Abstract

Penelitian ini mengemukakan dua cars terapi pada karsinoma hepatoselular lanjut seria membandingkan hasil yang dicapai pada 20 pasien. Berdasarkan respon tumor masing-masing setelah terapi, temyata kemoterapi dengan infus berkelanjutan menunjukkan hasil yang lebih balk dan lebih efektif. Akan tetapi cars pengobatan itu juga menyebabkan efek samping yang lebih berat.Kedua cars terapi di alas memberikan angka ketahanan hidup (survival rate) 70% untuk 6 bulan dan 40% untuk 1 tahun. Sebagian besar pasien dalam penelitian ini mengalami gangguan fungsi hepar yang berat dan stadium penyakit sangat berperan terhadap hasil terapi.Key Words : hepatocellular carcinoma transcatheter arterial chemoembolizati on infusion chemotherapy anticancer drugs angiography

Abstract

The process of aging of an individual has begun since his existence in the womb. Old age forms the last stage of the process.In this aging process changes take place continuously affecting the morphology and function of the individual, his systems, and organs down to the ultrastructure and biochemical reactions, probably due to "intrinsic mutation"; some of those changes are universal and some are specifically affecting certain organ, tissue or cells.In the aging process the heart is not free from these change characteristics which affect the structure of the heart muscle and the vascular wall, and also their biochemistry. All these changes have their impact on the cardiac function.In old age differences are found in the heart from its previous stages of the aging process, which among others are:1. fragmentation of some myofibrils down to complete degeneration with its specific characteristics,2. decrease of the number of mitochondria: some mitochondria show signs of degeneration, 3. heart perfusion decreases down to 80%,4, degeneration of the coronary vessels.5. a change of noradrenalin metabolism so that in the heart muscle it remains 75%,6. degenerative changes of the cardiac valves.All these changes tend to decrease the function of the heart.Key Words: aging process - "intrinsic mutation" - aging heart - myocardium degeneration - senescence

Abstract

Bacground: " Gurah" is a traditional treatment for nasal and lower respiratory tract diseases or for establishing clearer voice, where the extract " Srigunggu" or "Sirgunggu" is used as nasal drop. The side effect of "gurah" on the mucocilliary system has not been known yet.Objective: The study was done to know the effects of "Gurah" on the mucocilliary transport, in chronic rhinitis patients.Method: The study was performed by pretest-posttest design.The study was performed at "Gurah" location of Kyai Hisyam, Giriloyo, Wukirsari, Imogiri, Kabupaten Bantul Daerah Istimewa Yogyakarta. The mucocilliary transport time of the subjects with chronic rhinitis were tested with saccharine method for three times i.e. on the second (Wt2) and the tenth day (Wt10) after "gurah". Procedure of the "gurah" was done by KH Hisyam himself, and the measurement of mucocilliary transport time was done by the author. The paired-t test was used to test the significant difference of the mucocilliary transport time between before and after "gurah".Result: On the second day after "gurah" there was significant difference of the mucocilliary transport time between before "gurah" and after "gurah" (P< 0.05), but on the tenth day after "gurah", there was no significant difference of the mucocilliary transport time (P>0.05).Conclusion: The mucocilliary transport time was prolonged significantly on second day after "gurah", but on the tenth day the mucocilliary transport time returned to the before gurah" value.Key word: "Gurah" - mucociliary transport time

Abstract

The aim of war is to inflict death in the greatest number and permanent injury among enemy troops, while the aim of medicine is to regain physical and mental health, to maintain medical wellbeing and to prevent diseases and physical disability. Therefore, the aims of war and medicine are in conflict.Paleinology is the science of war and peacewhich studies the causes of war and the conditions for peace and to search for means to realize those conditions, and for the necessary actions to prevent war and to promote peace.At present war is highly mechanized due to the development of technology, and this in turn is brought abou tby the advance of science. The mechanization of war makes possible mass killing, not limited anymore to combatants but includes civilians and children as well, who are ignorant of the purpose of the war. Weapon technology grows rapidly because it is not hampered by economic considerations as is business, but it is forced by political conflictss.Pure science changes human concepts concerning nature, himself and his fate. Applied science has greater influence and attracts wider attention, especially since it is applied to war. Abstract

All patients admitted to the Department of Internal Medicine, Gadjah Mada University Hospital, Yogyakarta, with typhoid fever from January 1976 to December 1977 have been reviewed.The diagnosis was made by clinical examination and laboratory data such as Widal reaction and culture of blood, stool and urine.In that period there were 163 typhoid fever cases, 86 males and 77 females, and the average age was 22.19 years. The average duration of hospitalization was 17.6 days. All cases had fever for several days and some of them got treatment before admission. Most of the patients with chloramphenicol treatment gave excellent result.All patients who died in hospital were in bad condition. Abstract

A diagnosis of thalassemias has advanced from clinical to molecular in concordance with the advances in molecular biology. Since the introduction of polymerase chain reaction procedure - a practical in vitro procedure of deoxyribonucleic acid amplification - various diagnostic methods have been developed, to detects either gene deletions or point mutations. In a population where the spectrum of mutations is not too heterogenous, direct methods such as dot blot or reverse dot blot hybridization, ligase chain reaction and amplification refractory mutation system may be applied with high effectivity dan efficiency. But, in a population where the spectrum of mutations is very heterogenous other methods such as mismatch analysis, denaturing gradient gel electrophoresis and single strand conformation analysis as the screening step followed by deoxyribonucleic acid sequencing are chosen. Each of the above methods has advantages and shortcomings, in relation to various problems among others the sensitivity, the specificity, the ease, the reproducibility and the cost.In this paper the molecular diagnostics, concerning the principle, the advantages and the shortcomings, espebially that have been used in the field are discussed.Key words: gene disorders - ligase chain reaction - dot blot and reverse dot blot hybridization - amplification refractory mutation system - denaturing gradient gel electrophoresis - single strand conformation analysis - chemical cleavage of mismatch

Abstract

Extirpation of eyelid benign tumor, while the vision is still good, must be followed by lid reconstruction to protect the eyeball. The means of such reconstruction to make the lid function are optimal. A case of lid reconstruction after lid benign tumor extirpation with the remain of skin and the orbicular oculi muscle has been reported. The reconstruction was carried out by replacing the lost tarsoconjunctival tissue with the opposite tarsoconjuncdval tissue. The blepharoraphy was maintained until the cicatrical tissue formation was completed. Abstract

In the female complication of the distal portion of the ureter injury is the ureterovaginal fistula, seen most often after the Wertheim hysterectomy. The true incidence of the disease is difficult to ascertain in the literature. Incontinence isthe first symptom of the ureterovaginal fistula patient. An IVP, RPG, cystography, cystoscopy and double dye test investigations are to be advocated for diagnosis of the disease.Ureterovaginal fistula is repaired by standard ureteroneocystostomy, incorporating an antirefluxing submucosal tunnel.Key Words: Wertheim hysterectomy - ureterovaginal fistula - incontinence - ureteroneocystostomy - ureter injury
