Abstract

The presence of immunocompetent cells in the respiratory tract and their cellular interactions are undoubtedly crucial, leading to the development of bronchial asthma in hypersensitive individuals. The cells comprise various populations, such as mast cells, basophils, eosinophils, neutrophils, macrophages, dendritic cells and lymphocytes, in which the T-cells are believed to play a central role. It has been demonstrated that CD4+ T-cells consist of CD45RO reactive population and CD45RA, each secreting different cytokines. In terms of cytokine pattern production, these T-cell subpopulations are similar, to those found in the murine system, i.e. TH1 and TH2 cells. The CD45RO+ IL-4 and IL-2 cells produce directly or indirectly that govern the proliferation and differentiation of B-cells to IgE-secreting plasma cells, by which IgE antibodies are responsible for the development of allergic type I reaction. In addition, the number of T-cells expressing activation markers, like IL-2R, VLA-1 and HLA-DR increases in the serum of asthmatic patients, as well as in their bronchoalveolar lavage and bronchial biopsies. Moreover, the CD4+ T-cells produce IL-3, GM-CSF, IL-4, IL-5, IL-6 and IL-10 that induce mast cell differentiation and activation and recruitment of eosinophils. The function of dendritic cells and macrophages in antigen presentation to either naive or activated T-cells should be noted. Finally, immunologic-based therapy for the airways hyperreactivity may be proposed. This concept includes the regulation of certain cytokine production governing TH1 subpopulations, and by eliciting an optimal mucosal IgA response to down-regulate the production of IgE antibodies.Key Words: bronchial asthma - immunocompetent cell interactions - T-cell subpopulations - T-cell cytokines -                 mucosal IgA response

Abstract

Telah menjadi kenyataan bahwa pada umumnya .umur tua membawa risiko lebih besar untuk mendapat penyakit kanker. Hal jig nyata benar dari data tenta.ng penyakit para penderita di rumah-rumah sakit, statistik dan sebab kematian, data mengenai periksa bedah mayat dan data dalam laboratorium-laboratorium untuk pemeriksaan jaringan tubuh.Tidak semua penyakit kanker dengan jenis-jenisnya yang sangat berlainan terjadi hanya pada orang yang telah lanjut usianya, bahkan kita kenal tumor ganas yang khusus timbul pada anak-anak seperti retinoblastoma, neuroblastoma, tumor Wilms atau nephroblastoma dan kanker kulit pada penyakit xeroderma pigmentosum.Sebagai contoh saya ambil data yang ada di Laboratorium Patologi kita selama 5 tahun (1970 sid 1974). Lihat Tabel 1 dan 2 dengan catatan bahwa umur penderita-penderita ini yang didapat dari keterangan-keterangan para dokter yang mengirim sediaan-sediaan untuk pemeriksaan, seperti kita semua ketahui, tidak semuanya tepat. Walaupun demikian secara kasar angkaangka tersebut dapat kita pakai.

Abstract

Chronic renal failure is still a major health problem in medicine world. An adequate treatment could only retard the progress of renal failure and delay the course toward end-stage renal disease, a condition that requires maintenance dialysis or kidney transplantation.Managable factors should always be identified and promptly treated. Dietary therapy may be able to cope with uremic symptoms, perhaps retards the progress of renal failure and improves nutritional status. All above mentioned will improve morbidity and mortality of the patients. A proper antihypertensive regiment not only protect the kidneys through blood pressure reduction but also gives independent renoprotective effects by improvement of renal hffect and electrolyte balance.emodynamics without negative metabolic effect and electrolyte balance.Key Words: end-stage renal disease -- predialytic phase -- conservative treatment -- dietary management -- hypertension

Abstract

Background: Since its introduction in 1974, the Glasgow Coma Scale (GCS) has gained widespread acceptance around the world as a means of assessing the level of consciousness of patients with head injury. The reports have been widely accepted as evidence that the GCS is reliable when used by physicians and nurses.Objectives: To determine the interobserver agreement on GCS between doctor and nurse on assessing the altered consciousness.Methods: Thirty-seven patients with altered consciousness, who were admitted to the Department of Neurology, Dr. Sardjito General Hospital Yogyakarta, were observers by a physician and a nurse. Each patient was assessed separately by the observers. Complete forms were not shown or discussed, but were collected and put in a sealed enveloped. Statistical analysis: The statistics used to measure the levels of agreement was based on the formula developed by Fleiss.Result: The agreement of all items was almost perfect (K range from 0.74 to 0.90; p < 0.0001). Conclusions: The GCS was reliable as an objective tool for assessing altered consciousness.Key words : inter-rater agreement - Glasgow Coma Scale - consciousness

