Abstract

Malayan filariasis is still the major public health problem in Indonesia. In endemic area, when microfilaremia and clinical manifestations are absent, the diagnosis of malayan filariasis is extremely important, particularly for filariasis control.With the development of hybridoma technology, it is possible to produce monoclonal antibody against any stage of Brugia malayi, which is capable of detecting the circulating parasite antigen in malayan filariasis sera.Sandwich ELISA, assay using Fab 4 monoclonal antibody could identify circulating parasite antigen in human sera. With this assay it was found that: 100% of sera from symptomatic-microfilaremic, 50% of sera from symptomatic-amicrofilaremic, 82,1% of sera from asymptomatic-microfilaremic and 24,4% of sera from asymptomatic-amicrofilaremic residents of filariasis-endemic areas positively contained circulating antigen.In comparison with the conventional method, the sensitivity and specificity of this new technique are 85,2% and 75,4% respectively.From this study it is concluded that Sandwich ELISA is proved to be very sensitive as an immunological test to detect filarial infection without any clinical manifestation or microfilaria in the blood.Key words : malayan filariasis - circulating antigen - monoclonal antibody - Sandwich ELISA - asymptomatic-amicrofilaremic.

Pediat. 81:178-84 photo-therapy (6 hours light on, 6 hours light off 48 hours) in the diminution of serum total and unboundbilirubin levels has been studied in 26 randomized premature infants. Total-bilirubin level was determined spectrophotometrically and unbound-bilirubin level by the peroxidase oxidation method using an UB-analyzer. During phototherapy there was significant reduction of total and unbound-bilirubin levels in both groups. However, the diminution of both groups was not significantly different. After photo-therapy was completed there was significant reduction of total and unbound-bilirubin levels in both groups. The diminution of total-bilirubin level until the 4th day after treatment was not significantly different in both groups. Diminution of unbound•bilirubin of intermittent group was significantly greater than that of the continuous group in the 3rd and 4th days after treatment. In conclusion both phototherapy methods were equally effective in diminution of total and un• bound •bilirubin levels in premature infants. Abstract

This article defines professionalism relevant to the medical profession and the trend towards deprofessionalization in medicine. Subsequently humanitarianism is described, followed by the nature and causes of dehumanization in medicine. Rehmnanizing efforts are expected to be enhanced by certain on-going trends in the society.The author further presents the issue of human rights, both natural and cultural, and their relation to the right to health care, whence patient rights developed. In this connection the problem of justice in the allocatiOn of health resources is encountered.Private medical practice in itself does not interfere with either professionalism or humanitarianism as long as distributive justice and patient rights arc observed.Lastly, potential future problems related to medical professionalism and humanitarianism are brought into focus, such as the growing urban slums, rural poverty, and opportunity for practice and employment. It is emphatically noted that sporadic medical interventions among underserviced communities are not effective, except from the vantage point of publicity.Key Words: medical professionalism - patient rights - justice in medicine 7 dehumanization - private practice

Abstract

Chronic renal failure is still a major health problem in medicine world. An adequate treatment could only retard the progress of renal failure and delay the course toward end-stage renal disease, a condition that requires maintenance dialysis or kidney transplantation.Managable factors should always be identified and promptly treated. Dietary therapy may be able to cope with uremic symptoms, perhaps retards the progress of renal failure and improves nutritional status. All above mentioned will improve morbidity and mortality of the patients. Uraian ini meneatat adanya perhatian yang meningkat terhadap pengobatan tradisional di dunia didalam dasawarsa terakhir, dan berbagai macam reaksi yang timbul terhadap kebangkitan itu dari kalangan kedokteran modern. Penulis menganggap aspek holisma sebagai hal yang.ter• penting pada pengobatan tradisional yang harus diambil oleh kedokteran modern, .diantara berbagai perbedaan yang terdapat diantara kedua system itu. Aspek ini menjadi lebih penting, karena adanya hyperspesialisasi dan modemisasi dalam kedokteran, massifikasi pelayanan kesehatan serta industrialisasi, dan terurama terasa dalam kedokteran populasi dan kedokteran pedesaan. Dengan pengharripiran holistis, pasien baik berupa individu maupun populasi dilihat sekurang.kurangnya pada tiga peringkat system hayat. Holisma juga akan melihat manusia sebagai makhluk biokultural yang berevolusi dan memperlakukan tiap-tiap pasien secara unik dan with. Holisma tak dapat tidak akan memperhatikan ethika kedokteran dan keadilan distributif dalam pelayanan kesehatan. Berbagai proses dalam berhadapan dengan penyakit sejak dari diagnosis sampai rehabilitasi akan dilihat dalam matrix yang Inas. Abstract

