Abstract

Atrial fibrillation is very rare in children. Two children are described, although both were initially in congestive heart failure due to active carditis and were difficult to manage, but all have completely recovered. In addition to digitalis therapy all cases had quinidine, with therapeutic goal of controlling the heart rate rather than abolishing the fibrillation. One case died at home, 12 days after discharge from hospital and one recovered from fibrillation until now.Mitral and tricuspidal an nuloplasty have been performed to the second case, and the result is very satisfactory.Key Wards: atrial fibrillation — rheumatic hear t diseases — rheumatic fever— congestive heart failure — quinidine therapy

Abstract

Diagnosis sebagian besar limtoma gastrointestinal hanya dapat ditegakkan dengan laparotomi. Duapuluh empat penderita limtoma gastrointestinal primer diteliti. Penderita dikelompokkan menurut umur, jenis kelamin, keluhan, adanya simtom B (demam, penurunan berat badan, dan keringat malam), lokasi tumor, tindakan bedah, diagnosis histopatologis menurut International Working Formulation, dan terapi.Dari 16 orang penderita laki-laki, umur antara 6,5 - 76 tahun dengan rerata 44,7 tahun, lama keluhan pada 10 penderita tidak Iebih dari 6 bulan, hanya terdapat 8 penderita dengan simtom B.Lima betas penderita dengan keluhan massa pada abdomen, 5 dengan gangguan pola pencernaan, 4 dengan obstruksi, dan sakit perut pada 10 penderita. Biopsi laparotomi dikerjakan pada 16 penderita dan reseksi hanya dapat dikerjakan pada 6 penderita. Dua penderita dilakukan apendektomi karena gejala yang mirip dengan apendisitis. Menurut International Working Formulation, 17 penderita dengan derajat keganasan sedang, 5 dengan derajal keganasan tinggi dan 2 dengan derajat keganasan rendah. Hanya 9 penderita mendapat kemoterapi dengan sikiotostamid, vinkristin dan prednisolon dan hanya 3 penderita mendapatkan 3 siklus atau Iebih kemoterapi.Key Words : gastrointestinal lymphoma - laparotomy - chemotherapy - working formulation.

Abstract

Multiple mini punch grafts is the placing of mini size of full thickness skins on to ulcer bed. Theyconsist of epidermal and dermal component composed with hair follicles and other skin appendiceswhere epidermal stem cells are located. The epidermal stem cells are the best source of epidermalcells in reconstruction of skin equivalent that is usually used for replacing classic split thicknessskin graft in recovering extensive ulcer. In this article, the application of multiple mini punchgrafts onto extensive ulcer is reported. A case of extensive ulcer was suffered by a 6-year-oldboy whose left foot is injured in a traffic accident. His toes had already been amputated bysurgeon but a classic skin graft failed to recover the ulcer. Multiple mini punch grafts had beenharvested from his inguinal and buttock skin and they were placed onto his ulcer. Pre and postmini punch grafting photographs were reviewed. After eight weeks, placed multiple mini punchtissues onto large ulcer reveals lateral extensions and more than 90% of epithelialization. Multiplemini punch grafts can be used as a method to cover large ulcer.Key words: mini punch grafts-large ulcer-epithelialization-epidermal-stem cells

Abstract

Keratitis ulcerosa is a common disease in Yogyakarta and surrounding: This disease will cause blindness without good and right management. Usually it is encountered in rural areas caused by mud or pin, some materials in the field, for example wood or leaf and other corpora aliena.When complication occurs, perforation happens, resulting in total blindness.The treatment is easy when administered immediately. Sometimes there are difficult cases of keratitis ulcerosa which are resistent to treatment and need referral to the eye hospital after simple treatment is found to be unsuccessful in one week. From data presented the simple treatment is successful enough.Key Words: keratitis ulcerosa - health center - blindness - hypopyon - Aureomonas hydrophila