Abstract

The paper presented a brief review of the role of hyperglycemia in the development of complications in the diabetic patient. The chronic diabetic complications tend to occur in patients with longstanding diseases. Althoueh the exact mechanism responsible for the development of each complication is not fully clarified, the [actors such as accumulation 01 soronol derived from hyperglycemia through polyol pathway, glycoproteins, myo-inositol deficiency, glycosylated non-enzymatic proteins, ketogenesis, hyperlipidemia, increased adhesiveness of platelet and hyperviscocity are thought to play significant roles in the pathogenesis. If the degree of metabolic abnormalities can be reduced by dietary manipulation and insulin administration, the rate of development of diabetic complications should also be reduced. This hypothesis has been shown to be correct by several investigators. Others disagree with that. However, whatever the cause of the development of chronic diabetic complications were it is believed that diabetic control is important and that each diabetic patient should be encouraged to learn as much as possible about diabetes mellitus in that they will have a better understanding of the necessity as well as the desire for good control. If complete normalization of blood glucose and other metabolic abnormalities can not be achieved, the chronic diabetic complications will continue to occur. And in the severe cases, the result of the treatment may not influence the nature of the complications. Abstract

Hemorrhagic diathesis in pediatrics, covering the mechanism of hemostasis and the diagnostic and therapeutic aspects, is discussed.Hemorrhagic cases due to various causes are not uncommon in children. It is essential to understand aspects of hemorrhagic diathesis to be able to manage such cases efficiently. The diagnostic approach should be in an ordered fashion. By such a way and by understanding the principles of management, even with limited facilities, we can diagnose and treat many cases and at least recognize difficult cases and act appropriately before referring.Key Words: hemorrhagic diathesis - mechanism of hemostasis - hemophilia - purpura - hypoprothrombinemia

Abstract

Interaction of Tumor-Associated Macrophages (TAMs) with tumor cells gives insights into tumor progression andinto a novel therapeutic strategy. In papillary thyroid cancer, patients with tumors containing TAMs had a betterprognosis than patients without TAMs . In prostatic cancer, the reduced of total TAMs can be used as a novelprognostic marker. In melanoma maligna, high number of TAMs was statistically significant associated with poorresponse to treatment . Abstract

Two cases of pericarditis with effusion admitted to the Department of Child Health, Gadjah Mada University Hospital were presented. The suspicion was based on the presence of pericardial friction rub for the first case, and the presence of cardiac enlargement and liquid line (on X-ray) for the second case. Pericardiocentesis was performed to confirm the diagnosis and for treatment.An accurate etiological diagnosis could not be made due to lack of laboratory facilities. The treatment was based on clinical and pathological findings

Abstract

Cardiovascular disease is the most common cause of death in patients maintained on hemodialysis (HD) of end stage renal disease (ESRD). This study was aimed at evaluating the regression of left ventricular mass (LV mass) and blood pressure among ESRD patients underwent HD. A randomized double-blind placebo controlled clinical trial had been done on 32 patients who underwent HD with acetate. Left ventricular mass was examined with Echocardiography interdialitic time. Fifteen patients (11 males and 4 females) with r-HuEpo therapy (group I) and 16 patients (9 males and 7 females) with placebo (group II) were recruited in the study. The mean age group I was 52.27 ± 14.69 years and Group II: 46.25 ± 9.01 years (95% CI:-2.82 - 14.86; p = 0.117. There was no significant difference in hemoglobin concentration (Hb), hematocrit (Hct), systolic and diastolic blood pressure; LVM at baseline between two groups. After 5 weeks of therapy with r-HuEpo 2000 Unit 2 times/week, the change of mean in hemoglobin concentration in Group I increased: 8.05 ± 1.62 mg/dl to 8.39 ± 2.160 mg/dl and in Group II decreased 7.38 ± 1.2 to 7.13 ± 1.75 mg/dl (p = 0.083,NS), Hct: group I increased 22.54 ± 7.83% to 25.71 ± 6.66%; group II decreased 23.99 ± 4.69 to 21.51 ± 5.29 (p = 0.061,NS) where as systolic blood pressure in Group I increased from 134.615 ± 21.454 mmHg to 140.67 ± 27.12 mmHg, group II increased 144.37 ± 18.24 mmHg to 146,88 ± 26,01 mmHg (p = 0.520, NS); diastolic blood pressure in Group I decreased from 83.076 ± 11.821 mmHg to 79.67 ± 13.43 mmHg, group II 91.25 ± 10.87 mmHg to 90.63 ± 15.69 mmHg (p = 0.046, SI; and the LV mass in Group I reduced from 359.15 ± 107.49 g to 316.32 ± 67.43 g, group II 345.21 ± 89.27 to 340.65 ± 97.84 g (p = 0.434, NS) respectively. In conclusion, after 5 weeks of therapy r-HuEPO tended to reduce left ventricular mass in ESRD patients with maintenance hemodialysis. Left ventricular mass reduction in this patients may be associated with diastolic blood pressure reduction, increase of hemoglobin concentration and increase of hematocrit decrease of end diastolic diameter.Key words: r-HuEPO - left ventricular mass - hemodialysis - echocardiography - end stage renal disease
