Abstract

Stunting is still become a major public health that causes impaired physical and cognitive growth in children, thus having a long-term impact on productivity, quality of human resources, and the country's economic growth. Data from the 2023 Indonesian Toddler Nutritional Status Survey (SSGI) shows stunting prevalence in East Kalimantan Province, with 22.8% in 2021 and 23.9% in 2022. The study aims to obtain a prediction model of stunting in children 0-23 months in East Kalimantan Province. The study used SSGI 2022 as a data source, an observational study with a cross-sectional approach, and was analyzed using binary logistic regression. The study results showed that the binary model regression model had a classification accuracy of 83.2% and could explain 59.4% of the variation in the incidence of stunting in children aged 0-23 months. The findings indicate that Low Birth Weight (LBW) and maternal height significantly affect stunting in children 0-23 months. Therefore, special attention is needed for adolescents and pregnant women through nutritional education programs to grow to optimal height and for pregnant women to give birth to healthy babies.

Abstract

This study aims to compare and assess the efficiency of manual and digital payment methods used by hospital cashiers. The shift towards digitization in health service administration, has become more relevant due to the demand for faster, more accurate, and transparent transactions. This descriptive quantitative study used primary data on the average length of transactions as well as secondary data on the number of transactions per month at RSAU dr. M. Salamun Hospital in the months of March and April. In addition to a 40-second time saving compared to cash transactions, the results demonstrated that digital payments processed using QRIS have the potential to streamline services by reducing wait times, particulary during peak periods. QRIS usage increased during the two months under study, despite cash’s continued dominance. Full adoption is hampered, nevertheless, particularly in the pharmacy unit where there are issues with tax integration and insufficient corporate procedures. Both staff and younger patients reported that they favored digital methods since they were quick and easy, while elderly patients opted to stick with cash. The results of the study show that QRIS offers a more effective alternative. Nonetheless, wider implementation of QRIS requires technology integration and policy alignment. Future research should look at the effects of digital technologies on patient satisfaction and workflow optimization across various hospital departments.

Abstract

Leprosy is an infectious disease caused by Mycobacterium leprae that primarily affects the peripheral nerves, skin, and other body tissues. This study aimed to examine the association between various risk factors and the incidence of leprosy. A case-control design was employed, comprising a case group of 15 multibacillary leprosy patients and a control group of 45 household members without leprosy. Data analysis included univariate and bivariate analyses, with the latter performed using the chi-square test. The findings revealed significant associations between leprosy incidence and several risk factors: age (P = 0.004, OR = 13.391; CI = 1.621–110.596), lack of knowledge (P = 0.042, OR = 4.75; CI = 0.957–23.572), duration of contact (P = 0.007, OR = 0.182; CI = 0.049–0.668), close contact (P = 0.007, OR = 0.182; CI = 0.049–0.668), and socioeconomic status (P = 0.021, OR = 5.688; CI = 1.149–28.163). These results highlight the necessity of enhancing educational and counseling initiatives for at-risk populations, particularly those residing near leprosy patients, while also encouraging individuals affected by leprosy to complete their treatment regimen.

Abstract

According to the Permenkes policy Number 3 of 2023, JKN participants can upgrade their treatment class by paying the difference either independently, through their employer, or through additional insurance. The decision to upgrade their treatment class can be influenced by several factors, such as social, personal, economic, and cultural factors. This study aims to analyze the factors that influence JKN patients' decisions to upgrade their treatment class at Dr. R. Sosodoro Djatikoesoemo Bojonegoro Regional Hospital. This study uses a quantitative cross-sectional approach. The population of this study were all JKN class 1 and 2 patients with a sample size of 92 respondents. The independent variables of this study were income, perception of services, availability of treatment rooms and ownership of additional insurance. The dependent variable was the decision to upgrade the class of care. This study used logistic regression analysis. This study was conducted at Dr. R. Sosodoro Djatikoesoemo Bojonegoro Regional Hospital. The results of bivariate analysis showed that the four independent variables had a significant effect on upgrading the in-patient care, i.e income p-value 0.000 (OR 4.312), perception of service p-value 0.000 (OR 1.362), availability of treatment rooms p-value 0.000 (OR 1.458), and ownership of additional insurance p-value 0.009 (OR 3.496). Whilst the multivariate analysis showed the most dominant factor was income p-value 0.000 (OR 4.571). In this study, it is expected that hospitals can consider service improvement strategies and understand patient segments which can later increase hospital revenue and overall service quality.

Abstract

The increasing burden of infectious diseases such as Tuberculosis (TB) and non-communicable diseases such as Diabetes Mellitus (DM) places significant pressure on the National Health Insurance (JKN) financing system. This study aims to analyze the healthcare financing patterns of TB and DM using BPJS Kesehatan secondary data from 2022 through a clustering analysis approach. This quantitative study employs BPJS Kesehatan Sample Data, covering 34 provinces in Indonesia. Data processing was performed using Stata 14.2 for case proportion calculations and RStudio for cluster analysis with the K-Means algorithm. The optimal number of clusters was determined using the Gap Statistic and Within-Cluster Sum of Squares (WCSS) methods. The results indicate three regional clusters: a moderate-burden cluster (Sumatra), a high-burden cluster (Java), and a low-burden cluster (other provinces).

