Abstract

Abstract: Recently, forensic toxicology has been an interesting concern, especially in exposing the phenomena associated with the law. Using the forensic toxicology approach, several cases of lead (Pb) poisoning have been widely revealed. In this present study will be investigate the interaction between Pb and amino acid gastric mucosal constituent proteins, especially cysteine and tyrosine groups. This research is a pure experimental research with posttest control group design, which is divided into 4 groups with 6 rats (Rattus novergicus) in each group. Treatment in each group as follows; P0 was control group were given 2 ml of distilled water; P1 = administration of Pb 0.1 g/L; P2 = Pb administration of 1 mg/L; and P3 = Pb administration of 10 g/L for 4 weeks repectively. According to the results, it can be concluded that Pb-Protein interaction tends to binding of Pb-Cysteine rather than Pb-Tyrosine Keywords: Gastric Mucosa, Lead, Protein

Abstract

Dengue Haemorrhagic Fever (DHF) is a dangerous infectious disease caused by the dengue virus and is transmitted through the bite of the mosquito Aedes aegypti. DHF control depends on controlling mosquitoes and larvae. Currently larvicidal control still uses temefos, although some studies have reported the presence of resistance. Larvicides that WHO recommended as a substitute temefos is permethrin. In addition there are some new larvicides derived from the class of insect growth regulator (IGR) which contain active pyriproksifen and active ingredients methopren. This study aims to determine the effectiveness of larvicides and effective concentration to kill 50% of larvae of Aedes aegypti from the sub Banjarmasin West. This study is an experimental study design with Posttest-Only Control Group Design with permethrin dose group was 0.0015 g / l; 0.0018 g / l; 0.002 g / l; 0.0022 g / l; 0.0025 g / l; and 0.0030 g / l, for pryriproksifen use dose groups of 0.5 ppm, 0.1 ppm, 0.05 ppm, 0.01 ppm, 0.008 ppm, 0.005 ppm, and methopren with dosis0 group g / l, 0, 0063 g / l, 0.0025 g / l, 0.005 g / l, 0.01 g / l, and 0.1 g / l. Each treatment group was exposed to larvae of Aedes aegypti larvae and change shape as well as the deaths were followed for 10 days. The results showed that the effective dose of permethrin kills 50% of larvae (Lethal Concentration / LC50) 24 hours ranged from 24 hour LC50 0.01439 - 0.01829 mg / l, with an average of 0.01671 mg / l and 24-hour LC99 ranged from 0.02856 - 0.03852 mg / l with an average of 0.03196 mg / l. This study showed that the larvae of Aedes aegypti in the district of West Banjarmasin are resistant to permethrin. As for pyriproksifen larvicides effectively killed larvae by 70% after seven days of exposure, and for larvicidal with active methopren killed 50% of larvae on the eighth day. This study shows that larviciding of IGR group can be an alternative in the area that has been declared resistant to abate as in the sub-district of West Banjarmasin. Keywords: effectiveness, permethrin, pyriproksifen, methopren, Aedes aegypti

Abstract

Abstract: Smoking and Chronic Obstructive Pulmonary Disease (COPD) become a global problem in the world. The increasing number of smokers in the world also led to significantly increased morbidity and mortality due to COPD. Many attempts were made, ranging from government policy that prohibits smoking in public places, to pharmacological therapy, and psychotherapy for smokers. However, often there is a failure to quit smoking. Dependence on nicotine is an important factor that causes someone failed to stop smoking. As for the pharmacological therapy is currently widely used to intervene in nicotine dependence, such as nicotine replacement therapy, bupropion, and varenicline. However, existing therapies have not given satisfactory results in reducing smoking rates and the number of COPD significantly. Therefore, the anti-nicotine vaccine that is currently in development and research, such as NicVAX, NIC002, TA-NIC, and SEL-068, become a new perspective for smokers to stop smoking and prevent long term recurrence. Keywords: nicotine, anti-nicotine, smoking, COPD

