Abstract

Current approaches to human gene therapy focus on insertion of a desired gene into autologous cells such as fibroblasts. hepatocytes, bone marrow stem cells or lymphocytes. An alternate strategy to gene therapy through genetic modification of the patients own cells is to implant into different recipients the same engineered cell line under immunologically isolated conditions. This in vivo study shows that human growth hormone (hGH) as a novel reporter gene product from microencapsulated genetically modified mouse cells is detectable in the circulation of allogenic recipient mice. On day 3 of post implantation no hGH was detected, 3 ng/ml of hGH was detected on day 12 and increased during the next 9 days up to 10-fold. The high level of hGH was maintained during days 21 - 33 post implantation and went down afterward to the baseline on day 45. No hGH was detected in the control mouse implanted with the same amount of free recombinant cells.Key Words: Gene therapy - autologous cells - genetic modification - recombinant cells - lymphocytes

Abstract

Two cases of glaucomas were found by papil disc examination. In the elder patient, more than 40 years old, glaucoma was diagnosed by tonometric examination. In the younger patient, less than 40 years old, suspected glaucoma was diagnosed by water drinking test. Abstract

A literature study on the management of premature rupture of the membrane (PROM) is carried out to find out the best method and its impact on maternal and neonatal morbidity.PROM is defined as rupture of the fetal membrane before the onset of true labor. The incidence of PROM varies from 0,7 to 10%. Premature labor and its consequences to the newborn as well as maternal and neonatal infections are complications attributed to early rupture of the membrane.Conservative management for PROM before 37 weeks of gestation is a method used by most obstetricians. Patients should be staying in bed as long as the amniotic fluid is still coming out, no vaginal examination performed, and she should be closedly monitored for signs of infection and prophylac tic antibiotics given. Once the signs of i nfection develop, termination of pregnancy should be carried out and massive antibiotics should be given. Risk of infection becomes evident if rupture of the membrane goes beyond 12 to 24 hours so that prophylactic antibiotics should be given before 12 hours of rupture. Most of the cases will go to labor before 48 hours, although in some cases labor can be postponed for 19, 33, 39, 44, and 55 days. If labor can be postponed for more than 48 hours, the risk of suffering From respiratory distress syndrome (RDS) in neonates is getting lower.The management of premature rupture of the membrane after 37 weeks of gestation depends on the condition of the cervix. Patients with ripe cervix can be induced immediately, but patients with unripe cervix should be managed conservatively.Key Words: PROM - RDS - prophylactic antibiotics - perinatal mortality - maternal infection

Abstract

Stool specimens for microscopic examination collected from orphanages in Yogyakarta indicated the species and the overall percentages of prevalence of human intestinal parasites. These samples were examined in our laboratory by direct smear and concentration methods. Abstract

There has been a growing number of evidence that calcium antagonists provide a salutary effects in preserving kidneys against acute renal ischemia in patients at increasing risk. Their beneficial effects on cellular and mitochondrial calcium may explain the effects on renal hemodynamics and metabolics. It seems, that they do not directly vasodilate kidney vessels but alter the response towards vasoconstrictor agents. This effect may mediate diuretic and natriuretic effect of calcium antagonists. Effects on tubuloglomerular feedback and autoregulation mechanism in the kidneys play an important role in this regards. The above mentioned effects may provide a significant kidney protection and retard the progress of kidney disease.The results of recent studies show that calcium antagonists exert potentially. beneficial effects on several clinical settings including radiocontrast-induced nephrotoxicity and transplant-associated renal insufficiency, particularly cyclosporine A nephrotoxicity and ischemia during harvesting of kidneys.Key words: kidney disease - calcium antagonists renoprotective - hemodynamics - kidney function

Abstract

Based on technique of cancer registration there are three types of cancer registry. These types are: 1.pathology department-based cancer registry 2.hospitl -based cancer registry 3. population-based cancer registry. Each of these types has its own method and characteristics. The aim of pupulation-based cancer registry is ib evaluate and control the trends of cancer in the community. This field has very close relation with cancer epidemiology. The prospect of the population-based cancer registry in Indonesia depends on various conditions. It is concluded that the Special Province of Yogyakarta could fulfill the conditions for the purpose of developing population-based cancer registry in Indonesia, while Bali is an ideal place for population-based cancer registry in Indonesia. Therefore, other conditions are needed for this purpose. Abstract

Background: hemophilia is the most common hereditary bleeding disorder in children, and can create challenges in the treatment of complicated bleeding episode. Circumcision and other surgical procedures in persons with hemophilia require anticipatory management plan to prevent and treat bleeding prior to, during and following surgery. Prolonged bleeding can be managed by supplement therapy with Factor VIII, administered intravenously by either bolus (BI) or continuous infusion (CI) to achieve hemostatic plasma Factor VIII levels.Objectives: the aim of this study was to compare plasma activity level of factor VIII in relation to the method of intravenous administration.Methods: commencing 4 days post circumcision, five sequential measurements of plasma Factor VIII levels were done at 6 hourly intervals in 4 boys with hemophilia A who received intravenous Factor Vill, either by 81 or Cl.Result: four patients given continuous infusion showed lower level of Factor VIII compared to bolus therapy. Conclusion: bolus therapy has more increased level of Factor VIII compared to continuous infusion. Bleeding was not found in both therapies (Cl or BI). Key words: AHF - continuous infusion - bolus - plasma level

Abstract

Galur sel hasil rekayasa genetik yang berada dalam kondisi terisolasi secara imunologis diharapkan dapat diimplantasikan kepada beragam penerima. Dengan demikian tersedia strategi altematif bagi terapi gena, yang umumnya bergantung kepada modifikasi genetik terhadap sel dari penderita sendiri. Penelitian ini mengamati ekspresi hormon pertumbuhan manusia (HPM) dari hasil rekayasa genetik sel mencit yang ditempatkan di dalam membran imunoprotektif secara in vitro.Sel mencit Ltk" ditransfeksi dengan plasmid pNMG yang mengicode gena HPM dan gena resistensi terhadap G418. Sel-sel yang mensekresi HPM dalam jumlah banyak dipilih untuk dibungkus dengan mikrokapsul berdiameter 300nm yang terbuat dari bahan alginate — polylysine — alginate. Prosedur pengkapsulan tidak mengganggu kelangsungan hidup sel, terbuicti bahwa lebih dari 95% sel masih tetap hidup seusai pengkapsulan. Sepanjang percobaan selama 3 minggu angka sel yang hidup dapat dipertahankan sekitar 90%, jumlah sel di dalam mikrokapsul meningkat lebih dari sepuluh kali lipat, dan HPM di dalam media juga meningkat konsentrasinya. Membran mikrokapsul cukup permeabel, dari perhitungan pada awal percobaan lebih dari 80% HPM yang disekresi dapat berdifusi ke dalam media. Dapat disampaikan bahwa produk gena baru dari galur sel baku hasil rekayasa genetik dapat dihantarkan melewati mikrokapsul dari bahan alginate-polysine-alginate secara in vitro paling tidak selama tiga minggu. Selain itu, prosedur yang dikerjakan cukup memadai bagi kelangsungan hidup sel, lingkungan dalam mikrokapsul dapat mendukung pertumbuhan sel, dan membran kapsul bersifat permeabel bagi produk gena baru.Key words: genetically modified cells-plasmid pNMG-human growth hormone-immuno protection-microcapsule
