Abstract

This research aims to discover the factors that influences and causes any defects on hollow products of PT. X. We applied DMAIC method to solve the problems. Pareto Chart and Fishbone are utilized for prioritizing and Fishbone discovering the root of the problem, consecutively. As the results, the recommendations that can be given are standardization on some processes such as patri, cukit, and drying, periodical cleaning and maintanance on the machines, calibration on the micrometer, utilizing box for storing the goods, and provide a specific training for the operators in the rangkai and patri departments.

Abstract

Quality is a very important thing to be considered in every company because it relates to customer satisfaction. However, defective products often occur in the production activities, the management attempts to decrease defective products since it causes the loss in time, material, and funds used. Measurement System Analyze is applied as the first stage to perform an analysis toward the ability of the operator, technicians, and QC inspector. The next stage is to make Fishbone Diagram to find out the roots problems of defective products on Blow Molding area. The Measurement System Analyze results shows that 61.7% of the entire operator still couldn’t perform an inspection correctly. The inspection’s methods of the technicians and QC inspector are also needed to be changed because it still generates very high calculation variation. The result of Fishbone Diagram finds out that defective products are caused by these factors, such as machine, method, human, and material.

Abstract

Human Resources (HR) Department is department which handled all the employee’s needs from many aspects, which is training, medical, uniform, etc. All employee fulfillments done by a workflow that called Standard Operating Procedure (SOP). HR Department always doing development by doing improvement in fulfillment employee’s needs and this process happened fast enough that affect to SOP discrepancy between HR sub-Department. There are six SOP’s that is not corresponding between HR sub-Department generates misunderstanding in practice, such as Training, Medical, Uniform, Special Leave, ID Card, Pension Fund SOP. Therefore a new SOP that corresponding is needed to accommodate all the changes that happened. Now the SOP that is not corresponding, in the future needs PIC who responsible to update the SOP by using method and template that already provide so that will be corresponding/uniformity SOP between HR sub-Department.

Abstract

PT Sumber Rubberindo Jaya is a company that produces motorcycle tires. This company has implemented occupational safety and health but with limitation in providing personal protective equipment. Currently, some export customers require occupational safety and health as a condition of export. Therefore this research is to improve the occupational safety and health in the company. HIRARC method is used to analyze the safety and health of the company. The result shows 24% activities are categorized as low risk, 48% are categorized as moderate risk, 28% are categorized as high risk. There are 8 moderate risk activities from all areas and 9 high risk activities from raw materials, mixing, cutting, building drum, curing. Proposed improvements are given to fixing the floor, giving personal protective equipment (safety shoes, helmet) and safety sign, the path the forklift and various other things.

Abstract

This article PT Famiglas Mitra Mandiri is an industrial manufacturing company that work in the body construction. The company itself, produce the main body part for the heavy vehicle such as trucks or bucks. Besides, the company also produced material that related to that sector. The Owner of the company complains that they have some issues about the work accident. The work accident happens frequently almost 2-3 accident each month. As a result, the company have to pay extra money for the medical expensive. This issues is happen because, until now the company doesn’t have a management standard for health and safety. To resolved this issues, the implementation of management standard for health and safety need to be implement immediately. The data was collected using qualitive method in the company. The observation including interview, scoring and discussion with the company staff. The observation is to ensure that the company is able to use SMK3 system and to obey the legal rules by the government based on the Peraturan Pemerintah nomor 50 tahun 2012. As a result of the observation of SMK3, is it clearly to be seen that the company already fulfilled almost 72.46% (121 from 167 sub-criteria).

Abstract

One of the issues of corporate refinement is to improve customer service by reducing the loss of shipment goods. This problem is caused by three factors namely the warehouse in Surabaya, transporter, and end customers. During the loading process in warehouses in Surabaya, the loading worker are required to make a report that will highlight the quantity tally. One of the methods used to analyze the problem is the FMEA method. The FMEA method is categorized into three phases, i.e. identification of problems, assessment of the results of identification and determination of repair problems. The results of the FMEA method evaluation will be used as a guide for improvement and are expected to help reduce the frequency of loading of goods caused by warehouses in Surabaya.

Abstract

Management at finished goods warehouse owned by PT. FSCM Manufacturing Indonesia is less effective. That problem makes the warehouse becomes disorganized. The suggested designs to improve warehouse management include optimizing the warehouse capacity, redesigning of the warehouse layout, and improving goods placement system. Additionally, measurement of warehouse performance makes warehouse management better. As the results, after the implementation of the new design, the warehouse is now well organized and itsperformance is improved.

Abstract

This study was done to make a method in Yield material factor collection system, to know the cause of deviation of production output with Yield material, to know Yield standard still relevant or not and to search first data of production activity at press manufacturing area. First problem is workers are not writing down on worker daily report the actual output of production. Second problem is to know the cause of deviation of production output with Yield material. Third probelm is to know wether Yield standard used to relevant with current condition or not. Last problem is workers do unnecessary activity in production proses. Improvement for first problem is making a new method using work instruction. Improvement for second problem is using fishbone diagram to know the root cause of the problem. Third problem is searching actual Yield, if actual Yield different with standard Yield then make a new Yield standard for each type of component. Last problem is only taking the first data in the form of SWCS and the suggestion is to make a new work instruction contains activities which workers have to do and a warning if workers are not doing activities as written on work instruction.

Abstract

The library attendance at Petra Christian University is decreasing. We are interested in finding the factors affecting that attendance. In this study, we applied the LIBQUAL one of library quality measurements, and added some external and internal factors to answer the quest. Additionally, we also used the Confirmatory Factor Analysis (CFA) and Structural Equation Modelling (SEM) as tools for analyzing the constructs. As the result we found out that the factor that affecting library attendance is library as place, and the factor that doesn’t affect library attendace is teacher motivation.

Abstract

UD X is a company that work in bicycle selling and distribution. Some of problems that occur in UD X’s warehouse is the long pick-up time. The other problem is inability of the warehouse to do FIFO system because the warehouse is using stacking system. This research is done by making a simulation in the current condition to obtain the pick-up time. The proposal of the layout is done by grouping the goods by throughput-to-storage to get the good’s throughput speed. Proposal alternative also made by the single racking with front end depot system and single aisle with font end depot racking system, and tested by simulation. Single racking with front end depot is the best proposal from the simulation test with pick-up time 22,017 seconds better than the current condition. Investation cost is Rp128.200.000,- and the breakeven is 21,6 years. Investation is considered as not feasible since the cost isn’t match with the savings that occur.