Abstract

Abstract: Asthma is a chronic inflammatory disease of the respiratory tract. The purpose of this study was to determine the description of the diagnosis and management of asthma inpatient at RSU Royal Prima Medan in 2018. This research was conducted using a case study design method, descriptive - retrospective, sample size 100 data of medical records of patients taken by purposive sampling, namely medical records the most complete data and analyzed with descriptive statistics. The results show that the youngest is 9 years old and the oldest is 85 years old, with 44 men and 56 women. The most common complaint is 74 shortness of breath and the most pharmacological management is beta2 agonist 100%. The conclusion was that the distribution of asthma was higher in women (56%) than in men (44%) and the most common complaint was shortness of breath (74%) and the most used combination treatment was Beta 2 + Corticosteroid + Ammonium Chloride Agonists. and Diphenhydramine + Paracetamol + Antibiotics (35%). Keywords: Asthma, Diagnosis and Management, Royal Prima Hospital Medan

Abstract

Abstract: Diabetic foot is a complication of diabetes mellitus which is characterized by ulceration in the legs. The severity of the diabetic foot can be determined by the Wagner-Meggit system which in this study uses 5 categories namely degrees 1 - 5 degrees. ABI (Ankle-Brachial Index) is an evaluation of peripheral arterial disease in diabetic foot patients. LDL is the main factor of atherosclerosis which causes macrovascular disorders in diabetic foot. The purpose of this study was to determine the correlation between ABI and LDL on the severity of diabetic foot based on the Wagner-Meggit classification. The method used is observational analytic with cross sectional approach. A total of 35 samples were obtained by consecutive sampling. Data analysis was performed using the Spearman test and logistic regression. A significant correlation was obtained between ABI and the severity of diabetic foot (p = 0.06; r = -0.424), there was no correlation between LDL and the severity of diabetic foot (p = 0.09; r = -0.395). The results of logistic regression analysis found no correlation between ABI (p = 0.163; OR = 0.008) and LDL (p = 0.69; OR = 0.984) with the severity of the diabetic foot. Abstract

Abstract: In urology, urogenital disorders include all male and female urinary tract and male genital organ abnormalities. These disorders can be an infections, blockages due to foreign bodies, dysfunction due to pathological or degenerative processes, trauma, neoplasms, or congenital disorders. This research was a retrospective descriptive study to determine the number of patient visits with urogenital disorders. The place of this research was ER of DHAAN General Hospital, Tanah Bumbu District, South Kalimantan Province, Indonesia. The research data was taken from data on new patient visits to the ER in 2017. The number of patient visits with urinary tract disorders was 475 cases: male 57.1% and female 42.9%. The most cases based on the type of disorder was UTI (38.9%), second was urolithiasis (25.3%), and third was CKD (23.4%). While the number of patient visits with male genital disorders was 79 cases. Most cases was BPE (86.1%). Knowing the number of patient visits with urogenital disorders will make the clinicians be directed in preparing themselves for urogenital emergencies in their respective health services. Keywords: DHAAN, number of visits, urogenital disorders, urology.

Abstract

Abstract: The correct positioning of the femoral tunnel and endobutton position determine the success of ACL reconstruction (ACLR). Improper femoral tunnel placement most common errors in failed ACLR. However, plain post-operative imaging in the early state is not routinely performed. EndoButton position evaluation by XRay was made because this technique was cheap and reliable in Indonesia. There is no consensus on the ideal positioning of EndoButton in compare with functional outcome after ACLR.  The study aimed to evaluate anatomic positions of tunnel and fixation devices, and also provide a baseline examination for future controls regarding tunnel placement and fixation devices, and also evaluation functional outcome after ACL procedure and also evaluation of functional outcome after ACLR with IKDC score. Between February 2019 to July 2019, 20 patients aged 17 to 59 years, with post ACL reconstruction were involved in this study. We are evaluating sex, age, affected the side of the knee, complication after an operation, related activity with injury, and evaluation postoperatively with femoral angle measurement Xray AP and lateral view. We also evaluated and correlate post ACRL and functional outcomes with the International Knee Documentation Committee (IKDC) score. The mean age of patients was 28,38 ± 9,3 years, (87,15% men and 12,5% women). There is no complication after the operation in all patients. Nine patients (56,3%) affected on the left side, and seven patients (43,8%) affected right-sided. All patients are performed single-bundle augmentation techniques for ACL reconstruction. Five (31,3%) patient has history fall and eleven (68,8%) patients caused by sports activity. All patient has Femoral angle measurement from knee AP view x-rays more than 17o, with minimum 39o and maximum 54o, with mean 46,63 ± 4,2. From lateral x rays, we interpreted nine patients (45%) interpreted as shallow and high, and 55% others interpreted as deep and low (optimal). The mean of IKDC score pre-op was 58,1±4,6 and means IKDC postoperatively 6 months was 84,68 ± 5,1. There was a significant difference in functional outcome between optimal endobutton and not optimal endobutton position group with p-value < 0,05. Early post-operative imaging (knee x-rays) provides useful information regarding tunnel placement and fixation devices. It also serves as a baseline examination for future reference. In this study, a single bundle ACL reconstruction shows a satisfying outcome. Keywords: Anterior cruciate ligament; knee; postoperative reconstruction; radiograph.

