Abstract

An observational study on factors influencing the survival of 40 patients with end stage renal failure, who had been chronically hemodialyzed at Sardjito General Hospital was performed. The major causes of end stage renal failure in these patients were: chronic glomerulonephritis (22 patients); Diabetes mellitus (9 patients); renal stones (5 patients) and miscellaneous (4 patients). Seventeen of those 40 patients had poor protein intake (42,5%) while the other 23 patients had sufficient protein intake. Patients having sufficient protein intake proved to have better survival than those having poor ones who died more in the first year from intractable congestive heart failure with sepsis. Meanwhile other factors such as the control of hypertension, causes of end stage renal failure and the age of patients were not proved to influence the patients survival. Survival rates of the patients with end stage renal failure who had chronic hemodialysis at Sardjito General Hospital from 1 January 1986 to 31 December 1991 was 62.2% in the first year and 45% in the second year. Most patients died in the first year from intractable congestive heart failure with sepsis, and might becaused by poor protein intake,Key words : end stage renal failure - chronic hemodialysis - protein intake - survival rate.

Abstract

The relationship between blood pressure and body weight, body height and upper arm circumference was studied in 506 school children, consisting of 227 boys and 279 girls, aged 6-14 years. Blood pressure was measured by using mercury sphygmomanometer. measurements were conducted once in the sitting position, and the subjects were selected at random from 7 000 school children in the district of Sewon, Bantul, Yogyakarta. The results found showed that there are correlations between either systolic or diastolic blood pressure with body weight, body height and upper arm cir cumference, especially in the ages of 15 co 14 years. Abstract

Background: Iron absorption is determined by iron status, heme- and nonheme-iron contents and amounts of various dietary factors that influence iron absorption. Few studies have examined the influence of food to iron absorption but only limited information is available about the net effect of these factors.Objectives: Objective of this study was to know the influence of Ferrous Sulphate @ 300 mg ingestion shortly after and 2 hours after meal on pharmacokinetic profiles of iron in serum sample in 12 women with Hb >_ 12 g/dLMethods: The study was carried out in a cross over design in which each of participants underwent 2pharmacokinetic studies. One tablet of Ferrous Sulphate @ 300 mg was taken by each subject shortly after and 2 hours after standard meal at the first and second study. Series of serum samples were taken at0, 0.25, 0.5, 1, 1.5, 2, 2.5, 3, 4, 6, 8, 10, and 12 hours after iron administration. The concentrations of ferric iron in serum were measured using Vitors Fe Slides System, while pharmacokinetics parameters were calculated using a non-compartmental method.Results: Pharmacokinetic parameters obtained from the first and second study were compared using t test with the following results (meant SEM): C,,, 275.92 ± 28.97 & 284.58 ± 30.44 ug/dL, T.3.50 ± 0.29& 3.50 ± 0.38 hours, K. 0.3235 t 0.0011 & 0.3816 ± 0.0010 hours-, Ka, 0.1138 ± 1.5696 & 0.1152 ±1.3331 hours T,201 7.80 ± 1.45 & 7.03 ± 1.02 hours and AUCo-12 2493.43 ± 397.54 & 2331.15 ± 283.64 ug/dL.hours. No difference of all pharmacokinetic parameters were detected by intervention typeConclusions: It can be concluded that after ingestion of Ferrous Sulphate @ 300 mg in women with Hb 12 ug/dL shortly after and 2 hours after meal did not show significant diferrences on pharmacokinetic profiles of iron in serum sampleKey words: iron absorption - pharmacokinetic - ferrous sulphate - absorption rate - Cmax

Abstract

In this experiment the fastigial projection to the paramedian reticular nucleus was investigated by means of retrograd axonal transport of horseradish peroxidase (HRP) in the rabbit cerebellum. Following HRP injection in various regions of the paramedian reticular nucleus of the medulla oblongata, labelled cells were found in the fastigial nucleus on both sides with a greater preponderance on the contralaterai side. Most of the labelled cells were distributed in the rostra] and medial regions of the fastigial nucleus, after HRP injection in the area of the paramedian reticular nucleus, proved that the fastigial neurons project their axons to the paramedian reticular nucleus.Key Words: fastigiobulbar pathways - horseradish peroxidase - fastigial nucleus - paramedian reticular nucleus - medulla oblongata

Abstract

Background: Neonatal sepsis is known as a leading cause of neonatal morbidity and mortality. It is hard to diagnose clinically sepsis due to its nonspecific manifestations. Blood culture has been the gold standard for neonatal sepsis. Unfortunately, it takes time to get the result, it is costly, and it is not easily performed. Buffy coat smear with Gram stain in attempt to diagnose neonatal sepsis is less costly and easily performed.Objective: To know the diagnostic value of buffy coat smear with Gram staining to diagnose neonatal sepsis.Methods: This cross sectional study was carried out at the Maternal-Perinatal Installation of Dr. Sardjito Hospital, Yogyakarta, between October 131, 2003 to March 3111, 2004. The inclusion criteria was clini-cally sepsis neonates (0-28 days old). Neonates with blood disorder, congenital anomaly, or had got antibiotic previously were excluded from this study. The buffy coat smear with Gram staining was examined and than was blindly validated with blood culture.Result: There were 127 neonates enrolled in this study. Buffy coat smear with Gram stain to diagnose neonatal sepsis produced sensitivity 77% (95% Cl, 65-88), specificity 85% (95% Cl, 77-93), positive predictive value (PPV) 78% (95% Cl, 67-90), negative predictive value (NPV) 84% (95% Cl, 76-92), positive likelihood ratio 5,24 (95% Cl, 2,98-9,24), and negative likelihood ratio 0,3 (95% Cl, 0,16- 0,45).Conclusion: Buffy coat smear with Gram stain was sufficiently accurate as diagnostic test for neonatal sepsis. The diagnostic value of buffy coat is expected to be useful for clinicians to predict neonatal sepsis earlier and guide them in making decision for clinical management.Keywords: neonatal sepsis - diagnostic test - buffy coat - gram positive staining

Abstract

Fifty two patients with typhoid fever were studied. Hematomegaly occurred in 7I % of the patients. Hepatic enlargement was noted in the first week as well as during the course of the disease but the maximum incidence was found in the second week. Almoimal liver function tests were found in both groups, with and without hepatomegaly. Jaundice was present in 2 cases.Biopsies showed focal necrosis, cellular infiltration. Kupffer cell proliferation and hydropic degeneration. Typhoid nodules were demonstrated in 2 out of the 7 cases.The prognosis of typhoid fever with liver abnormalities is good and requires no further treatment.Key Words: hepatomegaly - abnormal liver function test - jaundice - liver biopsy - nonspecific reactive hepatitis
