Abstract

Microhybrid composite resin is the recent development of hybrid composite resin which has a particle size 0,6-0,8 µm and the microfiller size is 0,04 µm. Microhybrid composite resin consist of up to 78% of filler. It increases the mechanical feature of the composite. Bonding is the material that adhere composite resin to email or dentin, in order to reduce the microleakage restoration. The aim of this study was to evaluate the microleakage differences between the fifth generation bonding and seventh generation bonding in fifth class restoration of microhybrid composite resin. It was in vitro experimental laboratories. The samples were post extraction tooth that prepared into fifth class cavity of Black classification. There were two groups, the first group was using fifth bonding generation and the second was seventh bonding generation. Microleakage was measured by observing the penetration of 2% methylen blue through by stereomicroscope. Data were analyzed by Mann-Whitney U. The result showed significant differences of microleakage between groups (p<0,05). This study was concluded that microleakage in seventh bonding generation more common than fifth bonding generation.

Abstract

Diabetes mellitus is one of major chronic health problems and the prevalence increases every year. Poor blood glucose control or uncontrolled hyperglycemia condition is the major reason diabetes mellitus can cause some complication in oral. As of this condition, numerous diabetes mellitus sufferers have poor quality of life because inadequate in mastication. For improving quality of life diabetic patients, they need denture to help them in mastication process. However, there some debates about denture for diabetic patients; because diabetes mellitus can be contraindication in prosthodontic treatment. Aim this study is to know the management in prosthodontic treatment of diabetic patients as rehabilitation and final treatment in dentistry. Diabetes mellitus patient management in prosthodontic are generally as followed medical history, check up blood glucose level, stress management, intra oral and radiography examination, oral and denture hygiene, diet consultation and denture elective. All of type of dental prosthesis can be considered to diabetic patients principally. Type of dental prosthesis that can be worn by diabetic patients are removable denture (partial and complete), fixed denture, overdenture, and implant. Pre treatment before making and inserting dental prosthesis must be finished. If there is dental caries, dental caries must be restored by appropriate restoration, such as filling, endodontic treatment, rigid restoration, etc. Moreover, the dentist must do plaque and calculus removing. The conclusion of this review is diabetes mellitus could be considered to take prosthodontic treatment, but the dentist must consider their blood glucose level.

Abstract

Perimenopause is trantition phase of nomal ovulation cicle to menopause that signed decreasingestrogen hormone production. In this phase, women will be physiology and psychology alteration related fluctuationof estrogen hormone. The estrogen level alteration is one of cause reducing of bone density in perimenopausewomen. When estrogen level decreases, it will induce osteoclast activity and finally cause bone resorption. In boneresorption osteoclast secretes protease that can dissolve collagen which is in organic matrix and free bone mineral.Mature collagen is bound by pyridinium crosslink that it will be degraded in bone resorption process, such asdeoxypyridinoline. Deoxypyridinoline have suggested as collagen degradtion marker of bone resorption in serumand urine. This study was objective to identify deoxypyridinoline in saliva of perimenopause women. The subjects ofthis study were perimenopause women who were 40-55 years old. The subjects had to fulfill informed consent and beundergone intra oral examination, including caries and periodontal index index. After that the subjects collectedtheir whole saliva. Deoxypyridinoline in saliva was identified by Liquid chromatography Mass Spectroscopy. The resultshowed that deoxypyridinoline level of perimenopause women was about 23.802 ppm. This level was higher thanproductive women. The conclusion was deoxypyridinoline level in saliva of permenopause women

Abstract

Tooth extraction is the most common dental treatment procedure in Indonesia. This is happened due to lack of knowledge and awareness of the effect of tooth extraction. Many patients requested dental extraction for some reason such as do not stand the pain anymore, dental treatment is expensive and long, althought the fact that such cases can still be treated. This condition called irrational extraction. Irrational extraction may cause loss of important componets for bone regeneration such as periodontal ligament stem cells/PDLSC, dental pulp stem cells/DPSC, extracellular matrix/ECM and local growth factor. PDLSC has potential differentiation into osteoblast, fibroblast periodontal ligament, and sementoblast. DPSC induces odontontoblast to form reparative dentin. Local growth factors are important component for hard and soft tissue regeneration. Due to loss of important component for bone regeneration, bone became small, thin, and brittle, which allows bone fracture. The purpose of this review is to explain the effect of irrational extraction on mesenchymal stem cell, periodontal ligament, ECM, and local growth factor. This knowledge is expected to reduce irrational tooth extraction.

Abstract

Drugs and health storage are components of public health services included dental health service in public health center and hospital. Public health center consider root canal treatment and medicament that used for that treatment. The objective of this study was to know treatment of pulp gangrene in four public health centers and a hospital. This study was descriptive observational   with cross sectional method. Population of this study was patients who diagnosed pulp gangrene in four public health center and a hospital on Juli to Agustus 2013.  This study used probability sampling for taking samples in population continued simple random sampling. The result showed the treatments of pulp gangrene were medication, extraction, and sterilization. Treatment in public health center focused in preventive treatment.

Abstract

Diabetes is a metabolic disorder characterized by high blood glucose levels or hyperglycemia. Diabetes can cause damage to multiple organs, such as the heart, eyes, kidneys, nerves, and vascular system, as well as an increased in periodontal disease. The periodontal disease mechanisms in diabetic conditions including, an increased of alveolar bone resorption, osteoclast formation, and osteoblast apoptosis. The activity of alveolar bone resorption were initiated by osteoblast which was secreted a key mediator of osteoclastogenesis process, namely the receptoractivator of nuclear factor-κB ligand (RANKL). This article will be discuss about the role of RANKL in the alveolar bone resorption process in diabetes conditions, which would be usefull for studying the diabetes complication such as alveolar bone resorption through the mechanism that related with the role of RANKL.