Untuk mengetahui fungsi hepar pada penderita gagal ginjal terminal dengan antibodi terhadap virus hepatitis C (anti HCV) positif yang menjalani hemodialisis, suatu penelitian potohOlintang telah dilakukan di Unit Hemodialisis. Rurnah Sakit Umum Pusat Dr: Sardjito, Yogyakarta.Selama penelitian ditemukan 58 penderita (42 laki-laki. 16 perempuan) dengan 32 (55%).anti HCV positif. Tidak ditemukan perbedaan yang bermakna antara kelompok dengan anti HCV positif dan kelompok dengan anti HCV negatif dalam kadar aspartate amino transferase (AST) (46,22 ± 20,85 UI/1 vs 40,15 ± 20,37 U1/1, p > 0,05), kadar alanine amino transferase (ALT) (45,44 ± 26,26 IU/1 vs 37,04 ± 23,03 U1/1, p > 0,05), kadar alkaline phosphatase (ALP) (156, 31 ± 141,141U/1 vs 144,57 ± 82,15.1y/l, p > 0,05),. dan jumlah hemodialisis (8 sampai dengan 733 kali). Kadar AST yang rneningkat di afas 1,5 kali nilai normal cenderung ditemukan lebih besar tapi secara statistik tidak signifikan pada kelompok dengan anti HCV positif dibanding dengan kelompok dengan anti HCV negatif (56.0% vs 30,7%, p > 0,05) yang mungkin akibat jumlah sampel yang kecil. Frekuensi transfusi ditemukan lebih tinggi secara bermakna pada kelompok dengan anti HCV positif dibanding kelompok dengan anti HCV negatif (3,53 ± 3,47 vs 2,23 ± 1,47 kali, p < 0,05).Key Words : hepatitis C virus-anti HCV antibodies -end stage renal disease-blood transfusion- hemodialysis

Abstract

Telah diukur tekanan darah pada 506 anak sekolah yang berumur antara 6-14 tahun dan terdiri atas 227 anak laki-laki dan 279 anak wanita. Sampel ini diambil secara duster dan acak dari suatu populasi anak sekolah di pedesaan. Tekanan darah ini diukur pada anak sekolah clari 17 sekolah dasar kecamatan Sewon, Bantu!, Daerah lstimewa Yogyakarta. 17 sekolah ini merupakan sampel dari seluruh sekolah dasar yang ada di kecamatan Sewon.Alat yang dipakai untuk mengukur adalah sphygmomanometer bahan air raksa, dan pengukuran dilakukan oleh hanya sacs orang. Semua anak mula•mula diukur tekanan darahnya pada posisi duduk, kemudian pada posisi telentang.Hasilnya pada posisi duduk tekanan darah sistolik berbeda secara bermakna antara anak lakilaki dan wanita, kecuali pada anak umur 6 tahun. Demikian juga tekanan darah diastolik, kecuali pada anak umur 14 tahun.Tekanan darah pada posisi duduk dan tidur berbeda bermakna balk pada laki-laki maupun pada wanita.Key Words: blood pressure - rural primary school children - systolic pressure - diastolic pressure

Abstract

Background: Momordica charantia L. has been used traditionally as medicinal plant in Indonesia. The seed extracts have active substances in the form of ribosome-inactivating proteins (RIPs. Almost all RIPs can eliminate virus infection and kill cancer cells. Some cancer cells like Raji cell line and HeLa cell line are derived from Burkitts lymphoma and cervical carcinoma, respectively. Both cancers shows a striking association with Epstein-Barr Virus (EBV) and human papilloma virus (HPV).T47D cell line is derived from breast cancer and is not related to certain type of virus.Objective: This study was focused on the effect of RIP from M.charantia L. on Raji, HeLa and T47D cell lines.Methods: Crude extract from M.charantia L. seeds was dissolved in 0.15 M NaCI, and then precipitated with 30-60% saturated ammonium sulphate. Precipitant was then purified with column of CM-Sepharose CL-6B with 0.5 M NaCI gradient. Cell lines grown in growth media with crude extract and active fraction of column. Lethal Doses 50% (LD50) was calculated directly for Raji cell line and used MTT for HeLa and T47D cell lines.Results: The study shows that 0.25M NaCI column fraction containing 32kDa protein had an activity on DNA supercoiled cleavage. The difference of LD50 between crude extract with 32kDa protein was not significant (p>0.05) for Raji cell line and HeLa cell line, but there was significant difference for T47D cell line. T47D cell line shows DNA fragmentation as sign of apoptosis.Conclusion: Raji cells shows more sensitive than T47D cell lines did, whereas HeLa cell line was resistant to the crude extract and 32kDa protein. In this work, the death of T47D cell line was estimated through apoptosis.Key words: column fraction - Momordica charantia L. - Raji - T47D cell lines - apoptosis.
