Abstract

Introduction: Penile fracture is a rather infrequent occurrence. The incidence of penile fractures is predominantly associated with instances of direct trauma that occur during sexual intercourse. This case report provides a case of a middle-aged man with penile trauma after a straddle injury that mimics penile fracture symptoms. Case(s) Presentation: A 42-year-old male came to primary healthcare due to pain in penis while sexual intercourse with women on top position. He complained the penis was immediately detumescence, swelling, and bent into an “L” shaped position. At the physical examination, vital signs and general examinations were normal, but at the genital site, there was a hematoma at the right side of corpus cavernous midshaft of the penis. This hematoma also made a curve in the penis that caused angulation of the penis into an “L” shape. Discussion: Ultrasound examination is needed to evaluate the patient thus, he was referred to the hospital with penile angulation and penile hematoma as the main diagnosis. Ultrasound imaging showed free fluid at the outer part of the tunica albuginea of a penis. Structural damage was not examined in the urethra and tunica albuginea of the corpus cavernosum. Conclusion: This patient's symptoms of pain, angulation, detumescence, and swelling are similar to penile fracture. However, we noticed that penile fracture is a tear of the tunica albuginea of the corpus cavernous that has to be proved by imaging diagnostic tools. Keywords: Penile fracture, hematoma, trauma.

Abstract

Introduction: Tuberculosis is an infection that involves multiple organs. Testicular tuberculosis is a rare type of extra-pulmonary tuberculosis, accounting for only 3% of total genital tuberculosis cases. Objective: We report a case of testicular tuberculosis in a 40 year old male patient with severe pain in the right testicle for the last one month. Case(s) Presentation: Patients with a history of pulmonary tuberculosis and undergoing intensive phase anti-tuberculosis drug therapy. The patient has a fever accompanied by redness, swelling, and puffiness in the scrotal area. Laboratory examination, leukocytosis and no increase in ESR. Testicular ultrasound examination showed swelling in the right scrotum area as well as normal size and hydrocele in the left testicle. Discussion: The patient was diagnosed with testicular tuberculosis and underwent orchidectomy on the right testicle, followed by anti-tuberculosis drug treatment for six months, then continued with an anatomical pathology examination. Anatomical pathology results showed specific chronic granulomatous inflammation due to tuberculosis in the right testicle. Conclusion: The combination of discontinuation and OAT needs to be done immediately to prevent further complications. Keywords: Tuberculosis, testicles, urogenital, orchidectomy.

Abstract

Introduction: Benign prostatic hyperplasia (BPH) is a common chronic condition frequently associated with the progressive development of voiding and obstructive lower urinary tract symptoms (LUTSs). Objective: This research aims to assess the effectiveness of the Rezūm system for BPH. Material & Methods: A literature search was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Clinical baseline characteristics, international prostate symptom score (IPSS), maximum flow rate (Qmax), postvoid residual (PVR), and quality of life (QoL) were analyzed using Revman 5.3. The risk of bias assessments was conducted by two independent authors and resolved through discussion. Results: A total of 288 studies were retrieved from PubMed, Cochrane, and Google Scholar. Four studies, including 590 patients, were analyzed, with a follow-up period of up to 12 months after treatment using the Rezūm system. The mean (SD) change from baseline to follow-up in IPSS improved from 20.58 (5.9) to 9.53 (6.7); Qmax increased from 9.2 (3.2) to 13.48 (6.5); PVR decreased from 128.2 (137.8) to 76.0 (88.8); and QoL improved from 4.6 (1.2) to 2.0 (1.4). The greatest improvement was observed in QoL (56.52%) followed by IPSS (55.88%), Qmax (46.74%), and PVR (40.72%). Conclusion: The Rezūm system is an effective therapy for BPH, providing significant relief of LUTSs. Keywords: Benign prostatic hyperplasia, LUTS, Rezūm system

Abstract

Introduction: The paediatric urology fellowship program was designed to accelerate the learning curve of junior urologist in various paediatric urology procedures, including hypospadias repair. Objective: This study aimed to provide insight in the learning curve of hypospadia repair during the paediatric urology fellowship program. Material & Methods: Three paediatric urology fellows performed 176 hypospadias repairs at four Jakarta, Indonesia, centers throughout their six-month fellowship training program between January 2022 and January 2023. Secondary repair or the initial stage of staged repair were excluded from the analysis. Complications such as urethrocutaneous fistula formation and wound dehiscence requiring secondary repair were documented. The learning curve of each fellow in hypospadias repair was documented. Results: Each fellow performed a median of 58 procedures (range: 56–62) during their training period. A total of 34 (19.3%) experienced complications, including 19 cases of fistula formation and 15 cases of wound dehiscence, over a mean follow-up period of 24.1±17.1 weeks. There were no significant differences in demographics, distribution of hypospadias, or complication rates observed among the fellows. Notably, there was a significant improvement in the learning curve for each fellow after performing 30 consecutive cases, achieving a success rate of over 84%. The average success rate showed a significant increase from 74.4% to 87.2% after 30 cases (p=0.032). Conclusion: Significant improvement of hypospadia repair success rate was observed among paediatric urology fellows after performing 30 consecutive hypospadia repair during their training. This improvement suggests that the fellowship program likely played a pivotal role in accelerating their learning curve. Keywords: Fellowship, hypospadias, learning curve, urethrocutaneous.

