Abstract

Background: Aplastic anemia (AA) is a hematologic disorder characterized by hypoplastic bone marrow and peripheral pancytopenia. The main symptoms are fatigue and bleeding. In the oral cavity, gingival bleeding is the common complaint.Case Management: A 18-year-old female patient referred from internal medicine department due to swelling of palate since 2 days before that interrupted eating activity. She routinely consumes Sandimun®100 mg tablets. A 2 x 1 cm black blood clot in anterior hard palate and ulcerative lesion at dorsal tongue regio 45 surrounded by necrotic area were found on intra oral examination. Patient was diagnosed with necrotizing ulcer-related neutropenia accompanied by AA-associated hemorrhagic bullae in the hard palate. Patient treated with mouthwash containing 0.1% hyaluronicacid and 1% feracrylum.Discussion: Immunosuppressant is the AA gold standard therapy. Cyclosporine (CsA) as imunosupressant is common drug to prevent T cells from attacking stem cells in bone marrow. The side effects of CsA is gingival hiperplasia.Conclusions: Gingival bleeding and gingival hiperplasia due to thrombocytopenia and the use of cyclosporine are vicious circles that need to be aware. With comprehensive management, good oral conditions will be obtained, therefore the quality of life can increase.

Abstract

Background: The aim of the case report is to demonstrate the importance of knowing the root canals configuration in the maxillary second premolars. Maxillary second premolars usually have one root canal, however in several cases there is more than one root canal with a variety of configurations. Method: multiple visit root canal treatment with a crown down technique on the maxillary second premolar with unusual root canals configuration which is confirmed using radiograph. Result: Based on radiograph, it was identified Vertucci type V root canal on the maxillary second premolars. After biomechanical preparation, obturation was conducted. When patient controlled, there were not problem and inflammation. Conclusion : This report described and discussed about the possibility of root canals variation on the maxillary second premolars. Careful examination using radiograph and deep knowledge, it is identified by the clinician in treating root canal treatment on that premolars.

Abstract

Background: Oral lesion is one the clinical signs found in chronic myeloid leukemia (CML). CML is a myeloid cell neoplasm that have oral manifestations such as gingival hyperplasia, spontaneous haemorrhage, petechiae, oral infection, mucosal pallor, and ulceration. The purpose is to find out how to diagnose and oral lesions associated CML to occure, so the dentist could provide the right therapyCase Management: 46 years old male was referred from Internal Medicine Department with diagnosis CML. The patient complained of pain at the right upper gum. Cheilitis exfoliative was found at the extra oral examination, while intra oral examination found ulcer at the palate. Blood test result showed decreases of Haemoglobin, Hematocrite, erythrocytes which showed iron deficiency anemia causes disruption of oral mucosal integrity, thereby facilitating oral ulcerations. Based on history, clinical examination and laboratory investigation, intra-oral abnormality was diagnosed as oral lesion associated CML. Triamcinolone acetonide 0.1% in orabase as theraphy oral lesion, vaseline album for management exfoliative cheilitis, and Surbex Z® as multivitamin. Oral ulcer improved within 6 weeks.Conclusion: The diagnosis oral lesions associated CML can be diagnosed based on history, clinical examination and laboratory investigation. Oral ulcers in this patient caused by iron deficiency anemia.

