Abstract

Principally, complications of post operative cataract surgery with pseudophakos implantation cover those complications due to the operation, and those due to the existence of pseudophakos within the eye ball. In Dr. Sardjito hospital, this operation has been done since 1988. There are many kinds of complication of this operation. This retrospective study is limited to those complications in the form of anterior uveitis, endophthalmitis, secondary glaucoma, and endothelial decompensation which occurred within 1992-1994 period.A study has been performed on 405 cases, consisting of 397 cases with posterior chamber pseudophakos implantation and 8 cases with posterior chamber pseudophakos implantation. In the same period, a study on 432 cases with extra capsular cataract extraction without pseudophakos implantation was performed. The complication in the group with pseudophakos implantation were: anterior uveitis (2.96%), secondary glaucoma (1,48%), endhopthalmitis (0,99%), endothelial decompensation (0.74%) and other complications such as iris prolapse and subluxation of pseudophakos. The complication in the group without pseudophakos implantation were anterior uveitis (0.23%), endothelial decompensation (0.23%), and prolapse of iris 0.46%. In this study, the complication in the form of endophthalmitis occurred 1 week after surgery in 3 cases, and 2 weeks after surgery in one case. One case was caused by fungi infection. The complication in the form of secondary glaucoma mostly related with the occurence of post anterior uveitis surgery. The anterior uveitis in post cataract surgery with pseudophakos implantation was considered to be related with the kind of lens implanted.Key words : post operative complications -- cataract surgery -- pseudophakos implantation -- anterior uveitis -- kind bf lens implanted

Abstract

Until now coronary heart disease is the number one killer in the world. Angina pectoris is one of the clinical manifestations of coronary heart disease. If there is an insufficient oxygen supply to meet the need ischaemic myocard will happen.Life style changing, drug treatment and operative procedure are applied for the management of pain caused by myocardial ischaemia. According to many big anti-thrombotic trials statin, ACE inhibitor, calcium inhibitor, beta blocker and nitrate are very helpful in the management of chronic coronary artery disease. Myocardial laser revascularization, enhanced external counter pulsation and spinal cord stimulation are alternative procedures in which we can handle it with common management. While conventional therapies are continued to be improved, strategies to increase myocardial circulation by stimulating formation of collateral vessels around the obstructed coronary arteries are also in development. These approaches include angiogenesis, protein transfer, gene therapy and cell therapy. It is hoped that those approaches will safely and effectively applied in the management of chronic coronary artery disease. Key words: Coronary heart disease - chronic stable angina - myocardial ischaemia - new approaches

Abstract

This paper described methods which can be used to construct "behavioural objectives" at the level of a Department of Social and Behavioural Sciences within a Faculty of Medicine, and also in the choice of Specific curricular content. An attempt is then made to(i) Identify the basic skills that studies in the social and behavioural science might seek to develop in medical students ;(ii) Develop a set of Departmental Objectives which arise out of these basic skills, so that courses in these areas are as realistic and as practical as possible ;(iii) Extend this approach and show how it might be used as a method for developing a specific and measurable curriculum in the social and behavioural sciences ;. (iv), Identify some basic practical problems arising from a decision, by a Faculty of Medicine, to incorporate aspects of the social and behavioural sciences into the curriculum.For illustrative purposes each of the above tasks has been applied to the Indonesian context. It may thus be expected to have direct practical application to nther, similar developing countries ; but, of course, the approach to curriculum design will have more widespread application.

Abstract

Dilakukan peninjauan atas 1516 kasus persalinan di Bagian Obstetri-Ginekologi Rumah Sakit Universitas Gadjah Mada, yang dikumpulkan secara bebas dalam waktu delapan belay (!8) bulan (dari bulan Oktober 1969 sampai bulart April 1971), tentang hubungan antara paritas dan perdarahan postpartum.Dari peninjauau tersebut dapat diatnbil kesimpulan, bahwa makirt tinggi paritas wanita makin tinggi pula frekwensi perdarahan postpartum khususnya perdarahan kala III dan IV.Frekwensi perdarahan postpartum di Bagian Obstetri-Ginekologi Riunah Sakit Universitas G-adjah Mada cukup tinggi (17,7%), sehingp perlu Fieningkata.n perhatian dan sikap aktip dalim mengadakan tindakan pencegahan atau pertolongan bila terjadi perdarahan postpartum.

