Abstract

Malayan filariasis is an important public health problem in Indonesia. A diagnosis of filariasis is normally based on clinical, parasitological and immunological examinations but the methods have limitations. There is an urgent need to improve immunodiagnostic techniques for malayan filariasis using monoclonal antibodies to detect circulating antigen. Recently, many types of monoclonal antibodies have been produced following immunization of mice with various stages of filarial antigens. Those monoclonal antibodies have different characteristics.The objective of this study was to explore the characteristic of specific monoclonal antibodies against the surface coat of infective stages of Brugia malayi larvae.This monoclonal antibody recognized antigen of 40.000 MW protein which is present among the antigens of surface coat of Brugia malayi larvae, as detected by an immunoblotting technique. Classification using EUSA techniques indicates that those antibodies are IgGi, IgG2b and IgG3 subclasses. Abstract

The paper describes the epidemiology and pathology of depression. The history of therapy against depression and in particular the neurotransmitters hypothesis is presented. A clinical trial was carried out in the Out-Patient Department of the Central General Hospital Dr. Sardjito, Yogyakarta. Twenty depressed patients with ages of 15 — 25 years were treated with Nomifensine, during two weeks period, with a daily dose of 50 mg in the morning and 50 mg at noon. Intensity of depression was tested with Hamilton Depression Rating Scale, before treatment, a week after treatment and finally two weeks after. A pairwise retest of differences between two item differences, was chosen for analysis of the means of the initial —, mid —, and terminal depression intensity. There is a significant decrease in intensity of depression between the initial and terminal evaluation. Remarkable side effects were absent in almost all the patients treated. Nomifensine in daily dose of 2 X 50 mg a day is effective in the treatment of depression. In most instances reduction in total score can be expected within 1 - 2 weeks of drug treatment. Abstract

Thalassemia is a great health problem in the affected countries, especially in the developing countries including Indonesia. There is no curative therapy up to now, while the supportive therapy should be done life4ong and such therapy brings many problems to the patients and an enormous burden to the community who should afford the health services.Ninety thalassemia syndrome, at Dr. Sardjito General Hospital during the period of 1986-1992 were retrospectively studied. The diagnosis was based on clinical manifestations, routine blood examination, HbF investigation either with alkali denaturation test or electrophoresis and on the evidence of trait in the parents. Thirty six patients had undergone molecular studies. Aspects of therapy were also studied.The study found that 57 and 33 patients were major thalassemia and thalassemia-hemoglobin E disease, respectively. There was no difference between major thalassemia and thalassemia-hemogolobin E disease in term of male and female proportion (p > .05), and on the level (p > .05). Foutty eight patients came from Yogyakarta Province, the rest came from various parts of Java, especially nearby Yogyakarta Province. Compared with the estimation, we conclude that many thalassemic patients had not yet been covered. The expenses for supportive therapy were so great, while most of the patients belonged to the middle to low sosioeconomic level, so that only 30-40% had come regularly for routine transfusion. Among the 36 patients who underwent molecular studies, 31 have been genotypically identified. Molecular diagnostic was aimed at the first step of prenatal diagnosis.Key words: thalassemia syndrome-thalassemia trait-blood transfusion-molecular study-prenatal diagnosis

The paper described a Short review of mechanism of glucose intolerance in most non-innilin dependent diabetics which is now clear that is not caused by defect in insulin secretion.alone. Genetic predisposition, impaired glucose-mediated insulin secretion, excessive basal hepatiC gluciase production (HGP) and cellular resistance to insulin action have been docuthented in NIDDM:The increase in basal HGP is the primary factor responsible for fasting hyperglycemia, where as both insulin resistance in peripheral tissues and relative defective insulin secretion contribute to the impaired glucose disposal following oral or intravenous glucose administration. Abstract

Background: Aedes albopictus play important role in the spread dengue virus, it is spread widely in Indonesia. Geographical condition of Indonesia which shows varying climatology, biogeography and environmental factors seem to be very possible to cause the difference in the genetic structure of Ae. albopictus.Objective: The aim of this study is to determine the genetic polymorphism of each Ae. albopictus population.Methods: Genomic DNA was extracted from individual mosquitoes from 4 geographic populations of Ae, albopictus and amplified in PCR reactions using single primers of arbitrary nucleotide sequence.Result: The DNA polymorphism level of Ae. albopictus population originated from Yogyakarta is 80.77%, followed by population originated from Padang is 57.14%, Banjar is 40.74% and Timika is 37.50%. Conclusion: The genetic structure of the Ae. albopictus populations shows obvious differences, and they shows that they are from different group and separated with the various difference percentage.Key words: genetic polymorphism - population - Ae. albopictus - dengue virus - RAPD-PCR.

Abstract

Post-traumatic syndrome is a constellation of symptoms that developed in a person usually following a relatively minor head injury. It can grossly in capacitating for the patient and can develop a chronicity that makes it almost a change in personality. The commonest symptoms are headache, and dizziness; but impairment of sleep, decreased concentration and libido, anxiety, depression, irritability and sensitivity to noise are also seen.The cause of the syndrome is a ,contentious issue. The question whether it is primarily of psychological or organic origin remains unanswered in many cases. Organic or phychogenic, each symptom by itself is of little consequence, but in combination they may be disabling enough to prevent useful employment. Furthennore, the development of the syndrome of any head injury is not easy to be predicted.Key Words: post-traumatic syndrome headache neurotic personality depression amnesia

Abstract

In this article we reported our experience with Clonidine in treating hypertensive patients in the Department of Internal Medicine, Gadjah Mada University Hospital, Pugeran, Yogyakarta. 25 patients suffering from severe or moderate hypertension of 13 females and 10 males were included in the study. The treatment began with a small dose of 0.075 mg bid. This dose if necessary was increased every 3-7 days. In some patients diuretics were added.Side effects and effect of treatment on the blood pressure are described and discussed.Key Words: hypertension - clooidine - diuretics - renal failure - antihypertensive drugs

Abstract

A prospective study on 229 acute stroke patients was conducted to develop a Gadjah Mada Stroke Algorithm (GMSA). This algorithm was proposed as a clinical strategy for distinguishing intracerebral haemorrhage from acute ischaemic or infarction stroke after onset of stroke in 3 hospitals in Yogyakarta from 16th December 1989 until 15th November 1991. The following investigations have been made: (a) interobserver reliability for questionnaire and clinical examination of stroke patients, (b) interobserver reliability in the interpretation of CT-Scans of stroke patients, (c) validity of every clinical symptom against CT-Scans to define intracerebral haemorrhagic stroke, and (d) validity of seven multiple parallel tests against CT-Scans to define intracerebral haemorrhagic stroke. The GMSA was developed by one of seven multiparallel tests which has the highest validity. A multivariate statistical analysis and validate study showed that decreasing consciousness, headache, and Babinskis reflex at the onset are significantly related to intracerebral haemorrhagic stroke. This study showed that the GMSA was reliable and valid for distinguishing intracerebral haemorrhagic stroke from acute ischaemic or infarction stroke.Key words: Gadjah Mada stroke algorithm - diagnostic test - intracerebral haemorrhagic stroke
