Objective: The aims of this study were to investigate the role of LMP1 expression on the status of Nand M that take place either directly or sequentially through its relation with the expression IL4, IL6. IL 10. IFN-gamma. Gran2yme B and with protease MMP2. MMP9. either in the serum or in the tumour tissue. Methods: Forty one 41 EBER-RISH confirmed NPC cases with parallel serum were used for this study. Immunohistochemistry was used to study the expression of proteins in the tissue. The serum level of proteins were determined with ELISA. Statistical analysis used Lisrel 8.50. Results: No correlation between the expression of LMP1 with the status of Nand M of the tumour either directly or that sequentially took place through the expression of the cytokines and MMPs either in the tumour tissue or the serum level. The serum level of MMP9 was correlated with the serum level of IL6, the status of N. and the expression of IL 10. Status N was correlated with serum level of MMP2. Conclusion: There is a process that sequentially took place through the increase of the cytokines. MMPs, either in the tumour tissue or serum level and N. M status. LMP1 expression was not involved in the process. Key words: NPC - metastasis - MMP - cytokine

Abstract

Drug utilization has been defined as "the marketing, distribution, prescription and use of drugs in a society, with special emphasis on the resulting medical, social and economic consequences". The aim of drug utilization study is to achieve optimal use of drugs in a society, i. e. safe, efficacious, rational and cost-effective. Various benefits can be obtained from drug utilization study which by all means may increase the quality of patient care.Drug utilization study may be classified into two types based on its specific aim. First routine monitoring of drug use in a society which is aimed at examining the general pattern of drug use and predicting the forthcoming trends, and second special drug utilization study which is aimed at investigating specific study questions and hypothesis.Review of the pattern of drug utilization is likely becoming and effective feedback to the prescribers and drug policy makers towards more rational use of drugs.Key Words: drug utilization - patient care - drug policy - pharmacoepidemiology - drug abuse