Abstract

Pharingitis karena Streptococcus punya hubungan erat dengan terjadinya glomerulonephritis dan demam rheumatik. Incidensnya bevariasi antara 0,3 - 3%. Di Indonesia incidens demam rheumatik masih tinggi, sehingga dengan demikian kita dapat mengambil kesimpulan bahwa inciderns infeksi streptococcus juga tinggi.dengan demikian kita dapat mengambil kesimpulan bahwa incidens infeksi streptococcus juga tinggi.Gejala klinis dari pharingitis karena steptococcus tidak khas, sehingga sukar untuk didifferensiasi dengan pharingitis karena virus. Ada tiga tingkatan gejala klinis, yalah berat, medium dan ringan, dan semuanya ini dapat merupakan pendahuluan dari demam rheumatik.Infeksi streptococcus yang berulang-ulang akan memberatkan kerusakan valvula pada kelainan jantung rheumatik, sehingga pada kelainan ini perlu dilakukan profilaksi. Tapi profilaksi ini tak perlu dikerjakan pada penderita glomerulonephritis, karena padanya infeksi ulang tak memberatkan.Bakteri streptococcus dapat diklasifikasikan sebagai berikut:a). Didasarkan pada sifat-sifat haemolisisnya pada agar darah kambing, maka bakteri ini diberi nama: alpha-, beta-, dan gamma-haernolisis.b). Didasarkan pada test serologis maka diberi nama grup A — Q (tanpa I dan J), sedang grup A sendiri dibagi jadi paling sedikit 45 tipe (subgrup).Strain group A dapat melepaskan sejumlah antigen extracellular.Pengobatan: Bila kita melakukan pengobatan awal dan cukup berarti kita mencegah terjadinya demam rheumatik dan kelainan jantung rheumatik. Obat-obat yang paling baik untuk infeksi ini yalah: penicilin, erythromycin dan Lincomycin. Sulfa tidak dapat digunakan sebagai pengobatan, karena resikonya lebih besar daripada kegunaanya. Tapi sulfa dapat digunakan sebagai profilaksi.

Abstract

Diabetes mellitus (DM) is a metabolic syndrome characterized by hyperglycemia and glucose intolerance, due toinsulin resistance, insulin deficiency, or both. Diabetics retinopathy (DR) is a DM complication due to retinal abnormality,that causes vision reduction and even blindness. The association between DR and aldose reductase C-106T (ALR C-106T) gene polymorphism has been reported in previous studies. This genetic polymorphism increases the sorbitollevel inside erythrocyte and pericyte in the retinal membrane that leads to weakness of retinal capillary vessel andmicroaneurism. The aim of this study was to know the presence of ALR C-106T gene polymorphism and its frequencydistribution among diabetics Javanese patients in Dr. Sardjito General Hospital Yogyakarta, Indonesia. In addition,this study also aimed to analyze the difference of erythrocytes osmotic fragility (EOF) among ALR genotypes in type2 diabetics patients with DR and without DR and to analyze whether ALR genetic polymorphism is a risk factor ofDR in type 2 diabetic patients. This was a case control study that involved 40 diabetics patients with DR as case and40 diabetics patients without DR as control groups. The C-106T ALR gene polymorphism was determined bypolymerase chain reaction-restriction fragment length polymorphism (PCR–RFLP) method. Erythrocytes osmoticfragility was analyzed using spectrophotometer. Genotype and allele distributions were analyzed using x2 and otherdata were analyzed using independent t-test and Mann-Whitney, with p<0.05 was considered as significantlydifferent. The results showed that in type 2 diabetics patients with DR, 33 patients (82.5%) were CC homozygoteindividuals and 7 patients (17.5%) were CT heterozygote individuals. In type 2 diabetics patients without DR, 27patients (67.5%) were CC homozygote individuals and 13 patients (32.5 %) were CT heterozygote individuals. Thegenotype and allele distributions were not significantly different between two groups (p=0.121 for genotype,p=0.151 for allele). Odds Ratio of genotype was 2.270 while allele was 2.023. Erythrocytes osmotic fragility of CCgenotype was higher than CT genotype (p=0.047). In conclusion, there was no significant difference between CCand CT genotype distribution among type 2 diabetics patients with and without DR. Erythrocyte osmotic fragility ofCC genotype was higher than CT genotype. C-106T gene polymorphism was a risk factor of DR in type 2 diabeticpatients.Key words : ALR genes – polymorphism - type 2 DM - diabetic retinopathy - erythrocytes osmotic fragility

