Abstract

Premature rupture of the membrane (PROM) still gives a great problem in obstetrics because of its high perinatal morbidity and mortality, and whether conservative or active management should be taken is still debatable. The aim of the present study is to evaluate a conservative management of PROM especially on infant morbidity and mortality. A total of 130 PROM cases with gestational age of 28 weeks or more and birth weight of 1000 grams or more were included into the study. Twin and major congenital anomalies were excluded. A total of 129 non-PROM cases during the same period were selected randomly serving as control. Data were processed with IBM compatible personal computer using version 2.0 SPSS program: t-test, chi-square test and relative risk (RR) were statistical analysis used in this study.Both groups were comparable, in terms of gestational age, birth weight, type of presentation, but not in parity. Results showed that the risk of getting neonatal infection was 1.82 (0.95-3.46). Although it was not statistically different, but i t was clinically important. Rupture of membrane more than 48 hours gave the highest risk of neonatal infection (RR 2.77; 95% CL 0.95-8.05). Neonatal asphyxia was more common in the PROM group compared to the non-PROM. One minute Apgar score indicated that the risk of severe asphyxia was 2.19 (L44-3.33) and the risk ofmoderate asphyxia was 1.84 (1.19-2.87). Evaluation after five minutes showed that such risk became 1.56 (0.12-19.19) and 3.13 (1.65-5.93) respectively. Perinatal death in both groups did not show significant difference, but neonatal death due to sepsis in the PROM group was about four-folds compared to the non-PROM. Death atrributed to respiratory distress syndrome did not show significant difference.The study concluded that conservative management resulted in a high infant morbidity, although the infant mortality was relatively the same. As neonatal asphyxia was more common in the PROM group, then adequate resuscitation should be prepared. Although prophylactic antibiotics had been given, neonatal death due to infection was still high.Key Words: PROM - neonatal infection - asphyxia - obstetrics - infant mortality

Abstract

Cimpaign against cancer is not only the duty of the medical professional but also the respon• sibility of the society. The activities of the Sardjito Teaching Hospital Cancer Team. "Kucala" Cancer Society, county hospitals, health centres and physicians in private practice are presented.It is clear that the role of the Cancer Team of the Medical Faculty/the SardjitO Teaching Hospital, "Kucala" Cancer Society and medical professionals are very important in the detection of cancer. Due to the lack of facilitiei and based on the situation of the Yogyakarta area, it seems that there are many problems to deal with.It is suggested that a well organized cancer society will be able to solve the problems.Key Words: cancer - community health - anti-cancer campaign - health organization -- early diagnosis

Abstract

The study was aimed at investigating whether there were any difference on lymphocyte growth in in vitro by using RPMI 1640 and green coconut juice as medium. Lymphocytes were collected from spleen and bone marrow of 8 weeks old albino rats (Rattus norvegicus). Rats were sacrificed, subsequently spleen and bone marrow were taken and prepared for single cell suspension. Cells were counted using vital dye (Tripan biru) at 5x106 cells/ml. Twenty petri dishes were used. Half of them were used for spleen and the rest for bone marrow cell culture. In every group of those cells, petri were divided into two groups. One group used RPMI 1640 and the other a green coconut juice. Every petri dish was filled with 2 ml of suspension in cell media. All of the petri dishes were incubated in the incubator at 37°C, 5% CO2 for seven days. After 48 hours and 72 hours culture cells were examined and the percentage of viable cells was counted. Analysis of the viable cells between lymphocyte culture and bone marrow cells using RPMI .1640 and a green coconut juice was compared using Students t-test and Anava. The results showed the increase of proliferation of viable cells from lymphocyte culture and bone marrow culture in both media hours were observed an after 72. As conclusion there is no significant difference between the growth of lymphocyte and bone marrow using RPMI 1640 and a green coconut juice (p>0,05).Key words: lymphocytes - cell culture - coconut juice - spleen cells - bone marrow cells

Abstract

CT scan is a reconstruction of tomographic slices of objects by a computer. The first application in the clinic was done in 1967 by the efforts of Godfrey Mounsfield in the Central Research Laboratory of EMI Ltd. in England. Its superiority compared with conventional tomography lies in the high power of resolution and its ability to distinguish soft tissues with 0.5% of the emission difference of X-ray, while with the conventional tomography the emission difference must be more than 5%. Abstract

Background: Tuberous sclerosis complex (TSC) is an inherited syndrome in which affected individuals are at increased risk for developing benign tumors. Mutation of the TSC-2 gene encoding tuberin on chromo-some 16p13.3 give rise to the clinical disorder of tuberous sclerosis characterized by the development of hamartomas.Objectives: In the present study, I conducted to examine whether overexpression of TSC-2 can affect the growth of human oral squamous cell carcinoma (OSCC) which have different expression level of p27KP. Methods: I constructed an expression vector containing sense-oriented rat TSC-2 cDNA with pcDNA3.1 (Invitrogen), and transfected to oral squamous cell carcinoma (B88) to regulate the expression of TSC-2 gene in each transfectant. Western blot analysis was carried out to determine the protein level of TSC-2. Therefore, Tumor volume and body weight of nude mice was examined using tumorigenicity assay.Results: Overexpression of TSC-2 exerted the growth inhibitory effect of oral squamous cell carcinoma. Sense-oriented B88-TSC-2 cancer cells, have high expression of p27KP1. Moreover, tumor induced by TSC2 up-regulated transfectans become much smaller than those of control cells.Conclusion: overexpression of TSC-2 may exert the antitumor effect on oral squamous cell carcinoma through p27KP induction. Key word: TSC-2 - transfection - oral squamous cell carcinoma - pcDNA3.1 - mutation.

Abstract

Left bundle branch block is very rare in children. A boy, 5 years old with diphtheritic myocarditis had left bundle branch block in ECG pattern during the first days in hospital. He had received basic DPT vaccination 4 1/2 years ago, when his age was three months.The prognosis of left bundle branch block in diphtheritic myocarditis is usually poor. However, as he received DPT vaccination in the last 5 years the prognosis was tending to good. After 11 days hospitalisation, the ECG pattern suddenly reversed to normal. The follow up- through outpatient department has no further complications, despite he discharged with still high LDH serum leveLThe purpose of this study is to report a very rare case, and this case hopefully to be the last diphtheritic disease in Yogyakarta, due to a very good coverage of vaccination.Key Words: diphtheria diphtheritic myocarditis left bundle branch block DPT vaccination LDH enzyme