Abstract

Indoor air quality is a crucial aspect of creating a healthy and productive work environment. Decreased air quality has the potential to cause various health problems in building occupants. One of the health problems that is often associated with poor indoor air quality is Sick Building Syndrome (SBS). This study aims to synthesize findings from various studies to provide a comprehensive understanding of environmental risk factors that contribute to the occurrence of SBS. This study is a literature review to review the results of previous studies related to indoor air quality and sick building syndrome. The search database used in searching for this article comes from national and international journals. The selection of articles is based on inclusion criteria, including publication within the last 10 years, a cross-sectional study design, and discussion of indoor air quality in relation to the occurrence of SBS. The results of the review of 10 articles showed that the factors that trigger the onset of sick building syndrome symptoms include temperature, humidity, lighting, particulates (PM2.5 and PM10), air flow rate, ventilation systems, and exposure to chemical contaminants. Environmental management as an effort to overcome the occurrence of sick building syndrome includes improving the ventilation system, controlling temperature and humidity, managing chemical contaminants, controlling particulates, and improving lighting. SBS symptoms are influenced by several environmental factors that can affect worker health and productivity. Therefore, environmental management is needed as a way to overcome the occurrence of SBS.

Abstract

Patient safety incidents are a critical issue in healthcare that can impact service quality and public trust in hospitals. Implementing a patient safety culture is one way to improve hospital services by raising awareness of error prevention and reporting. This study aims to improve reporting of patient safety incidents in the Central Surgical Installation of Mitra Plumbon Hospital, Cirebon, through the implementation of a culture of fairness. Using a quantitative, correlative descriptive method, this study involved 50 healthcare workers as respondents. The results showed a significant increase in reporting of patient safety incidents after the implementation of a culture of fairness, characterized by a shift from a moderate to a strong culture. A culture of fairness plays a crucial role in creating a safe work environment, where staff feel comfortable reporting incidents without fear of sanctions. This study is expected to contribute to efforts to improve patient safety in hospitals.

Abstract

This study aims to see the contributing factors to the maturity of patient safety culture. This study is a cross-sectional study utilizing secondary data from the results of the 2022-2024 patient safety culture survey totaling 3,892 samples. The results of the study found that the dimensions of staffing and work speed (p=0.026; r=0.038), the dimensions of response to errors (p=0.000; r=0.068), the dimensions of supervisors, managers or clinical leaders supporting patient safety (p=0.037; r=0.025), the dimensions of communication about errors (p=0.000; r=0.077), the dimensions of openness of communication (p=0.000; r=0.080), the dimensions of reporting safety incidents (p=0.001; r=0.055), the dimensions of hospital management support for patient safety (p=0.000; r=0.053), and the dimensions of handover and exchange (p=0.000; r=0.073) were related to the maturity of patient safety culture, while the dimensions of teamwork (p=0.523) and the dimensions of organizational learning-continuous improvement (p=0.249) were not related to the maturity of patient safety culture. These results highlight the need for hospital management to improve its personnel system and create a complaint system for staff reporting incidents where the parties involved can maintain confidentiality but remain open.

Abstract

This study aims to analyze the influence of transformational leadership on nurses' roles in professional ethical communication and its implications for hospital branding at Bunda Margonda General Hospital, Depok. A mixed methods approach was applied, combining quantitative surveys using MLQ, NELQ, and Aaker & Keller's brand equity scales, and qualitative semi-structured interviews with head nurses, staff nurses, marketing managers, and returning patients. A total of 114 staff nurses were included using total sampling. Path analysis revealed that transformational leadership significantly affects nurses’ ethical communication (β = 0.278; p = 0.003), and ethical communication significantly impacts hospital branding (β = 0.424; p = 0.000). However, transformational leadership does not directly influence branding (p > 0.05), but rather through the mediating role of ethical communication. Qualitative triangulation supports this finding, highlighting that the leadership of head nurses in cultivating ethical culture greatly influences hospital image. The study recommends routine training in transformational leadership and ethical communication for nurses as a strategy to improve service quality and institutional image.

Abstract

Hospitals and health care facilities generate increasing amounts of solid medical waste each year. Improper treatment of this waste poses a potential risk for disease transmission. The Ministry of Health Regulation No. 7 of 2019 classifies hospital waste as hazardous and toxic medical waste and regulates its management procedures accordingly. This study aims to analyze the management of hazardous and toxic solid medical waste at Purworejo Hospital, focusing on the stages of reduction, segregation, storage, transportation, and the use of personal protective equipment (PPE). This research employed a descriptive qualitative approach with a cross-sectional design. Data were collected through observations, interviews, and document reviews. The interviews involved both key informants and triangulation informants. The results revealed that the management of hazardous and toxic solid medical waste at Purworejo Hospital achieved a compliance rate of 74.58% categorized as “appropriate” and 25.442% categorized as “not yet appropriate.” This compliance rate remains below the required standard as stipulated in the Ministry of Health Regulation No. 7 of 2019. Non-compliance was identified in several aspects, including the absence of a designated pathway and special trolley for waste transportation, the lack of hepatitis and tetanus immunization for staff, and inconsistencies in the use of PPE.