Abstract

Phyllanthus niruri L. (meniran) is well known and can be used for traditional treatment of various diseases such as jaundice, diarrhea, and infection of tractus urinarius. The studies showed that the extract of Phyllanthus nirurl L. could be used as antihepatotoxics. This study was designed to investigate the effect of 10%, 20%. and 30% water-extract of Phyllanthus niruri L. herbs on the rats' liver treated with 15 ug aflatoxin 131 (AFB1) In 0,2 ml propylene glycol, orally by tube, everyday for 16 and 24 weeks. Employing the analysis variance, significant effect was observed in serum glutamate-pyruvate transaminase (SGPT) and liver's cytosol glutamate-pyruvate transaminase (GPT), but was not significant in liver's cytosol glutathione S-transferase (GST). However among inter group analysis, significant decrease in SGPT and liver's cytosol GPT observe only group treated with 20% water-extract of Phyllanthus niruri L. herbs and AFB1 compared to group treated with AFB1  (p<0,05). The study also demonstrated that 20% water-extract Phyllanthus niruri L. herbs could prevent metaplasia, hyperplasia, and proliferation of the bile duct epithellial cells and altered foci in the hepatocytes were caused by AFB1.Keyword : aflatoxin 131 - glutamate-pyruvate transaminase - glutathion S-transferase - Phyllanthus niruri L. - rat's liver

Abstract

To study the possibility of the influence of exercise on body posture, anthropometric measurements were taken in 40 male athletes and 41 male non-athletes, as well as in 26 female athletes and 24 female non-athletes, among students in Medan. The measurements included height, weight, arm and thigh circumferences, thorax diameters, and skinfold thickness. The results showed that there was a significant difference in the left arm circumference between male athletes and male non-athletes, whereas in the female none of the measurements taken showed any significant difference. The explanation of these results has to be sought by further studies with more consideration on the types of exercise performed. Abstract

Cimpaign against cancer is not only the duty of the medical professional but also the respon• sibility of the society. The activities of the Sardjito Teaching Hospital Cancer Team. "Kucala" Cancer Society, county hospitals, health centres and physicians in private practice are presented.It is clear that the role of the Cancer Team of the Medical Faculty/the SardjitO Teaching Hospital, "Kucala" Cancer Society and medical professionals are very important in the detection of cancer. Due to the lack of facilitiei and based on the situation of the Yogyakarta area, it seems that there are many problems to deal with.It is suggested that a well organized cancer society will be able to solve the problems.Key Words: cancer - community health - anti-cancer campaign - health organization -- early diagnosis

Abstract

Backgound: The nocturnally periodic form of B.malay/ was reported as health problem both in Cheju, Korea and Bireuen, Aceh, Indonesia. The filarial worm was transmitted respectively by Ae.togoi in Korea and An.barbirostris in Indonesia.Objectives: To know any difference of microfilarial behaviour in peripheral blood by meta-analisis on available data published by previous workers.Methods: Using Aikat and Das formula of statistical analysis.Results: B.malayi' s microfilariae tend to present at peripheral blood at day time among cases in Cheju, Korea more than those in Bireuen, Aceh, IndonesiaConclusion: B.malayi's microfilariae in Cheju, Korea still had tendency to show                  (nocturnally) subperiodicmore than those in Bireuen, Aceh. Key words: filariasis      - B.ma/ayi's microfilaria variant        - nokturnally periodic      - nocturnally subperiodic