Abstract

Abstract: This study aims to determine the association between prostate volume and PSA levels in BPH patients at Ulin Hospital Banjarmasin. This study used a cross-sectional retrospective approach and analytic method. A total of 52 samples were obtained by purposive sampling based on inclusion and exclusion criterias. The highest prostate volume of  20.00-39.99 cc was obtained from 16 patients (30.7%) and the lowest volume of  60.00-79.99 cc was obtained from 10 patients (19.2%). Examination of PSA levels were highest at 2.6-9.9 ng/mL from 24 patients (46.2%) and the lowest at >20.0 ng/mL (5.8%) from 3 patients. Data analysis using Pearson correlation coefficient showed a positive correlation between prostate volume and PSA levels in BPH patients at Ulin Hospital Banjarmasin with a p value = 0,000 and r = 0.616 (positive and strong correlation). Keywords: BPH, Prostate volume, PSA

Abstract

Abstract: Female genital tract anomalies have important effects on reproductive function, and usually recognized after puberty. The membrane canalization process end to various hymenal forms. Microperforate hymen appears as a partial obstruction depending on its  size. Most often, patient will present with menstrual disorders, dyspareunia, and infertility. The aim of this research was to report a case of spontaneous pregnancy in postoperative microperforated hymen in form of case report. A 27-year-old woman, P0A0, complained of pain during intercouse. Patient had been married for 1,5 years, had reguler menstrual cycles without any contraceptive method, but not conceived yet. Inspection showed an obstructed vaginal introitus with a small opening laterally at 3 o’clock, consistent witn microperforate hymen. During surgery, short vaginal introitus was observed and no vaginal canal was seen. A sound was inserted through a small opening of 1 mm in diameter laterally at 3 o'clock, followed by sufficient incision and excision of the distal vaginal tissue. Interrupted suture of the proximal and distal mucosa was performed. The vaginal portion as well as uterus appeared normal. Postoperative tissue healing was good. A spontaneous pregnancy occurred 56 days after the surgery and a healthy term baby was born by caesarean section. In the case of microperforated hymen, menstrual flow can be normal. The patient complained of dyspareunia or impaired sexual penetration leading to infertility, although the presence of a small opening may allow passage of sperm and spontaneous pregnancy may occur before the surgery. Abstract

Abstract: The purpose of this study was to determine the characteristics of nephrolithiasis patients at Ulin General Hospital Banjarmasin based on sex, age, and residence place of patients. This study was conducted at Ulin General Hospital Banjarmasin, using a descriptive method with a retrospective approach. A total of 100 nephrolithiasis patients were obtained by total sampling according to inclusion and exclusion criteria.  The results of this study showed that all nephrolithiasis patients in the 2017-2018 period were recorded, there were 58 male patients (58%), while there were 42 female patients (42%). The results showed that nephrolithiasis patients at Ulin General Hospital Banjarmasin were more than 55-64 years old (31%) and Banjarmasin was the area where most nephrolithiasis patients with a number of 35 patients (35%). This study showed the characteristics of age, sex, and residence place of patients were the risk factors that influence the incidence of nephrolithiasis at Ulin General Hospital Banjarmasin. Keywords: Patient Characteristics, Nephrolithiasis, Ulin General Hospital Banjarmasin.
