Abstract

Terapi einbolisasi intraarterial melalui kateter (transcatheter arterial embolization) merupakan upaya terapi alternatif pada penderita hepatoma yang sudah tidal( dapat dilakukan terapi operasi (unresectable hepatoma). Kemajuan dalam bidang kemoterapi mendorong dilakukannya terapi kombinasi embolisasi. dengan infus intraarterial berkesinambungan (continuous inkaarterial infusion). Basil yang dicapai dengan metode di atas cukup efektif, cumulative survival rate 66,6% untuk 6 bulan dan 55,4% untuk 1 tahun.Efek samping terapi kombinasi lebih banyak, tetapi masih dalam Batas-Batas toleransi. Sebab kematian penderita hepatoma dalam penelitian ini terutarna gagal hati (hepatic failure).Key Words: chemotherapy - arterial embolization - hepatoma - hepatic failure - infusion chemotherapy

Abstract

Many species of mosquito were made to bite cat which contain microfilaria of Brugia pahangi in infective stage.Microfilaria will develop to infective stage (L3) in non-cibarial armature mosquitoes and will not in mosquitoes which have cibarial armature. This period depends on the species of mosquitoes. Abstract

Mucopurulent cervicitis (MPC) is an endocervical inflammation that causes a variety of complications, including infertility. The diagnosis of MPC is usually confirmed by counting the number of leucocytes using a Gram stain of an endocervical swab. This method requires trained personnel, microscopic equipment and time to read the result, so the detection of MPC in primary health care setting is often difficult. The leucocyte esterase dipstick (LED) is an easy and rapid method designed for detecting leucocytes, although its role for diagnosing MPC needs to be clarified. In order to test whether LED could be used as Gram stain substitute for diagnosing MPC, a diagnostic study was conducted among 145 female sex workers seeking reproductive health care from Griya Lentera STD clinic, managed by the Indonesia Planned Parenthood Association in Yogyakarta. Endocervical swabs were taken from each subject for Gram staining, a gonorhoeae culture and LED examination. The result showed that compared to Gram staining, the LED had 96.7% sensitivity, 64.81% specificity, a Positive Predictive Value (PPV) of 82.2% and a Negative Predictive Value (NPV) of 92.1 %. Compared to the gonorrhoeae culture, the LED had 100% sensitivity, 32.2% specificity, a PPV of 25.23% and a NPV of 100%. This study concluded that among women at high risk for STD infection the LED could be used as a Gram stain replacement in the diagnose of MPC and as an initial screening method to eliminate MPC caused by gonorrhoeae.Key words : mucopurulent cervicitis - gram stain - leukocyte esterase dipstick - gonococcal microculture -female sex workers.

Abstract

Pemulihan kesuburan setelah penggunaan IUD Multiload Cu-250 Pemulihan kesuburan, setelah penggunaan IUD Multiload Cu-250 dihentikan, terjadi datam waktu yang singkat. Hal ini merupakan kesimpulan penelitian reirospektip sclama tujuh tahun, mencakup 53 pemakai ML Cu-250 yang minta dilepas IUD-nya karena menginginkan anak lagi, dan meliputi 55 kehamilan yang kemudian terjadi. Rata-rata umur 25,02 tahun dan paritas 1,25: Lama pemakaian IUD antara 1,00 dan 66,42 bulan, rata-rata 27,28 bulan. Setang waktu antara pelepasan IUD dan hari pertama menstruasi terakhir sebelum konsepsi (REM-LMP) berkisar antara —0,51 dan 49,03 bulan, rata-rata 5.90 bulan. Empat wanita tidak lagi haid setelah IUD dilepas. Terjadi 35 (63.64%) kehamilan dalam waktu 6 bulan dan 7 (12,72%) dalam 6 bulan berikutnya. Dan 55 kehamilan yang terjadi 46 (83,63%) terjadi dalam waktu 1 tahun, dan 52 (94,54%) dalam waktu 2 tahun setelah IUD dilepas, Lama pemakaian ML Cu-250 tidak berpengaruh aims kelancaran pemulihan kesuburan, kecuali bila ada komplikasi infeksi panggul, Kehamilan dan anak yang dilahirkan tidak juga terpengaruh. Pada 14 wanita pemakai IUD di alas tiga tahun, lama rata-rata REM-LMP hanya 4,15 bulan. Kecuali 2 keguguran, 1 persalinan prematur dan 2 postmatur, 1 perdarahan postpartum atonis dan 1 kematian neonatal pada bayi dengan sindrom Down, semua kehamilan lain berlangsung genap bulan, dengan berat lahir rata-rata bagi 43 bayi 3387 g. Dari 42 anak yang diketahui jenis kelaminnya, 26 (61,90%) lelaki dan 36 (38,10%) perempuan. Angka banding bayi lelaki terhadap perempuan sebesar 162,50 ini jauh lebih tinggi dad angka banding normal sebesar 105 -106. Apakah fakta ini bermakna dalam hubungannya dengan pemakaian IUD bertemliege sebelumnya pantas diselidiki lebih lanjut. Abstract

