Abstract

The relationship between blood pressure and body weight, body height and upper arm circumference was studied in 506 school children, consisting of 227 boys and 279 girls, aged 6-14 years. Blood pressure was measured by using mercury sphygmomanometer. measurements were conducted once in the sitting position, and the subjects were selected at random from 7 000 school children in the district of Sewon, Bantul, Yogyakarta. The results found showed that there are correlations between either systolic or diastolic blood pressure with body weight, body height and upper arm cir cumference, especially in the ages of 15 co 14 years. Abstract

Background: Oxydative DNA damage, namely 8-OHdG, may occurred due to DNA oxydation by reactive oxygen species (ROS). Unrepaired 8-OHdG may initiate a carsinogenesis due to G to T transversion of gene point mutations. The 8-OHdG repair can be measured by determination of urinary 8-OHdG level. It is well known that ROS reaction with preexisting melanin may perform an immediate pigmentation darken-ing (IPD) and it can be measured by mexameter. The correlation between IPD and 8-OHdG repair capacity is still not known.Aim of Study: To know the correlation between IPD and 8-OHdG repair ability in order to predict the oxydative DNA repair capacity.Methods: Using of a quasi experimental design, 8-OHdG urine level alteration correlated with skin color alteration after oxydative stress among 30 healthy elementary-school children during sport activity pro-gram under sun exposure. 24 hours of skin color alterations as a IPD were measured by mexameter, and urinary-8- HdG- level were measured based on ELISA with 8-OHdG-Kit antibody. The correlation be-tween IPD and     8-OHdG urine level alteration was analyzed with Spearman test.Results: There was a very significant correlation between immediate pigmented darkening and urinary                      8- OHdG level alteration (r=0.8562; p<0.01).Conclusion: IPD represents 8-OHdG repair capacity after oxydative stressed under sun exposure Keyword: immediate pigmention darkening -  8-OHdG -mexameter - sun exposure

Abstract

Background: Malnutrition is a problem in under five-year old children. During the economic crisis, the nutritional status of this group has decreased. There are variety of risk factors related to severe malnutrition. Some risk factors are preventable. Recognition of preventable risk factors is important in the management of severely malnourished children in the community.Objective: To determine factors associated with severe malnutrition among under five-year old children in Yogyakarta during the economic crisis.Methods: A case control study conducted in Sleman and Bantul Districts. Thirty four severely malnourished children and 68 controls of well nourished children were enrolled in this study. There were 6 group of variables consisted of 21 factors analyzed in this study. A logistic regression analysis was done to calculate OR and 95%Cl.Results: Univariate analysis showed that low educated mother, mother who works outside home, bad water disposal, non-breastfed children, and tuberculosis were associated with severe malnutrition where their OR and 95%Cl were 2.96 (1.26 - 6.94), 2.79 (1.15 - 6.77), 12.33 (3.44 - 44.24), 4.13 (1.70 -10.02) and 9.03 (2.29 - 35.62) respectively. Using multivariate analysis only non-breastfed children, tuberculosis and bad water disposal related to severe malnutrition with OR and 95%Cl 7.01 (1.35 -36.48), 11.52 (1.43 - 92.64) and 32.64 (5.44 - 195.90) respectively.Conclusions: Breastfeeding plays important role to support nutrients and other benefits to nutritional status of the children during the economic crisis. Tuberculosis is still a main factor related to severe malnutrition compared to acute diseases.Keywords: malnutrition - under five - risk factors - breastfeeding - tuberculosis

Abstract

Non-ionic (Iopamiro) and ionic (Urografin 76%) contrast media were used for intravenous urographic examination. Three patients (6%) showed symptoms of side effect in ionic group; two of them without allergy history, but only one patient had an experience of allergy to shrimp, penicillin and dust. The symptoms of reaction were chills, dizziness and febris.There was no reaction in non-ionic group patients. Abstract