Abstract

Background: Intrinsic tooth discoloration is an aesthetic problem for most people. One of the Intracoronal bleaching treatment used hydrogen peroxide 35% and carbamide peroxide 35%. Tooth whitening material reported increase microleakage in composite resin. Purpose: This research aims to determine the differences effect of hydrogen peroxide 35% and carbamide peroxide 35 % toward microleakage in the nanohybrid composite resin. Method: This research was an experimental laboratory method with post test only control group design. The research was conducted by doing the root canal treatment on 27 samples of maxillary central incisor with the base, then the procedure of intracoronal bleaching was done using hydrogen peroxide 35 % as group 1, carbamide peroxide 35 % as group 2 and distilled water as control group. After nanohybrid composite resin restorative procedure, coated with nail varnish then immersion in methylene blue 5%. Furthermore, slit the teeth and methylene blue penetration depth measured using metalography  microscope. Result: Based on one way ANOVA test found that there was a difference in each group with a significance of 0,000 (p < 0,05) it can be interpreted that there are differences between groups. Based on LSD Post Hoc test, found that there was a difference in the effect on the group of hydrogen peroxide 35% and carbamide peroxide 35% with a significance of 0,004 (p<0,05). Conclusion: The results of this research can be concluded that there was differences in the effect of hydrogen peroxide 35% and carbamide peroxide 35% towardmicroleakage in the nanohybrid composite resin.Keywords: intracoronal bleaching, hydrogen peroxide 35%, carbamide peroxide 35 %, microleakage, nanohybrid compositeresin

Abstract

Background: Red dragon fruit peel mucoadhesive gel has the potential to dye dental plaque. The colour of a transparent dental plaque can be seen with visualize using dyes (disclosing agent). One of the natural products containing dyestuff betacyanin, namely the peel of the red dragon (Hylocereus polyrhizus). This research aimed to review the peels of red dragon betacyanin (Hylocereus polyrhizus) in the mucoadhesive gel formula as an alternative disclosing agent.Method: Research is done with the manufacture of thick peels of a red dragon extract with the maceration methods. In clinical trials carried out an application red dragon fruit peel mucoadhesive gel 10%, concentration 15 % and Three-Tone Plaque Disclosing Gel. The assessment of plaque was carried out using the O’leary method. Result: The results obtained were analyzed by the t-test method. The mean of plaque index scores with red dragon fruit peel mucoadhesive gel concentrations of 10% (22.08), 15% (26,29) and Three-Tone Plaque Disclosing Gel (66,21; 64,20). The effectiveness of the gel with a concentration of 10% is 33.87%, and 15% concentration is 40.32%. Conclusion: The mucoadhesive gel of red dragon fruit peel has the potential to colour dental plaque.

Abstract

Introduction: Gingivitis is an inflammatory gingiva which has early sign is a little bleeding and mild swelling of the gingiva and if this gingivitis is untreated it could be the periodontitis. The process of gingivitis can be prevented with an antibacterial compound.Purpose: The aim of this research is to determine the effectiveness of topical extracts of lemongrass and clove to cure gingivitis.Methods: This experimental research used post test control group design. The samples were 28 male Wistar rats 2-3 months that were divided into 4 groups. Group I as a positive control was given amoxicillin and metronidazole, group II was given lemongrass extract, group III was given clove extracts, and the fourth group was given a combination of lemongrass and clove extracts. The addition of an extract is done in twice a day for 2 days.Results: Healing of gingivitis seen from the number of polymorphonuclear leukocytes/PMN leukocytes. The mean number of PMN in group I = 5.00, group II = 9.43, group III = 8:57, and group IV = 5.86. Based on the Kruskal Wallis test there was not a significant difference of the PMN number withinfourth experiment group.Conclusion: The conclusion of this study was combination of lemongrass and clove extract had an effectiveness level that almost similar to metronidazole dan amoxicillin in decreasing the number of polymorphonuclear leukocytes in the gingivitis healing process.

Abstract

Allergic contact cheilitis is a common cause inflammation of the outer lip or vermillion margin. It is due to a type IV hypersensitivity reaction following contact with an allergen. Allergic contact cheilitis is more common in women than men. Lip cosmetics are the most common allergen source in women, while toothpastes allergy occur mostly in men. Purpose: To report the management of allergic contact cheilitis due to lip cosmetics. Case : A 22-years old woman complaint was dryness of the lips, cracking and peel off after using lip cosmetics since one week ago. Patient used new lips-cosmetics two weeks ago. No history to atopic allergy and no medication. Intra oral examination was normal. Management : patient was advised to avoid wearing lip-cosmetics. To relieve symptoms were given topical anti-inflammation non steroids, multivitamin and maintain to oral hygiene. Patient revered to patchtesting. Patient had strongly positive result to the tested lip cosmetics. Patient were given topical anti inflammation steroids and from dermatology department were given antihistamine tablets. Conclusion : Patch testing is the key to this diagnosis and for determining of contact allergen.

