Abstract

Adherence to medication in elderly patients is crucial to support their health, as many suffer from chronic non-communicable diseases requiring long-term treatment. The complexity and volume of medications can lead to patient fatigue. Identifying factors that affect adherence is key to addressing these challenges. This study aims to identify the factors influencing medication compliance among elderly patients at PKU Muhammadiyah Gamping Hospital’s outpatient clinic. This quantitative, cross-sectional study involved 80 elderly patients at the internal medicine clinic, selected through accidental sampling. Data was collected using the MMAS-8 (Modified Morisky Adherence Scale-8) and additional questionnaires to determine the factors influencing therapy adherence, analyzed through multiple linear regression. Results showed that compliance was low in 21.3% of patients, moderate in 43.8%, and high in 35.0%. Statistical analysis found significant influences from education level (p<0.05), illness duration (p<0.05), knowledge level (p<0.05), health service access (p<0.05), family support (p<0.05), healthcare workers’ role (p=0.029), and treatment motivation (p<0.05). Compliance among elderly patients at PKU Muhammadiyah Gamping Hospital falls in the moderate category, with these factors explaining 75.6% of adherence levels.

Abstract

Sidr leaf extract (Ziziphus mauritiana Lam.) is effective against hypercholesterolemia by blocking the HMG-CoA reductase. This study seeks to identify the characteristics of tablets formulated from the ethanol extract of Sidr leaf and their efficacy in lowering total blood cholesterol levels. The tablets were produced using the wet granulation method and processed into three formulations, F1, F2, and F3, with differing dosages of 100, 150, and 250 mg, along with placebo. The granules produced were evaluated for their physical characteristics. The tablets were evaluated for their physical characteristics and tested for effectiveness in lowering cholesterol levels. The efficacy of tablet-form sidr leaf ethanol extract was assessed by analyzing cholesterol levels in mouse serum samples compared to simvastatin. The data were statistically analyzed using one-way ANOVA and post-hoc Tukey HSD test. In the disintegration time test, F1 and F2 tablets satisfied the criteria. However, F3 exceeded the limit of 60 minutes. Among the three tablet doses, the F2 tablet (150 mg) demonstrated superior efficacy in lowering total cholesterol levels relative to the F1 and F3 tablets. Consequently, tablets containing Sidr leaf extract demonstrate potential for hypercholesterolemia management at an effective dosage of 150 mg, warranting further investigation through clinical trials.

Abstract

Parijoto fruit (Medinilla speciosa) contains flavonoid compounds that are potentially immunomodulators because they increase macrophage phagocytosis activity and lymphocyte proliferation in vitro. This study aims to determine the phytochemical compound content of parijoto fruit and its potential as an immunomodulatory agent in vivo. Parijoto fruit was extracted using 70% ethanol solvent. The ethanol extract of parijoto fruit (EEPF) was qualitatively identified by phytochemical screening. The immunomodulatory activity test in vivo was conducted using BALB/c mice, which were separated into five groups: untreated, Phyllanthus niruri L. (PN) extract (positive control), and test groups (125mg/KgBW; 250mg/KgBW and 375mg/KgBW). The treatment duration was 18 days, with the induction of the hepatitis B vaccine on days 7 and 14. An immunomodulatory activity test was conducted on the 19th day, which included a macrophage phagocytosis activity test, lymphocyte proliferation, and measurement of IgG levels. The identification results showed that EEPF contains flavonoids, tannins, and saponins. Macrophage phagocytosis activity showed that EEPF increased macrophages' phagocytosis capacity (PC) and phagocytosis index (PI). EEPF increased lymphocyte proliferation activity and IgG levels, significantly different from control and not substantially different from positive control. Collectively, EEPF increases immunomodulatory activity in vivo.

Abstract

Adolescents are a group that is very intensively exposed to changes from the Internet of Things, resulting in reduced exploration of nature, including medicinal plants. This study explores behavioral factors related to the use of herbals among Indonesian adolescents by applying the theory of planned behavior (TPB). The study was an analytic with a cross-sectional design. The independent variables were attitude, subjective norm, and perceived behavioral control. The dependent variable was behavioral intention. The samples were adolescents aged 10 to 19 years, determined from 200 samples in each of Indonesia's five major island groups, so 1,000 respondents were recruited. The instrument was a validated questionnaire that refers to the TPB constructs. Data were analyzed with a chi-square test and regression. 931 respondents participated in the research; the response rate was 93.1%. Approximately 85.1% of the participants expressed an intention to use traditional medicine. There is a statistically significant association between attitude, subjective norms, perceived behavioral control, and the purpose of using traditional medicine. The combined effect of the three constructs accounts for 25.9% of adolescents' propensity for using traditional medicines. All the factors contributed partially, with the subjective norm being the most significant contributor.

