Abstract

The major histocompatibility complex (MHC) class II antigens were originally described to be expressed on the surface of B lymphocytes, macrophages, skin Langerhans cells (LC) and dendritic cells (DC). Surprisingly, the class Il molecules of MHC are also found in the intestinal epithelium.The distribution and expression of la molecules on intestinal epithelial cells depend on the continuous exposure of antigen and the presence of certain stimuli, like interferon-gamma, inflammation process in the gut and graft-versus-host disease. The function of class II molecules on intestinal epitheliurn may be different from those molecules on professional antigen presenting cells. Luminal antigens which are processed by enterocytes will be delivered to CD8 cells in the vinous epithelium and led to immunosuppression, whereas antigen uptake by M cells in the Peyers patches will be presented to CD4 cell populations, and initiate the immune response.Key Words : antigen processing-MHC molecule-enterocytes-Peyers patches-mucosal immunity

Abstract

Timolol maleat, a new beta-adrenergic blocking agent, lowers intraocular pressure by decreasing actuous productiOri. It available in 0.25%;and 0.50% solution as an eye drop. The systemic effect of the timolol maleat 0.26% eye drops were observed among healthy medical students: The pulse rate and systolic blood pressure were insignificantly decreased but diastolic blood pressure was, insignificantly increased.Key Words: timolol maleat -- beta-adrenergic blocking agents -- blood pressure -- pulse rate -- intraocular pressure

Abstract

Background: Pregnant women in malaria-endemic areas are uniquely susceptible to infection with Plasmodium falciparum and this susceptibility is greatest during first pregnancies. Malaria causes serious complications in pregnant women, especially in those who have a low level of acquired immunity before pregnancy.Objective: to assess the risk predictors for malaria during pregnancy and the role of chloroquine in low endemic area of malaria in Jepara districtDesign: A longitudinal study of all pregnant mother was carried out in Batealit and Mayong I subdistricts of Jepara district between June 1997-August 1999. All pregnant women were screened for peripheral parasitaemia through active surveillance. Women who had parasitaemia were treated with chloroquine for 3 days. Blood smears were then examined on days 4, 7, 14 and 21 after completion of the chloroquine course. All women irrespective of the blood smear results at enrolment were followed up once every two weeks until delivery.Results: Among 3099 pregnant women enrolled in the study, only 58 women had malaria infection, giving an incidence rate of 1.9/100 pregnant women. In this study low income is a significant risk predictor for malaria (OR= 11.03; 95%Cl: 3.91-31.08). Women who had reported a history of malaria or history of taking antimalarial drugs 6 months before their last menstrual period (LMP) showed an increased risk of developing malaria during pregnancy (OR= 10.56; 95%Cl: 4.57-23.72 and OR = 10.90; 95%Cl: 4.4825.61) respectively. Among those infected by P.falciparum and P. vivax and treated with chloroquine, complete parasite clearance was found in day 7.Conclusions: This study shows that low income, history of malaria within 6 months before LMP and history of taking antimalarial drugs within 6 months before LMP are best predictor for malaria in pregnancy in low endemic area. Abstract

The harniful health effects of tobacco smoking have been reported by many experts. Death rates are unifomily higher among smokers than among non-smokers, in both sexes and whatever the age at death. Cigarette smoking is also considered to be responsible for the great majority of cases of lung cancer. There is evidence that the influence of smoking in ischaemic heart disease is not only independent, but also synergistic with other risk factors such as hypertension and high blood cholesterol. Surveys in many countries show that smokers cough more than non-smokers and produce more sputum. Cigarette smoking acts independently and synergistically with other risk factors contributing to bronchitis.Many experts agree that the smoking problem is now a worldwide epidemic and causing serious diseases. Therefore, studying the problem associated with tobacco should include, specifically the harmful health effects of smoking.The study was carried out in Yogyakarta among senior high schools student. The aim of the study is to test the hypothesis that there is a relationship between neurotic tendency with smoking. Randomized sampling method were done for the analytic-descriptive design of the study.Results showed that there is a significant difference between group of smokers and non-smokers according to the tendency of neurosis (p<0.05). Results also showed that there is a correlation between the tendency of neurosis and smoking (Contingency-Coeff.=0.44)Key Words: smoking - smoker prevalence - smoker relatives - neurosis - psychiatric intervention

