Abstract

The clinical impression of association between hypertension and diabetes mellitus has not been consistently confirmed by epidemiological studies. The variety of methodological problems in this area of research includes the choice of appropriate patients and control group, the measurement techniques used, and consideration of many possible confounding variables.Hypertension in diabetes mellitus patients contributes substantially to the increased cardiovascular morbidity and mortality, and should be actively treated as accurately as diabetes mellitus itself. The first therapeuticstep is reduction of overweight, saltintake, and stopsmoking; the omission of interfering drugs and adequate education. Prescription of diuretic drugs or alternatively of calcium antagonists, vasodilators of hydralazin or prazosin. The adds of minoxidil or cap topril to the previousdrugsshouldonlybe taken after evaluation of the overall situation. Abstract

Previous studies showed that susceptibility to leprosy and antibody response toward M. leprae in Javanese population was under control of HLA-DR alleles. To investigate whether this susceptibility was also associated with HLA-DQ, the study had been continued with phenotyping of HLA-DQ alleles and antibody assay to the same population which consisted of 79 leprosy patients, 41 tuberculoid (TT/BT) and 38 lepromatous (LL/BL) type, and 50 healthy controls. The HLA-DQ typing had been performed by using a sequence specific oligonucleotyping (SSO). method, while the anti M. leprae antibody had been tested by ELISA and INHIBITION-ELISA.The results show that HLA-DQB501 is associated with leprosy, either tuberculoid or lepromatous type (OR 3.27; 95% Cl 1.42-7.60). When all HLA-DQ1 alleles are analyzed, a significant association is found only with lepromatous leprosy (OR 9.18; 95% Cl 1.89-86.30). IgG antibody anti 36 kD M. leprae is found to be associated with HLA-DQ102. The level of this antibody is higher in HLA-DQ102 positive individuals compared to those negative one (P). No correlation is found between HLA-DQ alleles and the seropositivity of either IgM or IgG.In conclusion, the susceptibility to leprosy in this population is also controlled by genes in HLA-DQ locus. This study also supports the previous findings that HLA-DQ1 is a universal marker for the susceptibility to lepromatous leprosy, while the infection with M. leprae per se is not controlled by HLA genetic factor.Key words : leprosy - M. /eprae - genetic factor - HLA-DQ - HLA-DQ1 - HLA-DQB501

Abstract

To evaluate the clinical efficacy of rooming-in program, sixty one term-newborn infants with normal deliveries, birth weight 2500-4000 g, Apgar scores 5 minute were studied. The infants were randomly assigned into two groups, 31 rooming-in and 30 not rooming-in group (mother separated from the newborn for 3 days duration). In the third day of life, there were no statistically significant differences in both groups on serum bilirubin levels (p>0.05), cumulative weight loss (p>0.05) and incidence of hyperbilirubinemia (p>0.05). The intensity of hyperbilirubinemia was not related to weight loss (p>0.05). The episode of gastroenteritis was non-existent in the rooming-in group whereas two infants in the not rooming-in group suffered from it. Abstract

This article discussed the existence or non-existence of the right to die. Death has various facets, namely the biomedical, social, historical, moral and religious. Man has the right to live or not to be killed, but not the reciprocal right to die or to kill, although in borderline cases this is debatable. The right to die is executed by way of suicide and voluntary euthanasia.Suicide is committed due to hopelessness, shame or guilt feeling, terminal illness, loyalty to a dead person, and for political reasons. It can be performed by an individual, a pair (suicide pact), a family, a group or en masse. The medical profession should prevent all suicidal attempts and help the surviving victim, since it is its duty to prolong life and ameliorate suffering.Euthanasia is either positive or negative (not infrequently, and inaccurately, termed active or passive), voluntary or involuntary, direct or indirect, and by not implementing extraordinary or ordinary procedures in life saving. Reasons are put forward why euthanasia is condoned or condemned. The moral difference between the two forms of euthanasia are questioned by some serious ethicists. Conditions for negative euthanasia are presented if it is performed selectively, for which the demand is unfortunately real.Key Words: right to die-euthanasia-suicide-patient rights - bioethics

Abstract

Diagnosis sebagian besar limtoma gastrointestinal hanya dapat ditegakkan dengan laparotomi. Duapuluh empat penderita limtoma gastrointestinal primer diteliti. Penderita dikelompokkan menurut umur, jenis kelamin, keluhan, adanya simtom B (demam, penurunan berat badan, dan keringat malam), lokasi tumor, tindakan bedah, diagnosis histopatologis menurut International Working Formulation, dan terapi.Dari 16 orang penderita laki-laki, umur antara 6,5 - 76 tahun dengan rerata 44,7 tahun, lama keluhan pada 10 penderita tidak Iebih dari 6 bulan, hanya terdapat 8 penderita dengan simtom B.Lima betas penderita dengan keluhan massa pada abdomen, 5 dengan gangguan pola pencernaan, 4 dengan obstruksi, dan sakit perut pada 10 penderita. Biopsi laparotomi dikerjakan pada 16 penderita dan reseksi hanya dapat dikerjakan pada 6 penderita. Dua penderita dilakukan apendektomi karena gejala yang mirip dengan apendisitis. Menurut International Working Formulation, 17 penderita dengan derajat keganasan sedang, 5 dengan derajal keganasan tinggi dan 2 dengan derajat keganasan rendah. Hanya 9 penderita mendapat kemoterapi dengan sikiotostamid, vinkristin dan prednisolon dan hanya 3 penderita mendapatkan 3 siklus atau Iebih kemoterapi.Key Words : gastrointestinal lymphoma -- laparotomy -- chemotherapy -- working formulation.
