Abstract

Background: In developed countries, survival rate of preterm baby with gestational period more than 26 weeks has increased. Most studies on developmental outcomes were conducted on preterm baby with gestational period of less than 32 weeks. On the other hand, in developing countries the mortality rate of preterm baby with gestational period of less than 32 weeks is still high. Objective: To assess whether the incompleted gestational age is a risk factor for developmental disorder at 6 up to 36 months of age. Methods: Sixty four subjects born between 2001 April 1 to 2004 January 31 in Dr. Sardjito Hospital were enrolled, consisted of 31 children with incompleted gestational age (exposed group) and 33 children with completed gestational age (unexposed group). The developmental milestone was assessed using the DenverDeve/opmenta/ Screening Test/DDST (Denver //). Age line adjusment is used for exposed grou when the subject was born 4 weeks before completed gestational age and less than 3 years old. Results: There was no statistically significant diferrence on the prevalence of developmental, disorder at 6 to 36 months of age between exposed group and unexposed group in every area i.e. for gross motor: RR (95% Cl): 0,87 (0,24-3,19); language RR (95% Cl ): 1,48 (0,30-7,23 ) and personal social RR (95% Cl): 1,10 (0,10-17,83) or developmental conclusion: RR (95% Cl): 0,75 (0,23-2,48Conclusion: On the basis of developmental screening using DDST (Denver 11) incompleted gestational age (32-36) weeks was not a risk factor for developmental disorder at 6 to 36 months of age. Key words: incompleted gestational age - development disorder - Denver Developmental Screening Test/ DDST (Denver ll).

Abstract

Abstract

Since the first description in 1925, thalassemia has been studied intensively and extensively. Thalassemia is inherited by Mendelian recessive genes, in which there is a mutation or deletion of the DNA nucleotide or results in defective production of one or more globin chains of the hemoglobin. There are two important forms of tha/assemias, a- and ft-thalassemia that are health problems in the tropical belt, extending from Africa and Meditenanian countries up to West Melanesia. The molecular studies of thalassemia and other hemoglobinopathies had discovered such a lot of abnormal hemoglobin structures and pathogenesis of the disease, so that the hemoglobin is now indeed a paradigm for our understanding of gene action at the molecular level. The thalassemia occurs essentially due to deletion, whereas 13thalassemia is mostly due to mutation of the gene(s). The understanding of the molecular pathogenesis of thalassemia is very essential for the diagnosis of the trait and especially for prenatal diagnosis, and in the future might be for genetic therapy.Key Words: thalassemia gene deletion gene mutation polymerise chain reaction haemoglobin

Abstract

This article describes the process and mechanism of phagocytosis, especially their immunological aspects. Phagocytosis is one of the bodys second line of defence against foreign particles, e. g. infectious and non-infectious material. The macrophage system, mononuclear and polymorphonuclear cells are responsible for phagocitizing and processing those particles, moreover they serve the processed material to the specific immunocompetent system. In addition CO the normal mechanism of phagocytosis, this article describes some disturbances which interfere with each step of phagocytosis as chemotaxis, opsonization and intracellular killing; this inadequacy results as a certain disease.Key Words: mononuclear phagocyte system - non-specific immune response - inflammation - receptor of the phagocyte cell - opsonin

Abstract

Simple glaucomas are charaterized by an elevated intra ocular pressure (10P) and, are an important cause of blindness in the world. The blindness is due to the optic nerve atrophy and is found more frequently in women. It has been reported that high activity of G6PD isozyme can reduce the ocular pressure in the calf eyes. The activity of G6PD isozyme in the blood of simple glaucoma patients has been examined, and compared to those of normal persons. This study consisted of 76 patients of simple glaucoma (41 men and 35 women) and 30 normal persons as a control group. The 10P of both patients and control group was measured by using a Schiotz tonometer. The average of the G6PD isozyme activity was 9.40 ulg Hb in the simple glaucoma and 10.94 u/g Hb in control group. The two G6PD isozyme activity showed a significant difference. It can be concluded that the activity of G6PD isozyme of simple glaucoma patients were lower than those of normal persons without glaucoma symptoms.Key Words : isozyme - G6PD - glaucoma

Abstract

Fever is a feature of a variety of infectious and non-infectious diseases. Recognition of drug fever in hospitalized patients is critical. If the condition is not appreciated, the patient may be subjected to prolonged hospitalization and a variety of unnecessary antibiotics, laboratory, and radiologic studies.Fever is considered to be drug-induced, -an incompletely understood allergic hypersensitivity reaction to medication-, if other causes can be eliminated, if the temperature elevation is temporally associated with the administration ofsensitizing medication, and if the fever disappears within 72 hours after discontinuation of the medication. Any drug has the capacity to induce fever as an adverse reaction. This article reported three cases of drug fever. Patients appeared "inap p ropria tely well for the magnitude. of their temperature elevation. They typically felt comfortable. Administration of antipyretic medications made the patients very uncomfortable and induced shaking chills. Their body temperature normalized within 24 hours after discontinuation of sensitizing medication in two patients. However, in one patient, the drug was not discontinued and the fever sustained, because she needed the drug.Key Words: drug fever - adverse reaction - allergy - hypersensitivity - antipyretics

Abstract

This article discusses the aims and various kinds of biomedical examinations and their evalua • Lion, primarily to assist those preparing for such undertakings. Among those aims, examination as a measure for protection of the public against incompetence and for quality control seems to be gradually disemphasized.Practical hints and "pearls" are given for passing various examinations, ranging from quizzes and pre-practical tests to subspecialty and doctorate examinations, and from viva voce and objective examinations to clinical and thesis examinations. Candidates are advised to know beforehand the system of evaluation and grading used in their examinations which could vary from pass/fail to 0-100 scale marking, and from continuous, comprehensive assessment to grading on the class curve. The discussion dwells on special techniques and procedures, such as rapid reading, analytical reasoning. the diagnostic process, and "post-mortem" evaluation of ones performance.Key Words: biomedical examination - viva voce - clinical examination - problem solving -diagnostic process

Abstract

The management of soft tissue sarcoma of the extremities is very complicated, particularly if the tumor is large and has infiltrated into the deep structures. Forequarter amputation is still one best choice for either curative or palliative purposes, although this operation is very mutilated. This report is aimed at presenting the benefit of forequarter amputation in patients with soft tissue sarcomas in the humeri region. Two patients with extrasceletal myxoid chondrosarcoma and pleomorphic rhabdomyosarcoma in the humeri region were managed with forequarter amputation and neoadjuvant chemotherapy. The patients condition and quality of life were better after the amputation.Key Words : soft tissue sarcoma - forequarter amputation - neoadjuvant chemotherapy - extrasceletal myxoid chondrosarcoma - pleomorphic rhabdomyosarcoma