Abstract

Indonesia is a tropical country whose population is potentially exposed to ultraviolet (UV) radiation. Continuous exposure to UV radiation triggers the formation of free radicals that cause damage at the cell, tissue and organ levels. Dandang gendis (Clinacanthus nutans) is known for its diverse pharmacological activities, including its role as a source of antioxidants. This study aims to evaluate the antioxidant activity of specific fractions of dandang gendis plant through experimental methods. The fractionation process was carried out by stepwise maceration method. Antioxidant activity test was conducted using DPPH (2,2-diphenyl-1-picrylhydrazyl) method, which measures the ability of the extract to capture DPPH free radicals. The results obtained were IC50 values for vitamin C control at 1.25 μg/mL, ethanol fraction at 36.63 μg/mL, ethyl acetate fraction at 5.07 μg/mL, and n-hexane fraction at 40.19 μg/mL. These findings indicate that the ethyl acetate fraction of Dandang gendis has very strong antioxidant activity. Therefore, it can be concluded that the ethyl acetate fraction of Clinacanthus nutans leaves shows very strong potential antioxidant activity, with the lowest IC50 value compared to the ethanol fraction and the n-hexane fraction, so it has a great opportunity to be utilized as an external antioxidant.

Abstract

Vetiveria zizanioides (L.) Nash is one of the plants with high flavonoid compound content with antioxidant activity. This study aims to formulate a spray gel of the root of Vetiveria zizanioides (L.) Nash infusion determines UV protection and observes the level of irritation caused by the application.  This study used formulas at variations concentrations FI 1%, FII 5%, and FIII 10%. The formulas observed flavonoid content, sun protection factor (SPF) value, and irritation index. Flavonoid content was tested using the Willstatter method. The SPF value was determined in vitro using UV-spectrophotometer, while the irritation test was conducted using the Draize test method. The SPF value test results showed that Formula 0 (F0), FI, FII, FIII, and the commercial preparation had SPF values of 0.355, 2.856, 5.324, 6.634, and 13.438, respectively. Meanwhile, the irritation test obtained an irritation infection of 0.3 in the commercial preparation and 0.6 in F3. Therefore, FII has the potential to be UV-protected and have a minimal irritating effect.

Abstract

The expense of managing CKD patients ranks second highest behind cardiovascular illness, with hypertension being the primary contributor to renal impairment. Maximizing the efficiency of therapeutic expenditures is essential. This study is to evaluate the cost-effectiveness of the combination of amlodipine 10 mg - candesartan 16 mg compared to the combination of candesartan 16 mg - furosemide 40 mg in patients with chronic kidney disease and hypertension. The research employed a non-experimental observational cohort design. The study was performed at both a private and a governmental hospital in Yogyakarta, Indonesia. Data were gathered retroactively using complete sampling. The inclusion criteria comprised outpatients with CKD-hypertension from January 2019 to October 2022, aged 18 years or older, who were undergoing combination therapy. The efficacy of therapy was evaluated by determining the number of samples achieving therapeutic objectives after 2 to 4 weeks. The mean expenditure was derived from monthly antihypertensive costs. The analysis of cost-effectiveness employed the Average Cost-Effectiveness Ratio (ACER) and the Incremental Cost-Effectiveness Ratio (ICER). The study comprised 50 samples, with 51% male participants and 66% aged 60 or older. Amlodipine and candesartan demonstrated superior efficacy (77%) and a reduced average cost of 101,095 IDR. The ACER for amlodipine and candesartan was 1,306 IDR, whereas for furosemide and candesartan it was 1,571 IDR. The amalgamation of amlodipine and candesartan is more economically advantageous than furosemide and candesartan.

Abstract

The escalating utilization of antipsychotics undeniably contributes to the heightened risk of drug side effects. This study aims to identify antipsychotic side effects and their causality. This study belongs to observational research with a descriptive cross-sectional design. The participants were patients prescribed antipsychotic drugs at Hospital X Yogyakarta between April and July 2022 who met the specified inclusion criteria. This study utilized accidental sampling, generating 61 patients as the participants. Data collection was carried out prospectively through interviews and medical records. The Naranjo algorithm was used to analyze causality. The predominant drug type administered to patients was atypical antipsychotics, with a total of 72 prescriptions. Risperidone accounted for the highest number of prescriptions, totaling 37 (22.16%). Weight gain contributed to the highest percentage of reported side effects, with 16 (16.33%) instances. The side effects with the highest causality score of 7 (probable) were breast secretion, tremors, palpitations, weight gain, and decreased blood pressure. Weight gain was the most frequently observed side effect when taken together. The maximum score for the causality association of side effects was an average of 7, indicating a probable connection. This study provides insight into the numerous side effects associated with prescription antipsychotic drugs.
