Abstract

Presbyopia is a problem of old people. Usually asthenopic symptoms begin when the patients approach 40 years old. Refraction anomalies of hypermetropia and myopia influence directly the measure of reading spectacles, so that both these anomalies influence the possibility of asthenopic symptoms. Data from Out•Patient Department of Ophthalmology, Dr. Sardjito Hospital, during 6 months were 92 presbyopic•emmetropic, 15 presbyopic-myopic, and 75 presbyopic-hypermetropic. The asthenopic symptoms which excite patients to go for medical consultation were not in conformity with the theory concerning asthenopic symptoms. Abstract

The paper presented a brief review of the role of hyperglycemia in the development of complications in the diabetic patient. The chronic diabetic complications tend to occur in patients with longstanding diseases. Althoueh the exact mechanism responsible for the development of each complication is not fully clarified, the [actors such as accumulation 01 soronol derived from hyperglycemia through polyol pathway, glycoproteins, myo-inositol deficiency, glycosylated non-enzymatic proteins, ketogenesis, hyperlipidemia, increased adhesiveness of platelet and hyperviscocity are thought to play significant roles in the pathogenesis. If the degree of metabolic abnormalities can be reduced by dietary manipulation and insulin administration, the rate of development of diabetic complications should also be reduced. This hypothesis has been shown to be correct by several investigators. Others disagree with that. However, whatever the cause of the development of chronic diabetic complications were it is believed that diabetic control is important and that each diabetic patient should be encouraged to learn as much as possible about diabetes mellitus in that they will have a better understanding of the necessity as well as the desire for good control. If complete normalization of blood glucose and other metabolic abnormalities can not be achieved, the chronic diabetic complications will continue to occur. And in the severe cases, the result of the treatment may not influence the nature of the complications. Abstract

The aim of the diabetic treatment is to restore glucose intolerance to normal. There are abundant evidence that chronic hyperglycemia, from whatever cause, leads to chronic complications, such as angiopa thy, retinopathy, nephropathy and neuropathy. Prophylactic treatment should include good diabetic control and avoidance of other factors involving pathogenesis of atherosclerosis, e. obesity, smoking and hypertension. To achieve this aim, all diabetics, regardless of their age or the severity of their diabetes, must take an active role in managing their disease, and they must have adequate information about the importance of dietary management and must be instructed in suitable diet planning. Diet remains the cornerstone of the treatment of diabetes in adults. Meal-schedule, caloric intake and the composition of the diet must be a relative constant, against which is computed the necessary medication and the individuals exercise habits.Education of diabetic patients and their families should begin at the time of diagnosis. In long term diabetes education, the basic concepts of management should be reviewed together with patients during clinic visits at stated interval.Ketonuria usually means that insulin will be necessary.Key Word: diabetes mellitus — home glucose monitoring — dietary management — glocuse intolerance — diabetic education.

Abstract

Fracture of the penis is a rupture of the corpus cavernosum due to blunt trauma. In the West the penile fracture is commonly resulted from trauma in sexual intercourse but in the Middle East and Mediterranean is predominantly due to masturbation. The other causes are unconscious nocturnal manipulation or fall onto the erecting penis. We present a penile fracture case, one of the less frequent urological traumas, referred to the Sardjito Hospital. The report illustrated a 23 year old circumcised male, who had suffered from penile fracture while his penis was erecting. He was referred to the hospital by a general practitioner two days after the incident. Immediate surgical repair was done with the indication of specific history and severe hematoma in the penis as the clinical presentations. The patient was discharged on day 8 postoperation. A mild hematom was present in the wound and no sign of infection.Discussion about penile fracture written in the literature by authors is presented.Key words: penile fracture, causes, urologist problem, surgical repair.

Secara global dapat kita katakan, bahwa data yang diperoleh dari catatancatatan dalam status-status itu secara kwalitatif adalah konsisten dengan landasan teoretis, tetapi secara kwantitatif tidak dapat kita nilai, karena data perbandingan kurang atau tidak ada. Data khusus untuk P.D.R. tidak kita ketahui, dan ketemendapatkan gambaran perbandingan yang lebih lengkap, lebih valid serta reliable perlu diadakan studi klinis dan epidemiologis lebih lanjut tentang P.D.R. dan depresi pada umumnya.Dalam hubungan itu alangkah baiknya apabila kita dapat meningkatkan observasi kita ke arah pengukuran kwantitatif, agar mendapat data obyektif yang dapat dibandingkan secara nasional dan internasional. Abstract

Levels of blood Hb, glucose and lactic acid were estimated two times to 49 athletes of badminton and tennis (13-27 years old). The estimations were done before and after physical performance test. The loads of physical performance test for the subjects were different to each other, depending on their capacities. The changes of Hb concentration were used to correct the changes of glucose and lactic acid concentrations due to the changes of blood volume. The Hb concentrations of 35 subjects increased and the rest of 14 decreased; they were called group A (n=35) and group B (n=14) respectively. When blood glucose concentrations were observed from group A, 24 of them decreased (group At) while the remaining 11 increased. After it had been adjusted to the changes of blood volume, the blood glucose concentration of 4 among the 11 persons (group A2) became lower, while the other 7 subjects remained high (group A3). Blood glucose concentrations of group B increased even after adjusted to the changes of blood volume. The lactic acid concentrations of all subjects also increased after adjusted to the changes of blood volume.Key words: athletes blood haemoglobin glucose lactic acid physical performance

Abstract

Opioid receptor can be classified as p., 8, and K-opioid receptor that widely expressed in the CNS. The development of selective receptor agonist and cloning of each receptor have contributed greatly to our increasing knowledge of the neuropharmacological profile of each opioid receptor type. This review focuses on the functional interaction among these opioid receptor types that contribute to opioid dependence especially in psychological dependence. Several lines of evidence provide arguments to support substantial roles for p-opioid receptors and the possible involvement of 8-opioid receptors in the development of physical and psychological dependence on morphine. Many studies have pointed to the mesolimbic dopaminergic system projecting from the ventral tegmental area to the nucleus accumbens as a critical site for the initiation of psychological dependence on opioids. It is important to note as the broad existence of opposing interaction between p/8-, and K-receptors in the central nervous system. The activation of Kopioid receptors leads to the suppression of unpleasant p.- or 8-mediated side effects such as withdrawal and rewarding effect. Considering the functional interaction among opioid receptor types, the co-administration of morphine-like compounds with K-receptor agonists lead suppressing the side effects of p-opioid receptor agonist that benefit for the pain management.Key words: Opioid receptor - receptor types - psychological dependence - morphine