Abstract

Resi,stance to insulin-stimulated glucose uptake is present in the great majority of patients with impaired glucose tolerance or type 2 diabetes mellitus, in first degree relatives of patients with type 2 diabetes mellitus, as well as in approximately 25% of non-obese individuals with normal oral glucose tolerance. Deterioration of glucose tolerance can be prevented if beta cell is able to produce more insulin resulting hyperinsulinemia.The association between hypertension and diabetes mellitus is well recognized. Several reasons for the coexistence of these disorders have been postulated, i.e. as part of aging process, diabetes causes macrovascular and microvascular disease, renovascular stenosis which is enhanced by the development of hypertension. Insulin causes renal sodium reabsorption, stimulates the sympathetic nervous system, increases intracellular calcium, and is a vascular growth factor.Insulin resistance, hyperinsulinemia, impaired glucose tolerance, increased plasma triglyceride, and decreased HDL-cholesterol concentration have been shown to be associated with increased risk of coronary heart disease. Based upon these considerations, it raised the possibility that insulin resistance and hyperinsulinemia are involved in the etiology and clinical course of three major related disease; the type 2 diabetes mellitus, hypertension, and coronary heart disease.Key Words: hyperinsulinemia insulin resistance cardiovascular disease glucose homeostasis aging

Abstract

The corneal curvature is an important medium refractum because it gives the strongest refraction.The measurement of corneal curve has a direct connection with refraction. The measurements of the emmetropic, myopic, hypermetropic and astigmatic cornea have been presented. The mean of the emmetropic cornea! curve was 42.796 D, that of the myopic was 44.275 D. Statistically, there was significant difference between those of emmetropic and myopic cornea.The mean curve of hypermetropic eyes was 42.843 D. However, statistically there was no difference between the emmetropic and hypermetropic corneal curve.In astigmatism there were always differences between the vertical and horizontal curves. Key Words: corneal curve - emmetropy - myopy - hypermetropy - astigmatism

Abstract

In the natural environment, human embryos will be only in the uterine cavity at the morula or blastocyst stage. However, in vitro culture of embryos in assisted reproductive technologies remain replacing back embryos at or before four cell stage due to some problems involved. In this case report we aimed to culture and transfer embryos at blastocyst stage. A sub-fertility couple, wife at 33 years old, with bilateral fallopian tube damage. Husband has normal semen analysis. Of 9 oocytes retrieved from ovum pick up (OPU), seven were fertilized. On day 3 observation, five embryos showing considerably good in grade to allow further development. On day 5, four out of 5 embryos were formed blastocysts of which 2 were transferred, and the other two blastocysts were cryopreserved. Blastocysts of this reported patient were formed at the rate of 57.1% (4/7). Pregnancy was confirmed by positive urine semi-quantitative test, two intrauterine gestational sacs with 3 heart pulsations detected by ultrasonography. We reported a pregnancy obtained from our first human blastocyst culture and transfer to the couple undergoing in vitro fertilization (IVF) program at Permata Hati Infertility Clinic, DR Sardjito Hospital/Faculty of Medicine Gadjah Mada University, Yogyakarta.Key words : blastocyst culture - assisted reproductive technologies - in vitro fertilization

Abstract

This study examined the effect of parity, maternal age, and interval between pregnancies on body- weights of ill infants of a rural village in Central Java during the first 12 months of life. With a few exceptions, differences in these pregnancy factors were not accompanied by differences in body-weight growth.The pregnancy data were dichotomized at 2, 3, and 4 pregnancies for parity, at 35 years for maternal age, and at 17 months for pregnancy interval.Key Words: parity - birth intervals - age-specific birth rate - infant morbidity - infant nutrition
