Abstract

Community Health Center Poasia, as a public service, aims to provide quality health services. The existence of complaints from patients related to the services received can have an impact on patient visits, which in 2023 decreased by around 21%. One of the causes of decreased patient visits is that patients are not satisfied with the services at the Poasia Health Center. The quality of health services can be measured using the Servqual and ZoT methods. The Servqual method measures the GAP value between the performance value and the expected value, followed by ZoT to see whether the tolerance limit range of patient dissatisfaction is still acceptable. Respondents will be interviewed using a questionnaire. The results of this study showed that the GAP value in the responsiveness dimension is still negative (-0.04), indicating that some patients are dissatisfied with the services received. The GAP value for the other four dimensions is positive, indicating that patients are satisfied with the services received at the health center. The ZoT value in the tolerance limit range is -0.01-2.24. The negative ZoT value indicates that some patients are dissatisfied, and the positive ZoT value of the patient is still within the tolerance limit for receiving minimum service. This study concludes that quality health services will provide satisfaction to patients.

Abstract

Antibiotic resistance in the treatment of diabetic wound infections is common. Piper betle leaf extract has antibacterial, anti-inflammatory, and antioxidant activities. This study aims to formulate a spray gel from Piper betle leaf extract and test its effect on diabetic wounds infected with Methicillin-Resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa. The gel was made with 10% (F1), 15% (F2), and 20% (F3) extracts. The organoleptic physical characteristics, pH, homogeneity, viscosity, spray pattern, adhesive spreadability, drying time, and physical stability were tested. All rats were given alloxan 170 mg/kgBW, fasting glucose levels were measured, and then grouped into control, MRSA-exposed, and P. aeruginosa-exposed groups. The negative and positive controls were also included in the test. A 2 cm wound with a depth of 0.3 cm was given bacteria, except for the normal control. The length of the wound was measured, and the wound reduction was calculated as a percentage of wound closure and statistically analyzed. All the formulas met the physical criteria, significantly closing MRSA wounds by 100%. F2 and F3 significantly closed P. aeruginosa wounds by 89.88% and 93.13%, respectively. Collectively, the Piper betle leaf extract spray gel had an effect that could relieve diabetic wound infections.

Abstract

Healthcare expenditure due to diabetes mellitus in Indonesia reaches approximately US$143 per person. The largest portion of this cost is attributed to medications. Therefore, ensuring the cost-efficiency and effectiveness of drug use is critically important. This study aims to determine and analyze the cost-effectiveness of antidiabetic drug therapy in patients with type 2 diabetes mellitus in the outpatient department of Bahteramas Hospital in Southeast Sulawesi Province. Descriptive methods and quantitative analysis of medical record data from Type 2 diabetes mellitus (DM) patients and treatment costs at Bahteramas Hospital in 2023. A total of 61 patients were included in the study based on the inclusion criteria. Based on the average cost-effectiveness ratio (ACER), glimepiride was more efficient, with a value of IDR 18,389, compared to metformin, which had an ACER of IDR 25,405. However, based on the incremental cost-effectiveness ratio (ICER), metformin was more cost-effective, with a value of IDR 50,070. While the biguanide group (metformin) appears more cost-effective than the sulfonylurea group (glimepiride) based on ICER, the overall findings indicate that, based on ACER, glimepiride (sulfonylurea) is more cost-effective. Therefore, glimepiride can be recommended as a single oral antidiabetic therapy at Bahteramas Hospital.

Abstract

Honey is a natural product rich in bioactive compounds with various pharmacological and anti-inflammatory effects. Bee species and nectar sources influence the composition of bioactive compounds in honey. This study aimed to identify bioactive compounds and evaluate the anti-inflammatory activity of honey from Apis mellifera and Apis dorsata. Compound identification from both honey types was performed using Gas Chromatography-Mass Spectrometry (GC-MS). At the same time, anti-inflammatory activity was assessed through the human red blood cell (HRBC) membrane stabilization method and protein denaturation inhibition assay, with sodium diclofenac as a positive control. GC-MS analysis revealed various bioactive compounds, notably 2-Propenoic acid, 3-(4-methoxyphenyl)-2-ethylhexyl ester, consistently present in both honey types. In the HRBC assay, Apis mellifera honey showed the highest stabilization value of 94.14% at a concentration of 6.25 mg/L honey solution, surpassing sodium diclofenac (87.96%), while Apis dorsata honey reached 85.21%. All samples achieved over 95% stabilization at 100 mg/L. In the protein denaturation assay, Apis dorsata honey exhibited stronger anti-inflammatory potential with an IC₅₀ of 12.85 ± 1.477 mg/L compared to Apis mellifera (25.35 ± 0.024 mg/L). In conclusion, both types of honey possess promising natural anti-inflammatory potential through membrane stabilization and protein protection mechanisms.

Abstract

The pharmaceutical business in Indonesia, including in Kendari City, has shown a positive growth trend in recent years. Pharmacies compete by applying different marketing strategies, with some adopting a conventional model and others utilizing a modern retail approach, such as self-service pharmacies. Pharmacy X, a modern self-service pharmacy, is perceived to have a strong brand image and standardized services. This study evaluates the influence of brand image and perceived service quality on customer loyalty in a modern pharmacy setting. Data were collected via a Google Form questionnaire from 100 customers who had visited twice during the period of October to November 2024. The data were analyzed using descriptive statistics and Structural Equation Modelling (SEM) with SmartPLS. Results revealed that brand image significantly affects customer loyalty, while service quality does not show a significant impact. Brand image contributed to customer loyalty with an influence value of 33.3% based on the R-squared value, indicating a moderate effect, and a Q-squared value of 0.275, reflecting moderate predictive accuracy. Brand image is critical in enhancing customer loyalty at Pharmacy X, while service quality may require further improvement or reassessment in its current implementation.