An epidemiological study on Chronic Otitis Media (COM) in children in Bantu[ regency, especially its prevalence in relation to socioeconomic status and index ofachievement has been done. The study used two-step cluster random sampling. The subject of the study was 16 primary schools with 2896 children, taken out from 17 districts consisting of 514 primary schools with a total number of 90 000 children.The study was carried out for six months from May 1990, and the result showed that the prevalence of COM was 5.28%, consisting of 2.10% serous COM and 3.18% suppurative COM, and middle ear cholesteatoma was found in three of them. A number ofrisk factors, the educational level and job of the father, the mothers educational level, the economic status based on the value of their property, nutrition and tidiness, the status of children, and the size of the family were also studied.The resultof the study showed that the economic status (P <0.05) and the mothers educational level (P < 0.01) had a significant indirect relation to the prevalence of COM. The index of achievement of children in natural sciences, Indonesian language, and mathematics was not influenced by the serous COM, but was significantly (P < 0.01) influenced by the duplex suppurative COM.The study of increasing the mothers educational level, the economic status, and the managementofCOM cases by giving hearing aid or developing national program ofmicracar-sugeryservices are suggested.Key Words: chronic otitis media - school age children - middle ear cholesteatoma - index of achievement - micro-ear-surgery

Abstract

Commemorating the centennial of the birth of the reknown Indonesian feminist, R. A. Kartini, the author touched upon the discrimination against women and the achievement of the emancipation movement in the medical profession. The author then reviewed the natural vigour of the female sex, starting with the thesis of Greulich and sex dimorphism. Subsequently, sex ratio and sex differences in mortality, life expectancy and prevalence of various diseases are discussed. Abstract

This study was carried out to evaluate the diagnostic agreement between measurement of uterine fundal height with measurement of fetal biometry detecting intrauterine fetal growth retardation.The study consisted of two phases, i.e. determination of standard of uterine fundal height and a test of the standard against ultrasonography, that was currently regarded the most reliable tool in the detection of intrauterine fetal growth retardation. Data was analyzed using agreement analysis with Kappa statistics and intraclass correlation coefficient.The first phase showed a standard curve for the growth of uterine fundal height at 10th, 50th and 90th percentiles. This curve was constructed from 1,024 measurements of uterine fundal height on 487 pregnant mothers at 24-42 weeks of gestational age visiting antenatal clinics at Dr. Sardjito Hospital, Yogyakarta, from 21 March to 4 June 1992. There was a positive correlation between gestational age and uterine fundal height (r=0.7362, p<0.01). The average gain in uterine fundal height was 0.7 cm per week, with a steep increase at 24th to 36th week of gestational age and a tendency to level-off after that.Agreement of the diagnosis was tested on 85 mothers living in Lendah Subdistrict, Kulon Progo District, who at the time of the study were 24-40 weeks pregnant. Eight mothers were found to have intrauterine growth retarded infants, either using uterine fundal height measurement with under 10th percentile criteria of abnormality, or ultrasonography. There was a perfect agreement between the two in the detection of intrauterine growth retardation (Kappa=1, p<0.001) and an excellent correlation between uterine fundal height measurement and the measurement of fetal biometry (intraclass correlation coefficient = 0.8655).It was concluded that uterine fundal height measurement represented an excellent alternative in the monitoring of fetal growth and the detection of intrauterine fetal growth retardation starting from 24th week of gestational age.Key words: intrauterine fetal growth retardation - uterine fundal height - ultrasonography - agreement analysis

Abstract

A survey on anemia has been conducted on 94 school children in Yogyakarta of whom 46 were from rural and 48 from urban private elementary schools. The subjects ranged in age from 6 to 12 years. The frequency of anemia showed significant difference between the two groups (p < 0.01). The result showed that 39.13% of the rural and 6.25% of the urban school children were anemic. More girls were anemic than boys, namely 13.83% of the girls and 8.5% of the boys (p < 0.05). Amongst the causes of anemia in the rural area is thought to be worm infestation and other factors enhancing iron deficiency. It is suggested that iron supplement to adolescent girls is important. Abstract

Background: The body flexibility of elderly people tends to decrease. One of the causes is the increased dehydration of the tissues that leads to adhesion formation.Objective: To investigate the effect of moderate intensity of low impact dance on body flexibility in untrained elderly women.Methods: Pretest and posttest control design. A total amount of thirty, healthy women, untrained, 60 - 70 years old was divided into three groups. The first group was treated by exercise three times a week, the second group two times a week and the third group was the control group. The exercise used was D series of fitness dance for elderly created by PERWOSI Yogyakarta which is a low impact aerobic dance with moderate intensity (70 - 79 % Maximal heart rate), 40 - 54 minutes/day. Three groups are treated for eight weeks.Results: The result showed that the first group revealed a significant increase in body flexibility (p<0,05) in the fourth week. The second group also revealed statistically significant increase in body flexibility (p<0,05) in the eighth week. The changes of body flexibility of the first group was higher and significant different compared to the second group.Conclusion: The low impact aerobic dance with moderate intensity, 40 - 54 minutes/day, performed three times a week in untrained elderly women increase the body flexibility earlier and higher than that of twice a week. Key words: moderate intensity low impact aerobic dance - exercise frequency - body flexibility-elderly women
