Abstract

Objectives: to analyze the relationship between passive smoker of pregnant women with low birth weight (LBW).Materials and Methods: This study is an observational analytic research with case control design, which is LBW as case group and normal birth weight as control group. The location is in Wonokusumo Health Centre, Surabaya, period January 2016 – December 2017. Study sample was 68, consisting of 34 cases and 34 controls. The dependent variable is LBW, while the inde-pendent variable is passive smoking of pregnant women. Sample was excluded multiple births and congenital defects. To determine a significant level, the data collected will be tested with the statistical test Chi-square at significance level ?=0.05.Results: The results showed that a majority (61.5%) of passive smoker of pregnant women are cases. After Chi-square test obtained by value p=0.027 (p?0,05) and OR analysis obtained by 3.04 (CI 95% 1.117 – 8.274), which means there is a relationship between passive smoker of pregnant women with LBW.Conclusion: Passive smoker of pregnant women is relationship with decreased birth weight.

Abstract

Objectives: This research want to identify some factors that increasing risk of maternal death in pregnant woman with heart disease.Materials and Methods: This study was a retrospective analytic study with cross sectional design, with total sampling 92 patient with heart disease in pregnancy at maternity room Dr. Soetomo hospital, Surabaya, during periode January until December 2017.Results: Prevalency of pregnancy with heart disease in dr. Soetomo hospital about 0,5% with mortality that caused by heart disease 14% from all maternal death. Factors that increase risk of maternal death are non adequate antenatal visit number (OR 1,7;CI 95%), delay of detecting heart disease (OR 2,5;CI 95%), complication such as severe pulmonal hypertension (OR 61,4;CI 95%), Eisenmenger syndrome (OR 2,9;CI 95%), Decompensatio Cordis Functional Class IV (OR 1,2;CI 95%), and Thrombo-embolism (OR 9;CI 95%).Conclusion: Mortality rate in pregnancy with heart disease is 14% of all maternal mortality. Risk of maternal death increased by non adequate antenatal visit and delay in heart disease detection, and also medical complications. Detection of heart disease since before pregnant with pre-conceptional councelling and a good managed multidisciplinary antenatal care supposed to decrease morbidity and mortality.

Abstract

Objectives: The purpose of this study was to analyze the effects of mung bean sprout extracts on Follicle stimulating hormone (FSH) and estrogen hormone in female Wistar rats (Rattus norvegicus) exposed to monosodium glutamate (MSG).Materials and Methods: This true experimental study was conducted by using post-test only control group design in the laboratory for animal experimentation of Faculty of Veterinary Medicine - Airlangga University in which Wistar rats (Rattus norvegicus) aged 2 months weighing 150-200 grams were used in this experiment. The samples comprised of 5 rats distributed in each group, totaling 7 groups. I Control Group (P1) was provided with Aquades for 37 days; II (P2) was provided with Aquades for 7 days + MSG 0.03 mg/g of weight on day 8-37; III (P3) was provided with extract of mung bean sprouts 72mg/200g of weight on day 1-37 + MSG 0,03mg/g of weight; IV (P4) was provided with extract of mung bean sprouts 144mg/200g of weight on day 1-37 + MSG 0.03mg/g of weight; V (P5) was provided with Aquades for 7 days + MSG 0.7 mg/g of weight on day 8-37; VI (P6) was provided with extract of mung bean sprouts 72mg/200g of weight on day 1-37 + MSG 0.7mg/g of weight; VII (P7) was provided with extract of mung bean sprouts 144mg/200g of weight on day 1-37 + MSG 0.7mg/g of weight.Serum samples were taken for Follicle Stimulating Hormone (FSH) and Estrogen with ELISA method. Data analysis to test the differences between groups was done by using one way Anova statistical test.Results: MSG dosage 0,03mg/gBB or 0,7mg/g BB was significant difference in FSH (p = 0,011) and estrogen (p = 0,008).Conclusion: obtained from this research that giving green bean sprout extract influence to the level of FSH and estrogen hormone.