Abstract

Background: Estrogen plays a key role in human physiological processes. Polymorphisms of estrogen receptors have been implicated in the development of numerous diseases. The aim of this study was to evaluate the frequency of ERα gene Pvull and Xbal polymorphisms and assessing their association with CD4+ T-cell counts in HIV-infected children on highly active antiretroviral therapy.Methods: CD4+ T cell counts were determined using the FACS count system. ERα PvuII and XbaI polymorphisms were analyzed by PCR-RFLP.Results: This study enrolled 34 HIV-infected children on HAART. The frequencies of the PvuII and XbaI gene polymorphisms were PP 41,2%, Pp 26,5%, pp 32,4% and XX 35,3%, Xx 17,6%, xx 47,1% respectively. CD4+ T-cell counts were significantly associated with XbaI polymorphisms (p<0.05), but not PvuII polymorphisms (p>0.01).Discussion: Host genetic factor polymorphism is an important determinant of HIV  disease progression and treatment response. The ERα Pvull and Xbal polymorphisms can increase risk for the development of HIV-related complication,including oral diseases.Conclusion: The ERα gene XbaI polymorphism was significantly associated with CD4+ T-cell counts. It may explain the role of estrogen in the regulation of HIV replication. Studying human genetic variation in HIV-infected individuals is important to guide a new therapeutic approach.

Abstract

Background: In bone tissue engineering, scaffold is required with a large porosity of 80-90%. The porosity of the hydroxyapatite/alginate scaffold can be affected by the alginate concentration used. The objective of this study was to determine whether hydroxyapatite/alginate scaffolds with alginate concentration of 1% and 3% could be synthesized and to determine whether the use of low concentration alginates in hydroxyapatite/alginate scaffolds could increase thescaffold porosity.Methods: The method used in making scaffold was freeze drying method, while the method used to measure porosity was liquid displacement methodResult: In this study, it was found that hydroxyapatite/1% alginate scaffolds porosity had greater porosity compared with hydroxyapatite/3% alginate scaffolds with signifcant difference.Conclusion: So, it can be concluded that the use of lower alginate concentration can increase porosity in hydroxyapatite/alginate scaffolds.

Abstract

Background: Abscess is infection caused of anerob bacteria. Maxillofacialabcess aetiology was oral focus infection like dental gangren. Unproportionaltreatment could make complication that makes sistemic condition patient worst,gaster perforation. Objective : to present treatment bucal abscess case thatexpanding to colli, thoraks and complication based on mistherapy about NSAIDwith gaster perforation.Case and management: A 59 years old man with chief complain colli abscessand suspect gaster perforation with diffuse abdominal pain and distensiongaster since three days. Intraoral inspection was multipel dental gangren.Ultrasonography showed gaster perforation. Incicion drainase was done anderadication oral focus infection with repair gaster perforation under generalanaesthesia prosedure. Medication treatment with ceftriaxone injection 2x1 gram, and paracetamol infus 3x 500 mg. Bacteria kulture was negatif andhospitalization patient until 1 week.Discussion: Gold standart abscess therapy is incision drainase, but there wereclinician not aware abiut that and choosed konservative treatment with antibioticand analgesic, so that less maximal theraphy. Complication NSAID analgesicwas gaster iritation, so unproportional drug treatment can lead gaster ulcerationor perforation.Conclusion: It is important and consider drug choise for abscess treatment toavoid worst complication development