Abstract

The key of the development of gastric ulcer is that the unionised substances or the lipid soluble substances which can cross the lipoprotein part or the cell membrane cause a rapid back diffusion of H-ions from the lumina I part of the stomach to the mucosa] layer, and a rapid Na-ions secretion. It causes cell damage and histamin release. Histamin causes mucosal edema and increases gastric acid secretion. Strong and repeated insults of histamin release may develop mucosal erosions and then ulcers.It is suggested that the development of gastric ulcer is promoted among others by acid hypersecretion, mucous hypersecrerion, delayed mucosal cell turn-over, SIgA deficiency, rnucosal exposure to mucosa] damaging substances, and prolonged and repeated mucosa] ischemia.The pain of gastric ulcer is probably due mainly to the action of acid or other irritants on a sensitive region of the mucous membrane.Key Words : gastric mucosal barrier - I-12 receptor - cephalic phase of gastric secretion - emotional stress - cigarette smoker

Abstract

Fetal monitoring was carried out in 63 high risk pregnancies at Dr. Sardjito Hospital. Cardiotocographic examination (CTG) was used to evaluate 20 patients, and biochemical fetoplacental test. Urinary estriol (E3) and serum hPL were used to evaluate 43 patients. There were patients with postterm, preeciampsia, eclampsia and prolonged labor. Diagnostic test to predict fetal distress was analyzed for both CTG and biochemical test. CTG had a sensitivity of 87.5%, specificity of 58.3%, and accuracy of 70%. Urinary estriol level had a sensitivity of 25%, specificity of 89.7% and accuracy of 83.7%. The sensitivity, specificity and accuracy of serum hPL level were 25%, 92.3%, and 86%, respectively. There was correlation between relative birth weight and serum hPL level (r-z 0,3363), relative birth weight and urinary estriol (r=0.3427).Biophysical assessment Is more favourable compared to hormonal measurement in the detection of fetal distress.Key words: biophysical - hormonal assessment - fetal distress

Abstract

Glaukom merupakan salah satu dari lima penyebab utama kebutaan di Indonesia. Timolol merupaltan obat glaukom yang belum lama dipergunakan di Yogyakarta. Timolol tergolong betablocker yang dapat menurunkan tekanan bola mata dengan cara menghambat produksi humor aquosa pada corpus ciliare.Telah dilakukan percobaan pada orang normal kelompok muda berumur antara 10-20 tahun, kelompok normal lanjut usia berumur 40 60 tahun, clan kelompok penderita glaukom primer. Pada ketiga kelompok tersebut diberi tetesan maleat timolol 0,5%. Terjadi penurunan tekanan bola mata yang jelas setelah satu jam. Dua jam setelah tetesan masih terjadi penurunan tekanan bola mata yang nyata pada kelompok normal muda usia dan kelompok penderita glaukom primer. Dengan perhitungan statistik penurunan tersebut ternyata bermakna.Key Words: timolol maleate - beta blocker drugs - glaucoma - intraocular pressure - tonometer

Abstract

Racial discrimination is usually elicited by social, economic, religious or political reasons, and then justified by quasi-scientific arguments, c. q. by using anatomical and anthropological data.Almost all parts of the body have been used to justify racial discrimination, particularly easily visible physical traits. Not infrequently data are unhesitantly manipulated•in order to arrive at conclusions in tune with racist ideas nurtured. Physical traits are purposefully correlated with mental and behavioural traits to give a negative image of the race disliked.The progress of science seems to be not proportional to the decrease in racial discrimination. The number of anthropologists and the history of anthropology in a country do not influence the extent of racial discrimination. Scientific data will always be used for pragmatic ends in. intergroup socioeconomic competition. To abolish or substitute the term race or the race concept will not ne• cessarily abolish racial discrimination. It is hoped that in the future groups which have experienced discrimination will not resort to retaliation and use the same methods, even if the biological concept of race is maintained.Key Words: racism -- human races -- miscegenation - racial characteristics - heterosis
