Abstract

The Integrated System for Recording and Reporting (ISRR) Sukamerindu Public Health Center of Bengkulu there are several problems, namely the provision of inaccurate data / information, reporting that is often not timely and there is no standard mechanism. This study aims to analyze the application of ISRR from the aspect of input (human resources, funding, equipment), process (recording, reporting), and output (presentation of data & information, dissemination, utilization). The research method is qualitative research with a phenomenological approach through in-depth interviews with 3 key informants and 6 ordinary informants. Data processed by the Content analysis method and SWOT analysis to determine the formulation of strategies for the problems found. The results showed that ISRR Sukamerindu Public Health Center had not run optimally, this was due to lack of human resources, lack of coordination between data managers and program holders, lack of equipment, manual recording and recapitulation with personal laptops, there were still delays in sending reports and data became less accurate. The data presentation is more complete, the distribution is only to the Bengkulu Department of Health, and the data is used for evaluation for health policy stakeholders. Abstract

Coding is an activity to determine the diagnosis code and action based on the diagnosis and action given by the doctor to the patient carried out by the coder. Determination of neoplasm codes will produce topographic codes that contain the location of neoplasm and morphological codes that show the nature and behavior of neoplasms. Bhayangkara Hospital in Semarang has not implemented a morphological codes so that the code for neoplasm currently only uses topographic codes. This study aims to describe the accuracy of the topographic code of the main diagnosis of neoplasms in Bhayangkara Hospital Semarang. This study was conducted in March 2019 at Bhayangkara Hospital Semarang by observing medical record documents of neoplasm cases and interviews with coding officers. Data processing is done by editing, tabulating, cross check and analizing data. The research shows that there are 50% of topographical code is accurate and 50% topographical code is inaccurate. Inaccuracy of topographical code occurred because there is no policy the implementation of morphological code, because the morphological code does not affect the rates of health services and because the writing of the diagnosis is unreadable. Keyword : Coding neoplasms, topographic codes, accuracy

The increase in TB case is caused by inadequate monitoring of treatment, unavailability of standardized recording and reporting system. This research was conducted to evaluate the completeness TB form design in providing more complete information. This research was administered in the form of a case study. The results showed the completeness of identification review reached 96%, recording review of 90%, reporting review of 64%,and 0% for authentication review. Incomplete information led to inaccurate TB control information. This incompleteness was associated toimproper design of the form, especially the unavailability of form filling instructions, names and signatures of officersin the anatomic aspect. In the aspect of content, lack of clinical data was also found. Regarding to the results of this research, revision in the design of TB Treatment and Prevention Card is needed in order to accommodate the data completeness of TB patients. This research also proposed a better design of TB Treatment and Prevention Card Keywords : Quantitative Completeness, Tuberculosis, Form Design, TB Treatment Card (TB.01P)