Abstract

Objectives: Surgical wound metastases in stage 1 endometrial cancer are possible, with a variety of different pathophysiological possibilities. Comprehensive management is needed to keep the patient on the possibility of a good prognosis.Cases Report: During January 2015 – January 2018 at dr. Soetomo Hospital, there were 2 cases of metastatic endometrial cancer in the laparotomy wounds by which the condition is very rare. Case 1, The patient was diagnosed with endometrial carcinoma following the results of curettage. Anatomical pathology examination was done and obtained grade 2 endometrioid adenocarcinoma. In Case 2, the patient underwent Total Abdominal Hysterectomy and Bilateral Salpingo-Oophorectomy in 2013 at Mojokerto General Hospital, indicating Uterine Fibroids and Ovarian Cysts. The results of anatomical pathology examination were unknown. On April 2017, the patient complained abdominal swelling since 3 months ago.Conclusion: In January 2015 - January 2018, 2 cases of metastatic endometrial cancer was found in a former laparotomy operation where this condition is very rare in endometrial cancer cases with low grade ,so that follow-up, monitoring and more vigilance are required in patients with low-grade endometrial Ca who have finished undergoing a surgery and chemotherapy. Rapture or mass resection, followed by external radiation, may be performed in patients with recurrence in the laparotomy wound area or in patients with high risk factors for endometrial cancer such as a history of estrogen use, tamoxifen, nullipara, obesity, diabetes mellitus, and family history of endometrial cancer. Currently, there is no fixed procedure (guideline) in RS. Dr. Soetomo to overcome recurrences especially in the scars of cancer surgery.

Abstract

Objectives: Cesarean delivery rates have increased remarkably and cause a major public health concern. This study aims to evaluate the feasibility of using the Robson Ten Group Classification System (TGCS) for cesarean delivery (CD) indications at institutional level.Materials and Methods: Database of all women delivering at Cipto Mangunkusumo Hospital, Jakarta, Indonesia were analayzed. The CD rates overall and in each Robson group were calculated, as was the contribution of each group to the overall CD rate. In addition, the CD indications in each group were analyzed.Results: Approximately almost half (48.04%) of women delivered by CS in our study. Groups 10 was the largest groups representing 27.82% of the obstetric population. The second and third largest were group 3 and 1, which represents 18.00% and 17.34%, respectively. The highest relatively contribution of CS rate were group 10, 1, and 3 with the percentage of 28.24%, 17.59%, and 15.19%; contributively.Conclusion: The TGCS can be applied at institutional level. It helps in planning strategies for specific subgroups of women to reduce CS rate and improve outcomes.

Abstract

Objectives: Maternal Mortality Rate (MMR) is closely related to maternal care. MMR in Indonesia based on the 2015 IDHS is 359 per 100,000 live births. By increasing the utilization of MCH handbooks, MMR would be decline. Therefore, this research investigated the relationship between of the utilization of MCH handbooks and attitudes pregnant women regarding pregnancy and childbirth complications.Materials and Methods: We conducted a cross sectional research with 54 pregnant women during March - June 2018 at the Sooko Health Center, Mojokerto. The independent variable was the utilization of MCH handbooks and the dependent variable was the attitude of pregnan women regarding pregnancy and childbirth complications. Data analysis included descriptive and bivariate analysis.Results: The results showed that 79.6% of respondents used the MCH handbook well. The results of the chi square analysis test showed a p value of 0.027 (<0.05).Conclusion: MCH handbook utilization effected the attitude of pregnant women regarding the complication of pregnancy and childbirth. Pregnant women who read and utilize MCH handbook would be have better alertness about the risk of complication so they would make the right decision for their pregnancy.

Abstract

This narrative is not a review, it is just a personal account of a series of pivotal studies that directly influenced my evolving research interest in the pathogenesis and prevention of preeclampsia since the early 1980s’.Studying in Leiden (The Netherlands) in the early 1970’s, my introduction into obstetrics was not very positive. The obstetrical teaching, we received, as 3rd year medical students was boring. The focus was very much on the individual plusses and minuses of types of obstetrical forceps. Preeclampsia was a disease caused by ‘salt excess’, strict salt restriction the key concept in prevention of preeclampsia in The Netherlands.One of my lecturers told us that disseminated intravascular coagulation (DIC) was the key causal pathophysiologic mechanism in preeclampsia. At this stage one did not realize that the ‘DIC’ observations were all based on autopsy findings in women dying of eclampsia and/or HELLP (although that acronym was only introduced by Weinstein1 in 1982). It was Prof Jack Pritchard2 who – already in these years – made the observation that we ‘never die alone – DIC is always there’. For me the ‘DIC’ lectures certainly triggered my interest in the haemostatic system.

Abstract

Objectives: To prevent Herlyn-Werner-Wunderlich syndrome’s complication in pregnancyCase Report: Presenting 2 cases pregnant with Herlyn-Werner Wunderlich syndrome, which is both of the case diagnosed at teenage and becoming pregnant then delivered by cesarean due to severe preeclampsia.Conclusion: High suspectious is needed to diagnose Herlyn-Werner-Wunderlich syndrome and early diagnosis is better prognosis. The gold standard diagnostic is MRI. The treatment is resection of the vaginal septum, drainage, and marsupialization. Preconception Counseling is important to prevent or to reduce the pregnancy complications.
