Abstract

Objective: To compare the advantages and disadvantages of male circumcision techniques. The articles of male circumcision techniques were investigated from October 2018 to December 2018 through Google Scholar and Proquest. Material & Methods: There are various methods of circumcision, these methods can be grouped into two: using shield and clamp devices and conventional methods (dorsal slit and excision). Results: The result from this review article are the technique using shield and clamp devices such as the Smart Klamp have several advantages, that are minimal bleeding, rarely injuring the glans penis and faster, but the disadvantage is taken more costs, and often occurs edema in the circumcision area. Conclusion: In general, the circumcision method using shield and clamp devices is more beneficial and more practical than conventional methods.

Abstract

Objective: We evaluated the comparability and repeatability of analyses based on uroflowmetry reports among urologists in Indonesia. Material & Methods: We assessed the inter-observer agreement when interpreting urodynamic examinations. Four urologists analyzed 20 sets of uroflowmetry data and gave their interpretations of the normality of the curve, reason of abnormality, grade classification, and pattern of the uroflowmetry curves. The consensus among observers was analyzed using the kappa statistic. Results: The kappa values for the analyses of the normality of the curves indicated fair to the moderate agreement. Agreement on the reason of abnormality showed poor to fair consensus. The shape of the flow curve had kappa values ranging from 0.047 to 0.225, indicating poor to fair consensus. Based on grade also showed kappa value from 0.047 to 0.169, indicating a poor agreement. Conclusion: Interpretations of uroflowmetry tracings showed only the poor to a fair agreement despite the normality of the uroflow curve. Variability in interpretation can strongly impact patient treatment. Therefore, further work is needed to standardize the reporting and interpretation of uroflowmetry studies to optimize patient care.

Abstract

Objective: To determine Type-2 diabetic patients and its characteristics to determined risk of Erectile dysfunction (ED) and the role of EHS score as more practical tools to screening ED among T2DM patients. Material & Methods: The cross-sectional observational study was carried out at the Internal medicine Unit of Manambai Abdulkadir and Dompu district Hospital, out of 45 patients were included on this study. The data were collected from June 01 – August 01 2017. A structural questioner was used to collect the data and was analyzed using SPSS 2.0. person analysis correlation and logistic regression were used to find the Odds Ratio (OR). Results: We found that 38 out of 45 (84.4%) patients (mean age 57.2 ± 7.1) have Erectile dysfunction, 26 patients uncontrolled diabetes, 5 (11.1%) treated as CHF, 4 (8.9%) CKD, and 7 (15.6%) neuropathy diabetic. There is high correlation between random glucose level and IIEF-5 score (r=0.5, p=0.01). The overall odd ratio of ED in this studies was 4.3 (95% CI: 0.73 to 25.1) for uncontrolled diabetes, 2.5 (95% CI: 0.1 to 51.1) for Treated CHF, 2 (95% CI: 0.01 to 41.6) for CKD, and it was 1.2 (95% CI: 0.1 to 11.5) for neuropathy diabetics. Conclusion: From this study, we found that most diabetic patients have ED, and there is high correlation between random glucose level and simplified IIEF-5 score, EHS performed similar result on diagnoses ED compared to IIEF-5.

