Abstract

This study was undertaken to compare the causes of abnormal vaginal bleedings in 1964 before the family planning program in Indonesia was introduced and in 1983 when this program was widely accepted:The samples were taken from the files of the Department of Pathology, Gadjah Mada University Medical Faculty, during the period of July 1 — December 31 in the years 1964 and 1983.The results suggested that in 1964 these bleedings were mainly caused by conception (52.9%), while in 1983 only 19.7% were caused by conception. In 1964 most dysfunctional bleedings were found in the perimenopause groups and in 1983 in the child bearing groups.Extended studies utilizing larger samples could provide further information about the role of ex - ogen hormons for contraception and therapy or hormons for cosmetic purposes, and also about endogenic factors; it would be necessary to work together with other specialists as well as psychologists.Key Words: abnormal vaginal bleeding - endometrial curettage - family planning program

Abstract

Abstract

Five major structural types of immunoglobulin in man can be distinguished. i. e. immunoglobulin G (abbreviated to IgG), IgM, IgA, IgD and IgE. Each type of immunoglobulin can synthesize specific antibody according to the inducing antigen. The specificity of immunoglobulin molecule is found in the antigen-binding fragment.Actually the specificity and the diversity of the immunoglobulin is controlled by the gene in the chromosome. The diversity and where the assembly of immunoglobulins is held are described.Key Words: immunoglobulin - antigen-binding fragment - central dogma - protein synthesis - germ line theory

Abstract

A 10 year old girl suffering from esotropia for distant vision since she was 4 years old has been reported. Distant correction of S+3 to S + 4 dioptries resulted in orthophoric eye whereas esotropia still developed during the close work. By adding S+3 dioptries for close work this esotropia disappeared. Until now the patient has been using bifocals with comfort. Accommodative esotropia is a type of esotropia accommodative factor. This condition can be caused by hypermetropic refractive anomaly alone so that it is called refractive accommodative esotropia. Consequently corrective measure of underlying refractive errors will correct the eye deviation. Meanwhile esotropia that manifests during the accommodation alone, such as In close work and not due to refractive error is called non refractive accommodative esotropia. This type of esotropla is due to a higher AC/A ratio. Accommodative esotropia combined type is a combination of these two types of esotropia mentioned above. In this type, esotropia still occurs in close work (accommodation) although its refractive anomaly has been corrected. In this case, addition of positive lens 2-3 dioptries for close work (near vision) will cure this accommodative esotropia. The patient needs a bifocals.Key words : accommodative esotropia - non accommodative esotropia - AC/A ratio - distant correction - addition for close work.

Abstract

The effectiveness of trabeculectomy has been studied in 65 primary glaucomatous patients consisting of 30 men and 35 women. The trabeculectomy was able to control the intraocular pressure in 89% of cases and it improved visual acuity in 67.69% of cases. Based on the iridocorneal angle appearances, the cases of primary closed angle glaucoma were more frequent than those of primary open angle glaucoma. The most frequent complication found was hyphaema which occured in 23.08% of cases, but it I usually disappeared in 7 days post-operatively. Anterior uveitis was the second complication and this happened in 6.15% of cases. Other complications such as flat anterior chamber and cataract were very rare. Conjunctival fistula, bleb rupture and intraocular infection were not found here. Key Words: trabeculectomy - glaucoma - control of intraocular pressure - visual acuity improvement - hyphaemia

Abstract

A study on operative delivery was carried out during the period of 1988 to find out the fetal outcome and some of the risk factors. A cohort study was used, consisting of the exposed group (operative delivery, 212 cases) and the control group (spontaneous delivery, 212 cases) selected randomly using a computer generated random number. Only singleton pregnancy, gestational age 28 weeks or more or birthweight 1000 g or more were included into the study. All babies were followed up during their neonatal period in the neonatal care unit. Neonatal morbidity was defined as asphyxia, birth traumata and diseases such as fever, vomiting etc. found during the early neonatal period. Data were processed with IBM personal computer using version 2.0 SPSS program. t-test, chi–square test and relative risk were statistical analyses used in this study.Result showed that the incidence ofoperative delivery during that period was 24.62%. The most common one was cesarean section (39.2%) and the least common was internal podalic version and laparotomy (1.9%). Age more than 30 years, parity more than two (gravidity more than three) and referral cases seemed to be the risk factors for termination of delivery by an operative procedure. The risk of deliveringasphyxic babies was much more common in the exposed group compared to the control. The rate of severely asphyxic babies was 50.5% compared to only 5.9% and the rate of slight asphyxia was 38% compared to 11.3% respectively for the exposed and control groups. Mean one minute Apgar score was 3.87 vs 7.29 and mean five minutes Apgar score was 6.89 vs 9.15. The number of babies suffering from one or more morbidities were 94.0% in the exposed group vs23.6% in the control group. There was a significant difference in term of perinatal mortality but not in neonatal mortality, although it seemed to be clinically important. The study concluded that although fetal outcome in the operative delivery was worse, but with careful maneuver and intensive neonatal care the risk of neonatal mortality could be minimized.Key Words: high-risk labor - operative delivery - perinatal morbidity and mortality - asphyxic babies Apgar score

Abstract

Pharyngeal tuberculosis is a rarely diagnosed case or it is frequently ignored and confused with other diseases such as midline granuloma and fungal infections.Pharyngeal tuberculosis generally follows pulmonary tuberculosis while the incidence of pulmonary tuberculosis is still high. This report showed a male with distinct symptoms such as painful swallowing for 5 months and had small and shallow ulcers with rough edge and granulation tissue at the base, pale mucosa at the dorsal wall of pharynx, tonsils and soft palate. The differential diagnosis were midline granuloma, fungal infection and pharyngeal tuberculosis. The appropriate diagnosis of pharyngeal tuberculosis could be confirmed by the finding of a positive acid fast bacteria test or histopathological examination which showed pharyngeal tuberculosis. The patient was referred to the Pulmonary Disease Unit for treatment. After one month treatment the clinical features were better.Key words : tuberculosis - midline granuloma - fungal infection - pulmonary tuberculosis

Abstract

This articles described various definitions of peace and peace science, and put special emphasis on operational ones which are relevant for our survival. Our main and immediate concern is the threat of a nuclear war which endangers the global ecosystem. Several reasons are brought forth why this problem has not been widely discussed in the Third World. Furthermore, the articles pointed out that old ideas on the biological necessity and unavoidability of war are obsolete and incorrect, and the assumption that nuclear arms race is just a political game to preserve peace is no longer valid. Also, the possibility of human survival after a nucleanholocallst is categorically rejected. Finally, the responsibility of the university in the preservation of peace is proposed. Key Words: peace studies - structural violence - nuclear warfare - human survival - peace-oriented education