Abstract

Cyclooxygenase-2 is part of the cyclooxygenase enzyme that plays a role in the inflammatory process has produce of prostaglandins (PG) such as PGE2. Prostaglandin E2 plays an important role in the pathogenesis of periodontal disease because it leads to increased vascular permeability, vasodilatory, edema. This review intends to give an overview the role of COX-2  in oral diseases likely periodontitis to be better understood and the future of our research that may facilitate the development of diagnosis and therapies. Cyclooxygenase is enzyme that plays of catalyze biosynthesis of prostaglandins and contained in two forms of COX-1 and COX-2. Cyclooxygenase-2 activity has increased in inflammation especially wound formation and periodontitis. In periodontitis, invasion of lipopolysacharide can Increase COX-2 activity in smooth blood muscle that can lead atherosclerosis. COX-2 inhibition can lead healing process that characterized fibroblast formation.it is concluded that the presence COX-2 in gingival tissue and gingival crevicular fluid can be used as biomarkers about the severity of periodontal disease, healing process and risk of atherosclerosis.

Abstract

Until todays, dental caries disease is a major problem in a child's oral cavity. Children aged 6-10 years old are vulnerable to dental and oral diseases. The aim of this study was to describe the severity of dental caries in primary school children in 10 districts in Jember. An observational study with cross sectional approach was conducted in July-September 2015. Accidental sampling was 102 children of primary school grade 1 to grade 6 in 10 districts in Jember (Ambulu, Balung, Ledokombo, Tanggul, Mayang, Gumukmas, Jenggawah, Pakusari, Rambipuji and Sukorambi). The variables studied were deciduous tooth caries with four categories of severity i.e. pulp irritation (IP), pulp hyperemia (HP), gangrene of the pulp (GP), gangrene roots (GR). Data were presented in descriptive research results with a frequency distribution table and a bar chart. The results showed that most cases of primary teeth were gangrene tooth root 626 (38.1%), subsequently gangrene dental pulp 454 (27.7%), 360 dental pulp hyperemia (21.9%), and cases of irritation of the pulp were at least 202 teeth (12.3%). The high incidence of gangrene roots caused by deciduous teeth which have a tendency to be more susceptible to the development of caries lesions, lack of knowledge, awareness and motivation of parents of the importance of maintaining oral hygiene, the transition period the child's diet between home-cooked meals with school meals, which tend to choose cariogenic foods.

Abstract

Oral cavity diseases can be caused by the oral microbes  such as Streptococcus mutans, Porphyromonas gingivalis and Candida albicans. The oral microbes growth can be inhibited by chlorhexidine gluconate mouthwash, but its long term use have adverse effects, therefore it is important to do a further research to find effective antibacterial and antifungal as alternative substance mouthwash. Flavonoid compounds in tobacco leaf have potential as an antibacterial and antifungal. The objective was to analyze the inhibition effect of tobacco leaf extract to the growth of Streptococcus mutans, Porphyromonas Gingivalis and Candida albicans. This research used 20%, 40%, 60%, 80% and 100% ethanol extract of tobaco leaf,  1% DMSO as negative control and 0.2% chlorhexidine gluconate as positive control. Antibacterial and antifungal activities were tested by diffusion disc method with four repetitions. The observation was conducted by measuring the diameter of inhibition zone after incubating for 24 and 48 hours. The largest inhibition zone of Streptococcus mutans growth were producted by ethanol extract of tobacco leaf with 80% concentration, the largest inhibition zone on Porphyromonas gingivalis and Candida albicans growth were produced by ethanol extract of tobaco leaf with 100% concentration. Diameter of inhibition zone ethanol extract of tobacco leaf was equal with the diameter of inhibition zone of 0.2 % chlorhexidine gluconate. The conclusion described that ethanol extract of tobacco leaf can inhibit the growth of Streptococcus mutans, Porphyromonas gingivalis and Candida albicans.

Abstract

Nanofilled composite resin is a restorative material that can withstand mastication force in the posterior region. The compressive strength of composite resin is influenced by beverages consumption. Robusta coffee is the most consumed beverages by Indonesian society. It is an acidic-beverage that containing chlorogenic acid, therefore coffee solution can be expected to increase water absorption and solubility of restorative materials. This study was aimed to know the effect of robusta coffee solution to the compressive strength of nanofilled composite resin. Samples were 64 discs nanofilled composite resin, diameter 5 mm and thickness 2 mm. Samples were divided into 4 control group (immersed in aquadest) and 4 treatment group (immersed in robusta coffee solution). Every group contained of 8 samples and immersed for 1, 3, 5, and 7 days. Compressive strength was tested by using Universal Testing Machine. It showed average compressive strength value of control group was higher than treatment group. It increased on 3rd and 5th day then decreased on 7th day. Compressive strength value in teratment group approximately decreased on 3rd, 5th, and 7th day. Nonparametric statisic analysis Kruskal-Wallis and Mann-Whitney showed no difference among control group and treatment group (p>0,05) according to immersion time (days). It concluded that robusta coffee solution decrease the compressive strength of nanofilled composite resin.