Abstract

Objective: In this study, we try to describe the characteristics of patients with urethral trauma in Hasan Sadikin Hospital Bandung from 2013 to 2017. Material & Methods: The data were taken retrospectively from medical records in the Department of Urology with the permission of the ethical committee. The patient characteristics were then classified according to their age, etiology of trauma, location of trauma, grade of trauma, associated trauma, and initial management. Results: The number of trauma cases in Hasan Sadikin Hospital during 2013-2017 was 20.489, 477 of which (2.33%) were urogenital trauma. Of the total urogenital trauma, there were 124 patients with urethral trauma, male patients were more common (84.67%), with an average age of 34.67 (1-82) years. Seventy two patients (58.06%) were iatrogenic trauma (catheter instrumentation 44.35%, circumcision 6.45%, and others 7.25%), and fifty two patients (41.94%) were non-iatrogenic trauma (traffic accident 31.45%, falls from a height 7.25%, and occupational accident 3.22%). In non-iatrogenic trauma group, 40 patients (76.92%) had posterior urethral trauma and 12 patients (23.08%) had anterior urethral trauma. We found 22 (42.31%) of 52 patients with non-iatrogenic trauma were AAST grade I-II and 30 patients (57.69%) were AAST grade III-V. In patients with posterior urethral trauma, 25 patients (62.5%) had pelvic fractures. There were 10 patients (19.23%) who underwent primary endoscopic realignment within the first 72 hours while 30 patients (57.69%) underwent delayed urethroplasty 3 months after trauma, and the rest (23.08%) were treated conservatively. Conclusion: Urethral trauma in males occurs more frequently than in females. with the most common cause were catheter instrumentation (iatrogenic) and traffic accidents (non iatrogenic). Posterior urethral traumas had higher incidence than anterior urethral traumas, which were commonly associated with pelvic fractures.

Abstract

Objective: This study aims to determine the characteristics of trauma patients, especially ureteral trauma in Hasan Sadikin General Hospital and expected to be used as data for trauma nationally. Material & Methods: This study was descriptive. Retrospective data collection is taken from medical records of ureteral trauma from 2013 until August 2017 at Hasan Sadikin General Hospital, Bandung. The data obtained are gender, mechanism of trauma, and management of ureteral trauma. Results: A total of 20489 cases of trauma handled in Hasan Sadikin General Hospital, 2.3% is a case of urogenital trauma and 40 of which are cases of ureteral trauma (0.19%). Most ureteral trauma patients are female (90%). Based on the trauma mechanism, 39 patients iatrogenic trauma (97.5%) and 1 patient gunshot wounds (2.5%). From iatrogenic ureteral trauma, 36 patients (90%) had iatrogenic trauma from the gynecological procedure, 5% as a result of the urological procedure, and 2.5% due to gastrointestinal procedure. Management of ureteral trauma is 47.5% ureteroureterostomy and 25% ureteroneocystostomy. Conclusion: Most cases of ureteral trauma are experienced by women, and most commonly due to iatrogenic trauma. Iatrogenic ureteral trauma caused by gynecology procedure is the most common.

Abstract

Objective: This study aims to compare outcomes of supine percutaneous nephrolithotomy to prone percutaneous nephrolithotomy as a traditional approach in Hasan Sadikin Hospital Bandung. Material & Methods: Data was taken from urology’s database between 2003-2012, which contain data of prone position (2003-2007), and supine position (2008-2012). Three hundred forty-nine patients were included in this study. We reviewed data on demography, stone burden, operating time, stone-free rate, transfusion rate, hospital stay, and major complications. Results: Of 174 patients underwent prone position, and 175 patients were part of the supine position. Age means of prone position was 51.0 years old, the supine position was 50.5 years old. The number of male patients was 65.5% for prone, and 66.1% for supine. The mean stone's largest diameter of prone was 23.87 mm, supine was 22.36 mm. The operating time of prone was 107 minutes; supine was 90 minutes. The stone-free rate of prone was 94.7%; supine was 91.3%. The mean hospital stays of prone was 14.3 days; supine was 9.6 days. The transfusion rate of prone position was 8.9%, supine was 7.2%. No major complications were recorded. Conclusion: Higher stone-free rates are achieved with patients in the prone position during PCNL. Supine position has shorter operating time, and hospital stay, lower blood transfusion rates. The complication rate is not different between the two positions.