Abstract

Introduction: Kidney transplantation may encounter challenges from medical or surgical complications. Iatrogenic injuries are responsible for more than half of ureteral stricture cases, which are a rare (0.6% to 12.5%) yet complex challenge in the realm of urology surgery. Objective: In this report, we presented a case stricture ureteral resulting from iatrogenic trauma in the context of kidney transplantation that was treated using pyelo-native-ureteral anastomosis. Case(s) presentation: A 33-year-old woman post-kidney transplant with iatrogenic ureteral trauma and developed recurrent urinary tract infections (UTI), Retrograde Pyelogram (RPG) was performed showing a partial stricture of proximal and distal right ureteral. Pyelo-to-Native Ureteral Anastomosis was performed to overcome the complication. After 35 days, kidney function was improved, USG and RPG for the improvement of hydronephrosis. Discussion: Pyelo-to-native ureteral anastomosis is a feasible treatment option for complex post-transplant ureteral strictures, particularly when endourological approaches are unsuccessful. Conclusion: Pyelo-to-native ureteral anastomosis can be an alternative treatment for stricture following kidney transplantation. Keywords: Kidney transplant, ureteral stricture, pyelo-to-native ureteral anastomoses.

Abstract

Introduction: Hypospadias repair may cause long-term urinary dysfunction. Collagen, inflammation, and fibrosis may influence urethral healing and urinary flow, but their association with postoperative Qmax remains unclear. Objective: This study aims to investigate the relationship between collagen, inflammation, and fibrosis with maximum urinary flow rate in patients with hypospadias after urethroplasty. Material & Methods: This retrospective cohort study included 72 hypospadias patients who underwent surgery at Ngoerah General Hospital, Denpasar, from 2003 to 2013. Urethral tissue samples were histopathologically assessed for collagen, inflammation, and fibrosis. Urinary flow was evaluated 10 years postoperatively using uroflowmetry, with Qmax categorized based on the optimal ROC-derived cutoff. Associations were analyzed using chi-square and logistic regression. Results: The results showed a significant association between collagen and maximum urinary flow rate (RR=0.573; 95% CI=0.350–0.936; p=0.019), where low collagen levels were protective against weak urinary stream. Moderate-to-severe fibrosis increased the risk of weak urinary stream by 1.6 times (RR=1.659; 95% CI=1.071–2.568; p=0.026). However, no significant association was found between inflammation and maximum urinary flow rate (p=0.964). Logistic regression analysis indicated that patient age was significantly associated with maximum urinary flow rate (OR=1.32; p=0.002). Conclusion: There is an association between collagen levels and maximum urinary flow rate in patients after hypospadias surgery. However, no significant association was found between inflammation and fibrosis with urinary flow. Patient age was the dominant factor influencing urinary flow post-surgery. Keywords: Collagen, inflammation, fibrosis, maximum urinary flow rate, urethroplasty.

Abstract

Introduction: Urinary incontinence (UI) is common in women and may impair quality of life. Pelvic floor muscle dysfunction contributes to stress and mixed urinary incontinence. High-Intensity Focused Electromagnetic (HIFEM) therapy is a promising non-invasive modality for improving pelvic floor muscle function. Objective: This study aims to evaluate whether HIFEM can offer a more effective alternative for managing urinary incontinence. Material & Methods: 31 women were enrolled in this cohort study. Patients are assessed before and after intervention using the International Consultation on Incontinence Questionnaire—Urinary Incontinence—Short Form (ICIQ-UI-SF). The intervention is a 28-minute HIFEM stimulation with 33 sequences and intensities between 50 to 100% and sequential amplitude modulation, once a week within 4 weeks. Results: 26 participants (83.8%), dominated by mixed-type UI (17 patients), followed by 7 stress-type and 2 urge-type, have experienced an improvement in UI symptoms and showed improvement in ICIQ scores, while the other 5 participants (16.2%) showed insignificant progress after the intervention. The enhanced group reported a transition from moderate-severe symptoms to mild symptoms or complete recovery, and statistical analysis showed a significance value of p = 0.001 (p < 0.05). Conclusion: The HIFEM stimulation procedure significantly alleviates urinary incontinence patients, particularly the mixed-type. Keywords: HIFEM, urinary incontinence, pelvic floor muscles training.
