Abstract

Penelitian ini bertujuan untuk mengetahui hubungan komorbiditas dengan gambaran CT scan toraks pada pasien terkonfirmasi COVID-19. Penelitian ini dilaksanakan di Departemen Radiologi RSUP dr. Wahidin Sudirohusodo Makassar dengan jumlah sampel sebanyak 192 pasien terkonfirmasi COVID-19. Analisis karakteristik gambaran CT scan toraks pasien dilakukan pada lung dan mediastinal window untuk melihat gambaran Groundglass opacity (GGO),konsolidasi, crazy paving, halo sign, air bronchogram sign (ABS), fibrosis, nodul, limfadenopati mediastinum, dan efusi pleura. Perbedaan proporsi gambaran CT scan berdasarkan komorbiditas diuji dengan chi-square. Hasil penelitian menunjukkan terdapat hubungan yang signifikan antara pasien dengan riwayat komorbiditas terhadap gambaran GGO, konsolidasi, crazy paving, nodul, fibrosis, efusi (semua nilai p<0,05). Terdapat hubungan yang signifikan pada pasien dengan riwayat hipertensi dengan gambaran GGO, dan crazy paving (keduanya p<0,05), riwayat diabetes mellitus dengan gambaran konsolidasi (p<0,05), riwayat CKD dengan gambaran efusi pleura (p<0,05), riwayat keganasan dengan gambaran nodul (p<0,05), riwayat CAD dengan gambaran crazy paving (p<0,05), dan riwayat tuberkulosis dengan gambaran fibrosis, tree in bud dan nodul (p<0,05).

Abstract

Introduction: Previous studies have shown pulmonary vascular thickening found in COVID- 19 patients. In addition, the CT-Severity Score (CT-SS) scoring system have become a reference in assessing the severity of COVID-19. This study analyzes the relationship between pulmonary vascular thickening on CT-Scan thorax with CT-SS in patients with confirmed COVID-19.Materials and Methods: A retrospective cross-sectional study was conducted on 137 patients with confirmed COVID-19 at the Radiology Installation of Doctor Wahidin Sudirohusodo Hospital Makassar. Patients who were eligible and had undergone a chest CT-Scan were then measured for their pulmonary vascular thickness in the intralesional and extralesional areas of the secondary lung lobes. The CT-SS score was assessed to determine the degree of mild or severe disease based on the opacity of the involved lung segment. Independent T-test and Chi- square were used to determine the differences and relationships between the variables studied. Results: Based on CT-SS, most subjects were classified as mildly ill (CT-SS < 19.5, 78/137 people). The mean intralesional and extralesional vascular thickening was 2.65 ± 0.48 and 2.17 ± 0.36, respectively. There was a significant difference between vascular thickness between subjects with mild and severe CT-SS categories (intralesion: 2.4 ± 0.33 vs. 2.99 ± 0.47, p=0.001; extralesion: 2.04 ± 0.28 vs. 2.35 ± 0.39), Conclusion: Vascular thickening on CT-Scan chest images is related to the severity of COVID-19 as measured by CT-SS scoring. Abstract

ABSTRACT DWI with ADC mapping adds value to conventional MRI imaging by offering assessment of cellularity functionality with the potential to help differentiate between benign and malignant lesions. The aim of this study is to assess the accuracy of the ADC value MRI in predicting soft tissue tumors benign and malignant. This study was carried out by the Radiology Installation of Wahidin Sudirohusodo Hospital from February 2020 to August 2022 with a sample of 36 people and an age range of 9-74 years. The statistical method used was to calculate the value of sensitivity, specificity, Positive Predictive Value and Negative Predictive Value to assess accuracy as well as ROC and Mann Whitney tests. The results show that it consists of 36 samples with soft tissue tumors (malignant: 25 samples and benign : 11 samples). The mean value of ADC for malignant soft tissue tumors is 0.998 ± 0.526 x 10-3mm2/s and benign is 1.705 ± 0.665 x 10-3mm2/s. With a Cut Off value to distinguish malignant and benign soft tissue tumors of 1,265 x 10-3 mm2/s, with a sensitivity of 81.8%, specificity of 80%, Positive Predictive Value 64.3%, Negative Predictive Value 90.9%, Accuracy 80.5% and AUC 0.81. The accuracy of the ADC MRI value is 80.5%, indicating that the ADC value can be used as an additional modality to help predict benign and malignant soft tissue tumors. Abstract