Abstract

Objective: This research aims to identify factors influencing the degree of erectile dysfunction in bicycle racing athletes. Material & Methods: This study used quantitative analysis and cross-sectional research design. Samples included in this research were 54 respondents in accordance to the inclusion and exclusion criteria of this study, and were willing to participate in this research. The data were collected using the IIEF-5 (International Index of Erectile Function-5) questionnaire. The data were then edited, coded and analyzed using Chi-square (bivariate analysis). Results: The results showed correlations between the degree of erectile dysfunction in bicycle race athletes and age (PR= 1.182; 95%CI= 0.403-3.465; p= 0.976), cycling history (PR= 0.462; 95% CI= 0.490-4363; p= 0.687), cycling frequency (PR=1.400; 95% CI= 0.299-6.560; p= 0.72), cycling duration (PR= 0.333; 95% CI= 0.074-1.507; p= 0.165), furthest cycling distance (PR= 0.883; 95% CI= 0.267-2.919; p= 1.00), and type of saddle (PR= 0.271; 95% CI= 0.481-1.547; p= 0.221) respectively. Conclusion: There were no significant correlation (p= >0.05) in all variables analyzed in this study.

Abstract

Objective: To determine the effect of conditioned medium human Adipose-Derived Mesenchymal Stem Cells (CM-hADMSC) on apoptosis of urothelial bladder cancer cells. Material & Methods: Bladder (5637) cancer cell lines cultured in conditioned media harvested from human adipose-derived mesenchymal stem cells (hADMSC). Flow cytometry tests were carried out using the Flowcytometry Acquisition cell sorting (FACS) Calibur to measure apoptosis. Results: There was a significant difference in the percentage of late apoptosis in the group receiving culture medium treatment: CM-hADMSC 1: 1 to the entire study group. Further analysis revealed no difference in the average percentage of late apoptosis in groups exposed to culture medium: CM-hADMSC 1: 2 and culture medium: CM-hADMSC 1: 4 (p> 0.05). Conclusion: CM-hADMSC at a 1: 1 dose concentration to culture medium obtain a significant increase of apoptosis in bladder cancer cells.

Abstract

Objective: This study aimed to gives a perspective in CM-ADMSCs effect in urothelial bladder cancer viability. Material & Methods: Human bladder cell carcinoma type 5637 was used as the subject of this in vitro study. This study contains four different groups: untreated control group, Culture medium: hADMSCs with 1:1, 1:2, and 1:4 concentration group. Each group consists of 6 replications to prevent bias of the study. Viability was determined with MTT assay methods and evaluation performed after 48 h exposure of conditioned medium. Results: A post hoc test was conducted to analyze the data. The 5637 bladder cancer cell line demonstrated significantly decreased viability after exposure to culture medium: CM-hADMSCs 1:1 (p: 0.002) compared to the negative control group, but there are no significant differences in viability between the control groups with groups that were exposed to culture medium: CM-hADMSCs 1:2 and culture medium: CM-hADMSCs 1:4 with p: 0.480 and p: 0.060 respectively. Conclusion: Decreased viability of urothelial bladder cancer cells after exposure to CM-hADMSCs occurs at a concentration of 1:1 and Dosage addition more than 1:1 concentration doesn’t give any advantages.

Abstract

Objective: A comprehensive knowledge about BPH and hypertension is needed to reduce morbidity and mortality in patients with decreased kidney function due to the two diseases. Material & Methods: This research used the observational analytics method with cross-sectional design. This research used secondary data from medical records of BPH patients in Bina Sehat Hospital and Paru Hospital from January 1st to December 1st 2019. Results: Comparative serum creatinine test results in hypertensive BPH patients with non-hypertensive BPH patients using the Mann Whitney test, obtained p= 0.000. Based on these results it can be concluded that there are significant differences in serum creatinine between hypertensive BPH patients and non-hypertensive BPH patients. Correlation test results using the Spearman test between serum creatinine with blood pressure systole and diastole all samples obtained each p= 0.000 r= 0.399 for systolic blood pressure and p= 0.000 r= 0.337 for diastole blood pressure. Based on these results it can be concluded that there is a significant correlation between serum creatinine with systolic and diastolic blood pressure of the patient. Conclusion: Hypertension will worsen the kidney function decline in BPH patients seen from an increase in serum creatinine.
