Abstract

Fatigue experienced by most of the public transport drivers cause a decrease in the level of vigilance while driving . The demands of the company or demands to be able to make ends meet cause most of the public bus drivers keep working even in conditions of fatigue so that the condition often causes accidents . To determine the level of awareness of the driver while driving is done by spreading the data collection questionnaire and use vigilance test software . Data obtained by using software vigilance test is Average Reaction Time ( ART ) . The data is then analyzed to see regression correlation relation to the number of variables which are , long drive , long breaks and the number of breaks . From the analysis it was concluded that the ART is directly proportional to the length of time driving or driving , but inversely proportional to the length and number of rest breaks , where the longer breaks and more breaks then the value of ART will be smaller .

Abstract

In the cigarette industry for meeting consumer needs not only provide good taste, but also the fulfillment of the availability of goods. It required a good coordination between related parties such as suppliers, factory, distributors, retailers, and so on. Parties involved in the procurement of raw materials to the distribution of products to consumers is known as the supply chain. Lack of coordination will lead to information distortion phenomenon called the bullwhip effect. PT. U.S. Purwosari a cigarette factory with one of its products is PMI and product distribution in Borneo. At PMI Product frequent urgent packaging materials resulting in subsequent orders and production delays. Based on these in this study to identify the presence of the bullwhip effect on PT. U.S. to calculate its value and find the cause.

Abstract

Abstract The quality of health services is required by the health implementing-organization as a strategy to win the competition. In order to improve the quality of health services, there should be a sustainable improvement in all aspects of quality. This study aims to perform the identification of the quality of the existing services and determine the priorities for resolving the problem and improving the customer satisfaction. It applies Servqual method with five dimensions: tangible, responsiveness, reliable, assurance and empathy; and 25 research attributes. The questionnaires given to the respondents consist of three things: a questionnaire for the identification of the respondents, of the level of customer perception and of the level of customer expectation. The research results that the highest gap between the consumer perception and the consumer expectation found in the dimension of empathy and the attributes of the medical workers’ patience and amiability during health examination with the value of -1.05. These results indicate that the priority of health care quality improvement is by increasing the medical workers’ ability in doing health examinations with patience and amiability.

Abstract

Abstract The fresh fish demand increase in the Sidoarjo district can be seen from the fish production amount increase which could be found in the fish market. The fish market became a fresh fish distribution center from fish farmers to customers. To keep the qualities of fish and its distribution services, designing the system design of fish distribution is need to be conducted. The aim of this research is to design a system design of fish distribution by integrating physical distribution service quality (PDSQ ) methods and lean six sigma methods at a fish market in Sidoarjo. This research is conducted to give a suggestion for fisheries and maritime department of Sidoarjo in providing services to public. This research began by measuring the quality of service by using the PDSQ method, then the highest of negative gap value would be identified to find “the waste” by lean method during the distribution process from producer to consumer which affects the quality of service, and than measured capability process using the six sigma method. By using both methods in implementing the system design improvement of fish distribution in Sidoarjo, the results show that the quality of services has increased the distribution of fish by the presence of some improvements facilities. These can be seen from the value of fish qualities which has increased in the reckoning of customer satisfaction from the gap value of each dimensions namely timeline of -2.08  becomes -0.81, availability dimension of -3.36 becomes -1.7, and condition dimension of -1.96 becomes -1.17, and scores of  process capabilities of 4.11 becomes 4.43. The improvement of system design from this existing condition is by involving integrated service unit (UPT) of fish market as a control of the quality of fish and its distribution process to increase the customer satisfaction.

Abstract

Abstract Customer satisfaction was the main business factor. After sales service was one of the most important to ensure customer satisfaction. The main criteria for customer satisfaction we know as speed, fit for use, and fair. Human always use analogical method for make some decisions, such as use historical experience for fit the similar problem. If the old case in after sales service could be saved dan organized efectivelly and sistematix, so we can use it to solve the new problem that have similar type. Method that use old case as a reference to solve new proble in Artificial Intelligent (AI) we always call Case Based Reasoning (CBR). This paper use CBR System for car sales after service problem. We also use Esteem software for improvement tools. After use this tools we get some resume that process in sevice become effective and efficient.

Abstract

Abstract One of increasing in product can be undertaken by working up the activity that it has value added for each their products and getting rid of various kind of extravagances, or it is called by Lean Thinking. In this research, the writer uses An approach to integrate the concept of Lean Six Sigma, Multi Attribute Failure Mode Analysis and Fuzzy AHP is to identify the cause of Potential Defect to reduce the defect product on the particle board product.  From the results of the regressive analysis, it performs that the independent variable  (Potential Defect) takes an important role to the mount of production that it can be stared to the Defect which has a high quality. Furthermore, the defect is called as blister. So, in this research, the writer is interested to study the defect blister. The causative element of blister itself consists of  some elements. They are machine factor, human, and material elements. There are 17 elements that found in it, according to the result of analysis in using the concept of approachment Lean, Six Sigma, Multi Attribute Failure Mode Analysis and FUZZY AHP is useful to identify (the element M) that it has over size. So that, it is able to cause the failure  of potential defect with a good quality that it is as big as 0,087. So, it can be stated that the conformation of standard to the output chipper machine and do a compliance to the Gap knife ring to the Flaker machine, so that it is able to reduce the potential defect to the tune of 39,5%.

Abstract

Abstract The goal of lean six sigma method is to solve the problems of waste, especially defect. The goal of failure mode and effect analysis (FMEA) method is to be able to identify potential failures in the production process of e.g. liquid soap bottle in order to reduce production failure and to have reference from FMEA highest risk priority number for corrective actions. The goal of fuzzy method is to improve priority risks of FMEA method. By using fuzzy method the priority risks with the same value can be eliminated so that only one highest value can be used as a reference for corrective action. Result of lean six sigma method are four types of critical defects i .e. dirty black, lines in the wall of the bottle, neck narrowed / clogged, and uneven surfaces. One result of the FMEA calculation with the highest risk priority number (RPN) is dirty black defect. This is due to less attention to the composition of the material and contamination / burnt crust. The highest score of RPN is 48 and ranked as the first priority for actions. While result of fuzzy calculations is the highest fuzzy risk priority number (FRPN) with value of 3.327831. This is caused by dirty black defect due to less attention to the composition of the material. Therefore from these results of the highest causes of defects can be used as a reference for corrective action.

Abstract

Abstract In this study done on leather shoe company that uses flow shop scheduling strategies. Where the purpose of this study is to minimize makespan is the total time needed to complete the entire job. This is done because of the large demand for leather shoes that exceeds the production capacity and production scheduling by the company based on intuition. In solving the production scheduling problem that occurs in the company, Cross Entropy algorithm used method-Genetic Algorithm (CEGA) to minimize the makespan. From the results of calculations with CEGA method sequences obtained by scheduling makespan value of 5822 seconds with an efficiency of 6.79% when compared with the method of the company with sebasar makespan value of 6246 seconds.