Infeksi Coronavirus disease-19 (COVID-19) dilaporkan menimbulkan komplikasi kardiovaskuler. Epicardial adipose tissue (EAT) merupakan komponen dinding luar miokardium yang dapat diukur dengan CT scan thorax untuk menilai atenuasi begitupun dengan troponin I yang dapat mengalami peningkatan pada pasien dengan penyakit kardiovaskuler. Modalitas CT scan thorax tersebut dilaporkan dapat menjadi modalitas penunjang noninvasif untuk mengevaluasi keterlibatan kardiovaskuler pada pasien dengan COVID-19. Tujuan untuk mengetahui korelasi nilai atenuasi EAT pada MSCT thorax dengan nilai troponin pada pasien COVID-19. Penelitian dilakukan di Instalasi Radiologi RSUP Dr. Wahidin Sudirohusodo pada periode waktu Juli 2021 hingga Desember 2021 dengan jumlah sampel 33 orang. Terdapat 25 pasien laki-laki (75%) dan 8 pasien perempuan (25%) dari rentang usia 32 hingga 77 tahun. Analisa data menggunakan uji korelasi Spearman. Hasil penelitian menunjukan terdapat hubungan signifikan antara average atrioventricular sinistra (AVG AV sinistra) dan nilai troponin (p <0,032). Begitupun MIN Hounsfield Unit dengan nilai troponin (p<0,027) dimana peningkatan AVG AV sinistra dan MIN Hounsfield berbanding lurus dengan peningkatan nilai troponin. Abstract

Facet joint osteoarthritis terkait erat dengan kondisi penyakit degeneratif diskus yang berbeda, namun terkait secara fungsional yang memengaruhi struktur di aspek anterior kolumna vertebralis. Perubahan endplate tipe modic dideskripsikan sebagai perubahan dari intensitas sinyal dari endplate dan bone marrow dari korpus vertebra. Perubahan endp/ate tipe modic dan degenerasi facet joint sering terjadi bersamaan pada pasien dengan nyeri punggung bawah. Penelitian ini bertujuan mengetahui korelasi perubahan endplate tipe modic pada magnetic resenance imaging /umbosakral terhadap facet joint osteoarthritis berdasarkan Pathria grading. Penelitian ini dilaksanakan di Departemen Radiologi RSUP dr. Wahidin Sudirohusodo Makassar dengan jumlah sampel sebanyak 198 sampel yang masuk kriteria inklusi. Penilaian perubahan endplate modic vertebra lumbosakral didasarkan pemeriksaan MRI lumbosakral. Kemudian, menilai derajat facet joint osteoarthritis dengan Pathria grading pada level yang sama dengan perubahan endpate modic. Selanjutnya, dilakukan perbandingan perubahan endplate modic vertebra lumbosakral dan derajat facet joint osteoarthritis dengan Pathria grading. Hasil penelitian menunjukkan terdapat korelasi yang signifikan antara perubahan endplate modic vert.ebra /umbosacral dan derajat facet joint osteoart.hritis dengan Pathria grading (p<0,001, koefisien korelasi 0,631). Sernakin meningkat perubahan endplate modic vert.ebra lumbosakral, semakin meningkat pula derajat facet joint osteoart.hritis dengan Pathria grading. Disimpulkan bahwa perubahan endplate modic vertebra lumbosakral terkait derajat facet joint osteoarthritis dengan Pathria grading sehingga apabila ditemukan perubahan endplate modic, kemungkinan terdapat juga facet joint osteoart.hritis. Abstract

Objective: These clinical findings were subsequently corroborated by anatomic pathology (PA) examination.Methods: Lymphangiomatosis, on the other hand, is a rare condition characterized by multifocal lymphatic proliferation, typically diagnosed during childhood. It affects various parenchymal organs, including the lungs, liver, spleen, bones, and skin. Lymphangiomas manifest across a wide age spectrum among patients and occur at multiple anatomical sites, presenting a diverse range of clinical and radiological manifestations.  The management of these tumors is complex and often requires involvement of multiple medical specialists, including orthopedic surgeons, radiologists, general surgeons, neurosurgeons, and oncologists.Results: The primary aim of presurgical embolization is to achieve devascularization of the tumor before surgical resection, resulting in reduced intraoperative blood loss and fewer complications. It is advisable to perform early resection following embolization, as revascularization can occur within six weeks in some cases.Conclusion: Limited research has been conducted on the embolization of musculoskeletal tumors for neoadjuvant, therapeutic, and palliative purposes. Nevertheless, there is a shortage of data regarding the results of presurgical embolization for musculoskeletal tumors.