Muchlas 1 has adapted marital intimacy scale (10 questions) from 36-items of PAIR (Personal Assessment of Intimacy in Relationships) to suit with Javanese culture. In his dissertation, the scale was used to describe that one of the ways to increase the condition of mental health of mothers attending a course on mental health is by improving their marital intimacy. The result is not like what was expected, although the scale had been approved in the reliability and validity test. In his study on this scale, Sudiyanto2 said that although it had significant validity, the scale still had low internal consistency; there were normative questions, asking about morality instead of intimacy; several statements were inappropriate and the ten questions might not represent marital intimacy dimensions. The demand to revise this scale became urgent, as the scale is the only one planned for Indonesians and its practicality is required to evaluate the results of consultations on marital problems as well as mental health conducted by existing institutions, especially the Advisory Board of Marital and Separation Problems (BP4). Later Muchlas and Sudiyanto3 revised this scale of marital intimacy, and it became the revised scale of Marital Intimacy (15 questions). This new scale had better validation and internal consistency. In this study the revision of the scale of marital intimacy was used to measure the marital intimacy of worried patients who visited BP4 for consultation concerning with their separaty. Through an experimental study using psychotherapy, 100 random samples were divided into two groups; 50 samples belonged to a group provided with psychotherapy and 50 other samples belonged to a control group without psychotherapy. Their marital intimacy was measured using the new instrument before and after they were provided with psychotherapy. The result showed significant difference between the two groups and the validity of the marital intimacy scale used in this study was adequately good.Keyword : marital intimacy -- validity of the MIS -- REMIS -- psychotherapy

Abstract

The classification of primary glaucoma is based on the depth of the iridocorneal angle. Supartoto et al. found in Dr. Sardjito Hospital Yogyakarta, that the cases of angle-closure glaucoma were thee times more than the open-angle glaucoma.Kolker & Hetherington stated that the open-angle glaucoma are 60 - 70% of the primary glau- coma in adults: The study on the iridocorneal angle in normal eye of random samples at the Eye Clinic, Dr Sardjito Hospital was done. Abstract

Background: Urinary tract infection (UTI) is one of the important diseases causing significant morbidity in infants and children. The true incidence of UTI in children is difficult to estimate because infection in infants and children can be asymptomatic. Delay in the treatment of UTI can lead to recurrent infection resulting in renal scarring or complication with urosepsis. Physicians use a large number of different antimicrobials and give them for widely varying durations to children and infants with suspected UTI. More standardized use of the most effective, best tolerated and least costly treatment regimens would have considerable benefit both in reducing morbidity and health care costs.Objective: The aim of this study is to compare of ampicillin compared to cotrimoxazole oral treatment in UTI children's.Methods: This study was used clinical trial design. A total of 1-10 child patients in Dr. Sardjito Hospitalwith urinary tract infection older than 2 months age who enaolled in this study were randomly assigned to receive either ampicillin (57 children) or cotrimoxazole (53 children). Diagnosis was based on clinical and laboratory criteria. Laboratory diagnosis was established based on bacterial count and urine culture tests. The cure rates were assessed after seven day treatment based on clinical and laboratory evalua- tion. Chi-square test was applied to analyse the difference of clinical outcomes between ampicillin and cotrimoxazole groups. Logistic regression analysis was applied to analyse the relationship of variable that influence the outcomes. Cost analysis accounted by direct cost (medical, length of stay, laboratory cost)between two groupsResult: Mean of the length of stay of cotrimoxazole patient was shorter than ampicillin patients. It was also found that E coli was the most frequent (45.4%) microbes detected in the urine culture. There were no significant differences in the sensitivity of the microbes to both antibiotics. The results indicated that cotrimoxazole showed significant differences in the effectiveness (p<0.05).Conclusion: Cotrimoxazole treatment on UTI is as effective as ampicillin. Abstract

Background: Astigmatism change after cataract surgery is common, but if the astigmatism is too high it can affect the visual acuity. The astigmatism change depends on the incision methods, sclera! rigidity, and age.Objectives: To compare the astigmatism change between corneoscieral and sclera! incision methods after cataract surgery and intraocular lens implantation.Methods: Eighty patients after cataract surgery with IOL implantation were evaluated. Keratometry examination was performed pre operative, 2 weeks, 4 weeks, and 8 weeks post operative. All of the patients underwent cataract surgery by fornix base flap conjungtiva, 40 patients were performed corneoscieral incision, and 40 patients were performed sclera! incision. All of them was sutured with ethilon 10.0, consist of 6 stichesResults: The corneoscleral incision induced astigmatisms were 0-8.6 D(dioptri), while the scleral incision was 0-8.50 D. The mean astigmatism of the corneoscieral incision 2 weeks post operative was 3.08 D, 4 weeks was 2.72 D, 8 weeks was 2.13 D, while the astigmatism of the sclera! incision 2 weeks post operative was 3.35 D, 4 weeks was 2.32 D, and 8 weeks post op was 1.75 D. There is no statistical significant difference between the corneoscieral and sclera! incision.Conclusion: The mean astigmatism 2 weeks, 4 weeks, and 8 weeks after operation was decreased in both methods of incisions. Despite no statistical significant difference, the mean astigmatism was lower in scleral incision method.Keywords : astigmatism change - keratometry - cataract surgery - corneoscieral incision - sclera! incision
